Abstract
Set (conjunto de elementos) de ileostomía transcecal diseñado para drenaje de fluidos intestinales desde el intestino delgado (19), que comprende un catéter de globo (1) que es introducido percutáneamente en el intestino delgado (19) a través de una pared abdominal (9) y una pared (11) del intestino grueso, caracterizado por un globo de fijación (3) fijado a una porción distal del catéter de globo (1) de tal modo que éste se infle en el intestino grueso (8) para llenar su lumen completo, y un globo de bloqueo (2) fijado a una porción distal del catéter de globo (1) de tal modo que éste se infle en el intestino delgado (19) detrás de una válvula de Bauhin (20) para obstruir su lumen completo.
Description
Set de ileostomía transcecal.
Esta invención se refiere en general a
dispositivos médicos. Más particularmente, el campo de la técnica en
el que cae esta invención son los sets (conjuntos de elementos) de
ileostomía utilizados para drenaje y recogida de heces directamente
desde el íleo (es decir, se puentea el intestino grueso) a través de
la pared abdominal.
La ileostomía de bucle quirúrgica
desfuncionalizadora es una técnica estándar utilizada para diversión
temporal para resecciones del colon y del recto del lado izquierdo
en pacientes de alto riesgo. Cuando la ileostomía ya no es
necesaria, se la elimina. Un paciente deberá someterse a otra
cirugía - laparotomía - durante la cual se cierra una ileostomía de
bucle. Incluso tal operación está asociada con mortalidad y con una
morbilidad bastante alta.
La invención presentada se basa en un catéter de
globo especial que permite el drenaje total de fluidos
intestinales/heces del intestino delgado de tal modo que el colón
se puentee después de una resección. Una bolsa especial para la
recogida de heces es conectada al catéter de globo fuera del cuerpo
humano.
El documento WO 01/49224 describe un dispositivo
de puerto de ostomía continente que comprende una porción de
catéter con un cabezal proximal para sellar y anclar el dispositivo
y una cámara distal.
Este set de ileostomía transcecal presentado se
ha construido con la intención de cerrar la ileostomía sin cirugía,
es decir, ahorrar una operación a un paciente. La solución de la
presente invención permite, tras la retirada del catéter de globo,
cerrar la ileostomía por medio de un bucle biorresorbible que frunza
percutáneamente la pared del intestino grueso.
Además, el catéter de balón de la presente
invención está equipado con un balón más inflable; tras la inserción
del catéter de balón en el lugar deseado, el balón es inflado
dentro del intestino grueso, lo que impide un movimiento indeseable
del catéter de globo.
La presente descripción se refiere a ileostomía
transcecal según se reivindica posteriormente.
La figura 1 es una vista lateral en sección
transversal de la realización preferida del set de ileostomía
transcecal completo (sin la bolsa para heces, que no es una parte de
la invención).
La figura 2 es una vista en planta del soporte
de plástico de la figura 1.
La figura 3 es una vista lateral en sección del
soporte de plástico de la figura 1.
La figura 1 muestra el set de ileostomía
transcecal de la presente invención. Consiste en un catéter de globo
1 equipado con dos globos - balón de bloqueo 2 y balón de fijación
3 - en una porción distal del mismo, con una perforación 4 en el
mismo extremo distal. El catéter de globo 1 está dividido en tres
lúmenes - uno de ellos equipado con una válvula 5 del globo de
bloqueo en un extremo proximal, otro con una válvula 6 del globo de
fijación en un extremo proximal y el último terminado con una salida
de fluido intestinal 7 en su extremo proximal. El catéter de globo
1 está sujeto contra el movimiento dentro del intestino grueso 8 por
medio del globo de fijación inflado 3 y fuera del cuerpo de un
paciente en la superficie de una pared abdominal 9 por medio de un
soporte de plástico 10. Además de eso, el catéter de balón 1 se
sujeta a una pared 11 del intestino grueso utilizando un bucle
biorresorbible 12. Ese denominado bucle biorresorbible 12 pasa a
través de la pared 11 del intestino grueso y de la pared abdominal
9 y es atado al soporte de plástico 10 (como se muestra en la
figura 3).
El soporte de plástico 10 citado se fija al lado
exterior de la pared abdominal 9 utilizando aberturas de fijación
13 como se representa mejor en la figura 2; sirve para sujetar el
catéter de globo 1 y el bucle biorresorbible 12. El bucle
biorresorbible 12 pasa desde el lugar de la incisión hacia dentro de
la pared 11 del intestino delgado (a través de la cual se inserta
el catéter de globo 1) a través de la pared abdominal 9 y la
abertura 14 de las dos partes estable 15 y giratoria 16 del soporte
de plástico 10 y es atado al mismo (como se muestra en la figura
3). La parte giratoria 16 del soporte de plástico 10 se fija con un
saliente 17 que sirve de bloqueo cuando se aprieta el bucle
biorresorbible 12. Para apretar el mencionado bucle biorresorbible
12, se hace girar la parte giratoria 16 en el sentido de las agujas
del reloj; un movimiento en el sentido contrario al de las agujas
del reloj de dicha parte giratoria 16 es bloqueado por un sistema de
dentado con pestillo 18.
En una realización preferida según la presente
invención, el catéter de globo 1 se introduce a través de una
pequeña incisión en la pared 11 del intestino grueso, de tal modo
que su porción distal con el globo de fijación 3 se inserte en el
intestino grueso 8 y el globo de bloqueo 2 en el intestino delgado
19 detrás de la válvula de Bauhin 20 (como se muestra en la figura
1).
El bucle biorresorbible 12, por ejemplo sutura
quirúrgica monofilamentosa PDS (sutura quirúrgica de polidioxanona
absorbible) en esta realización, es especialmente situado alrededor
del catéter de globo 1 en forma de sutura del tipo de cuerda de
bolsa para sellar el espacio entre el catéter de globo 1 y los
bordes de la incisión en la pared 11 del intestino grueso. Los dos
extremos del bucle biorresorbible 12 son enfilados después a través
de la pared abdominal 9 de tal modo que formen un bucle de lazo
(como se muestra en la figura 3). Este bucle de lazo sujeta
fuertemente la pared 11 del intestino grueso a la pared abdominal 9
y permite un cierre de la incisión después de la retirada del
catéter de globo 1. El extremo proximal del catéter de globo 1 queda
sujeto por el soporte de plástico 10 fijado a la superficie del
cuerpo como se muestra en la figura 1.
Se pone una jeringuilla en la válvula 6 del
globo de fijación para inflar el globo de fijación 3 con el fin de
fijar el catéter de globo insertado 1 in situ. A
continuación, se pone una jeringuilla en la válvula 5 del balón de
bloqueo para inflar el globo de bloqueo 2 con el fin de obstruir
completamente el intestino delgado 19. El fluido intestinal es
forzado así a fluir a través de la perforación 4 del catéter de
globo 1 y a través de éste hasta una salida de fluido intestinal 7
en el extremo proximal del catéter de globo 1 (que está conectado a
una bolsa ileostómica común que no es una parte de esta
invención).
Cuando ya no sea necesario este dispositivo y
tenga que retirarse el catéter de globo 1, tanto el globo de
bloqueo 2 como el globo de fijación 3 se desinflan primero
utilizando de nuevo una jeringuilla. El catéter de globo 1 se
retira entonces del cuerpo de un paciente simplemente tirando.
Para evitar una fuga de fluido intestinal a
través de la incisión cuando se retira el catéter de globo 1,
deberá cerrarse la pared 11 del intestino grueso. Girando la parte
giratoria 16 del soporte de plástico 10 en el sentido de las agujas
del reloj desde la posición por defecto 19 hasta la posición del
bucle apretado 20 (como se muestra en la figura 2), el bucle
biorresorbible 12 frunce la pared 11 del intestino grueso hasta que
esté completamente cerrada.
Una vez cerrada, dicha pared 11 del intestino
grueso se cura en varios días; se retira el soporte de plástico 10.
El bucle biorresorbible 12 llega a resorberse posteriormente in
situ, no siendo necesaria así una intervención adicional.
Claims (2)
1. Set (conjunto de elementos) de ileostomía
transcecal diseñado para drenaje de fluidos intestinales desde el
intestino delgado (19), que comprende un catéter de globo (1) que es
introducido percutáneamente en el intestino delgado (19) a través
de una pared abdominal (9) y una pared (11) del intestino grueso,
caracterizado por un globo de fijación (3) fijado a una
porción distal del catéter de globo (1) de tal modo que éste se
infle en el intestino grueso (8) para llenar su lumen completo, y un
globo de bloqueo (2) fijado a una porción distal del catéter de
globo (1) de tal modo que éste se infle en el intestino delgado (19)
detrás de una válvula de Bauhin (20) para obstruir su lumen
completo.
2. Set de ileostomía transcecal según la
reivindicación 1, caracterizado porque el catéter de globo
(1) comprende tres lúmenes, de los cuales uno está equipado con una
válvula (6) del globo de fijación en su extremo proximal que sirve
para el inflado del globo de fijación (3), otro está equipado con
una válvula (5) del globo de bloqueo en su extremo proximal que
sirve para el inflado del globo de bloqueo (2) y un lumen más está
equipado con una salida de fluido intestinal (7) en su extremo
