Description

"Cotyle scellé"
La présente invention concerne un cotyle scellé pour une prothèse de hanche.
Il existe différents types de prothèses de hanche utilisant les uns des cotyles vissés et, les autres, des cotyles scellés. Chaque technique a ses avantages et ses inconvénients.
Le cotyle vissé se compose d'une partie extérieure métallique, munie de moyens de vissage dans l'os du bassin et recevant à l'intérieur une coupelle en matière synthétique qui elle-même reçoit la rotule ou boule de la tête de prothèse du fémur.
Ce type de cotyle présente en général l'inconvénient de n'avoir aucune souplesse.
Les cotyles scellés sont réalisés actuellement en matière synthétique tel que du polyéthylène haute densité. Malheureusement, à l'usage, ce cotyle est trop souple et, sous l'effet des contraintes exercées par la boule de la prothèse du fémur, il engendre des efforts de cisaillement au niveau du scellement (ciment), ce qui crée un risque de descellement du cotyle.
Actuellement, l'état du développement de ces deux techniques fait choisir, de préférence, un cotyle présentant une certaine souplesse. Malheureusement, le cotyle en matière synthétique connu ne donne pas entière satisfaction à cause du risque de descellement.
La présente invention a pour but de créer un cotyle à sceller, remédiant aux inconvénients des cotyles connus, qui soit souple tout en évitant le risque de descellement.
A cet effet, l'invention concerne un cotyle caractérisé en ce qu'il se compose d'une coupelle formant une cavité sphérique pour recevoir la boule de la tête de fémur et ayant sur sa surface extérieure des moyens d'ancrage pour le scellement, ainsi que d'une armature métallique, cette armature se composant d'une partie centrale prolongée par plusieurs branches, et la face externe de la coupelle comporte une empreinte au moins partiellement creuse, de forme correspondant à celle de l'armature pour recevoir celle-ci.
Ce cotyle présente une excellente souplesse qui adapte le cotyle au travail de l'os de la hanche, dans lequel il est scellé, sans pour autant présenter le risque de cisaillement détruisant le scellement.
Suivant une autre caractéristique de l'invention, la coupelle comporte en son sommet de la surface extérieure un téton ayant à sa base une gorge et l'armature comporte dans sa partie centrale un orifice de dimensions correspondant à celles du téton pour permettre la fixation de l'armature par enclipsage sur le téton, les branches de l'armature se logeant dans les rainures correspondantes formant l'empreinte.
Suivant une autre caractéristique de l'invention, l'empreinte de l'armature reçoit les branches de l'armature de façon libre, seules les extrémités des branches étant reçues dans des logements situés au niveau du bord de la coupelle.
Suivant une autre caractéristique de l'invention, les moyens d'ancrage de la surface extérieure de la coupelle sont constitués par des rainures circulaires.
Suivant une autre caractéristique de l'invention, le cotyle comporte une lèvre périphérique bordant la cavité dont la forme est supérieure à celle d'une demi-sphère.
Suivant une autre caractéristique de l'invention, la surface extérieure est munie d'un bord périphérique.
Suivant une autre caractéristique de l'invention, le téton d'accrochage de l'armature est creux.
Suivant une autre caractéristique de l'invention, l'armature comporte cinq branches.
La présente invention sera décrite plus en détail à l'aide des dessins annexés dans lesquels

Selon les figures 1 et 2, le cotyle se compose d'une coupelle 1, en forme de dôme, ayant une cavité sphérique 2 dont le volume est légèrement supérieur à celui d'une demi-sphère de centre 0 pour recevoir la boule de la tête de fémur (non représentée). Cette cavité est bordée par une lèvre périphérique 3 qui la referme légèrement. Extérieurement, la coupelle comporte un bord périphérique 4 de façon à former un logement qui retient la boule.
La surface extérieure 5 de la coupelle 1 comporte des éléments d'ancrage constitués par des rainures périphériques 6, 7, 8. En son sommet, la coupelle comporte un téton 9 dont la base forme une gorge 10.
Le cotyle comporte également une armature 11 composée d'une partie centrale 12 et de branches 13. La partie centrale 12 de préférence circulaire ou polygonale, comporte un trou 14 de dimensions légèrement inférieures au brossage périphérique 15 formé par le téton 9 et la gorge 10 de façon à pouvoir s'enclipser sur ce téton 9 et s' > accrocher.
Pour recevoir les branches 13 qui, selon le mode de réalisation représenté. sont au nombre de cinq (figure 3) en forme de marguerite, la surface extérieure 5 de la coupelle est munie de rainures méridiennes 16, de forme correspondant à celle des branches 13 de façon à recevoir celles-ci en creux.
De façon préférentielle, pour permettre le mouvement relatif de l'armature et de la coupelle, les branches 13 sont de largeur constantes et il en est de même des gorges méridiennes 16.
La partie centrale 12 se loge dans la rainure périphérique 6 entourant le téton 9. Cette rainure 6 et les rainures méridiennes 16 forment l'empreinte recevant l'armature 11.
Selon le mode de réalisation représenté, les extrémités 17 de chaque branche 13 de l'armature viennent librement dans un logement 18 réalisé dans le bord 4 de la coupelle 1.
Le cotyle décrit ci-dessus se fabrique en deux parties, la coupelle et l'armature ; la coupelle 1 peut être réalisée par moulage avec éventuellement usinage des rainures périphériques et méridiennes.
L'armature est réalisée en un métal tel que
ou un alliage ; elle est mise en forme sphérique ou assimilée puis mise en place dans l'empreinte de la surface extérieure de la coupelle ; sa fixation est assurée par enclipsage sur le téton 9 et réception des extrémités 17 des branches 13 dans les logements 18.

2) Cotyle selon la revendication 1, caractérisé en ce que la coupelle comporte en son sommet de la surface extérieure (5) un téton (9) ayant à sa base une gorge (10) et l'armature (11) comporte dans sa partie centrale (12) un orifice de dimensions correspondant à celles du téton (9) pour permettre la fixation de l'armature (11) par enclipsage sur le téton (9), les branches (13) de l'armature (11) se logeant dans les rainures (16) correspondantes formant l'empreinte.
3) Cotyle selon l'une quelconque des revendications 1 et 2, caractérisé en ce que l'empreinte de l'armature (11) reçoit les branches (13) de l'armature (11) de façon libre, seules les extrémités (17) des branches (13) étant reçues dans des logements C8) situés au niveau du bord (4) de la coupelle (1).
4) Cotyle selon la revendication 1, caractérisé en ce que les moyens d'ancrage de la surface extérieure de la coupelle (1) sont constitués par des rainures circulaires (6, 7, 8).
5) Cotyle selon la revendication 1, caractérisé en ce qu'il comporte une lèvre périphérique (3) bordant la cavité (2) dont la forme est supérieure à celle d'une demi-sphère.
6) Cotyle selon la revendication 1, caractérisé en ce que la surface extérieure est munie d'un bord périphérique (4).
7) Cotyle selon la revendication 2, caractérisé en ce que le téton (9) d'accrochage de l'armature (11) est creux.
8) Cotyle selon l'une quelconque des revendications 1 à 7, caractérisé en ce que l'armature (11) comporte cinq branches (13).
