Abstract

Cette tige fémorale de prothèse totale de hanche à fixation primaire comporte: - un col prothétique (4) destiné à dépasser du fémur et dont l'extrémité (5) reçoit une sphère, - une tige proprement dite (1), destinée à être enfouie dans le fémur à renforcer, comportant une portion proximale (2) trochantéro- diaphysaire évasée dans le plan antéro-postérieur, et une portion distale (3), et comportant sur ses deux faces latérales, principales des rainures parallèles (8, 9). Lesdites rainures (8) sont orientées perpendiculairement à l'axe F du col prothétique (4) en zone proximale (2). La tige comporte également sur ses deux faces latérales principales en zone médiane (10) des rainures longitudinales (9).

Description

TIGE FEMORALE POUR PROTHESE TOTALE DE HANCHE
L'invention concerne une tige fémorale perfectionnée pour prothèse de hanche. Elle concerne également les prothèses de hanche équipées de telles tiges.
De manière connue et pour l'essentiel, une tige fémorale pour prothèse de hanche comprend
- tout d'abord, une tige fémorale, destinée à être insérée dans le fémur, et plus précisément dans le canal médullaire de celui-ci
- une cupule cotyloidienne, destinée à être engagée dans la cavité cotyloïdienne de l'aile illiaque de l'articulation de la hanche considérée
- enfin, un col prothétique, coiffé d'une sphère, destiné à relier la tige à la cupule pour assurer ainsi l'articulation proprement dite.
Différentes techniques de mise en place de telles prothèses sont à ce jour connues. Parmi celles-ci, la technique dite "à fixation primaire", dans laquelle la tige est fixée en force dans l'os, notamment par vissage, s'est largement développée. Cette technique présente néanmoins l'inconvénient de provoquer des douleurs postopératoires, résultant a priori des pics de contraintes formés au niveau du pas de vis. Afin de diminuer ces douleurs, on a alors proposé une tige fémorale de prothèse totale de hanche à fixation primaire, présentant un profil longitudinal effilé depuis la tête jusqu'à la pointe, ayant une section trapézoïdale, et dans laquelle d'une part, la courbure antérieure de la tige vue dans un plan longitudinal coïncide avec la courbure du cintre obturateur du fémur par rapport au cotyle pour faciliter la standardisation, et d'autre part, la portion proximale trochantero-diaphysaire de la tête de la tige est évasée dans le plan antéro-postérieur.
Par "cintre obturateur" du fémur, on désigne classiquement la ligne radiologique idéale qui s'étend du bassin jusqu'au fémur7 et qui correspond à la partie supérieure du trou obturateur en se continuant par le bord inférieur du col du fémur, puis par le bord médial de la métaphyse fémorale proximale. En d'autres termes, on cherche à se rapprocher le plus possible de la forme anatomique de la partie supérieure du fémur.
L'un des problèmes majeurs auquel sont confrontés les praticiens réside dans le fait que la fixation et le maintien de la tige en place dans le canal médulaire du fémur n'est pas toujours satisfaisante. En effet, on observe à la longue un descellement partiel de la tige, nécessitant de procéder à une nouvelle intervention chirurgicale, soit pour resceller la prothèse, soit pour sceller une nouvelle prothèse.
On a alors proposé de ménager au niveau des deux faces principales de la portion proximale de la tige une pluralité de rainures longitudinales parallèles. De la sorte, on aboutit à une meilleure repousse osseuse, et de fait, à un meilleur accrochage de l'os. Néanmoins, lors de certains mouvements de l'articulation, notamment des mouvements latéraux, on s'est aperçu que ces simples rainures longitudinales sont quelquefois inefficaces pour empêcher un descellement progressif de la tige.
L'invention vise à pallier l'ensemble de ces différents inconvénients. Elle propose une tige fémorale de prothèse totale de hanche à fixation primaire comportant
- un col prothétique destiné à dépasser du fémur et dont l'extrémité reçoit une sphère
- une tige proprement dite, destinée à être enfouie dans le fémur à renforcer, comportant une portion proximale trochantérodiaphysaire évasée dans le plan antéro-postérieur, et une portion distale, et comportant sur ses deux faces latérales principales des rainures parallèles.
Cette tige fémorale se caractérise
- en ce que lesdites rainures sont orientées perpendiculairement à l'axe du col prothétique en zone proximale
- et en ce que ladite tige comporte également sur ses deux faces latérales principales en zone médiane des rainures longitudinales.
En d'autres termes, I'invention consiste à munir de manière supplémentaire de rainures parallèles la partie proximale de tige pour prothèse totale de hanche connue par ailleurs, ces rainures étant oirentées perpendiculairement à l'axe du col prothétique, permettant de la sorte de favoriser la repousse et donc l'ancrage osseux à ce niveau, et s'opposant ainsi aux contraintes conduisant au descellement de la tige, y compris lors des mouvements latéraux de l'articulation.
Avantageusement, toute la zone proximale et médiane au niveau desquelles sont ménagées les dites rainures, subit un grenaillage afin de conférer à cette partie une surface micro-poreuse, destinée à favoriser de manière supplémentaire la repousse et donc l'accrochage osseux.
Avantageusement, les rainures longitudinales de la zone médiane sont parallèles à l'axe de la tige.
Dans une autre forme de réalisation, la partie proximale et la partie médiane de la tige au niveau de laquelle sont ménagées lesdites rainures est revêtue d'un dépôt de poudre d'hydroxy-apatite.
La manière dont l'invention peut être réalisée et les avantages qui en découlent ressortiront mieux de l'exemple de réalisation qui suit donné à titre indicatif et non limitatif à l'appui des figures annexées.
La figure 1 est une représention schématique d'une vue latérale d'une tige conforme à l'invention.
La figure 2 est une coupe partielle transversale réalisée selon la ligne I - 1 de la figure 1.
La tige conforme à l'invention est désignée par la référence générale (1) et est réalisée en un alliage métallique, notamment à base de titane. Elle est effilée de la tête (2) jusqu'à la pointe (3), la tête (2) étant reliée au col prothétique (4) de manière continue. Ce dernier se termine par un embranchement tronconique (5), sur lequel vient se loger la sphère d'articulation (non représentée) et dont l'angle au sommet est typiquement voisin de 5".
L'extrémité de la portion proximale au niveau de sa liaison avec le col prothétique (4) et toute la portion distale de la tige sont lisses. En revanche, le reste de la tige, notamment la zone proximale (2), définie entre la face antéro-postérieure (7) et la courbure (6) de la tige, et la zone médiane (10) subissent soit un grenaillage, soit sont revêtues d'un dépôt de poudre d'hydroxy-apatite, projetée par plasma, et ce de manière à présenter un état de surface microporeux, facilitant outre la stabilité secondaire, la reprise de l'os sur la prothèse et donc la stabilité ultérieure de la tige.
De manière connue, la courbure antérieure de la tige (1) désignée par la référence (6), vue dans le plan longitudinal coïncide avec la courbure du cintre obturateur du fémur par rapport au cotyle et est typiquement voisine de 138".
Selon l'invention, la partie proximale (2) et la partie médiane (10) de la tige, présentent sur leurs deux faces principales latérales une pluralité de rainures parallèles respectivement (8) et (9). Ces rainures de largeur égale mais de profondeur pouvant éventuellement varier, sont également de longueur variable. En zone proximale, ces rainures (8) sont orientées perpendiculairement par rapport à l'axe F du col prothétique (5).
En revanche, en zone médiane les rainures (9) parallèles entre elles sont alignées selon l'axe longitudinal de la tige. Ces différentes rainures sont destinées à favoriser la reprise et l'ancrage osseux une fois la tige en place dans le canal médullaire du fémur. Compte tenu de l'orientation respective des rainures (8) et (9), on obtient une meilleure résistance aux contraintes lors des efforts effectués au niveau de l'articulation et ce, quelque soit la direction de fonctionnement de la dite articulation.
De la sorte, les prothèses à fixation primaire ainsi réalisées, dont la fixation est assurée par ciment, présentent l'avantage outre d'être très simple à mettre en oeuvre, de présenter une grande stabilité dans le tissu osseux compte tenu de leur revêtement mais également de la présence de ces rainures favorisant l'ancrage osseux et constituant des points de résistances aux contraintes exercées à leur niveau.

Claims (4)

REVENDICATIONS
1/ Tige fémorale de prothèse totale de hanche à fixation primaire comportant
- un col prothétique (4) destiné à dépasser du fémur et dont l'extrémité (5) reçoit une sphère;
- une tige proprement dite (1), destinée à être enfouie dans le fémur à renforcer, comportant une portion proximale (2) trochantérodiaphysaire évasée dans le plan antéro-postérieur, et une portion distale (3), et comportant sur ses deux faces latérales principales des rainures parallèles (8,9), caractérisée
- en ce que lesdites rainures (8) sont orientées perpendiculairement à l'axe F du col prothétique (4) en zone proximale (2)
- et en ce que ladite tige comporte également sur ses deux faces latérales principales en zone médiane (10) des rainures longitudinales (9).
2/ Tige fémorale selon la revendication 1, caractérisée en ce que la zone proximale (2) et la zone médiane (10) au niveau desquelles sont ménagées les rainures parallèles (8,9) subissent un grenaillage afin de conférer à cette partie une surface micro-poreuse, destinée à favoriser de manière supplémentaire la repousse et l'accrochage osseux.
3/ Tige fémorale selon l'une des revendications 1 et 2, caractérisée en ce que les rainures parallèles longitudinales (9) de la zone médiane sont parallèles à l'axe de la tige (1).
4/ Tige fémorale selon l'une des revendications 1 à 3, caractérisée en ce que la partie proximale (2) et la partie médiane (10) de la tige (1) au niveau desquelles sont ménagées lesdites rainures (8,9) sont revêtues d'un dépôt de poudre d'hydroxy-apatite.
