Abstract
Description
現象について検討し、VPBを誘発しないトリガ式撮像プロトコルを開発することができるかを調べるために、雑種のイヌ(体重9〜32kg、平均体重22kg)でトリガ式コントラスト心エコー検査(TCE)を行った。動物をフェンタニル及びペントバルビタールで麻酔し、室内空気を用いて人工呼吸器によって機械的に換気した。本プロトコルは地域の倫理委員会によって承認されたものであり、全ての処置が致死的であり、現行の指針及び法規に従って行われた。
トランスデューサを慎重に配置した後にVPBは全ての動物で観察された。VPB検査の至適撮像平面は、解剖学的構造単独では識別することはできず、VPBを最も引き起こし易かった設定でTCEによって誘導しなければならなかった。トランスデューサの取替え時には、以前にビデオテープに記録した撮像平面との慎重な比較を行って、撮像平面の変化をできるだけ小さくした。
破壊流入撮像は、超音波造影剤を用いた灌流撮像で広く用いられる方法となりつつあるため、P3−2を用いたTIS試験を陽性対照としてP4−2でのTRIプロトコルを3匹の動物で試験した。結果を表1に示す。TRIの間の9000回を超える破壊パルス及び68000回を超える撮像パルスによって3回のVPBがトリガされるに留まった。P3−2トランスデューサでのTISはVPBの頻度が2桁大きかった。全てのトリガを収縮末期に行った。TIS法の図解については図3のAを、TRI法の図解については図3のBを参照されたい。図3のCに示すように、収縮初期のトリガを与えるとVPBの回数がさらに減少したと考えられる。
Claims (11)
- 超音波造影剤を投与したヒト又はヒト以外の動物の被検体の心臓のトリガ式超音波撮像方法であって、高エネルギー超音波パルスが心臓の不応期に収まるように該パルスを開始する、トリガ式超音波撮像方法。
- 高エネルギー超音波パルスを繰返してパルス系列を形成し、該パルス系列の最初のパルスが心臓の不応期に収まるように該パルスを開始する、請求項1記載のトリガ式超音波撮像方法。
- 最初の高エネルギー超音波パルスが心臓の心電図のQ−R−S区間に収まる、請求項1又は請求項2記載の方法。
- 最初の高エネルギー超音波パルスが心臓の心電図(ECG)のR波と同時に発生する、請求項1乃至請求項3いずれか1項記載の方法。
- さらに低エネルギー撮像パルスを、高エネルギー超音波パルス又はパルス系列の後に開始する、請求項1乃至請求項4いずれか1項記載の方法。
- 低エネルギー撮像パルスが心臓の心電図のT波又はその近傍で開始する、請求項5記載の方法。
- 用いる超音波法が破壊流入撮像、トリガ式補給撮像及びリアルタイム灌流撮像から選択される、請求項1乃至請求項6いずれか1項記載の方法。
- 心筋灌流の評価に用いられる、請求項1乃至請求項7いずれか1項記載の方法。
- 請求項1乃至請求項8いずれか1項記載の方法における超音波造影剤の使用。
- 高エネルギー超音波パルスが心臓の不応期に収まるように該パルスを開始する方法において心筋の灌流を測定又は評価するためヒト又はヒト以外の動物の被検体の血管系に投与される画像強調用組成物の製造における超音波造影剤の使用。
- ヒト又はヒト以外の動物の身体に予め投与される超音波造影剤の超音波誘発破壊又は変性方法であって、心臓の不応期に収まるようにパルスを開始する高エネルギー超音波パルスで身体の心臓の目標領域を照射して、不整脈を引き起こす危険性を最小限に抑制して造影剤の破壊又は変性を可能にする、方法。
