Description

本発明を完全に理解するには、涙器系の解剖学と生理学を考察する必要がある。涙腺
の眼窩部は上側頭眼窩に位置し水層状涙液膜を生成する。涙は涙腺の眼窩部からの小導管、
隣接眼瞼涙腺を通り上結膜円蓋で空になる。上眼瞼と下眼瞼の小さな副涙腺も又涙の生成に寄与する。涙は目表面を洗った後内側上眼瞼と内側下眼瞼の涙点及び涙小管に排出する。涙はこの涙小管から涙嚢、鼻涙管、鼻の順で流れる。

本発明のバルーンカテーテルは予定のDCRの口領域に経鼻的に導入できる。カテーテルは繊細な涙小管を通して導入するバルーンカテーテルより大きい収縮輪郭、従ってより大きな膨張直径を有する。本発明のバルーンカテーテルはガイドワイヤーを必要とせず、従ってガイドワイヤーが繊細な涙小管を傷つける機会はない。より大きい直径のバルーンDCRの口は術後狭窄になりにくく手術成功率は高くなる。

Claims (23)
- 患者の涙嚢と患者の鼻腔との分離壁にある狭い小さな準備鼻開口拡張用バルーンカテーテルで、近位端、近位部、遠位末端及び遠位部を有するチューブ状カテーテル本体と、この遠位部が細長い穴を有し、この遠位部周囲に配置した膨張可能なメンバーと、この細長い穴のこの遠位末端に近い位置で閉鎖したこのカテーテル本体と、加圧流体をこのカテーテル本体のこの近位端に加え、この加圧流体をこの細長い穴を通して流してこの準備開口の拡張にこの膨張可能なメンバーを膨張する手段、及びこの膨張可能なメンバーが収縮したときに、このカテーテルをこの鼻腔からこの外側鼻壁のこの準備開口を経てこの涙嚢へ押し込むに十分な堅さと管強度を持つハイポチューブからなるこのカテーテル本体からなるバルーンカテーテル。
- このカテーテル本体は肉厚が少なくとも0.035インチのステンレススチールからなる請求項1のバルーンカテーテル。
- カテーテル本体が外径0.083インチ、内径0.039インチ、肉厚0.044インチを有する請求項2のバルーンカテーテル。
- この遠位部がこの近位部に対し角度70°乃至115°で曲がった円曲部を有する遠位部を持つ請求項1のバルーンカテーテル。
- この遠位部がこの近位部に対し90°の角度である請求項4のバルーンカテーテル。
- このカテーテル本体がこの遠位末端で閉鎖した請求項1のバルーンカテーテル。
- このカテーテル本体がこの遠位末端から離れた位置で且つこの細長い穴とこの遠位末端間で閉鎖し、この遠位部の末端が開いた遠位部分を備え、この末端が開いた遠位部分がプローブ末端と連携してこのカテーテル本体の誘導を助ける請求項1のバルーンカテーテル。
- 患者の涙嚢と患者の鼻腔を分離する外側鼻壁にある準備開口拡張用バルーンカテーテルで、近位端、近位部、遠位末端及び遠位部を有するチューブ状カテーテル本体と、この遠位部が細長い穴を有し、この遠位部周囲に配置した膨張可能なメンバーと、この細長い穴のあるこの遠位末端に近い位置で閉鎖したこのカテーテル本体と、加圧流体をこのカテーテル本体のこの近位端に加え、加圧流体をこの細長い穴を通して流してこの準備開口の拡張用のこの膨張可能なメンバーを膨張する手段、及びこの近位部に対し70°乃至115°の角度で曲がった円曲部を有する遠位部からなるバルーンカテーテル。
- この遠位部がこの近位部に対し90°の角度である請求項8のバルーンカテーテル。
- この膨張可能なメンバーが遠位首部、中心部及び近位首部からなりこの近位首部がこの遠位部の近位端、この近位部の遠位末端及びこの曲部と接着剤で接合し、この遠位首部がこの細長い穴とこの遠位末端間のこの遠位部の一部と接着剤で接合し、その結果このより長い中心部がこの遠位部上に延びる事ができる請求項8のバルーンカテーテル。
- この膨張可能なメンバーが遠位首部、中心部及び近位首部からなり、この近位首部がこの遠位部分の近位端、この近位部の遠位末端及びこの曲部と接着剤で接合し、この遠位首部がこの細長い穴とこの遠位末端間のこの遠位部の一部と接着剤で接合し、その結果より長いこの中心部がこの遠位部上に延びる事ができる請求項8のバルーンカテーテル。
- 患者の涙嚢と鼻腔を分離する外側鼻壁に比較的大なる開口を形成する方法で、管状カテーテル本体が近位部、曲部及びこの近位部に対して70°乃至115°の角度を持つ遠位部、この遠位部周囲に取り付けた膨張可能なメンバー、この膨張可能なメンバー内のこの遠位部壁に開けた細長い穴からなり且つこの管状カテーテル本体がこの細長い穴の末端位で閉鎖したバルーンカテーテルを供与し、且つ加圧流体をこの管状カテーテル本体の近位端に供与してこの膨張可能なメンバーを膨張する手段を供与し、一個或いはそれ以上の器具を用いて開口がこの収縮した膨張可能なメンバーをこの遠位部にぴたりとはめ込める大きさで患者の内嚢壁、涙腺窩及びこの側鼻壁に通じる狭い小さな準備開口を形成し、このカテーテル本体のこの遠位部をこの準備開口にこの収縮した膨張可能なメンバーで押し込み、加圧流体をこのカテーテル本体のこの近位端及びこの細長い穴を通して導入しこの膨張可能な本体を膨張しこの準備開口を拡張する方法。
- 更にこの膨張可能な本体を収縮し、この遠位部の位置をわずかに変えこの膨張可能なメンバーを再度膨張させてこの準備開口の拡張を十分確実にする請求項12の方法。
- この膨張可能なメンバーが9バールで20秒間膨張する請求項12の方法。
- この準備開口の形成方法がボーマンプローブを患者の上涙点,上涙小管, 総涙小管及び涙嚢を通し、このプローブを内側涙嚢壁、涙腺窩骨及び外側鼻壁を経て患者の鼻腔に押し込んで小開口を形成する請求項12の方法。
- 更にこのプローブの押し込み操作を繰り返して内側涙嚢壁、涙腺窩骨及び外側鼻壁を経て鼻腔に通ずる数個の小開口を形成する事からなる請求項15の方法。
- 更にこのプローブをこの数個の開口内を上下に動かしてこの数個の小開口をこの準備開口に合体する事からなる請求項16の方法。
- 更に神経鈎をこの数個の小開口を通す事によりこの数個の小開口をこの準備開口に合体する事からなる請求項16の方法。
- このカテーテル本体がこの収縮した膨張可能なメンバーでこの遠位部をこの準備開口に押し込むに十分な堅さと管強度を有するハイポチューブからなる請求項12の方法。
- この近位部をこの遠位部がこの準備開口隣接位置に達する迄患者の鼻腔に入れ、この近位部をこの開口とこの遠位部と並ぶように回転しこの遠位部のこの開口への挿入を容易にする請求項12の方法。
- この管状カテーテル本体がこの遠位部に開放端部を備えるようにこのカテーテル本体の遠位末端の手前位置で閉鎖し、更にこの方法がプローブをこの開放端部に挿入しこの遠位部の目的位置への移動を容易にする事からなる請求項12の方法。
- 患者の涙嚢と鼻腔を分離する外側鼻壁に比較的大なる開口を形成する方法で、管状カテーテル本体が近位部分、遠位部、この遠位部周囲に取り付けた膨張可能なメンバー、この膨張可能なメンバー内のこの遠位部壁に開いた細長い穴からなり、この管状カテーテル本体がこの細長い穴の末端位置で閉鎖したバルーンカテーテルを供与し、且つこのカテーテル本体がこの膨張可能なメンバーが収縮したとき、このカテーテルをこの鼻腔から狭い小さな準備開口を経てこの涙嚢に押し込むに十分な堅さと管強度を有するハイポチューブから形成し、加圧流体をこの管状カテーテル本体の近位端に供与してこの膨張可能なメンバーを膨張する手段を供与し、一個或いはそれ以上の手術器具を用いて開口がこの遠位部とこの収縮した膨張可能なメンバーにぴたりとはめ込める大きさに、この外側鼻壁に通じる準備開口を形成し、このカテーテル本体のこの遠位部をこの準備開口にこの収縮した膨張可能なメンバーで押し込み、加圧流体をこのカテーテル本体のこの近位端及びこの細長い穴を通して導入しこの膨張可能な本体を膨張しこの準備開口を拡張する
方法。 - 更にこの膨張可能な本体を収縮し、この遠位部の位置をわずかに動かしこの膨張可能なメンバーを再度膨張してこの準備開口の拡張を十分確実にする事を含む請求項22の方法。
