Abstract
Sistema quirúrgico asistido por ordenador, que comprende: un aparato (100) para obtener imágenes de una zona de interés (640) de una parte de la anatomía de un sujeto, comprendiendo el aparato un fluoroscopio (110); una memoria (160) que contiene instrucciones ejecutables; y un procesador (150) programado mediante las instrucciones para: recibir una fluorografía de la zona de interés y un implante (610, 1654) posicionado en la zona de interés tomada en un primer ángulo con respecto al aparato, estando asociado el implante con un cuerpo de referencia (616); detectar la presencia del cuerpo de referencia en la fluorografía; determinar una posición y dimensión espacial del implante en función de una identificación y registro del cuerpo de referencia; superponer un implante virtual sobre la zona de interés en función de la posición y la dimensión espacial determinadas, para formar una imagen que muestra el implante virtual en relación con la zona de interés, y generar una señal de presentación visual asociada con la imagen superpuesta.
Description
Atendiendo ahora a la fig. 4A,, se ilustra un diagrama de flujo de alto nivel 400 de las etapas del procedimiento de implantación de un implante. Tal como se muestra, el procedimiento comienza con el posicionamiento de un implante principal en una zona de interés, en la etapa S402. Tal como se explica con mayor detalle más adelante, este posicionamiento inicial se realiza preferentemente empleando fluorografías tomadas en al menos dos
5 dimensiones o direcciones. Una vez que el cirujano considere satisfactoria la posición del implante principal, el sistema 100 genera una imagen que muestra la posición de un implante virtual y los correspondientes subimplantes virtuales con respecto a la zona de interés, etapa S408, basándose en las fluorografías y la posición de un cuerpo de referencia u objetos de referencia dentro del campo visual del fluoroscopio 110.
10 Mediante la imagen de los implantes virtuales, el cirujano puede fijar entonces el implante, mediante los subimplantes, por ejemplo, tal como se ilustra en S424. Una vez que los subimplantes (p. ej., tornillos) e implantes estén en su lugar, el sistema puede llevar a cabo una comprobación de calidad, en S428, detectando y presentando la ubicación real de estos implantes con respecto a su posición deseada. Esta comprobación de calidad resulta deseable, dado que durante la implantación, la posición de un implante o subimplante puede cambiar con respecto a
15 su posición ideal debido a las fuerzas mecánicas ejercidas durante, por ejemplo, la perforación o colocación de tornillos o a consecuencia de movimientos del paciente. A este respecto, también se pueden llevar a cabo comprobaciones de calidad, como en la etapa S428, durante la fijación del implante, en la etapa S424. Además, también se pueden llevar a cabo comprobaciones de calidad en fase posoperatoria mediante el sistema para detectar movimientos en el implante causados, por ejemplo, por la actividad del paciente.
20 De manera significativa, el anterior procedimiento 400 es de carácter reactivo, ya que no es necesario que el cirujano informe al sistema 100 de qué paso está llevando a cabo como parte del flujo de trabajo en quirófano. A este respecto, este sistema es compatible con el flujo de trabajo normal en quirófano y es capaz de determinar la etapa del flujo de trabajo en quirófano que se está llevando a cabo, por ejemplo, detectando la presencia de un
25 cuerpo u objeto de referencia.
Atendiendo ahora a la fig. 4B, se ilustran las subetapas o el procedimiento para posicionar o alinear el implante de acuerdo con la etapa S402 de la fig. 4A. Tal como se muestra, el procedimiento comienza con la inserción y el posicionamiento del implante principal en la zona anatómica de interés en la etapa S430. Para seguir con el ejemplo
30 ilustrativo, se emplea una placa intracapsular (ICP) para reparar fracturas del cuello femoral. De este modo, en la etapa S430, se insertaría la placa ICP en el paciente y se posicionaría de manera aproximada sobre el hueso, en este caso el cuello femoral. Esta etapa se puede llevar a cabo, por ejemplo, en conformidad con el flujo de trabajo normal en el quirófano, por ejemplo permitiendo que el médico recurra a la realimentación háptica para juzgar una posición inicial apropiada de la placa.
35 A este respecto, la fig. 5 ilustra la colocación de un implante de ICP 510 (p. ej., el implante principal) junto con subimplantes (es decir, los tornillos) para asegurar una fractura de cuello femoral. Tal como se muestra, la ICP 510 se fija al fémur 520 y se insertan unos tornillos a través del cuello y la cabeza de la ICP. Preferentemente, los tornillos que entran en la cabeza femoral se posicionan completamente dentro de la cabeza. En otro aspecto más, al
40 estar la ICP adaptada a la forma del fémur, los grados de libertad en el posicionamiento de la ICP sobre el hueso resultan limitados y se emplean como parte del procedimiento de alineamiento S402. Concretamente, la ICP solo se puede desplazar a lo largo (es decir, traslación) y/o rotar alrededor del eje del fémur. Además, la ICP está provista de unos orificios roscados, de manera que la posición/el ángulo de los tornillos con respecto a la placa es conocida.
45 Antes de la inserción del implante principal en la zona de interés, el implante principal 520 se conecta a un cuerpo u objeto de referencia. El cuerpo de referencia va fijado a (o forma parte de) el implante, pero también puede ir fijado a un dispositivo o instrumento de guiado (p. ej., una guía de perforación). De este modo, se puede determinar la posición del implante en función de la ubicación y la posición del cuerpo de referencia. Preferentemente, cada implante está asociado con un cuerpo de referencia diferente que es detectable por el sistema 100, en particular el
50 fluoroscopio 110. En una forma de realización preferida, el cuerpo de referencia comprende una pluralidad de marcadores de referencia esféricos insertados encima o dentro del instrumento (p. ej., dispositivo de guiado). Disponiendo los marcadores de referencia en un patrón predeterminado, pueden servir como identificadores para los diferentes instrumentos. A este respecto, los marcadores y el instrumento de referencia se pueden denominar convenientemente cuerpo de referencia, aunque son los marcadores de referencia los que proporcionan la
55 referencia.
Por ejemplo, la fig. 6A ilustra una vista lateral de un cuerpo de referencia 604 como parte del implante 610. En conjunto, el cuerpo de referencia y el implante se denominan en el presente documento sistema de implante 614. Como se muestra en la fig. 6A, el cuerpo de referencia 604 incluye uno o más marcadores de referencia 616 que

son detectados por el sistema de obtención de imágenes y utilizados como puntos de referencia o medición. Preferentemente, los marcadores de referencia comprenden esferas para hacer que la detección resulte más fácil dentro de un sistema de obtención de imágenes bidimensionales tal como un fluoroscopio. Además, la disposición de los marcadores de referencia dentro del cuerpo de referencia actúa como una firma que se usa para identificar el
5 cuerpo de referencia y el implante asociado. Dado que las dimensiones del cuerpo de referencia y el implante son conocidas y estos dispositivos están fijos unos con respecto a otros, la ubicación del implante se puede determinar con precisión detectando o registrando la ubicación del cuerpo de referencia. Como también se muestra en la fig. 6A, también se pueden colocar marcadores de referencia en el propio implante, pero no son necesarios.
10 La fig. 6B es una vista en perspectiva del dispositivo de guiado 604 y el implante 610 (que, para seguir con el ejemplo, es una ICP) en un estado separado. La fig. 6C muestra estos dos dispositivos en un estado unido. Tal como se muestra, el dispositivo de guiado 604 tiene un contorno adecuado para ajustarse a la ICP 610. Además, incluye unas aberturas que permiten el acceso a los orificios para los tornillos de la ICP 610 que se utilizan para asegurar el implante 610 tal como se explica más adelante con mayor detalle. Para permitir el procesamiento, el
15 cuerpo de referencia debe estar situado en el campo visual de la imagen con el implante y la zona de interés 640, que, para seguir con este ejemplo ilustrativo, incluye el cuello del fémur y la cabeza femoral. Como parte de esta inserción y colocación iniciales, el cirujano normalmente utilizará la realimentación háptica para determinar una ubicación de partida para el implante.
20 La fig. 7 muestra un instrumento 700 que se puede utilizar en la inserción y la colocación iniciales de la ICP 610 sobre un fémur 720. Además, al llevar a cabo esta colocación inicial, el cirujano puede utilizar el instrumento 700 o el cuerpo de referencia 604. A este respecto, el instrumento 700 también puede comprender un cuerpo de referencia colocando los marcadores de referencia apropiados encima o dentro del mismo.
25 Volviendo a la fig. 4B, una vez que el cirujano determina una ubicación inicial para el implante (y el cuerpo de referencia que lo acompaña), se toma una fluorografía de una zona de interés en una primera dimensión o dirección, en la etapa 434. Por ejemplo, se puede tomar una fluorografía en la dimensión anteroposterior o la dimensión axial. En referencia a la fig. 1, la vista anteroposterior se ilustra con el emisor 190 y el detector 192 alineados a lo largo del eje y, mientras que, en la vista axial, el emisor y el detector están alineados a lo largo del eje z. Tal como se muestra
30 en la fig. 8, se pueden tomar fluorografías desde dos diferentes dimensiones o direcciones cualesquiera 810A, 810B. Preferentemente, las dos imágenes se tomarán perpendiculares entre sí (es decir, formando entre ellas un ángulo próximo a 90 grados), pero esto no es imprescindible y bastará con cualquier ángulo.
Debido a que el implante y el cuerpo de referencia están situados en el interior del campo visual del dispositivo de
35 obtención de imágenes, y dada su proximidad a la zona de interés, el fluoroscopio 110 detecta la presencia del cuerpo de referencia, es decir, los marcadores de referencia. El ordenador 120 utiliza entonces los datos de imagen que recibe procedentes del fluoroscopio 110 para proporcionar una visualización de la ubicación del implante con respecto a la zona de interés. En particular, el registro de las imágenes fluoroscópicas se lleva a cabo a través del cuerpo de referencia. Como se explica anteriormente, el cuerpo de referencia se encuentra normalmente en una
40 posición fija con respecto al implante y al hueso. Además, se utiliza un disco en el intensificador de imágenes para revelar la imagen de rayos X. El disco suele ir unido directamente al intensificador de imágenes. Dicho cuerpo de referencia también se observaría en cada imagen y se podría usar para compensar la distorsión de la imagen y para la determinación del centro de, por ejemplo, el haz de rayos X.
45 La determinación del implante con respecto a la zona anatómica de interés se realiza mediante técnicas conocidas de procesamiento de imágenes basadas en la variación en la radiación espacial que alcanza el detector, incluida la radiación dirigida hacia la zona de interés y el cuerpo de referencia. Mediante la variación espacial, el ordenador es capaz de construir una imagen que representa de manera precisa la relación espacial entre el implante y la zona de interés (p. ej., fémur y cabeza femoral) en forma de imagen bidimensional.
Tras observar esta imagen, el cirujano puede determinar después si se debe reposicionar el implante, como en la etapa S438. Por ejemplo, el cirujano puede decidir ajustar la posición a lo largo del fémur aproximándola más a la cabeza femoral u otro grado de libertad. Si el cirujano decide que dicho ajuste esta justificado, reposiciona el implante tal como se muestra en la etapa S440 y se toman más fluorografías en la etapa S434. Por otra parte, si el
55 cirujano determina que no es necesario ningún ajuste en esta dimensión, el procedimiento continúa en la etapa S442 con la estabilización del implante. Por seguir con el ejemplo, la estabilización se podría efectuar mediante la introducción de una aguja de Kirschner (aguja K) a través de una o más aberturas en la ICP.
Con el implante fijado tal como se describe anteriormente, se puede tomar una fluorografía en una dimensión

esta zona, el cirujano puede ajustar manualmente la placa en S55 y observar una visualización actualizada volviendo a la etapa S52.
En las figs. 13A y 13B se muestra cómo se puede lograr el ajuste de la ICP 610 mediante la rotación externa 1300.
5 En particular, de manera similar a lo que ocurre en el caso del ajuste de traslación, si el cirujano cree que el implante no está alineado correctamente, puede ajustar las flechas 1300 para visualizar cómo se alinea el implante al girarlo. Como también puede observarse en la fig. 13B, se puede utilizar una zona de aceptación 1340 para mostrar el modo en que la rotación cambiaría la ubicación de los tornillos con respecto a una posición preferida. Como también se muestra en la fig. 13B, el movimiento de rotación se lleva a cabo preferentemente después de haber determinado
10 satisfactoriamente la ubicación a lo largo del cuello del fémur. De este modo, el cuerpo de referencia 604 se puede fijar al hueso mediante un clavo 1326. Este clavo 1326 se puede utilizar con un mecanismo de ruedas dentadas 1330 para girar con mayor precisión el implante tal como se muestra.
Como se explica anteriormente, el sistema puede considerarse reactivo, ya que reacciona ante el cirujano en lugar
15 de pedirle que lleve a cabo una actuación o que interactúe con el ordenador o el sistema. A diferencia de esto, las soluciones convencionales tienden a interferir en el flujo de trabajo en el quirófano, ya que solicitan al cirujano que interactúe con el CAS; lo cual alarga
20 el procedimiento quirúrgico y resulta más exigente en términos de equipo, por lo que el coste de los procedimientos quirúrgicos aumenta.
Tras concluir las etapas expuestas anteriormente en relación con la etapa S408, el procedimiento continúa hasta la etapa S424, en la que el implante se puede fijar a la zona de interés. Se pueden tomar más fluorografías en el
25 transcurso de la etapa S424 o después de la misma para comprobar la reducción de la fractura y la posición de la ICP y las agujas K o tornillos. Por ejemplo, en la figura 15 se muestra la inserción de agujas K 1620, 1630 y un movimiento detectado de la cabeza femoral 1610. Idealmente, el sistema detectará dicho movimiento y propondrá una medida correctiva como, por ejemplo, nuevas longitudes de los tornillos. Si es necesario, se puede llevar a cabo otro reposicionamiento del implante y reducción de la fractura tal como se explica anteriormente.
30 Como se explica anteriormente, se pueden insertar agujas de Kirschner (agujas K) a través de unas aberturas en el cuerpo de referencia 604. Más concretamente, como se muestra en la fig. 15, se puede insertar una primera aguja K 1630 para fijar la placa al hueso. (También se puede insertar una segunda aguja K 1620 a través de la fractura). Después se pueden insertar tornillos para comprimir la fractura (S424). Los tornillos pueden ser autorroscantes o
35 pueden insertarse a través de unos orificios perforados. Se proporciona preferentemente una aplicación informática de procesamiento de imágenes basada en la detección de bordes para detectar cualquier inserción y doblado de los tornillos o las agujas K. Dicha aplicación informática puede comprender un componente o rutina en un conjunto de instrucciones que llevan a cabo el procedimiento descrito anteriormente. La ICP posee unos orificios roscados para tornillos, de manera que la posición/el ángulo del tornillo con respecto a la placa son fijos. Las agujas K se pueden
40 retirar antes o después de haber insertado los tornillos.
Otra posibilidad consiste en que la ICP lleve unido directamente un aparato de guiado con una escala graduada en combinación con un orificio de forma alargada (en el que se puede insertar una aguja K) que puede servir de ayuda en el montaje y en cualquier ajuste posterior que el cirujano considere necesario. En la fig. 14 se muestra el
45 reposicionamiento mediante un aparato de aplicación en un único movimiento (one shot) de la técnica anterior 1400; que se sustituye preferentemente por el cuerpo de referencia 604 y otros accesorios 1326, 1330 expuestos anteriormente.
Atendiendo ahora a las figs. 16A a 16H, se muestra un uso alternativo de los procedimientos descritos
50 anteriormente. Como ya se explicará más detalladamente, estas figuras muestran la inserción de un clavo de bloqueo 1645, que se utiliza como parte de un sistema de reparación de fractura de cadera. En particular, y haciendo referencia a la fig. 16A, el procedimiento comienza con la inserción del clavo 1654 en el fémur 1658. Como también se muestra, el clavo 1654 está unido a un instrumento o dispositivo de guiado 1660. El dispositivo de guiado está provisto preferentemente de una pluralidad de marcadores de referencia, p. ej., cuatro o más, que funcionan
55 como un cuerpo de referencia que es detectable por el sistema de obtención de imágenes. De acuerdo con los procedimientos descritos anteriormente, en este estado inicial del procedimiento, el cirujano obtiene fluorografías a lo largo de una primera dimensión. Por ejemplo, se puede obtener una fluorografía a lo largo del eje anteroposterior del paciente o en cualquier otro ángulo que el cirujano considere adecuado.
