Abstract
【解決手段】フィルタ要素により覆われ展開可能な支柱アセンブリを介入処置の下流側及び遠位側に位置する展開部位に搬送されるように拘束シースの内部で圧縮されている体内管腔に使用する医療装置において、本体が第1の形状になるように応力を受けた時、及び本体にかかる応力が除去されて第2の形状になる時、線形擬似弾性動作を呈示するマルテンサイト相である冷間成形ニッケルチタン合金を含み、更に第1の形状において本体を少なくても部分的に包囲するシースを含んでいる医療装置。

Description
血液の流れが閉塞すると、患者に致命的な結果がもたらされることがあり得る。
塞栓予防装置及びシステムは、特に頚動脈処置に有用であるが、塞栓の危険がある血管介入処置においても同じく有用である。最近発売された、超弾性ニチノールを使用する塞栓予防装置は、Cordis CorporationのAngioguardとして知られている。
塞栓捕捉媒体から作られたフィルタエレメントは、展開可能な支柱アセンブリに取り付けられる。このフィルタエレメントは、傘の動きに似た展開可能な支柱の動きによって、折り畳まれた状態から展開された状態へと開く。
図1及び図2は、本発明の特徴を具体化する塞栓予防装置10の好適な実施の形態を示している。図1及び図2に示す実施の形態の特定の一例において、塞栓予防装置10はフィルタアセンブリ12として表される本体を備えている。フィルタアセンブリ12は、展開可能な支柱アセンブリ14と、フィルタ要素16とを有している。フィルタアセンブリ12は、細長い管状シャフトの遠位端に回転可能に取り付けられている。例えば、図1及び図2に示すシャフトはガイドワイヤ18からなる。
フィルタエレメント16は、コーン形状部分50と一体に設けられるとともにフィルタアセンブリ14の支柱28が展開されたときに実質的に円筒形状に展開する、短い近位部分52を有している。コーン形状部分50の短い近位部分52に配置された入口開口51は、塞栓細片27を集め、塞栓細片27をフィルタエレメント16内に差し向ける。
同様に、各支柱28の特定の寸法および形状は、本発明の範囲から逸脱することなく変更することができる。
更に、中央部分62により、支柱28はより大きな容積に展開できるようになる。こうして、必要な場合、より大きなフィルタエレメントを支柱アセンブリ14に配置することが可能となる。
次いで、レーザ切断したニチノール管材料30を所望の展開寸法まで冷間加工する。こうして、所望の展開寸法を、レーザ切断された管に付与あるいは設定することができる。
x軸とy軸とは、それぞれ、零から320ksiの応力単位と、0から9%のひずみ単位を標示している。

Claims (24)
- 支柱から形成された本体と、
前記本体を少なくとも部分的に包囲するシースと、
を備え、
前記本体は線形弾性のニッケルチタン合金を有し、この合金は少なくとも1つの三元系要素を含むことを特徴とする管腔用装置。 - 前記三元系要素は、パラジウム、プラチナ、クロム、鉄、コバルト、バナジウム、マンガン、ホウ素、銅、アルミニウム、タングステン、タンタル、ジルコニウムならびにこれらを組み合わせたものからなる群から選択されることを特徴とする請求項1記載の管腔用装置。
- 前記三元系要素は、プラチナ、パラジウム、タングステンならびにこれらを組み合わせたものからなる群から選択されることを特徴とする請求項2記載の管腔用装置。
- 前記本体の支柱は拡張可能な支柱アセンブリを含み、この拡張可能な支柱アセンブリは前記の線形弾性のニッケルチタン合金を有することを特徴とする請求項1記載の管腔用装置。
- 前記シースは少なくとも部分的に弾性であることを特徴とする請求項1記載の管腔用装置。
- 前記本体は薄壁を有するとともに本体を通過する開口を有していることを特徴とする請求項1記載の管腔用装置。
- 塞栓防止装置であることを特徴とする請求項1記載の管腔用装置。
- 大静脈フィルターであることを特徴とする請求項1記載の管腔用装置。
- 中隔用装置であることを特徴とする請求項1記載の管腔用装置。
- ステントであることを特徴とする請求項1記載の管腔用装置。
- 管腔用装置の使用方法であって、
a)支柱から形成された本体およびこの本体を圧縮部分において少なくとも部分的に包囲するシースを備えた管腔用装置を準備する工程であって、本体は線形弾性のニッケルチタン合金を有し、この合金は少なくとも1つの三元系要素を含むような本体およびシースを備えた管腔用装置を準備する工程と、
b)管腔用装置を哺乳類の管腔内に配置する工程と、
を備えたことを特徴とする管腔用装置の使用方法。 - シースを後退させて管腔内で管腔用装置を拡張させる工程をさらに備えたことを特徴とする請求項11記載の管腔用装置の使用方法。
- 管腔から管腔用装置を取り除く前に、シースによって本体を少なくとも部分的に包囲することにより、拡張した管腔用装置を縮める工程をさらに備えたことを特徴とする請求項12記載の管腔用装置の使用方法。
- 前記の少なくとも1つの三元系要素は、パラジウム、プラチナ、クロム、鉄、コバルト、バナジウム、マンガン、ホウ素、銅、アルミニウム、タングステン、タンタル、ジルコニウムならびにこれらを組み合わせたものからなる群から選択されることを特徴とする請求項11記載の管腔用装置の使用方法。
- 前記本体の支柱は拡張可能な支柱アセンブリを含み、この拡張可能な支柱アセンブリは前記の線形弾性のニッケルチタン合金を有することを特徴とする請求項11記載の管腔用装置の使用方法。
- 管腔用装置は塞栓防止装置であることを特徴とする請求項11記載の管腔用装置の使用方法。
- 管腔用装置はフィルター、中隔栓またはステントであることを特徴とする請求項11記載の管腔用装置の使用方法。
- 管腔用装置の製造方法であって、
a)支柱から形成され、少なくとも1つの三元系要素を含むような線形弾性のニッケルチタン合金を有する本体を準備する工程と、
b)前記本体を少なくとも部分的に包囲するシースを準備する工程と、
c)前記本体を所望の形状に形成する工程と、
d)前記本体を前記シース内で少なくとも部分的に包囲する工程と、
を備えたことを特徴とする管腔用装置の製造方法。 - 前記本体は、冷間成形により所望の形状に形成されることを特徴とする請求項18記載の管腔用装置の製造方法。
- 前記の少なくとも1つの三元系要素は、パラジウム、プラチナ、クロム、鉄、コバルト、バナジウム、マンガン、ホウ素、銅、アルミニウム、タングステン、タンタル、ジルコニウムならびにこれらを組み合わせたものからなる群から選択されることを特徴とする請求項18記載の管腔用装置の製造方法。
- 前記本体の支柱は拡張可能な支柱アセンブリを含み、この拡張可能な支柱アセンブリは前記の線形弾性のニッケルチタン合金を有することを特徴とする請求項18記載の管腔用装置の製造方法。
- 前記本体は薄壁を有し、さらに本体を通過する開口を有しており、この開口はレーザー切断、エッチングまたは打ち抜きにより形成されることを特徴とする請求項21記載の管腔用装置の製造方法。
- 管腔用装置は塞栓防止装置であることを特徴とする請求項18記載の管腔用装置の製造方法。
- 管腔用装置はフィルター、中隔栓またはステントであることを特徴とする請求項18記載の管腔用装置の製造方法。
