Abstract
内視鏡下での組織吻合等において、操作性が良い外科手術処置具を実現する。
【解決手段】
本発明の外科用処置具(301)は、挿入部(302)と、挿入部の一端に設けられた操作部(303)と、挿入部の他端より延出するように設けられた処置部(304)と、処置部に設けられ、それぞれ挟持面を有する2つの挟持部材(308)と、操作部に設けられ、2つの挟持部材の少なくとも1方を動かして開閉操作するための開閉操作部材(305)と、操作部に夫々設けられ、処置部を所定の角度に変更するための角度変更操作部材(306)と、操作部内に設けられ、角度変更操作部の操作に連動する被制動部材(361)と、操作部内に設けられ、開閉操作部材の動作に連動して、被制動部材を制動する制動手段(362)とを具備し、処置部は、制動手段によって被制動部材が制動されることにより、所定の角度を維持する。

Description
(第1の実施の形態)
以下に、本発明の第1の実施の形態を説明する。
図1は、第1の実施の形態に係るニードルドライバの正面斜め一側方からみた外観斜視図である。図2は、本実施形態のニードルドライバの正面図、図3は、本実施形態のニードルドライバを一側方(左側方)からみた左側面図、図4は、本実施形態のニードルドライバを他側方(右側方)からみた右側面図である。
図6から図11は、ニードルドライバ301の処置部304を含む先端部分の構造を説明するための図である。
従って、図11に示すように、シース311の内側には、同軸に、湾曲力伝達パイプ315と、回動力伝達パイプ313と、牽引ワイヤ314とが配置されている。
上記回動力伝達パイプ313は、当該回動力伝達パイプ313の軸を回動中心として回動摺動可能に挿通され、湾曲力伝達パイプ315は、当該湾曲力伝達パイプ315の軸方向に進退可能に挿通されている。
図10に示すように、先端ハウジング部材312は、先端ハウジング部材312の基端側においてシース311の内周面に嵌合する。先端ハウジング部材312の中央部と先端部内に、湾曲力伝達パイプ315の一部、回動力伝達コイル317、ジョイント部材318、リンク部材319及び湾曲部ベース部材320の一部とが配設される。先端ハウジング部材312の2つの腕部312aと312bの間に、湾曲部ベース部材320が配設され、湾曲部ベース部材320と先端ハウジング部材312とがピン324によって連結されている。具体的には、湾曲部ベース部材320と先端ハウジング部材312とは、2つの腕部312aと312bに嵌入するピン324が湾曲部ベース部材320に設けられた孔に嵌入されることによって連結され、湾曲部ベース部材320は、ピン324の軸を回動中心として、回動可能となっている。
回動部ベース部材325は、ステンレス製であり、回動部ベース部材325内には、先端側の開口部から、処置部304の挟持部308の一方の可動挟持片326の一部が内挿されている。可動挟持片326は、ステンレス製であり、基端側に内向フランジ部を有する円筒部材である。
可動挟持片326の基端側の底部には、牽引ワイヤ314が挿通可能な孔が設けられている。牽引ワイヤ314の先端部には、先端部を溶融して形成した末端肥大部314aが形成され、その末端肥大部314aが、可動挟持片326の底部の内側に固定されている。従って、牽引ワイヤ314が、操作部303側に引っ張られたときに、可動挟持片326も操作部303側に移動する。
操作部303において、開閉ボタン305を操作していないとき、バネ333は、可動挟持片326の底部を押圧するが、可動挟持片326のフランジ部326cの先端側面が固定挟持片331のフランジ部331aの基端側面と当接して、それ以上は伸びることができないので、圧縮された状態のままである。従って、操作部303の開閉ボタン305を操作していないときに、可動挟持片326と固定挟持片331のそれぞれの平面は密着するように押圧されているので、針をしっかりと挟持することができる。また、開閉ボタン305を押すと、可動挟持片326が、固定挟持片331から基端側に向かって移動するので、可動挟持片326と固定挟持片331のそれぞれの平面間に挟持された針を放したり、針を挟持するためにそれぞれの平面の間を離したりすることができる。
上述したように、固定挟持片331の基端側の円柱部分は、可動挟持片326の孔部に挿通され、その円柱部分が回動部ベース部材325に固定されているので、固定挟持片331は、回動部ベース部材325に対して固定された位置関係を有する。言い換えれば、固定挟持片331は、湾曲部ベース部材320に対しても長軸方向に固定された位置関係を有する。
針が挟持された状態において、あるいは針が挟持されていない状態において、回動ダイヤル307が回動されると、軸部材である回動力伝達パイプ313が軸を回動中心として回動するために、回動力伝達パイプ313に固定された回動力伝達コイル317が回動し、回動力伝達コイル317に固定された回動部ベース部材325も回動する。回動ダイヤル307が回動された量に応じて、回動力伝達パイプ313が回動するので、回動ダイヤル307が回動された量に応じた回動量が、処置部304へ伝達される。その結果、挟持部308を構成する固定挟持片331と可動挟持片326は、回動部ベース部材325の回動に連動して、回動部ベース部材325と共に回動する。
角度可変レバー306を挿入軸方向の先端側から基端側に向かって移動させることによって、図14に示すように、処置部304を含む先端部分は湾曲する。角度可変レバー306を挿入軸方向の先端側から基端側に向かって移動させると、湾曲力伝達パイプ315は、ジョイント部材318を先端側に押し、その結果、ジョイント部材318は、リンク部材319を押す。押されたリンク部材319は、さらに、湾曲部ベース部材320を押すが、湾曲部ベース部材320は、先端ハウジング部材312にピン324によって連結されているため、湾曲部ベース部材320は、ピン324を回動中心として回動する。
なお、調整ダイヤル309のダイヤル頭部309aは、その外周部分を把持し易くするために、操作部303の両側面から突出するように、操作部303の幅方向の長さよりも長い外径を有している。
こうして、スラスタ353は、2つの螺子ピン354a,354bの軸が留パイプ346の長軸に直交するように、留パイプ346に嵌着される。
図27に示す角度可変レバー306の状態、つまり、2つの角度可変レバー306が紙面の左側(操作部303の先端側)に傾いている状態において、処置部304は、その長軸が挿入部302の軸にと略同じ軸上にある状態、すなわち、挿入部302に対して略直線となる状態である。
図30に示すように、開閉ボタン305が術者によって矢印A方向に押し込まれると、図31に示すように、板バネ363は、ブレーキシュー362を矢印B方向へ押圧する。この矢印B方向へ押圧されたブレーキシュー362は、その一面と接触するブレーキバー361を矢印B方向へ押圧する。
以下に、本発明の第2の実施の形態を説明する。尚、図36から図47を参照して本実施の形態のニードルドライバについて説明するが、第1の実施の形態と同じ構成には同一符号を用いて、それらの説明を省略し、異なる構成、作用及び効果についてのみ説明する。
穴部382は、その頂面382aが先端側に向けて斜面となるような横断面形状が略台形をしており、リンク部材385の一端部分が収容されている。この穴部382の頂面382aは、リンク部材385の一端部とが当接し、且つ、摺動する摺動面を構成している。
図49に示すように、開閉ボタン380の押し込み量が増大するにつれて、板バネ395がブレーキバー361に対して押圧力が増大し、ブレーキバー361に係る制動力が増加する。上述したように、開閉ボタン380の押し込み操作による動作直後、リンク部材385の板バネ側当接部386により、急激に板バネ395がブレーキバー361に対して押圧力が増大して、板バネ395は、ブレーキバー361を制止させるに十分な所定の制動力F3を発生する。

Claims (6)
- 挿入部と、
該挿入部の一端に設けられた操作部と、
前記挿入部の他端より延出するように設けられた処置部と、
該処置部に設けられ、それぞれ挟持面を有する2つの挟持部材と、
前記操作部に設けられ、前記2つの挟持部材の少なくとも1方を動かして開閉操作するための開閉操作部材と、
前記操作部に設けられ、前記処置部を所定の角度に変更するための角度変更操作部材と、
前記操作部内に設けられ、前記角度変更操作部の操作に連動する被制動部材と、
前記操作部内に設けられ、前記開閉操作部材の動作に連動して、前記被制動部材を制動する制動手段と、
を具備し、
前記処置部は、前記制動手段によって前記被制動部材が制動されることにより、前記所定の角度を維持することを特徴とする外科用処置具。 - 前記制動手段は、前記開閉操作部材と前記被制動部材との間に設けられ、前記開閉操作部材の動作に連動するカム機構を具備することを特徴とする請求項1に記載の外科用処置具。
- 前記カム機構は、前記開閉操作部材により前記2つの挟持部材の少なくとも1方が動かされる前に、前記制動手段が前記被制動部材を制止するために必要な所定の制動力を付与するような前記輪郭曲線を備えた原節を有していることを特徴とする請求項2に記載の外科用処置具。
- 前記カム機構と前記被制動部材の間に弾性要素を有する請求項2又は請求項3に記載の外科用処置具。
- 前記被制動部材は、所定の摩擦力が与えられるように、前記操作部内に設けられていることを特徴とする請求項1から請求項4のいずれかに記載の外科用処置具。
- 前記制動手段は、前記開閉操作部材により前記2つの挟持部材の少なくとも1方が動かされている間において、常に前記被制動部材を制止していることを特徴とする請求項1から請求項5のいずれかに記載の外科用処置具。
