Abstract

Dispositif concernant une prothèse totale de hanche à géométrie variable et angulation multiple. L'invention concerne une prothèse totale de hanche permettant d'adapter, sur une tige intra fémorale perdue d'emblée en début d'intervention dans la diaphyse évitant ainsi l'hémorragie per-opératoire, différents cols fémoraux sans limitation d'angulation et aussi d'obtenir une position col cotyle, la plus anatomique. Elle est constituée d'un élément vertical (2) présentant sur sa surface haute une rainure (4), un ou plusieurs orifices (5) permettant de recevoir une pièce intermédiaire (1) inverse et réciproque à la surface (4) et (5) de l'élément vertical (2) et permettant aussi de choisir la pièce intermédiaire (1) en fonction de la meilleure angulation en regard de l'axe col cotyle. Le dispositif selon l'invention est particulièrement destiné à obtenir lors d'une pose de prothèse totale de hanche la position la plus anatomique en regard de l'axe col cotyle.

Description

La présente invention est relative à une prothèse de hanche à géométrie variable et angulation multiple destinée à la réparation des fractures du col du fémur et adaptée à toute pathologie coxo-fémorale. Lors de fracture du col du fémur il est généralement nécessaire d'implanter une prothèse destinée à remplacer ce col, et venant starticuler dans la cavité cotyloidienne de l'os iliaque Il existe déjà un grand nombre de prothèses qui comprennent toutes un élément vertical qui est implanté dans le fémur et un élément oblique solidaire du fémur plus mince faisant fonction de col et portant à son extrémité libre un élément arrondi coopérant avec la cavité cotyloidienne de l'os illiaque.
Ces prothèses de l'art antérieur présentent toutes un inconvénient dans la mesure où leur implantation, leur maintient dans le corps du fémur, et la position du col en regard de l'axe tête cotyle n'est pas totalement satisfaisant. Il n'est pas rare qu'il existe un jeu préjudiciable à la bonne tenue de la prothèse dans le corps du fémur, et un positionnement du col désaxé de l'axe tête cotyle.
Aussi un des buts de la présente invention est il de fournir une prothèse de hanche dont l'implantation est aisée, dont la tenue est totalement solidaire du fémur, et dont la partie col permet du fait de sa mobilité de positionner ce col exactement dans l'axe du cotyle ce qui permet d'obtenir un positionnement axial optimal en regard de l'axe tête cotyle.
La présente invention basée sur un sytème mécanique permet d'associer deux pièces entre elles tout en assurant une solidité optimale. Les buts sont atteints par une prothèse de hanche composée de deux partie
1) Une partie intra fémorale de forme troncônique elliptique de différentestailles permettant un remplissage optimum du fût fémoral et perdue d'emblée en début d'intervention dans la diaphyse évitant ainsi l'hémorragie per-opératoire et facilitant les manoeuvres dans un champs resté libre.
2) Une pièce intermédiaire miniaturisée, posée dans un plan oblique aussi bien que sur un plan horizontal avec antéversion, inclinaison variable du col, longueur variable du col le col étant médialisé.

Ainsi que l'on peut le voir sur ces figures la prothèse de hanche à angulation multiple selon la présente invention est composée de deux parties : une partie col (1) à angulation et une partie intra fémorale (2) s'alliant entre elles par un système mécanique (3).
Un élément vertical (2) de forme troncônique elliptique présentant sur sa partie haute une rainure (4), un trou (5) cylindrique ou conique dans l'axe col prothèse, un trou (6) désaxé de l'axe col prothèse mais restant à 1350 et parallèle à l'axe col. La forme tronconique elliptique de la partie haute de l'élément vertical permet de par son profil une restriction importante de sacrifice osseux.
Une pièce intermédiaire selon la figure 1 présentant dans sa partie basse (7) une rainure (8) inversée réciproque à la rainure (4), un téton (9) positionné dans l'axe et inversé réciproque au trou (5) de l?L partie verticale (2).
Un trou (9) dans la partie en regard du grand trochanter (10) désaxé de l'axe col-prothèse mais parallèle à l'axe col et présentant sur sa partie haute (12) un cône (13) permettant de noyer une vis (14). Le système mécanique selon la présente invention assure un contact parfait en regard des contraintes de flexion et rotation entre la pièce intermédiaire et l'élément vertical.
La progressivité dimensionnelle de l'élément vertical (2) assure un remplissage parfait du fût fémoral tout au long de celui-ci.

Claims (4)

REVENDICATIONS
1) Prothèse de hanche à angulation multiple composée de deux parties (1,2), une première partie intra-fémorale (2) constituée d'un élément vertical de forme tronconique elliptique et une seconde partie (1) formant pièce intermédiaire ou col, prothèse de hanche caractérisée par le système mécanique (3) de montage à ajustement bloquant des deux parties (1,2), système constitué d'une part par la partie haute de la partie intra-fémorale (2) qui comporte une rainure (4), un trou (5) dans l'axe du col de la prothèse et un trou (6) désaxé par rapport à l'axe du col de la prothèse mais cependant parallèle à celui-ci, et d'autre part par ra partie basse de la pièce intermédiaire (1) qui comporte une rainure (8) inversée réciproque de la rainure (4), un téton (9) positionné dans l'axe du col de la prothèse et inversé réciproque au trou (5), et un trou réalisé dans la partie en regard du grand trochanter (10), désaxé par rapport à l'axe du col de la prothèse, mais parallèle à celui-ci, et destiné à recevoir une vis (14).
2) Prothèse selon la revendication 1 caractérisée en ce que le trou (5) de la partie haute de la partie intra-fémorale (2) est cylindrique.
