Abstract

Il s'agit d'une armature pour implant cotyloïdien de prothèse de hanche, destinée à être fixée à l'os et à réceptionner un cotyle, l'armature (1) comprenant un premier (2) et un deuxième (3) renforts disposés en croix et qui possèdent des branches (4 à 7) rabattues les unes vers les autres pour définir un logement de réception du cotyle, et une première patte (11) en forme de crochet et une deuxième patte (12) qui permettent la fixation de l'armature à l'os, la deuxième patte étant en forme de disque liée par un coude (17) à l'autre (5) des branches du premier renfort (2). L'une (5) des branches du deuxième renfort comprend un orifice (20) qui définit un plan (A) avec le coude de la deuxième patte. Un retour (14) de la première patte (11) est décalé par rapport à ce plan en étant sensiblement parallèle à ce plan.

l
La présente invention est relative à une armature pour implant cotyloïdien de prothèse de hanche, notamment destiné à être utilisé lorsque l'os du bassin délimitant la cavité cotyloïdienne est détérioré.
Les armatures couramment rencontrées dans de tels implants, comprennent habituellement: - un premier et un deuxième renforts qui sont disposés sensiblement en croix et qui possèdent des branches rabattues les unes vers les autres pour définir un logement de réception d'un cotyle de forme sensiblement hémisphérique, les deux renforts se croisant au sommet du 10 logement hémisphérique, et - une première patte et une deuxième pattes qui permettent la fixation de l'armature à l'os et qui font saillie latéralement depuis chacune des branches du premier renfort selon des directions sensiblement diamétralement opposées, la première patte ayant la forme d'un crochet qui comprend un retour solidaire de l'une des branches du premier renfort et un rabat prolongeant le retour et muni d'un orifice de passage de vis de fixation, la deuxième patte étant en forme de disque liée par un coude à l'autre des branches du premier renfort, dans laquelle sont ménagés au moins deux 20 orifices de passage de vis de fixation.
Même si de telles armatures donnent entière satisfaction quant au positionnement du cotyle par rapport à l'os de la cavité cotyloïdienne, les pattes de fixation de l'armature ne permettent au chirurgien d'implanter l'armature en choisissant au mieux la position angulaire en fonction de l'anatomie du patient. 25 Ceci a pour inconvénient d'engendrer de nombreux problèmes de luxation une fois l'implant introduit dans le patient.
La présente invention a donc pour but de pallier à ces inconvénients en fournissant une armature favorisant un meilleur positionnement pour lutter plus efficacement contre les problèmes de luxation. 30 A cet effet, selon la présente invention, l'une des branches du deuxième renfort comprend un orifice qui définit un plan avec le coude de la deuxième patte, et le retour de la première patte est décalé par rapport à ce plan en étant sensiblement parallèle à ce plan.
Ainsi, grâce à ces dispositions, le chirurgien a une plus grande 35 liberté d'action dans le positionnement de l'armature par rapport à l'os de manière à minimiser des problèmes de luxation ultérieurs.
De manière préférée, le retour de la première patte et le logement hémisphérique sont situés de part et d'autre du plan défini par le coude et l'orifice ménagé dans le deuxième renfort.
Encore de manière préférée, l'autre branche du deuxième renfort comprend un orifice de passage de vis de fixation.
En variante, l'autre branche du deuxième renfort est plus courte que les autres branches des premier et deuxième renforts.
De préférence, des plots font saillie depuis les premier et deuxième renforts, en direction de l'intérieur du logement hémisphérique.
De manière préférée, l'armature est réalisée en matériau métallique formé d'un alliage de chrome-cobalt.
En variante, le sommet du logement comprend un trou taraudé permettant le montage de l'armature sur un ancillaire de mise en place dans une cavité cotyloïdienne.
De toute façon, l'invention sera bien comprise à l'aide de la description qui suit en référence au dessin schématique annexé représentant à titre d'exemple non limitatif, une forme de réalisation préférentielle d'une armature d'implant cotyloidien selon l'invention.
La figure 1 est une vue en perspective de l'armature pour implant 20 cotyloidien selon la présente invention.
La figure 2 est une vue de côté de l'implant de la figure 1.
La figure 3 est une vue en coupe transversale de l'implant de la figure 1.
L'armature 1 selon la présente invention représentée aux figures 25 est destinée à faire partie d'un implant cotyloïdien (non représenté) qui est généralement utilisé au sein d'une prothèse de hanche et fixé à l'os d'un bassin humain dont la cavité cotyloïdienne est détériorée. De tels implants sont bien connus de l'homme du métier et l'on pourra se référer à l'art antérieur pour de plus amples détails au sujet des parties constitutives de l'implant autres que 30 l'armature représentée aux figures. Notamment, l'armature est destinée à réceptionner un cotyle par exemple à cimenter.
De manière connue en soi, l'armature 1 comprend un premier renfort 2 et un deuxième renfort 3 qui sont disposés l'un par rapport à l'autre en croix et qui possèdent chacun deux branches respectivement notées 4, 5 et 6, 35 7. Les quatre branches 4 à 7 sont rabattues les unes vers les autres pour définir un logement de réception d'un cotyle (non représenté) de forme sensiblement hémisphérique. De telles armatures en forme de croix sont couramment appelées croix de Kerboull.
Le premier renfort 2 et le deuxième renfort 3 sont ainsi sensiblement disposés à 900 l'un de l'autre et se croisent au sommet 10 du logement 9.
De manière encore habituellement rencontrée, l'armature possède une première patte 11 et une deuxième patte 12 qui s'étendent respectivement à chacune des-branches 4, 5 du premier renfort 2. Elles font saillie latéralement depuis ce renfort selon des directions sensiblement diamétralement opposées. 10 La première patte 11 est de forme en crochet dont la section transversale est en U. Cette patte 11 possède ainsi un retour 14 qui est solidaire de la branche 4 du premier renfort 2, ainsi qu'un rabat 15 qui prolonge ce retour 14 et qui est muni d'un orifice 16 pour le passage d'une vis de fixation de l'armature 1 sur l'os.
Par ailleurs, la deuxième patte 12 qui s'étend depuis l'autre branche 5 du premier renfort 2 est en forme de disque et est liée à cette branche par un coude 17. Cette deuxième patte 12 possède également au moins deux orifices 18, 19 destinés aux passages de vis de fixation de l'armature dans l'os.
Selon une caractéristique essentielle de la présente invention, l'une 6 des branches du deuxième renfort 3 comprend un orifice 20 pour le passage de vis de fixation qui définit, avec le coude 17 de la deuxième patte 12, un plan A. Le retour 14 de la première patte 11 est décalé par rapport à ce plan et est sensiblement parallèle à ce plan.
Le plan A s'étend de préférence parallèlement à la base du logement hémisphérique 9. Le retour 14 et le logement hémisphérique 9 sont situés de part et d'autre de ce plan A. Ainsi, le décalage de niveau créé entre le retour 14 de la première patte 11 en forme de crochet et le coude 17 de la deuxième patte 12 en forme 30 de disque permet une plus grande latitude d'implantation de l'armature pour éviter des problèmes ultérieurs de luxation pour le patient.
De préférence, l'autre branche 7 du deuxième renfort 3 comprend également un orifice de passage 22 pour des vis de fixation.
Afin de faciliter le positionnement de l'armature 1, cette branche 7 35 est plus courte que les autres branches 4 à 6 du premier renfort 2 et du deuxième renfort 3.
Ainsi, l'orifice 22 ménagé dans l'autre branche 7 du deuxième renfort 3 n'appartient pas au plan A et est situé à l'opposé du retour 14 par rapport à ce plan.
Afin de faciliter l'immobilisation d'un cotyle à l'intérieur du logement 5 9, quatre plots 25 à 28 font saillie depuis chacune des branches 4 à 7 des premier et deuxième renforts 2, 3. Un ciment peut être ainsi placé entre le cotyle et l'armature 1.
L'armature est par exemple réalisée en un matériau métallique tel qu'un alliage chrome-cobalt.
Un trou taraudé 30 est ménagé dans le sommet 10 du logement hémisphérique 9 pour permettre le montage de l'armature 1 sur un ancillaire de mise en place (non représenté) de l'armature dans une cavité cotyloidienne.
Comme il va de soi, l'invention ne se limite pas à la forme de réalisation préférentielle décrite ci-dessus à titre d'exemple non limitatif. Elle en 15 embrasse au contraire toutes les variantes de réalisation dans le cadre des
revendications ci-après.

Claims (6)

REVENDICATIONS
1 - Armature pour implant cotyloidien de prothèse de hanche, destinée à être fixée à l'os et à réceptionner un cotyle, l'armature (1) comprenant: - un premier (2) et un deuxième (3) renforts qui sont disposés sensiblement en croix et qui possèdent des branches (4 à 7) rabattues les unes vers les autres pour définir un logement (9) de réception du cotyle de forme sensiblement hémisphérique, les deux renforts (2, 3) se croisant au sommet 10 (10) du logement hémisphérique (9), et - une première patte (11) et une deuxième patte (12) qui permettent la fixation de l'armature (1) à l'os et qui font saillie latéralement depuis chacune des branches (4, 5) du premier renfort (2) selon des directions sensiblement diamétralement opposées, la première patte (11) ayant la forme d'un crochet qui comprend un retour (14) solidaire de l'une (4) des branches du premier renfort (2) et un rabat (15) prolongeant le retour (14) et muni d'un orifice (16) de passage de vis de fixation, la deuxième patte (12) étant en forme de disque liée par un coude (17) à l'autre 20 (5) des branches du premier renfort (2), dans lequel sont ménagés au moins deux orifices (18, 19) de passage de vis de fixation.
caractérisée en ce que l'une (5) des branches du deuxième renfort (3) comprend un orifice (20) qui définit un plan (A) avec le coude (17) de la deuxième patte (12), et en ce que le retour (14) de la première patte (11) est 25 décalé par rapport à ce plan en étant sensiblement parallèle à ce plan.
2 - Armature selon la revendication 1, caractérisée en ce que le retour (14) de la première patte (11) et le logement hémisphérique (9) sont situés de part et d'autre du plan (A) défini par le coude (17) et l'orifice (20) ménagé dans le deuxième renfort (3).
3 - Armature selon la revendication 2, caractérisée en ce que l'autre branche (7) du deuxième renfort (3) comprend un orifice (22) de passage de vis de fixation.
4 - Armature selon la revendication 2 ou la revendication 3, caractérisée en ce que l'autre branche (7) du deuxième renfort (3) est plus 35 courte que les autres branches (4 à 6) des premier (2) et deuxième (3) renforts.
- Armature selon l'une des revendications 1 à 4, caractérisée en ce que des plots (25 à 28) font saillie depuis les premier (2) et deuxième (3) renforts, en direction de l'intérieur du logement hémisphérique (9).
6 - Armature selon la revendication 1 à 5, caractérisée en ce qu'elle est réalisée en matériau métallique formé d'un alliage de chrome-cobalt.
7 - Armature selon l'une des revendications 1 à 6, caractérisée en ce que le sommet (10) du logement (9) comprend un trou taraudé (30) permettant le montage de l'armature (1) sur un ancillaire de mise en place dans une cavité cotyloïdienne.
