Abstract
Un ensayo de estratificación de un sujeto con respecto a enfermedad cardiaca, comprendiendo el ensayo la determinación en una muestra de dicho sujeto de los niveles de al menos dos o más analitos lipídicos no modificados seleccionados de la lista que consiste en: los analitos lipídicos alquilfosfatidilcolina (APC), bis(monoacilglicerol)fosfato (BMP), ceramida (Cer), diacilglicerol (DG), dihexosilceramida (DHC), gangliósido GM3 (GM3), factor de activación de lisoplaquetas (LPAF), monohexosilceramida (MHC), fosfatidilcolina de cadena impar (oddPC), fosfatidiletanolamina (PE), fosfatidilglicerol (PG), fosfatidilinositol (PI), fosfatidilserina (PS), triacilglicerol (TG) y trihexosilceramida (THC) enumerados en la Tabla 1; fosfatidilcolina (PC) 30:2, PC 34:0, PC 40:7, PC 40:5, PC 44:12; esfingomielina (SM) 15:0, SM 16:1, SM 18:1, SM 18:0, SM 20:1, SM 22:1, SM 24:2, SM 24:1; lisofosfatidilcolina (LPC) 14:0, LPC 15:0, LPC 16:1, LPC 18:1, LPC 20:5, LPC 20:3, LPC 20:2, LPC 20:1, LPC 20:0; éster de colesterol (CE) 14:0, CE 15:0, CE 16:2, CE 16:0, CE 17:1, CE 20:1, CE 17:0, CE 18:0, CE 20:3, CE 20:2, CE 22:5, CE 22:4, CE 22:3, CE 22:2, CE 22:1, CE 22:0, CE 24:6, CE 24:5, CE 24:4, CE 24:3, CE 24:2, CE 24:1, y CE 24:0; en el que el nivel de un analito lipídico individual es diferente entre sujetos normales y con enfermedad cardíaca y en el que el nivel de los analitos lipídicos en el sujeto con respecto a un control proporciona una indicación de la presencia o ausencia de enfermedad cardíaca.

La referencia a un "control" incluye, en términos generales, datos que el experto en la materia usaría para facilitar la interpretación exacta de los datos técnicos. En un ejemplo ilustrativo, se compara el nivel o los niveles de analito/s lipídico/s de un sujeto con el respectivo nivel o niveles del/de los mismo/s analito/s lipídico/s en una o más cohortes (poblaciones/grupos) de sujetos de control seleccionados entre una cohorte de sujetos vulnerables, en la que los sujetos han sido diagnosticados con la enfermedad cardiaca inestable, una cohorte de sujetos no vulnerables, en la que los sujetos han sido diagnosticados con enfermedad cardiaca estable, una cohorte de sujetos normales, en la que se ha predeterminado que los sujetos no padecen enfermedad cardiaca, y una cohorte de sujetos con enfermedad cardiaca que comprende los miembros de las cohortes vulnerables y no vulnerables. Como se describe y/o como se reivindica en este documento, el control puede ser el nivel o la proporción de uno o más analitos lipídicos en una muestra del sujeto de ensayo tomada en un punto temporal previo. Por lo tanto, se puede usar un cambio temporal en los niveles de analito para identificar la vulnerabilidad o proporcionar una correlación en cuanto al estado de las enfermedades cardiacas. Se pueden seleccionar umbrales que proporcionen una capacidad aceptable de predecir el riesgo de diagnóstico o pronóstico, el éxito del tratamiento, etc. En ejemplos ilustrativos, se calculan las curvas de las características operativas del receptor (ROC) trazando el valor de una o más variables frente a su frecuencia relativa en dos poblaciones (denominadas arbitrariamente grupos de "enfermedad" y "normal"
o "de bajo riesgo" y de "alto riesgo", por ejemplo). Para cualquier analito o clase de lípido particular, la distribución del/de los nivel/es para los sujetos de las dos poblaciones es probable que se superponga. En dichas condiciones, un nivel de ensayo no distingue absolutamente "enfermedad" y "normal" o "vulnerable" y "no vulnerable" con un 100 % de precisión, y el área de superposición indica que el ensayo no puede distinguir entre los grupos. Por consiguiente, en algunas realizaciones, se selecciona un umbral o un intervalo, por encima del cual (o por debajo del cual, dependiendo de cómo varíe un nivel de analito lipídico con la enfermedad cardiaca o el pronóstico) se considera que el ensayo es "positivo" y por debajo del cual se considera que el ensayo es "negativo". Como se describe en el Ejemplo 4, se usaron ensayos no paramétricos para establecer la significación estadística de las diferencias entre los distintos niveles de analitos en los diferentes grupos de control (véase la Tabla 16). También se usó un análisis de regresión lineal para identificar los analitos lipídicos que son predictores independientes de la asignación a los grupos. Se encontraron varios analitos lipídicos como predictor independiente de la EAC estable o inestable, en concreto, PI 34:0, DHC 18:1, modCer 703,6/5,87, SM 22:1 y GM3 18:0, Del mismo modo, se pudieron distinguir veintiún analitos lipídicos individualmente entre los pacientes de control y EAC (Tabla 12, modelo 6). El análisis multivariado es particularmente adecuado para el desarrollo de un modelo predictivo basado en los perfiles de lípidos en plasma. Se examinó un intervalo de modelos que incluían diferentes números de analitos lipídicos (1, 2, 4, 8, 16, 22, 64 …329) bien solos o con factores de riesgo tradicionales por su capacidad para distinguir un determinado grupo (Tablas 18 a 20). Se usaron los valores de estos modelos para realizar los análisis ROC con el fin de determinar la gravedad y la especificidad de los modelos (véase el Ejemplo 6, Figura 6). Por consiguiente, como se ha demostrado, en el presente documento se puede usar todo un intervalo de analitos lipídicos o seleccionar subconjuntos particulares de analitos lipídicos capaces de distinguir entre determinados grupos.
Como alternativa, o además, se pueden establecer umbrales mediante la obtención de un nivel de analito del mismo paciente, con el que después se pueden comparar los resultados. En estas realizaciones, el individuo, en efecto, actúa como su propio "grupo de control". En los marcadores que aumentan con la gravedad de la enfermedad o el riesgo de pronóstico, un aumento a lo largo del tiempo en el mismo paciente puede indicar un empeoramiento o desarrollo de la enfermedad o el riesgo de enfermedad o un fracaso de un régimen de tratamiento, mientras que una disminución a lo largo del tiempo puede indicar la remisión de la enfermedad o el éxito de un régimen de tratamiento. El experto en la materia aplicará otros controles adicionales de forma rutinaria. En un ejemplo ilustrativo, se determinan los niveles de una variedad o grupo de analitos lipídicos de una o más clases de lípidos y se comparan con niveles predeterminados de uno o más grupos de sujetos de control. Los analitos lipídicos que, en el presente documento, se determinan como no correlacionados con la enfermedad cardiaca o las placas inestables se pueden incluir como controles internos, siendo también, por tanto, útiles en algunas realizaciones.
En algunas realizaciones, los niveles de analito lipídico de los grupos de control se usan para generar un perfil de niveles de analito lipídico que reflejen la diferencia entre los niveles en dos grupos de control. Por lo tanto, un analito lipídico en particular puede ser más abundante o menos abundante en un grupo de control en comparación con otro grupo de control. Los datos se pueden representar como una puntuación distintiva global o el perfil se puede representar como un código de barras u otra representación gráfica. Los niveles de analito lipídico de un sujeto de ensayo se pueden representar del mismo modo, y de manera similar al distintivo, se puede determinar el alcance o nivel de "ajuste" con un código de barras distintivo u otra representación gráfica. En otras realizaciones, se analizan los niveles de un determinado analito lipídico o clase de lípido y se determina una tendencia a la baja o al alza en un nivel de analito determinado. El perfil lipidómico permite además la determinación de criterios de valoración en los estudios farmacotraduccionales. Por ejemplo, los ensayos clínicos pueden tardar muchos meses o incluso años en establecer los parámetros farmacológicos de un medicamento para su uso en el cuidado coronario. Sin embargo, estos parámetros pueden estar asociados con un perfil lipidómico asociado con un estado de salud. Por lo tanto, se puede acelerar el ensayo clínico seleccionando primero un medicamento y los parámetros farmacéuticos que producen un perfil lipidómico asociado con el estado de salud deseado.
Por consiguiente, en el presente documento, se describe un método para determinar la eficacia farmacológica de un medicamento para su uso en el tratamiento de la enfermedad cardiaca, comprendiendo el método la selección de un medicamento y sus parámetros de concentración y/o de formulación que proporcionan un perfil lipidómico asociado
