Abstract
Un aparato (1) para tratar un área de bifurcación en el que un conducto del cuerpo principal (2) se separa en al menos dos conductos secundarios (3) y en el que el conducto principal (2) aumenta, en sección transversal, hacia la unión con los múltiples conductos, comprendiendo dicho aparato (1) un primer cuerpo de stent expansible, configurado para ser enganchado en el conducto primario (2) en el área de bifurcación caracterizado porque: dicho primer cuerpo de stent es auto-extensible y tiene, en un estado predeterminado y totalmente auto-expandido, una forma sustancialmente cónica y un primer extremo (6) que tiene un diámetro mayor que un segundo extremo (5), y una sección transversal del primer extremo sustancialmente mayor que una sección transversal de uno de los conductos secundarios 3; teniendo el primer cuerpo del stent una forma para ser enganchado a la sección transversal creciente del conducto principal (2), en el área de una zona de transición ensanchada de la bifurcación que separa el conducto principal (2) de los conductos secundarios (3), en la dirección de al menos un conducto secundario (3), abriéndose dicho primer extremo (6) en la dirección de dichos conductos secundarios (3) al expandirse, y teniendo el primer cuerpo del stent forma para ser independiente de cualquier otro cuerpo del stent, y ser libre de cualquier medio para conectarse a cualquier otro cuerpo del stent, cuando el primer cuerpo del stent está en un estado predeterminado totalmente auto-expandido, caracterizado porque dicho cuerpo del stent está cortado de un conducto único.
Description
Prótesis endovascular para una bifurcación.
La presente invención se refiere a un aparato y
equipo para el tratamiento de conductos corporales en el área de
una bifurcación, es decir, en el área donde un conducto principal se
separa en dos conductos secundarios. También se refiere al
equipamiento para posicionar este aparato.
El aparato y equipo de la invención puede
utilizarse para el tratamiento de bifurcaciones vasculares, en
particular las bifurcaciones carótidas, femorales, ilíacas,
poplíteas, renales o coronarias, o bifurcaciones no vasculares,
como bifurcaciones traqueales o biliares, por ejemplo, entre los
conductos comunes hepático y cístico, o en el área de la
bifurcación del tracto biliar principal.
El tratamiento puede consistir en reestablecer
el diámetro apropiado de la bifurcación, en casos de
arteriosclerosis o proliferación celular interna, en rectificar una
disección localizada o no localizada en la pared del conducto, o,
en casos de aneurisma, en recrear una bifurcación de diámetro
normal, mientras se elimina la bolsa aneurismática.
Se conoce para tratar el estrechamiento de un
vaso sanguíneo rectilíneo mediante un aparato tubular expandible
radialmente, comúnmente denominado stent. Este aparato se
introduce en estado no expandido en el lumen interno del vaso, en
particular por vía percutánea, hasta llegar al área de
estrechamiento. Una vez en su sitio, se expande de tal modo que
puede soportar la pared del vaso y así, reestablecer la sección
transversal adecuada del vaso.
El aparato puede estar realizado en un material
no elástico, y en este caso, se expande mediante un globo hinchable
al que está acoplado, o puede autoexpandirse, es decir, estar hecho
de material elástico que se expande espontáneamente cuando se
retira de una vaina que lo mantiene en estado contraído.
Las patentes estadounidenses números 4,733,665 y
4,886,062 ilustran aparatos existentes y técnicas de posicionamiento
correspondientes.
Un stent convencional no es totalmente
adecuado para el tratamiento de un estrechamiento situado en el
área de una bifurcación, ya que su acoplamiento tanto en el conducto
principal como en uno de los conductos secundarios puede causar una
oclusión inmediata o retardada del otro conducto secundario.
Es conocido para reforzar una bifurcación
vascular mediante un aparato que comprende dos elementos, cada uno
formado por un bobinado helicoidal de un filamento de metal. Uno de
los dos elementos tiene dos partes de diámetro correspondientes
respectivamente al diámetro del vaso principal y al diámetro de uno
de los vasos secundarios, y su objeto es acoplarse, por un lado, en
este vaso principal y, por otro, en este vaso secundario. El otro
elemento tiene un diámetro correspondiente al diámetro del otro vaso
secundario y está enganchado al primer elemento, después de que
este haya sido colocado en su lugar, al acoplar uno o más de sus
vueltas en las vueltas del primer elemento.
Este equipo permite el refuerzo de la
bifurcación pero resulta inadecuado para tratar un estrechamiento
vascular o una lesión oclusiva, debido a su estructura y a la baja
posibilidad de expansión radial de sus dos elementos
constituyentes.
Asimismo, la forma del primer elemento no se
corresponde con la forma de una bifurcación, que tiene una zona de
transición ensanchada entre el extremo del vaso principal y los
extremos de los vasos secundarios. De este modo, este equipo no
permite el soporte total de esta pared o el tratamiento de una
disección en el área de esta pared. Además, el posicionamiento
separado de estos dos elementos parece relativamente difícil.
El documento US 20010037138 divulga un
procedimiento y un aparato para vasos bifurcados mediante
stent.
La patente estadounidense nº 5,683,449 a Marcade
(Marcade) divulga un injerto bifurcado formado por una serie de
componentes individuales que se suministran de manera intraluminal
separados unos de otros y, ensamblados después para formar una
estructura totalmente compatible. El sistema incluye un miembro de
base y uno o más injertos conectados a este. El sistema de Marcade
requiere una conexión solapada de sus componentes con el fin de
lograr una estructura totalmente compatible y bloquear los
componentes entre sí, garantizando de este modo un montaje seguro y
minimizando la posibilidad de fuga. Marcade divulga asimismo que los
sistemas y procedimientos que no emplean dicho sistema solapado son
propensos a sufrir averías agudas y crónicas, por lo que el injerto
podría desplazarse o colapsar.
La patente estadounidense nº 6,068,655 de Seguin
et al. (la patente 655) divulga un aparato que tiene un
primer segmento con forma cónica, al menos un segmento secundario,
y un enlace flexible entre segmentos. El enlace flexible permite el
ajuste de segmentos relativos entre sí, pero los mantiene en
conexión unos con otros, limitando su posicionamiento
independiente.
El documento WO 99/37242 divulga un injerto
reforzado que incluye una lámina flexible de material de injerto y
un alambre de refuerzo. El alambre de refuerzo se une a la lámina
flexible, por ejemplo, mediante puntos y la lámina se enrolla
consiguientemente en una forma tubular para ser utilizada en el
tratamiento de aneurismas, por ejemplo, en la aorta, mediante un
procedimiento endoluminal. En algunas realizaciones el injerto
reforzado es auto-expansible.
Los aparatos de la técnica anterior no son
suficientes para tratar áreas de bifurcación vascular porque
requieren una conexión, ya sea directa o indirecta, entre múltiples
segmentos del aparato. Esta conexión fuerza a los segmentos a
alinearse unos con respecto a otros, en contraste con alinearse de
acuerdo con la forma y curvatura de los vasos bifurcados que se van
a tratar. Esto disminuye la flexibilidad del aparato. Asimismo, los
segmentos conectados para ser directamente adyacentes o solapados
disminuye además la flexibilidad del aparato. Dichos aparatos son
problemáticos, por ejemplo, para tratar un conducto secundario
bifurcado que tiene un eje perpendicular al eje del conducto
primario.
La presente invención tiene por objeto superar
varias de estas desventajas poniendo a disposición un aparato con
el que es posible tratar una condición patológica en el área de
bifurcación, soportando totalmente la pared vascular y siendo
relativamente sencillo de colocar.
De acuerdo con un aspecto de la invención, se
proporciona un aparato para tratar un área de bifurcación de
acuerdo con la reivindicación 1.
El aparato al que se refiere la invención
comprende un segmento cónico con una forma para adaptarse a la forma
de la bifurcación en el área de una zona de transición ensanchada.
El segmento cónico tiene una forma que se adapta a la sección
transversal creciente del conducto principal en un área de, y
orientado a, la bifurcación vascular.
En las realizaciones, el extremo distal del
stent puede expandirse a un diámetro que sea al menos el 105%
del diámetro del extremo proximal del stent. En las
realizaciones, el extremo distal del stent puede expandirse
a un diámetro que sea al menos el 110% del diámetro del extremo
proximal del stent.
En las realizaciones, el aparato comprende solo
un cuerpo (es decir, segmento) que tiene, en un estado expandido
predeterminado, una forma truncada que se corresponde con la forma
de la bifurcación en el área de la zona de transición ensanchada
que separa el conducto principal de los conductos secundarios. De
este modo, solo se requiere un único stent para tratar una
bifurcación vascular, eliminando de este modo la necesidad de
inserción quirúrgica, posicionamiento y coordinación de múltiples
segmentos de stent.
En las realizaciones, el aparato comprende:
al menos un segmento radialmente expansible que
tiene, en un estado expandido predeterminado, una sección
transversal mayor que la sección transversal de uno de los conductos
secundarios; y
- un segmento que tiene, en un estado expandido
predeterminado, una forma truncada que se corresponde con la forma
de la bifurcación en el área de la zona de transición ensanchada que
separa el conducto principal de los conductos secundarios.
En estas realizaciones, los segmentos no se
solapan unos con otros, y no están unidos unos a otros y
preferiblemente no incluyen medios para unirse unos a otros. De
este modo, puede lograrse un posicionamiento óptimo de los
segmentos del stent de forma independiente unos de otros.
Para el propósito de esta aplicación, un "segmento no
vinculado" se define como un segmento independiente que no está
conectado ni directa ni indirectamente a ningún otro aparato
quirúrgico, como stents o segmentos adicionales, cuando el
segmento independiente está en su estado predeterminado, totalmente
expandido.
A efectos de simplificación, el segmento que
tiene, en el estado expandido, una sección transversal
sustancialmente mayor que la sección transversal de uno de los
conductos secundarios se denominará en lo sucesivo "segmento
secundario", mientras que el segmento que tiene, en el estado
expandido, una forma truncada se denominará en lo sucesivo
"segmento truncado."
De acuerdo con otro aspecto de la invención, se
proporciona un aparato para tratar un área de bifurcación de
acuerdo con la reivindicación 11.
En las realizaciones, el cuerpo cónico del
stent puede estar revestido con un elemento de revestimiento
en un equipo para tratar un área de bifurcación vascular. En las
realizaciones, el equipo puede incluir únicamente el cuerpo cónico
del stent y el elemento de revestimiento. Alternativamente,
el equipo puede además incluir al menos un cuerpo de stent
secundario.
El segmento secundario está concebido para ser
introducido en el conducto secundario en estado contraído y para
apoyarse, en el estado expandido, contra la pared de este conducto.
Esta expansión no solo hace posible tratar un estrechamiento o una
disección situada en el área de este conducto, sino también
garantizar una perfecta inmovilización del aparato en el
conducto.
En esta posición del aparato, el segmento
truncado se inclina contra la pared del conducto delimitando la
zona de transición ensanchada de la bifurcación que es capaz de
soportar totalmente. Un estrechamiento o una disección que tiene
lugar en este emplazamiento pueden, de este modo, ser tratados
mediante este aparato, con soporte uniforme de la pared vascular, y
así, sin riesgo de que se produzcan daños en la pared.
Los dos segmentos no vinculados, no solapados se
orientan de forma adecuada independientemente unos de otros, al
expandirse.
En las realizaciones, al menos el segmento
truncado está cubierto por una pared que le proporciona
impermeabilidad en una dirección radial.
Esta pared hace posible que atrape, entre él y
la pared del conducto, partículas que pueden originarse de la
lesión que se está tratando, como partículas arterioscleróticas o
aglomerados celulares, y evitar así la migración de estas
partículas en el cuerpo.
Además, el aparato puede permitir el tratamiento
de un aneurisma guiando el líquido a través de la bifurcación y
evitando así la tensión de la pared que forma el aneurisma.
Los segmentos pueden estar hechos de un tubo
único con diferentes modelos de cortes para generar diferentes
formas y tamaños.
El aparato puede comprender varios segmentos
secundarios, colocado uno detrás del otro, para garantizar el apoyo
complementario de la pared del conducto secundario. Con este mismo
fin, el aparato puede comprender, en el lado del segmento truncado
dirigido hacia el conducto principal, al menos un segmento
radialmente expansible que tiene, en el estado expandido, una
sección transversal que es sustancialmente mayor que la sección
transversal del conducto principal.
De acuerdo con las realizaciones de la
invención, los segmentos del aparato tienen una estructura de malla,
siendo las mallas alargadas en la dirección longitudinal del
aparato, y teniendo cada una forma sustancialmente hexagonal; las
mallas del segmento truncado tienen una anchura que aumenta
progresivamente en el sentido longitudinal del aparato, en la
dirección del extremo de este segmento que tiene la mayor sección
transversal en el estado expandido.
Este aumento en la anchura de las mallas es el
resultado de un incremento en la longitud de los bordes de las
mallas dispuestos longitudinalmente y/o un aumento en el ángulo
formado entre dos bordes enfrentados de la misma malla.
Además, el segmento truncado puede tener un eje
no coincidente con el eje longitudinal del segmento secundario,
sino oblicuo en relación a este eje, con el fin de adaptarse
óptimamente a la anatomía de la bifurcación que se va a tratar.
El aparato puede estar hecho de un metal con
efecto térmico de memoria. Este metal es preferiblemente la aleación
de níquel/titanio conocida por el nombre de NITINOL.
El equipo para posicionar el aparato comprende
medios para permitir la expansión de este aparato cuando este está
en posición. Estos medios pueden comprender una vaina móvil en la
que se coloca el aparato en estado contraído, cuando este aparato
está hecho de un material elástico o un núcleo de apoyo que
comprende un globo hinchable en el que se coloca el aparato, cuando
este aparato está hecho de un material no elástico.
En cualquier caso, este equipo comprende, de
acuerdo con la invención, medios con los que es posible identificar,
a través del cuerpo del paciente, la ubicación longitudinal del
segmento truncado, de modo que este pueda posicionarse
correctamente en el área de la zona ensanchada de la
bifurcación.
En el caso en que la expansión de este mismo
segmento no sea uniforme en relación con el eje del aparato, el
equipo comprende adicionalmente medios con los que es posible
identificar, a través del cuerpo del paciente, la orientación
angular del aparato en relación con la bifurcación, de modo que
parte de este segmento que tiene la mayor expansión puede colocarse
de manera adecuada en relación con la bifurcación.
Para garantizar que se ha comprendido
totalmente, se describe la invención de nuevo a continuación con
referencia a los dibujos adjuntos en forma de diagrama que
muestran, mediante un ejemplo no limitante, realizaciones de los
aparatos a los que se refiere la invención.
La figura 1 es una vista lateral de los mismos,
de acuerdo con una primera realización;
La figura 2 es una vista en perspectiva de
aparatos de la invención en un estado de contracción radial, y, en
corte transversal parcial, del equipo que permite su
posicionamiento;
La figura 3 es una vista seccional longitudinal
de una bifurcación que va a tratarse;
Las figuras 4 a 6 son vistas de esta bifurcación
similares a la figura 3, durante tres etapas sucesivas de
posicionamiento de aparatos de la invención;
La figura 7 es una vista, similar a la figura 3,
de una bifurcación que presenta un aneurisma, en la que se colocan
aparatos de la invención, y
la figura 8 es una vista lateral de aparatos de
acuerdo con una segunda realización.
La figura 1 muestra un aparato expansible 1 que
permite el tratamiento de conductos corporales en el área de una
bifurcación, es decir, como se muestra en la figura 3, en el área
donde un conducto principal 2 se separa en dos conductos
secundarios 3.
En las realizaciones, el aparato 1 comprende un
segmento cónico que tiene un extremo proximal 5 y un extremo distal
6, y al menos un/unos segmento(s) secundario(s) que
tienen un extremo proximal y un extremo distal 8, cada uno con una
estructura de malla, que no están solapados ni vinculados como se
ilustra en 9.
Las mallas 10 de estos segmentos son alargadas
en la dirección longitudinal del aparato 1 y tienen en cada caso
una forma sustancialmente hexagonal.
El extremo proximal 5 del segmento cónico tiene
una forma tubular y tiene un diámetro que es sustancialmente mayor
que el diámetro del conducto principal 2.
El extremo distal 6 del segmento cónico tiene
mallas 10 cuya anchura aumenta progresivamente, en comparación con
las mallas del extremo proximal 5, por un lado en el sentido
longitudinal del aparato 1, en la dirección del extremo del extremo
distal 6 situado en la parte opuesta al extremo proximal 5.
Puede producirse un incremento transversal en la
anchura de las mallas 10 a partir de un incremento en la longitud
de los bordes 10a de las mallas 10 dispuestas longitudinalmente, así
como un incremento en el ángulo formado entre dos bordes
enfrentados 10a.
De este modo, este segmento cónico tiene una
forma truncada con un eje que es oblicuo en relación con el eje
longitudinal del aparato 1. Esta forma se corresponde con la forma
de la bifurcación en el área de la zona de transición ensanchada 11
que separa el extremo del conducto principal 2 de los extremos de
los conductos secundarios 3.
En varias realizaciones alternativas, el
segmento cónico puede estrecharse continuamente desde el extremo
proximal al extremo distal, o la reducción puede variar de forma
adecuada.
El extremo proximal 7 y el extremo distal 8 del
segmento secundario tienen un diámetro idéntico, que es
sustancialmente mayor que el diámetro de uno de los conductos
secundarios 3.
El aparato 1 puede estar hecho mediante el corte
adecuado de una lámina de aleación de níquel/titanio conocida con
el nombre de NITINOL, a continuación se dobla el modelo resultante
en un círculo y se sueldan las partes de este modelo que se acercan
unas a otras.
Esta aleación es maleable a una temperatura
cercana a los 10ºC pero puede recuperar su forma neutra a una
temperatura sustancialmente correspondiente a la del cuerpo
humano.
La figura 2 muestra el aparato 1 en un estado de
contracción radial, obtenido mediante el enfriamiento de su
material constituyente. Durante esta contracción, los bordes 10a se
articulan en relación con los bordes transversales 10b de las
mallas 10, de tal modo que las mallas 10 tienen, en este estado de
contracción, una forma sustancialmente rectangular.
Debido a esta contracción, los segmentos 5
forman una sección transversal, que es menor que la de los conductos
2 y 3, y pueden introducirse en estos, como se describirá a
continuación.
El aparato 1 está acoplado en un núcleo de apoyo
central 15, y está contraído radialmente en este. Este núcleo 15
comprende un empalme axial, como un resalte (no visible en la figura
2) que tiene un diámetro menor que el del aparato 1 cuando este
aparato está expandido, pero mayor que el diámetro de este aparato 1
cuando este está contraído. Este empalme permite, por
consiguiente, la inmovilización del aparato 1 en el núcleo 15 cuando
este está contraído.
A continuación se acopla una vaina 16 en el
aparato 1 con el fin de mantenerlo en estado contraído. Esta vaina
16 puede incluir, por ejemplo, cuatro marcadores radiopacos 20, 21,
22, 23 impresos en ella, que contienen por ejemplo, un compuesto de
bario. Tres marcadores 20, 21, 22 tienen una forma anular 5 y se
extienden por toda la periferia de la vaina 16. Están situados,
respectivamente, en el área de los extremos libres de los
segmentos. El cuarto marcador 23 está situado sustancialmente en el
punto medio de la generatriz del segmento 6. Tiene forma de
diamante y poco espesor.
El núcleo 15 tiene un orificio axial
longitudinal que permite su acoplamiento en un alambre guía 25
(figuras 4 a 6). Este alambre 25 puede acoplarse, por vía
percutánea, en el conducto 2, mediante la zona 11, y a continuación
en uno de los conductos 3, a través del cual puede deslizarse, y
comprende un cono 26 de material sintético, situado delante del
montaje que consiste en el núcleo 15, aparato 1 y vaina 16.
La bifurcación 30 mostrada en la figura 3 tiene
excrecencias 31 que crean un estrechamiento en la sección
transversal, que impide que el flujo del líquido circule en los
conductos 2 y 3. En el caso de una bifurcación vascular, estas
excrecencias pueden deberse, por ejemplo, a arteriosclerosis o
crecimiento celular.
El aparato 1 permite el tratamiento de esta
bifurcación mediante el reestablecimiento del diámetro apropiado de
los conductos 2, 3 y de la zona ensanchada 11.
Según convenga, en función de si se requiere
refuerzo en los conductos secundarios, el segmento cónico puede
utilizarse en el área ensanchada de la bifurcación, o puede
utilizarse junto con uno o más segmentos secundarios.
En la práctica, como puede verse en la figura 4,
el montaje que consiste en un núcleo 15, un aparato 1 y una vaina
15 puede acoplarse en el alambre 25 hasta llegar al cono 26.
Mediante esta acción deslizante, este alambre 25 permite el
acoplamiento y a continuación, la guía de este montaje en el
conducto 2, la zona 11 y a continuación, el conducto 3. El cono 26
facilita el deslizamiento del montaje y reduce el riesgo de
trauma.
El marcador 22 hace posible visualizar, con la
ayuda de un aparato de radiografía adecuado, la posición del
segmento cónico, y de este modo, visualizar la localización del
segmento cónico de modo que puede posicionarse correctamente en
relación con la zona ensanchada 11.
Con los marcadores 20 y 21, es posible
garantizar que los segmentos están correctamente ubicados en el
conducto principal 2 y el conducto secundario 3.
El marcador 23 es, por su parte, visible en una
vista en planta o vista de canto, dependiendo de si está orientado
perpendicular o paralelamente al radio del aparato de radiografía.
De este modo, es posible identificar la orientación angular del
aparato 1 en relación con la bifurcación 30, de modo que la parte
del segmento cónico que tiene la mayor expansión puede situarse de
manera apropiada en relación con la zona 11.
La vaina 16, que tiene una longitud tal que se
abre más allá de la abertura que ha permitido la introducción del
montaje, se va retirando progresivamente, como se muestra en las
figuras 5 y 6, con el fin de permitir la expansión completa del
aparto 1. Este se recalienta por la temperatura corporal, y esto
permite su expansión, o puede expandirse por la simple fuerza del
muelle sin efectos de temperatura. Después de la expansión completa
del aparato 1, el núcleo 15 y el alambre 25 se retiran.
La figura 7 muestra que el aparato 1 también
puede utilizarse para tratar un aneurisma 40. Al menos una parte
del segmento cónico está cubierto por una película de poliéster 41,
impermeable al líquido que circula en los conductos, estando dicha
película cosida a ellos. Entonces, el aparato guía este líquido a
través de la bifurcación 30 y por consiguiente, evita la tensión de
la pared que forma el aneurisma 40.
La figura 8 muestra un aparato 100 de acuerdo
con la invención, que tiene segmentos 105, 106, 107, 108, con una
estructura similar a la de los segmentos 5 a 8 en el aparato
mostrado en la figura 1.
En el aparato 100, los segmentos individuales
incluyen seis puentes con forma de omega 190. La parte central
curvada 190a de estos puentes 190 tiene una elasticidad
multidireccional que permite la orientación longitudinal apropiada
de los diversos segmentos en relación mutua.
La ventaja de estos puentes 190 es que
proporcionan segmentos individuales, y de este modo, el aparato, con
continuidad longitudinal, que facilita el paso de dicho aparato en
una zona muy curvada y que elimina la necesidad de reducir esta
curvatura, que siempre es peligrosa en los casos de
arteriosclerosis, para permitir el paso del aparato.
De este modo, la invención proporciona aparatos
que permiten el tratamiento de una condición patológica en el área
de una bifurcación 30. Estos aparatos tienen las numerosas ventajas
indicadas anteriormente, en particular las de garantizar un apoyo
perfecto de la pared del vaso y de ser relativamente sencillas de
ubicar.
No es necesario afirmar que la invención no está
limitada a las realizaciones descritas anteriormente a modo de
ejemplo, sino que, en su lugar, permite todas las realizaciones
alternativas.
De este modo, los aparatos 1 y 100 pueden
comprender un segmento único, cónico, o un segmento cónico y uno o
varios segmentos secundarios colocados uno después del otro, para
garantizar un apoyo complementario y, si fuese necesario, para
incrementar el soporte del aparato en la bifurcación 30.
El núcleo 15 podría comprender un globo
hinchable, ya sea para efectuar la expansión del aparato 1, en el
caso en que este está hecho de un material no elástico, o para
garantizar la expansión total de un aparato
auto-expansible 1, de acuerdo con la invención
después de haber sido colocado en su lugar.
Los marcadores 20 a 23 podrían imprimirse en el
núcleo 15 o directamente en el aparato 1.
El segmento cónico podría tener un eje
coincidente con el eje longitudinal del aparato, y no oblicuo en
relación con este eje, si resulta necesario por la anatomía de la
bifurcación que va a tratarse.
Asimismo, el segmento secundario podría tener él
mismo, en el estado expandido, una forma ensanchada, correspondiente
a la forma de la zona de conexión ensanchada mediante la cual, en
determinadas bifurcaciones, los conductos secundarios 3 están
conectados a la zona de transición ensanchada 11. El segmento
secundario tendría, de este modo, una forma correspondiente a la
forma de esta zona de conexión ensanchada, y garantizaría un apoyo
perfecto del mismo.
El número de puentes 190 podría ser superior o
inferior a seis, y podrían tener una forma diferente a la forma de
omega, permitiendo su elasticidad multidireccional, y en particular
una forma V ó W.
Como se ha planteado, el cuerpo cónico del
stent puede estar revestido con un elemento de revestimiento
en un equipo para tratar un área de bifurcación vascular. En las
realizaciones, el equipo también puede incluir únicamente el cuerpo
del stent y el elemento de revestimiento. Alternativamente,
el equipo puede además incluir al menos un cuerpo de stent
secundario.
El sistema de stent descrito puede
adaptarse como se ha mencionado anteriormente para tratar cualquier
número de bifurcaciones en un paciente humano. Por ejemplo, las
bifurcaciones de las arterias coronarias izquierda y derecha, la
bifurcación de la arteria circunfleja, las bifurcaciones de la
carótida, la femoral, la ilíaca, la poplítea, la renal o la
coronaria Alternativamente, este aparato puede utilizarse para
bifurcaciones no vasculares, como las bifurcaciones traqueales o
biliares, por ejemplo, entre los conductos comunes hepático y
cístico, o en el área de la bifurcación del tracto biliar
principal.
Claims (15)
1. Un aparato (1) para tratar un área de
bifurcación en el que un conducto del cuerpo principal (2) se separa
en al menos dos conductos secundarios (3) y en el que el conducto
principal (2) aumenta, en sección transversal, hacia la unión con
los múltiples conductos, comprendiendo dicho aparato (1) un primer
cuerpo de stent expansible, configurado para ser enganchado
en el conducto primario (2) en el área de bifurcación
caracterizado porque:
dicho primer cuerpo de stent es
auto-extensible y tiene, en un estado predeterminado
y totalmente auto-expandido, una forma
sustancialmente cónica y un primer extremo (6) que tiene un diámetro
mayor que un segundo extremo (5), y una sección transversal del
primer extremo sustancialmente mayor que una sección transversal de
uno de los conductos secundarios 3; teniendo el primer cuerpo del
stent una forma para ser enganchado a la sección transversal
creciente del conducto principal (2), en el área de una zona de
transición ensanchada de la bifurcación que separa el conducto
principal (2) de los conductos secundarios (3), en la dirección de
al menos un conducto secundario (3), abriéndose dicho primer extremo
(6) en la dirección de dichos conductos secundarios (3) al
expandirse, y
teniendo el primer cuerpo del stent forma
para ser independiente de cualquier otro cuerpo del stent, y
ser libre de cualquier medio para conectarse a cualquier otro cuerpo
del stent, cuando el primer cuerpo del stent está en
un estado predeterminado totalmente auto-expandido,
caracterizado porque dicho cuerpo del stent está
cortado de un conducto único.
2. El aparato de la reivindicación 1, que
comprende además al menos un segundo cuerpo del stent
radialmente expansible, en un estado predeterminado totalmente
expandido, una forma sustancialmente cilíndrica, estando dicho
segundo cuerpo del stent configurado para ser enganchado en
dicho conducto secundario (3);
siendo la sección transversal de al menos un
segundo cuerpo del stent, en el estado predeterminado
expandido, menor que la sección transversal del primer y segundo
extremo (5, 6) del primer cuerpo del stent cuando el primer

3. El aparato de la reivindicación 2 en el que,
el primer cuerpo del stent está configurado para acoplarse
en el conducto principal (2) solamente, y estando al menos el
segundo cuerpo del stent configurado para ser enganchado en
un conducto secundario solamente (3).
4. El aparato de la reivindicación 2, en el que
dicho primer y segundo cuerpos del stent están hechos del
primer y segundo tubos de material, respectivamente, teniendo el
segundo tubo un diámetro mayor que el primer tubo.
5. El aparato de la reivindicación 4, en el que
dichos tubos están 9 10 hechos del mismo material.
6. El aparato de la reivindicación 2, que
comprende además una pluralidad de segundos cuerpos del stent
radialmente expansibles, pudiendo estar colocados uno detrás del
otro en un conducto secundario (3) y teniendo, en el estado
expandido predeterminado, una sección transversal que es
sustancialmente mayor que la sección transversal de dicho conducto
secundario (3).
7. El aparato de la reivindicación 2, teniendo
dichos primer y segundo cuerpos del stent una estructura de
malla formada por mallas (10) alargadas en una dirección
longitudinal del aparato, teniendo cada malla (10) una forma
sustancialmente hexagonal, en el que las mallas (10) de dicho
segundo segmento tienen una anchura que aumenta progresivamente en
la dirección longitudinal del aparato con las mallas (10) hacia el
extremo del primer cuerpo del stent, que tiene la mayor
sección transversal en el estado expandido.
8. El aparato de la reivindicación 2, que
comprende además al menos un tercer segmento radialmente expansible
que tiene, en un estado expandido, una forma sustancialmente
cilíndrica y una sección transversal que es sustancialmente mayor
que la sección transversal del conducto secundario (3).
9. El aparato de la reivindicación 1, en el que
el diámetro del primer cuerpo del stent disminuye
continuamente desde el primer extremo (6) al segundo extremo (5) de
la misma.
10. El aparato de la reivindicación 1, en el que
el primer extremo es sustancialmente cónico y el segundo extremo
(5) es sustancialmente cilíndrico.
11. Un equipo para tratar un área de bifurcación
vascular en la que el conducto principal (2) se separa en al menos
dos conductos secundarios (3) y en el que el conducto principal (2)
aumenta en sección transversal a la unión con los conductos
múltiples (3), comprendiendo dicho equipo:
un elemento de revestimiento, y un primer cuerpo
del stent radialmente auto-expansible
configurado para ser enganchado en el conducto primario (2) en el
área de bifurcación en la que un conducto del cuerpo principal (2)
se separa en al menos dos conductos secundarios (3) y en la que el
conducto principal (2) aumenta en la sección transversal con la
unión con los múltiples conductos (3); teniendo el primer cuerpo del
stent, en un estado predeterminado totalmente
auto-expandido, una forma sustancialmente cónica y
un primer extremo (6) que tiene un diámetro mayor que un segundo
extremo (5), y una sección transversal del primer extremo es
sustancialmente mayor que una sección transversal de uno de los
conductos secundarios (3), teniendo el primer cuerpo del
stent forma para adaptarse a la sección transversal creciente
del conducto principal (2), en el área de una zona de transición
ensanchada de la bifurcación, que separa el conducto principal (2)
de los conductos secundarios (3), en la dirección de al menos un
conducto secundario (3), abriéndose en la dirección de dichos
conductos secundarios (3) al expandirse, y teniendo el primer cuerpo
del stent forma para ser independiente de cualquier otro
cuerpo del stent, cuando el primer cuerpo del stent
está en un estado predeterminado totalmente
auto-expandido, caracterizado porque dicho
primer cuerpo del stent está cortado a partir de un conducto
único.
12. El equipo de la reivindicación 11,
consistiendo dicho equipo solo en dicho elemento de revestimiento
y dicho primer cuerpo del stent.
13. El equipo de la reivindicación 11,
comprendiendo además al menos un segundo cuerpo del stent
radialmente expansible que tiene, en un estado predeterminado
totalmente expandido, forma sustancialmente cilíndrica, estando
dicho segundo cuerpo del stent configurado para ser
enganchado en dicho conducto secundario (3); siendo la sección
transversal de al menos un segundo cuerpo del stent, en el
estado predeterminado expansible, menor que la sección transversal
del primer y segundo extremos (6,5) del primer cuerpo del
stent cuando cuerpo del stent está en su estado
predeterminado expandido.
14. El aparato de la reivindicación 1, en el que
el primer cuerpo del stent está cortado a partir de un
conducto único con diferentes modelos de cortes.
15. El equipo de la reivindicación 11, en el que
el primer cuerpo del stent está cortado a partir de un
conducto único con diferentes modelos de cortes.
