Abstract
Una composición dental que se obtiene de la mezcla de: - una fase sólida, que comprende de 30 a 80 % de al menos un polvo derivado del calcio seleccionado de silicato tricálcico o una asociación de silicato tricálcico y silicato dicálcico; en asociación con de 1 a 50 % de al menos un radioopacificador seleccionado del grupo que comprende óxido de bismuto o carbonatos de bismuto o una mezcla de los mismos, u óxido de circonio solo o en combinación con itrio; o una mezcla de óxido de bismuto o carbonatos de bismuto y óxido de circonio solo o en combinación con itrio; y de 1 a 20% de polivinilpirrolidona (PVP); y opcionalmente menos de 50 % de un relleno radioopaco que es carbonato de calcio; en donde dicho derivado del calcio, radioopacificador, PVP y relleno no radioopaco están todos en forma de polvo, en peso con respecto al peso total de dicha fase sólida; y - una fase acusa que comprende agua, de 0,1 a 15 % de al menos un agente reductor de agua que es un poli(met)acrilato de alcohol polihídrico, y de 10 a 50 % de al menos un acelerador de endurecimiento, en peso con respecto al peso total de dicha fase acuosa; la relación de la fase sólida con la fase líquida en el intervalo de 1,0 a 2,5; dicha composición tiene una resistencia a la compresión en el intervalo de 1 a 15 MPa, , medida en 24 horas después que termina el tiempo de endurecimiento.
Description
El retiro de la composición en 24 horas no origina ningún problema en cualquier modo de uso que se lleve a cabo, es decir, monobloque o monocono. El retiro se realiza fácilmente sin la desviación de la trayectoria del canal radicular.
El retiro de la composición que se lleva a cabo en 72 horas mostró lo siguiente: 5 -Los canales radiculares rellenos con la composición en monobloque pueden vaciarse por medio de instrumentos giratorios, -Los canales radiculares rellenos con un cono de Gutta Percha sellado con la composición se vacían fácilmente con una preservación excelente de la trayectoria del canal radicular (probablemente debido a las propiedades de la 10 composición las cuales previenen la desviación del instrumento giratorio y desempeñarían la función de guía).
El retiro de la composición que se lleva a cabo en 7 días mostró lo siguiente:
-Monocono: el retiro es fácil, el cono se adaptó perfectamente a la forma del canal mientras que no se adhiere a las 15 paredes -Monobloque: el retiro es rápido y fácil, las propiedades mecánicas de la composición se adaptan bien al retratamiento.
Segunda evaluación preclínica
20 Los simuladores endodónticos de una curva y de doble curva se llenaron con la composición, usada como una composición de sellado, con una de las siguientes técnicas: monocono o Thermafil (en comparación con el cemento sellador BC de Endosequence [comparación] y la resina Top Seal de Dentsply-Maillefer [control]).
25 El uso de la composición no se adapta a la implementación de una técnica de relleno con Gutta percha caliente (técnica de Schilder o System B), debido a que el calor excede los 200 °C.
Resultados:
30 Los ensayos para fijar el abrebocas Thermafil a la longitud necesaria fueron insatisfactorios cuando se utilizó el cemento BC (utilizado de acuerdo con el protocolo del fabricante), debido a la presencia de una columna del cemento de excesivo tamaño. La inserción completa de un cono de Gutta Percha clásico (es decir, diferente del recomendado para esta técnica) también originó problemas debido a la carencia de firmeza del cono de Gutta Percha. Mediante el uso de la composición, la fijación del cono Gutta Percha a la longitud necesaria, se hizo posible, e incluso fácil, en los canales
35 de una curva, así como en los canales de doble curva, donde se enfatiza que la técnica preferida fue el recubrimiento de las paredes con un léntulo seguido por la inserción de Gutta (en comparación con la técnica del relleno completo con el léntulo seguido por la inserción del cono Gutta Percha).
Los ensayos para fijar el abrebocas Thermafil a la longitud necesaria fueron exitosos después de que se realizó un 40 recubrimiento fino de las paredes con la composición y después de la extracción del exceso (el mismo protocolo que para la implementación de la resina Top Seal).
