Abstract
Description
(a)患者の心房中隔の組織に対して、1つまたは複数の電極を位置決めする工程;

増大したペーシング閾値(increased pacing threshold)
のうちの少なくとも1つに基づいて卵円窩を識別する工程;
からなる各工程を含むことを特徴とする患者の卵円窩を識別する方法が提供される。

Claims (20)
- 患者の卵円窩を識別する方法であって、
(a)患者の心房中隔の組織に対して、1つまたは複数の電極を位置決めする工程;
(b)該電極を心房中隔の組織に対して複数の位置に移動させながら、心房中隔の組織の単極及び/または双極電位図を獲得する工程;並びに
(c)以下のパラメータ:
単極電圧の低下、
信号分裂、
広がった信号、
低下した信号スルーレート、
低下した局所心筋層インピーダンス、
増大した位相角、及び
増大したペーシング閾値
のうちの少なくとも1つに基づいて卵円窩を識別する工程;
からなる各工程を含むことを特徴とする患者の卵円窩を識別する方法。 - 卵円窩が、双極電圧の低下に基づいても識別される請求項1記載の方法。
- 卵円窩が、以下のパラメータ:

(c)以下のパラメータ:

低下した局所心筋層インピーダンス、
増大した位相角、及び
増大したペーシング閾値
のうちの少なくとも1つに基づいて卵円窩を識別する工程;並びに

の各工程を含むことを特徴とする患者に経中隔穿刺を実施する方法。 - 前記1つまたは複数の電極がカテーテルの遠位端上に設けられ、かつ、前記位置決めする工程が該カテーテルの遠位端を患者の心房中隔の組織に対して位置決めする工程からなる請求項4記載の方法。
- 前記穿通する工程が、針を前記カテーテルの内部に通し、卵円窩を貫通して左心房の中に押し入れる工程を含む請求項5記載の方法。
- 2つの電極が前記カテーテル上に設けられ、該電極の一方が該カテーテルの遠位先端に配置され、該電極の他方が前記一方の電極に対して近位の該カテーテル上に配置される請求項5記載の方法。
- 単極及び双極電位図が得られ、かつ、前記カテーテルの遠位端が心房中隔の組織に接触していることを確認するために、該単極電位図中のST部の上昇を観察する工程をさらに含む請求項5記載の方法。
- 卵円窩が双極電圧の低下に基づいても識別される請求項4記載の方法。

該カテーテルは、カテーテルの遠位端が卵円窩の位置を特定するための電気生理学的マッピング用カテーテルとしてばかりでなく、カテーテルの内腔の中に位置決めした経中隔針上を伝ってカテーテルを押し入れることによって経中隔穿刺処置時に卵円窩を穿通するのに適切な拡張器としても使用可能であるように構成されている
ことを特徴とする経中隔穿刺に使用するためのカテーテル。 - 卵円窩が単極電圧の低下に基づいて識別される請求項1記載の方法。

をさらに含む請求項6記載の方法。 - 前記カテーテルの遠位端にはテーパが付けられ、さらに、前記第2電極が約2mmと約4mmとの間の間隔だけ前記第1電極から離間されている請求項11記載の方法。
- 前記カテーテルが前記第1及び第2電極と電気接続している第1及び第2電気リード線、並びに前記カテーテルの近位端に第1及び第2ケーブルをさらに備え、前記第1及び第2ケーブルが前記第1及び第2電気リード線と電気接続しかつ電位図を記録する装置に接続するように構成されている請求項17記載の方法。
- 患者の卵円窩の位置を特定し、かつ、卵円窩の経中隔穿刺を実行する経中隔装置であって、
(a)遠位端を有する中空シース;
(b)該シースの中に挿入されるように構成した内部カテーテルであって、該カテーテルは、内腔と、該カテーテルの遠位端に位置決めした第1電極と、該カテーテル上に位置決めされかつ前記第1電極から近位に離間された第2電極とを有し、該カテーテルは、該カテーテルの遠位端が該シースの遠位端を越えて突き出した状態で該カテーテルが該シースの中に挿入可能であるように該シースよりも長い、内部カテーテル;並びに
(c)該電極と電気接続している電位図を記録する記録装置;
を備え、
前記カテーテルは、経中隔針を、前記針の先端が前記カテーテルの遠位端を越えて突き出すまで前記内腔に押し通すことができるように構成され、
さらに、前記経中隔装置が、以下のパラメータ:
単極電圧の低下、
信号分裂、
広がった信号、
低下した信号スルーレート、
低下した局所心筋層インピーダンス、
増大した位相角、及び
増大したペーシング閾値
のうちの少なくとも1つに基づいて、使用者が患者の卵円窩を識別できるように構成されている
ことを特徴とする経中隔装置。 - 前記カテーテルの遠位端にはテーパが付けられ、かつ、前記第2電極が前記第1電極から約2mmと約4mmとの間の間隔だけ離間されている請求項19記載の経中隔装置。
