Abstract

Selon l'invention, la tige (1) comprend : - des faces antérieure et postérieure (15, 16; 21, 22) qui, au niveau de la partie métaphysaire (2a), convergent l'une vers l'autre depuis le côté externe vers le côté interne de l'articulation, en formant entre elles un angle de l'ordre de 30, et qui divergent depuis la base de cette partie métaphysaire (2a) vers le sommet de celle-ci, également en formant entre elles un angle de l'ordre de 30, et- des nodules (20) aménagés dans une portion supéro-interne des faces antérieure et postérieure (15, 16) de ladite partie métaphysaire (2a), c'est-à-dire sur la zone de ces faces (15, 16) s'étendant sensiblement entre le plan antéro-postérieur contenant l'axe longitudinal de la tige (1) et le bord interne de ces faces (15, 16).

Description

La présente invention concerne une tige fémorale de prothèse de hanche, destinée à être implantée sans ciment.
Une tige fémorale se compose classiquement d'une queue médullaire effilée, destinée à être engagée dans le canal médullaire du fémur, d'un col, qui reconstitue le col anatomique du fémur, et d'une tête sphérique fixée sur l'extrémité libre du col, qui reconstitue la tête du fémur. Cette tête sphérique est destinée à être reçue dans une cavité correspondante délimitée par un élément cotyloïdien fixé au bassin.
Une technique classique de fixation d'une tige fémorale consiste à employer un ciment synthétique polymérisable. Cette technique n'est toutefois pas dénuée d'inconvénients tels que ablation osseuse importante, nécrose osseuse résultant de l'échauffement du ciment lors de sa polymérisation, risque de mauvaise répartition du ciment ou de bulles d'air pouvant conduire à des mouvements de la tige par rapport à l'os et à un descellement.
Pour remédier à ces inconvénients, il a été envisagé de fixer une tige fémorale sans ciment, par repousse osseuse au travers d'un revêtement poreux que comprend la queue médullaire. Cette repousse osseuse implique une immobilisation préalable de la queue médullaire, généralement réalisée par coincement de cette queue dans le canal médullaire.
Avec ce type de prothèse, le problème est d'obtenir un bon équilibre entre l'appui de la tige contre l'os, pour permettre une bonne répartition des contraintes exercées sur la tige, et le coincement de la tige par rapport à l'os, pour une parfaite immobilisation de celle-ci le temps que la repousse osseuse s'opère.
Un coincement par des appuis sur des surfaces trop localisées se traduit par des douleurs et peut engendrer des dégradations de l'os.
Un coincement insuffisant, en revanche, n'assure pas une bonne immobilisation de la tige et nuit à la réalisation d'un bon ancrage par repousse osseuse.
Les tiges existantes de ce type ne donnent pas entière satisfaction sur ce point, et la présente invention vise à remédier à cet inconvénient pratique essentiel.
A cet effet, la tige qu'elle concerne comprend
- des faces antérieure et postérieure qui, au niveau de la partie métaphysaire de la tige, convergent l'une vers l'autre depuis le côté externe vers le côté interne de l'articulation, en formant entre elles un angle de l'ordre de 3 , et qui divergent depuis la base de cette partie métaphysaire vers le sommet de celle-ci, également en formant entre elles un angle de l'ordre de 3", et
- des nodules aménagés dans une portion supérointerne des faces antérieure et postérieure de ladite partie métaphysaire, c'est-à-dire sur la zone de ces faces s'étendant sensiblement entre le plan antéro-postérieur contenant l'axe longitudinal de la tige et le bord interne de ces faces.
La forme précitée de la partie métaphysaire de cette tige assure un parfait blocage de la tige dans le canal médullaire par effet de coin, avec obtention d'un équilibre optimal entre l'appui de la tige contre l'os et le coincement de cette tige dans l'os.
Les portions des faces antérieure et postérieure comprenant les nodules viennent, par cette forme, étroitement en appui contre l'os spongieux. Lors de la repousse osseuse, ces nodules permettent une parfaite intégration de la tige à l'os, sans risque d'amorce de rupture de la liaison os-queue médullaire lors du travail de la tige en compression.
Avantageusement, la tige comprend une collerette faisant saillie de sa face interne, conformée de manière à venir en appui contre l'os lorsque la tige est mise en place. Cette collerette permet de répartir la charge que subit la prothèse sur une surface d'os relativement importante.
Avantageusement, la tige présente, au niveau de la partie diaphysaire de sa queue médullaire, des faces antérieure et postérieure parallèles à l'axe longitudinal de la tige, et des faces interne et externe qui, au niveau de cette même partie diaphysaire, convergent en direction de l'extrémité distale de la queue médullaire, en formant chacune, avec l'axe longitudinal de la tige, un angle de l'ordre de 3 .
Cette partie diaphysaire permet ainsi un centrage avec coincement progressif de la tige dans le canal médullaire, ce qui évite l'apparition de douleurs distales lorsque la prothèse est en charge.
De préférence, les faces interne et externe de la tige présentent chacune, depuis la partie proximale de la queue médullaire jusqu'à l'extrémité distale de celle-ci, une forme convexe engendrée par un rayon de l'ordre de 8 millimètres. Ces faces permettent ainsi un appui de la tige contre l'os sur une surface importante et dépourvue d'arête vive, éliminant tout risque de prise d'appui localisée contre l'os, susceptible de générer des douleurs ou des dégradations de l'os.
De préférence, la partie métaphysaire et la zone proximale de la partie diaphysaire comprennent un revêtement constitué par une couche d'alumine et par une couche d'hydroxyapatite de calcium, tandis que le reste de la partie diaphysaire comprend uniquement une couche d'alumine. Ces matériaux assurent une parfaite ostéo-intégration de la queue médullaire.
Selon une forme de réalisation préférée de l'invention, la tige est constituée par assemblage de deux pièces faisant partie d'un ensemble de pièces assemblables qui comprend
- au moins une pièce constituant une queue médullaire, et
- un jeu de plusieurs autres pièces constituant des cols fémoraux, susceptibles d'être assemblés à ladite au moins une pièce constituant une queue médullaire, la partie de ces pièces constituant des cols qui supporte la tête fémorale ayant, d'une pièce à l'autre, des longueurs différentes et/ou des positionnements radiaux différents par rapport à un axe de référence contenu dans le plan sagittal médian de la pièce constituant la queue médullaire, et/ou des orientations différentes par rapport à ce plan sagittal médian.
Une pièce constituant une queue médullaire et une pièce constituant un col fémoral peuvent ainsi être sélectionnées en fonction de l'anatomie du patient et être assemblées pour constituer, à partir d'un ensemble de pièces en nombre relativement réduit, une tige précisément adaptée à cette anatomie.
Le jeu de plusieurs pièces constituant des cols fémoraux peut notamment comprendre
- une pièce à col "axé", c'est-à-dire dont l'axe de la partie supportant la tête fémorale prothétique est coaxial audit axe de référence
- une pièce à col "décalé", c'est-à-dire dont l'axe de la partie supportant la tête fémorale prothétique est parallèle audit axe de référence mais est décalé radialement par rapport à ce dernier, et
- quatre pièces à cols "antéversés", c'est-à-dire dont l'axe de la partie supportant la tête fémorale forme un angle avec le plan sagittal médian de la queue médullaire, cette partie étant orientée, après assemblage, du côté antérieur de la tige, deux de ces pièces étant destinées à des prothèses pour côté droit et les deux autres à des prothèses pour côté gauche ; l'une des pièces destinées à un même côté a un col "axé" et vautre un col "décalé" tels que précités.
L'assemblage d'une pièce constituant une queue médullaire avec une pièce constituant un col fémoral est de préférence réalisé au moyen d'un pion tronconique à faible pente, faisant saillie de l'une de ces pièces, et d'un puits tronconique de forme complémentaire, aménagé dans une autre de ces pièces, les axes de ce pion et de ce puits définissant l'axe de référence précité.
De préférence, la section transversale du pion et du puits est elliptique et le grand axe de l'ellipse est contenu dans le plan défini par l'axe longitudinal de la queue médullaire et par l'axe de ce pion et de ce puits.
Pour sa bonne compréhension, l'invention est à nouveau décrite ci-dessous en référence au dessin schématique annexé représentant, à titre d'exemple non limitatif, une forme de réalisation préférée de la tige fémorale de prothèse de hanche qu'elle concerne.
La figure 1 est une vue en perspective de deux pièces à partir desquelles elle peut être constituée
la figure 2 est une vue de côté de cinq pièces à partir desquelles elle peut être constituée
la figure 3 est une vue de profil d'une pièce à partir de laquelle elle peut être constituée, et
les figures 4 et 5 sont des vues en coupe de cette pièce selon respectivement les lignes IV-IV et V-V de la figure 3.
La figure 1 représente une tige fémorale 1 de prothèse de hanche, destinée à être implantée sans ciment.
Cette tige 1 est constituée par assemblage de deux pièces faisant partie d'un ensemble de pièces assemblables de manière modulaire. Cet ensemble comprend notamment, ainsi que cela apparaît aux figures 1 et 2
- un jeu de plusieurs pièces 2 de différentes tailles, constituant chacune une queue médullaire (une seule de ces pièces est représentée)
- un jeu de plusieurs autres pièces 3, 4, 5, 6 constituant des cols fémoraux, susceptibles d'être assemblés à chacune des pièces 2, et
- un jeu de plusieurs têtes fémorales 7 de différentes tailles, dont une seule est représentée.
Chaque pièce 2 comprend un pion tronconique 10 à faible pente, faisant saillie de la partie supérieure de la partie métaphysaire 2a de cette pièce 2, en formant avec l'axe longitudinal de cette pièce 2 un angle de l'ordre de 135". La section transversale de ce pion 10 est elliptique, et le grand axe de cette ellipse est contenu dans le plan défini par l'axe longitudinal de la pièce 2 et l'axe de ce pion 10.
Le pion 10 comprend un alésage taraudé axial 11 permettant le montage de la pièce 2 sur l'extrémité d'un instrument (non représenté) de mise en place dans le canal médullaire.
Une collerette 12 fait saillie de la face interne de la pièce 2, à la base du pion 10, cette collerette 12 étant conformée de manière à venir en appui contre l'os lorsque la tige 1 est insérée dans l'os.
Les faces interne et externe 13, 14 de la pièce 2 présentent chacune, depuis la partie métaphysaire 2a jusqu'à l'extrémité distale de la partie diaphysaire 2b, une forme convexe engendrée par un rayon de 8 millimètres.
Au niveau de la partie métaphysaire 2a, les faces antérieure et postérieure 15, 16 de la pièce 2 convergent l'une vers l'autre depuis le côté externe vers le côté interne de l'articulation, en formant entre elles un angle de l'ordre de 3 , ainsi que cela apparaît à la figure 4.
La figure 3 montre que ces mêmes faces 15, 16 divergent depuis la base de la partie métaphysaire 2a vers le sommet de celle-ci, également en formant entre elles un angle de l'ordre de 3 .
Des nodules 20 sont aménagés dans une portion supéro-interne des faces 15 et 16, c'est-à-dire sur la zone de ces faces s'étendant sensiblement entre le plan antéro-postérieur contenant 1 ' axe longitudinal de la pièce 2 et le bord interne de ces faces 15, 16.
Les faces antérieure et postérieure 21, 22 de la partie diaphysaire 2b sont parallèles à l'axe longitudinal de la pièce 2, tandis que les faces interne et externe 13, 14, au niveau de cette même partie diaphysaire 2b, convergent en direction de l'extrémité distale de la pièce 2, en formant chacune, avec l'axe longitudinal de celle-ci, un angle de l'ordre de 3 .
La pièce 2 comprend en outre, au niveau de la partie métaphysaire 2a et de la zone proximale de la partie diaphysaire 2b, un revêtement constitué par une couche d'alumine et par une couche d'hydroxyapatite de calcium. Le reste de la partie diaphysaire 2b comprend uniquement une couche d'alumine.
Chaque pièce 3, 4, 5, 6 constituant un col fémoral comprend une embase 30 et une partie 31 destinée à supporter une tête fémorale 7.
L'embase 30 comprend un puits tronconique de forme complémentaire de celle du pion 10, pour permettre l'assemblage de chaque pièce 3, 4, 5, 6 avec la pièce 2, par emmanchement.
La partie 31 est également de forme tronconique et est destinée à être emmanchée dans un alésage tronconique de forme complémentaire que comprend chaque tête fémorale 7.
Il apparaît à la figure 2 que le jeu de pièces 3, 4, 5, 6 comprend
- une pièce 3 à col "axé", c'est-à-dire dont l'axe de la partie 31 est coaxial à l'axe du pion 10 ;
- une pièce 4 à col "décalé", c'est-à-dire dont l'axe de la partie 31 est parallèle à l'axe du pion 10 mais est décalé radialement par rapport à ce dernier
- deux pièces 5, 6 à cols "antéversés", c'est-àdire dont l'axe de la partie 31 forme un angle avec le plan sagittal médian de la pièce 2, cette partie 31 étant orientée, après assemblage, du côté antérieur de la tige 1, la pièce 5 étant destinée à être utilisée pour une prothèse de côté gauche et la pièce 6 pour une prothèse de côté droit ; dans l'exemple représenté au dessin, la pièce 5 est à col "décalé" et la pièce 6 est à col "axé" mais l'ensemble de pièces comprend en outre une pièce à col "antéversé" pour côté droit et "décalé" et une pièce à col "antéversé" pour côté gauche et "axé".
L'invention fournit ainsi une tige fémorale ayant l'avantage essentiel de permettre un bon équilibre entre l'appui de la tige contre l'os, pour une bonne répartition des contraintes exercées sur la tige, et le coincement de la tige par rapport à l'os, pour une parfaite immobilisation de celle-ci le temps que la repousse osseuse s'opère.

Claims (9)

REVENDICATIONS
1 - Tige fémorale de prothèse de hanche, comprenant une queue médullaire effilée, destinée à être engagée dans le canal médullaire du fémur, un col, qui reconstitue le col anatomique du fémur, et une tête sphérique fixée sur l'extrémité libre du col, qui reconstitue la tête du fémur, tige (1) caractérisée en ce qu'elle comprend
- des faces antérieure et postérieure (15, 16 21, 22) qui, au niveau de la partie métaphysaire (2a) de la tige (1), convergent l'une vers l'autre depuis le côté externe vers le côté interne de l'articulation, en formant entre elles un angle de l'ordre de 3 , et qui divergent depuis la base de cette partie métaphysaire (2a) vers le sommet de celle-ci, également en formant entre elles un angle de l'ordre de 3 , et
- des nodules (20) aménagés dans une portion supéro-interne des faces antérieure et postérieure (15, 16) de ladite partie métaphysaire (2a), c'est-à-dire sur la zone de ces faces (15, 16) s'étendant sensiblement entre le plan antéro-postérieur contenant l'axe longitudinal de la tige (1) et le bord interne de ces faces (15, 16).
2 - Tige fémorale selon la revendication 1, caractérisée en ce qu'elle comprend une collerette (12) faisant saillie de sa face interne (13), conformée de manière à venir en appui contre l'os lorsque la tige (1) est mise en place dans l'os.
3 - Tige fémorale selon la revendication 1 ou la revendication 2, caractérisée en ce qu'elle présente, au niveau de la partie diaphysaire (2b) de sa queue médullaire, des faces antérieure et postérieure (21, 22) parallèles à l'axe longitudinal de la tige (1) et des faces interne et externe (13, 14) qui, au niveau de cette même partie diaphysaire (2b), convergent en direction de l'extrémité distale de la queue médullaire, en formant chacune, avec l'axe longitudinal de la tige (1), un angle de l'ordre de 3 .
4 - Tige fémorale selon l'une des revendications 1 à 3, caractérisée en ce que ses faces interne et externe (13, 14) présentent chacune, depuis la partie proximale de la queue médullaire jusqu'à l'extrémité distale de celle-ci, une forme convexe engendrée par un rayon de l'ordre de 8 millimètres.
5 - Tige fémorale selon l'une des revendications 1 à 4, caractérisée en ce que la partie métaphysaire (2a) et la zone proximale de la partie diaphysaire (2b) comprennent un revêtement constitué par une couche d'alumine et par une couche d'hydroxyapatite de calcium, tandis que le reste de la partie diaphysaire (2b) comprend uniquement une couche d'alumine.
6 - Tige fémorale selon l'une des revendications 1 à 5, caractérisée en ce qu'elle est constituée par assemblage de deux pièces (2 à 6) faisant partie d'un ensemble de pièces as semblables qui comprend
- au moins une pièce (2) constituant une queue médullaire, et
- un jeu de plusieurs autres pièces (3 à 6) constituant des cols fémoraux, susceptibles d'être assemblés à ladite au moins une pièce (2) constituant une queue médullaire, la partie (31) de ces pièces qui supporte la tête fémorale (7) ayant, d'une pièce à l'autre, des longueurs différentes et/ou des positionnements radiaux différents par rapport à un axe de référence contenu dans le plan sagittal médian de la pièce (2) constituant la queue médullaire, et/ou des orientations différentes par rapport à ce plan sagittal médian.
7 - Jeu de pièces selon la revendication 6, caractérisé en ce qu'il comprend
- une pièce (3) à col "axé", c'est-à-dire dont l'axe de la partie (31) supportant la tête fémorale (7) prothétique est coaxial audit axe de référence
- une pièce (4) à col "décalé", c'est-à-dire dont l'axe de la partie (31) supportant la tête fémorale (7) prothétique est parallèle audit axe de référence mais est décalé radialement par rapport à ce dernier, et
- quatre pièces (5, 6) à cols "antéversés", c'està-dire dont l'axe de la partie (31) supportant la tête fémorale (7) forme un angle avec le plan sagittal médian de la queue médullaire, cette partie (31) étant orientée, après assemblage, du côté antérieur de la tige (1), deux de ces pièces étant destinées à des prothèses pour côté droit et les deux autres à des prothèses pour côté gauche ; l'une des pièces destinées à un même côté a un col "axé" et l'autre un col "décalé" tels que précités.
8 - Jeu de pièces selon la revendication 6 ou la revendication 7, caractérisé en ce que l'assemblage d'une pièce (2) constituant une queue médullaire avec une pièce (3 à 6) constituant un col fémoral est réalisé au moyen d'un pion tronconique (10) à faible pente, faisant saillie de l'une de ces pièces (2), et d'un puits tronconique de forme complémentaire, aménagé dans une autre de ces pièces (3 à 6), les axes de ce pion (10) et de ce puits définissant l'axe de référence précité.
9 - Jeu de pièces selon la revendication 8, caractérisé en ce que la section transversale du pion (10) et du puits est elliptique et le grand axe de l'ellipse est contenu dans le plan défini par l'axe longitudinal de la queue médullaire et par l'axe de ce pion (10) et de ce puits.
