一种泪道阻塞硅胶条嵌入术组合器械，除本手术
常规器械外，其特征是有下鼻道窥镜和导管钩取器。
手术时利用下鼻道窥镜很容易找到导管；利用导管钩
取器，方便导管从鼻道拖引出，使手术操作简化。降
低了对手术条件及施术人员的技术要求，缩短手术时
间，减少治疗费用。
Description
本实用新型涉及一组泪道阻塞硅胶条嵌入术组合器械。
慢性泪囊炎泪道阻塞是一种常见病。其根治方法多采用泪囊摘除、泪囊鼻道吻合、逆行钻孔扩大鼻泪管等。手术器械多只用泪道冲洗针头、泪点扩张针、泪道探针等。这些手术难度较大。均需切开泪囊，在鼻骨上凿孔，术中病人痛苦，术后内眦部留有疤痕，病人必须住院，费用高。近年来有人报导用挂线、插管、硅胶管埋藏、环状硅胶管逆行插管术等，均已取得成功。但是其手术操作复杂，在下鼻道处，导引线不易窥见取出。本实用新型组合器械的目的就在于简化这些手术操作，降低对手术场地、施术人员的技术要求，而手术成功率反而得到提高。
本实用新型组合器械，除常规泪道阻塞硅胶条嵌入手术用器械（泪道冲洗针头、泪点扩张针、泪道探针、麻醉导管硅胶条）外，其特征是使用了下鼻道窥镜（见图5）和麻醉导管钩取器（见图1）。
下面结合附图来介绍这两种新器械的结构及工作原理。
图1是导管钩取器的主视图；
图2是钩取器主要工作部位（图1）的局部放大图；
由图1不难看出，导管钩取器有手柄〔1〕，它一般由塑料制造（也可用金属材料）。手柄〔1〕的一端固结有一段不锈钢管〔2〕，其长短、粗细合适。手柄〔1〕内套有推销〔3〕，推销〔3〕上固结有不锈钢针〔4〕；手柄〔1〕内弹簧〔5〕将推销〔3〕始终推出手柄〔1〕通孔外，钢针〔4〕在钢管〔2〕内通到钢管的前端。由图2可以看出，钢管〔2〕的前端有穿透的斜向孔槽与钢针〔4〕前端部在这里交汇，其方向斜向手柄一方。〔6〕是被钩取物件。
图3、图4是手柄的另外两种结构形式，它们其后都有一悬伸部。在图3手柄悬伸部上铆结有弹簧片〔7〕，图4手柄悬伸部装有杠杆〔8〕，借压簧〔9〕使其复位。
图5是下鼻道窥镜的结构图；
图6是图5的A－A剖视图；
从图5、图6可以看出下鼻道窥镜是一个轴向截面上单边开口的圆锥形筒，它一般用铜材制造，至少是其内表面经光亮镀铬处理，明亮如镜。
手术时，先按常规手术进行处理。当用泪点扩张针扩大泪小点及泪小管，以泪道探针扩通泪总管或鼻泪管，顺泪小管、泪囊、鼻泪管插入麻醉导管直达下鼻道后，将本实用新型的下鼻道窥镜插入下鼻道，借额镜投射过来的光，用窥镜窥视到麻醉导管，再将本实用新型的导管钩取器从下鼻道窥镜中间插入下鼻道，用其前端的斜钩槽钩住导管，用手指推动推销〔3〕，借固结在推销上的钢针〔4〕和钢管〔2〕将导管夹持住，拖引出鼻孔外；将细尼龙丝双叠逆行插入导管内一段，再将硅胶管切成条状结成环形，套在双叠尼龙丝内，引入鼻泪管嵌住。
经发明人许多例临床实践证明，过去手术需要比较好的手术条件（县级以上医院），现在在较简陋的条件下就能做了；过去需要有相当实践经验的医护人员施术，现在一般医护人员就能做了；过去手术需要几个钟头，现在只要几十分钟了；过去病人必须住院，现在不必住院了，因而给病人节约了医疗费用，而且器械本身的制造技术也简单，因此，本实用新型组合器械有着它自己的优点。
Claims (3)
1、一种泪道阻塞硅胶条嵌入术组合器械，包括泪道冲洗针头、泪点扩张针、泪道探针，其特征是有下鼻道窥镜、(麻醉)导管钩取器，下鼻道窥镜是轴截面单边开口的圆锥形筒，至少内表面光亮镀铬，导管钩取器有手柄[1]，手柄的一端固结有不锈钢管[2]，手柄内套有推销[3]，推销上固结有不锈钢针[4]，手柄内的弹簧[5]将推销顶出手柄通孔外，钢针套在钢管内直到钢管前部，钢管前部有斜向槽口与钢针前端部交汇。
2、按权利要求1的组合器械，其特征是导管钩取器的手柄有悬伸部分，在悬伸部分铆接有弹簧片〔7〕顶着推销。
3、按权利要求1的组合器械，其特征是手柄悬伸部装有杠杆〔8〕，杠杆可绕柱销转动，借弹簧〔9〕的作用复位。
