Description

The invention relates to a device for testing the wrist condition of a patient. When the forearm is turned from the maximum supine position (palm of the hand up) to the maximum pronation position (palm of the hand downward), the radius, one of the two bones of the forearm, rotates in relation to the ulna, the other forearm bone, around an oblique axis defined by a line connecting the center of the radial head (proximal radius) and the center of the the ulnar head (distal part of the ulna). The two bones are parallel in supination, while in pronation the radius crosses the ulna. This crossing of the bones implies that the distal portion of the radius rotates around the distal portion of the ulna, i.e., moves from one side of the ulnar head to the other side of the ulnar head, when 15 Forearm rotates from the pronation position to the supine position. This is illustrated in the appended FIG. 1, which shows the respective distal ends of the radius and the ulna, indicated by the markers 1 and 2, in the pronation (FIG. 1a), supination (FIG. 1c) and neutral (FIG. figure lb). When the forearm is horizontal and the hand holds a load, at least a portion of the vertical force generated by the load will be dissipated on the ulnar side face as shear stresses. In the neutral position, on the other hand, the force is mainly transmitted to the ulna in the form of compressive stresses. It is therefore understood that at each degree of prenupuncture, between the pronation and supination positions, the configuration of the stresses applied to the ulnar head will be unique. The test device according to the invention aims to test the resistance of the ulnar radio articulation, more generally the wrist, to a load in a large number of different positions.

To this end, there is provided a device for testing the condition of a wrist, characterized in that it comprises: a structure comprising a handle and a guide portion, a movable member guided on the guide portion, and a load suspended on the movable member, the guide portion being arranged so that the load remains substantially vertical to the handle during a rotational movement of the structure in a given plane. Such a device makes it possible, during a continuous rotational movement of the wrist of a patient, such as a prognosis movement, to subject the patient's hand to a force of gravity of constant intensity and moment relative to the patient. rotation axis of the wrist substantially zero. It is thus possible to reliably determine and compare the resistance of the wrist in all positions of this continuous rotational movement. In practice, the value of the load can be incrementally increased so as to determine the threshold up to which the patient will be able to support the load throughout the rotational movement without feeling any pain. This threshold of appearance of the pain is representative of the condition of the wrist. The higher the wrist is healthy, and the more particularly the cartilage at the level of the ulnar radio articulation is in good condition. Other characteristics and advantages of the present invention will appear on reading the following detailed description of a particular embodiment of the invention with reference to the appended diagrammatic drawings in which: FIG. 1, already commented on, shows the respective distal ends of the radius and ulna in the pronation positions (Fig. 1a), supination (Fig. 1c) and in the neutral position (Fig. 1b), Fig. 2 schematically shows the device according to the invention in partial cut view, 2909854 3 shows the device according to the invention held by the hand of a patient in three different positions of a pronosupination movement of the wrist, namely a pronation position (FIG. 3a), a neutral position (FIG. 3b). and a supine position 5 (Figure 3c). With reference to FIG. 2, a device for testing the state of a wrist according to the invention comprises a rigid structure 3 to which a load 4 is suspended. The rigid structure 3 comprises a straight part 5, a curved concave part 6 which connects the two ends of the right part 5 and a handle or to handle 7 located between the straight and curved parts 5, 6, the handle 7 being fixed by one of its ends to the right part 5 and extending orthogonally to the latter, the other end of the handle 7 remaining free. The curved portion 6 preferably has a circular arc shape, as illustrated, and its section is for example circular. The concave face 6a of this curved portion 6 cooperates with a groove of a pulley 8 to support and guide this pulley 8 so that it can move freely from one end to the other of the concave face 6e. The load 4 is suspended from the axis 9 of the pulley 8 by means of a wire or a rod 10 suspended by a hook 11 to a U-shaped part 12 of which 20; two ends are respectively fixed at the ends of the axis 9. In the example illustrated, the load 4 comprises a support member 13 retained by the end of the wire or the rod 10 opposite the hook 11. This support member 13 comprises a plate 14 on which can be placed and superimposed one or more removable weights 15 and a central guide member 16 cooperating with a central bore weights 15 for guiding and positioning these radially. The weights 15 can be placed on or removed from the tray 14 by unhooking the hook 11 from the workpiece 12 and moving the weights 15 along the wire or the rod 10, the hook 11 having a smaller size than the central bore of the weights 15 to allow their passage.

In order to determine the state of a patient's wrist, the patient is asked to hold the structure 3 in a vertical plane (plane of FIG. 2) by its handle 7, with the forearm in a horizontal position, then to turn the wrist between a pronation position and a supination position (see Figure 3). During this movement of rotation of the wrist, the structure 3 rotates in the aforementioned vertical plane and, under the effect of the vertical force applied by the load 4, the pulley 8 moves on the curve portion 6 so that the load 4 remains under the handle 7 and the hand, to the vertical thereof, and remains more precisely substantially centered on a vertical line 17 passing through the center of the handle 7. The wrist of the patient is thus subjected, during its movement, to a constant direction and intensity force and moment relative to the axis of rotation of the wrist substantially constant because substantially zero. Weights may be removed or added to adjust the value of the load 4. The amount of load from which the patient will feel pain at any time during wrist rotation will be representative of the condition of the wrist. The device according to the invention can be used, for example, before and after a wrist operation, to evaluate the improvement provided by said operation.
