Claims (1)
- 【特許請求の範囲】 1. 被検者の脳の異常活動の発生を検出する方法であって、 (a) 被検者の脳の活動を示す入力信号を信号プロセッサ内に受信し、 (b) 前記入力信号からてんかん発生成分を抽出しかつ強調するように構成さ れた第1フィルタを前記入力信号に適用することにより、前記入力信号内のてん かん発生成分を決定し、 (c) フォアグラウンド時期に対応する前記てんかん発生成分にオーダ統計量 フィルタを適用することにより前記入力信号のフォアグラウンド時期でのてんか ん発生活動を測定して、てんかん発生活動のフォアグラウンド測定値を生成し、 (d) 前記フォアグラウンド測定値が、前記異常活動の発生を示す閾値レベル に達するかどうかを決定し、かつ (e) 前記異常活動が発生している間に段階(a)〜(d)を実行する 段階を含む被検者の脳の異常活動の発生を検出する方法。 2. 段階(b)が、前記第1フィルタとしてディジタルフィルタを使用する段 階を含む請求項1に記載の方法。 3. 段階(b)が、前記ディジタルフィルタとして有限インパルス応答フィル タを使用する段階を含む請求項2に記載の方法。 4. 段階(b)が、前記ディジタルフィルタとして無限インパルス応答フィル タを使用する段階を含む請求項2に記載の方法。 5. 段階(b)が、前記第1フィルタとしてアナログフィルタを使用する段階 を含む請求項1に記載の方法。 6. 段階(b)が、前記第1フィルタの適用結果を二乗して、前記てんかん成 分を生成する段階を含む請求項1に記載の方法。 7. 段階(c)が、前記バックグラウンド時期に相当する前記てんかん発生成 分に前記オーダ統計量を適用することにより前記入力信号のバックグラウンド時 期でのてんかん発生活動を測定して、てんかん発生活動のバックグラウンド測定 値を生成する段階を含み、ここで前記バックグラウンド時期は前記フォアグラウ ンド時期の前に生じるものであり、 段階(d)が、前記バックグラウンド測定値に対する前記フォアグラウンド測 定値の比率が、前記閾値レベルに達するかどうかを決定する段階を含む請求項1 に記載の方法。 8. 段階(c)が、前記フォアグラウンド時期および前記バックグラウンド時 期をこれらの間に時間遅延を存在させるように構成する段階を含む請求項7に記 載の方法。 9. 段階(c)が、前記フォアグラウンド時期を約2秒、前記バックグラウン ド時期を約20秒、前記遅延を約1秒として構成する段階を含む請求項8に記載 の方法。 10. 段階(c)が、前記フォアグラウンド測定値および前記バックグラウンド 測定値を連続的に更新する段階を含む請求項7に記載の方法。 11. 前記閾値レベルが第1閾値レベルであり、段階(c)が、前記比率が第2 閾値レベルに達すると前記バックグラウンド測定値の更新を中断する段階を含む 請求項10に記載の方法。 12. 段階(e)が、発作の臨床学的成分の開始の前に段階(a)〜(d)を実 施して、この臨床学的成分を予測させる段階を含む請求項1に記載の方法。 13. 段階(e)が、発作の電気曲線記録図上の成分の開始の前に段階(a)〜 (d)を実施して、この電気曲線記録図上の成分を予測させる段階を含む請求項 12に記載の方法。 14. 段階(b)が、前記てんかん発生成分を抽出しかつ強調するように構成さ れた複数のフィルタを含むフィルタバンクから前記第1フィルタを選択する段階 を含む請求項1に記載の方法。 15. 段階(b)が、てんかん発生成分の最も大きな微分を提供する前記フィル タバンクからのフィルタとして前記第1フィルタを選択する段階を含む請求項1 4に記載の方法。 16. 前記信号プロセッサが前記フィルタバンクを記憶するためのメモリ手段を 含み、段階(b)が、前記フィルタバンクから複数のフィルタを検索し、前記複 数のフィルタを前記信号プロセッサ内の前記入力信号に適用し、かつ、てんかん 発生成分の最も大きい微分を提供する前記複数からのフィルタとして前記第1フ ィルタを選択する段階を含む請求項15に記載の方法。 17. 被検者のてんかん発生成分の微分を増すために前記第1フィルタを構成す る段階を更に含む請求項1に記載の方法。 18. 段階(b)が、有限インパルス応答フィルタおよび無限インパルス応答フ ィルタの一方を前記ディジタルフィルタとして使用し、かつ前記てんかん発生成 分を強調するために前記第1フィルタの適用結果を二乗する段階を含み、 段階(c)が、前記バックグラウンド時期に相当する前記てんかん発生成分に 前記オーダ統計量を適用することにより、前記入力信号のバックグラウンド時期 でのてんかん発生活動を測定して、てんかん発生活動のバックグラウンド測定値 を生成し、かつ前記フォアグラウンド測定値および前記バックグラウンド測定値 を連続的に更新する段階を含み、ここで前記バックグラウンド時期は前記フォア グラウンド時期の前に生じ、かつ前記フォアグラウンド時期および前記バックグ ラウンド時期はそれらの間に時間の遅延を有し、 段階(d)は、前記バックグラウンド測定値に対する前記フォアグラウンド測 定値の比率が前記閾値レベルに達するかどうかを決定する段階を含み、 前記閾値レベルが第1閾値レベルであり、段階(c)は、前記比率が第2閾値 レベルに達した場合に前記バックグラウンド測定値の更新を中断する段階を含む 請求項1に記載の方法。 19. 段階(a)が、被検者の脳の活動を検出し、かつこの活動を示す前記信号 を生成するように動作し得る少なくとも1個の電極から前記信号を受信する段階 を含み、前記少なくとも1個の電極が、頭皮電極および埋め込み型電極からなる 群の中から選択される請求項1に記載の方法。 20.段階(a)が、メモリ装置から前記信号を受信する段階を含む請求項1に記 載の方法。 21.段階(b)が、マイクロプロセッサおよびコンピュータからなる群の中から 選択される前記信号プロセッサで前記信号を分析する段階を含む請求項1に記載 の方法。 22.段階(b)が、被検者内に埋め込まれた信号プロセッサで前記信号を分析す る段階を含む請求項1に記載の方法。 23.段階(b)が、ウェーブレットフィルタを前記第1フィルタとして使用して 前記信号を分析して、前記信号の相当するウェーブレット係数を決定し、前記ウ ェーブレット係数を使用して、パワー密度分布を決定し、かつ前記パワー密度分 布を、前記分布による前記閾値レベルの交叉が発作発生を示す前記閾値レベルと 比較する段階を含む請求項1に記載の方法。 24.段階(b)が、前記第1フィルタとしてウインドウ付フーリエ変換および逆 フーリエ変換を使用して前記信号を分析する段階を含む請求項1に記載の方法。 25.前記異常活動としての発作発生の指示に応答して、医薬投与、被検者の脳の 一部の電気刺激、被検者の脳の一部の磁気的刺激、被検者の脳の一部の活動阻害 、被検者の神経の電気刺激、前記信号の記録、警告の賦活化、患者の生理的受容 体の刺激、被検者の脳の少なくとも一部の加熱、被検者の脳の少なくとも一部の 冷却、被検者の脳の一部の活動の促進、被検者の脳の一部の活動の抑制、および 被検者の脳の一部の切除からなる群の中から選択される出力を生成する段階を含 む請求項1に記載の方法。 26.前記出力を生成するために被検者に埋め込まれた装置を使用する段階を更に 含む請求項25に記載の方法。 27.前記第1フィルタが遺伝学的に設計されたフィルタを含む請求項1に記載の 方法。 28.被検者の脳内での発作発生を予測する方法であって、 (a) 被検者の脳の活動を示す入力信号を信号プロセッサ内に受信し、 (b) 被検者での発作発生を予示する少なくとも1つの前駆症状について前記 信号を分析し、かつ (c) 前記少なくとも1つの前駆症状が発生した際に、発作が発生する前に応 答出力を生成する 段階を含む被検者の脳内での発作発生を予測する方法。 29.段階(b)が、前記信号におけるてんかん様放電を検出し、前記スパイクの 各々について最適フィットの放物線を決定することにより前記放電の各々の鮮鋭 度を決定し、各鮮鋭度を時間ウインドウにおける他の放電の鮮鋭度と比較するこ とにより前記放電の各々の相対的鮮鋭度の指標を決定し、極性を有する前記スパ イクで、前記指標が前記所定レベルに達するかどうかを決定し、かつ前記所定レ ベルに達する前記スパイクが発作を予示する所定のパターンにフィットするかど うかを決定する段階を含む請求項28に記載の方法。 30.段階(b)が、スパイク形状係数を抽出するために信号分析フィルタを用い ててんかん様スパイクを検出することにより前記前駆症状を決定し、バックグラ ウンド信号情報に対する二乗したスパイク形状係数の比率を決定し、前記比率が 前記所定レベルを超えるかどうかを決定し、かつその場合には前記比率を超える スパイクが発作を予示するものとして決定されたパターンとフィットするかどう かを決定する段階を含む請求項29に記載の方法。 31.段階(b)が、バックグラウンドエネルギーと比較される現信号エネルギー の比率を決定することにより前記前駆症状を決定し、かつ前記比率が発作を予示 する前記所定レベルを超えるかどうか決定する段階を含む請求項28に記載の方 法。 32.バックグラウンドメディアン周波数に対するメディアン周波数の比率を決定 することにより前記前駆症状を決定し、かつ前記比率が発作を予示する前記所定 レベルを超えるかどうかを決定する請求項28に記載の方法。 33.被検者の脳の活動状態を分析する方法であって、 (a) 被検者の脳の活動を表す信号を信号プロセッサ内に受信し、 (b) 被検者の脳の現在の活動状態を示す情報の存在について前記信号プロセ ッサ内で前記信号を分析し、 (c) 前記活動状態が発生している間に段階(b)を実行し、かつ (d) 前記情報内における前記存在に応答して出力を生成する 段階を含む被検者の脳の活動状態を分析する方法。 34.段階(b)が、ディジタルフィルタ、非線形フィルタ、および適応型フィル タ、相関積分、アーク長さ微分および温度フィルタからなる群の中から選択され るフィルタを前記第1フィルタとして使用する段階を含む請求項1に記載の方法 。 35.前記方法が、てんかん発作の発生を前記発作として検出する方法を含む請求 項1に記載の方法。 36.被検者に関する他の生物学的信号を受信し、かつ発作の発生を検出する際に 前記生物学的信号を使用する段階を更に含み、前記生物学的信号が、呼吸活動、 グルコース、遊離基、ニューロトランスミッタ、血液、体液の濃度、脳温度、心 臓活動、筋肉活動、眼球活動、磁場、皮膚耐性、および電場からなる群の中から 選択される被検者の生物機能を表す請求項1に記載の方法。 37.発作またはてんかん様放電を示す被検者にバイオフィードバックを提供する 段階を更に含む請求項1に記載の方法。 38.時間および状態による重み付け平均化を用いて発生されたバックグラウンド 信号に対する前記フォアグラウンド時期における前記てんかん発生活動を測定す る段階を含む請求項1に記載の方法。 39.前記時間および状態による重み付け平均化として指数忘却時間平均化を用い る段階を含む請求項38に記載の方法。 40.被検者の脳での発作発生を検出するための発作の検出、予測および処理装置 であって、 被検者の脳の活動を示す入力信号を受信するための受信手段と、 前記入力信号からてんかん発生成分を抽出しかつ強調するように構成された第 1フィルタを前記入力信号に適用することにより、前記入力信号のてんかん発生 成分を決定し、 フォアグラウンド時期に対応する前記てんかん発生成分にオーダ統計量フィル タを適用することにより前記入力信号のフォアグラウンド時期のてんかん発生活 動を測定して、てんかん発生活動のフォアグラウンド測定値を生成し、かつ 前記フォアグラウンド測定値が、発作の発生を示す所定のレベルを超えるかど うかを決定するために、前記受信手段に結合された信号処理手段と、 発作が発生している間に発作の発生を示す出力を生成するための出力手段とを 含む発作の検出、予測および処理装置。
