Abstract
La presente invención se trata de una nueva endoprótesis femoral modular, empleada en amputaciones femorales la que se aplica en una primera fase con el montaje, realizado fuera de campo, mediante la unión de vástago (1) que puede presentar varias medidas con un cuerpo ranurado (6) y está dotado de un orificio roscado (5) donde se inserta el tornillo de cierre corto (4), un espaciador (2) con un sistema de amortiguación e integrador y se bloquea con un cilindro y el tornillo (4), rematándose con un tapón de cierre (10); este montaje permite una segunda fase que se trata de la extracción del bloqueo (10, 4) y la conexión al muñón de un alargador (3), un adaptador (9), con un tornillo de cierre largo (11).
Description
Endoprótesis femoral modular.
La presente invención se refiere a una nueva
endoprótesis femoral modular, que aporta una solución protésica
para favorecer la comodidad de estos pacientes, mejorar su calidad
de vida y la eficacia en la deambulación, mediante una endoprótesis
femoral con final en forma de cóndilos que permita el apoyo distal
y suprimir la presión isquiática, lo que permite aprovechar al
máximo las prestaciones del encaje sin molestias proximales. Esta
endoprótesis está anclada dentro del canal medular del fémur del
paciente, pudiéndose colocar en la intervención primaria (cuando se
produce la amputación) o secundaria (en una segunda intervención),
según cada paciente.
Su campo de aplicación es el campo de las
intervenciones quirúrgicas de endoprótesis y su fabricación.
Tras la amputación femoral, el paciente presenta
unas secuelas físicas y psíquicas que se debe tratar. Las secuelas
físicas responderán a la colocación de una prótesis y las psíquicas
pueden requerir de tratamiento especializado. Adaptarse a una
discapacidad física, como lo es una amputación, depende de una
amplia variedad de factores sociales, psicológicos y ambientales.
Desde un punto de vista psicológico, la respuesta de una persona a
una discapacidad está relacionada con su funcionamiento psicológico
anterior a su amputación.
Los componentes protésicos de la extremidad
inferior utilizados en la primera fase, las podemos dividir en:
- encaje
- estructura endoesquelética
- rodilla (amputación por encima de la
rodilla)
- pie
Los encajes son siempre molestos por los apoyos
necesarios para la buena sujeción de la misma al muñón del paciente
y dependen del nivel de amputación:
- nivel femoral: el paciente apoya en el isquion
y poco en el resto del muñón. Esto conlleva una molestia para el
paciente a la hora de andar, sea cual sea el tipo de encaje, y un
menor control del muñón por estar el fémur inmerso en un
"magma" de tejidos blandos. Dado que el nivel adecuado de
amputación es el tercio medio, queda suficiente espacio para la
colocación de la rodilla protésica.
- nivel desarticulación de rodilla: debido al
apoyo directo distal del muñón en el encaje y la transferencia de
peso en los cóndilos femorales, no es necesario un apoyo
isquiático, con lo que no tiene las molestias en la parte proximal
del muñón. La movilidad de la articulación de cadera no está
restringida, al contrario que en el nivel femoral, pudiendo evitar
dolores locales o a nivel lumbar. Ofrece una mayor comodidad en su
utilización debido a que el apoyo y la gran superficie de contacto
entre muñón y encaje, hacen que las presiones se distribuyan
uniformemente. Este nivel seria el más adecuado para el paciente
dado que tiene un mayor brazo de palanca que el femoral y el apoyo
es distal y no proximal, pero tiene el inconveniente de que en la
colocación de la rodilla se produce un alargamiento de la extremidad
amputada en la posición de sedestación.
En la actualidad algunos inventores han ideado
dispositivos para facilitar la colocación de prótesis femorales
como el expresado en la Pat. 4.158.895, en la que se reivindica un
anclaje de una prótesis artificial en el muñón específicamente en
miembros superiores, con el inconveniente de que No sirve para
miembros inferiores.
Por otro en la Pat. 3.947.897 se describe y
reivindica un anclaje de una prótesis artificial directamente a un
terminal que estaría ya insertado en el canal medular del muñón del
paciente. Además, de ser un sistema bastante complejo, no contempla
la posibilidad de la primera fase, es decir la posibilidad de poner
una prótesis que admita el apoyo distal como cualquier amputado de
fémur desarticulado y tampoco es de carácter modular.
Así mismo, en los documentos Pat.
64-85645 se describe un tipo de implante que no es
para miembros inferiores ni que pueda aguantar cargas soportadas
por el fémur.
Es decir, por la información de la que dispone
el inventor, las prótesis de los documentos mencionados no sirven
para apoyo distal ni se trata de prótesis modulares. Necesitan para
su uso tener una buena calidad del hueso del fémur amputado lo que
limita mucho los posibles candidatos a estas opciones y deja sin
respuesta a la gran mayoría de los amputado por encima de la
rodilla.
Con el fin de solventar los problemas
anteriormente descritos, se propugna en la presente invención la
colocación de una endoprótesis femoral modular de dos fases, que
tenga un pin externo de donde se pueda colgar la rodilla protésica
y se evite así la utilización de un encaje. Esta endoprótesis
también permitiría la percepción sensorial por el paciente de la
llamada osteopercepción o percepción de hueso, que ayuda al
paciente a saber donde coloca la prótesis en la deambulación.
Se trata de un tipo de endoprótesis modular, con
una primera intervención quirúrgica donde se implanta al paciente
la endoprótesis con forma de cóndilos femorales y en los pacientes
tributarios de una segunda intervención, se implanta un pin modular
que permite al paciente realizar una marcha sin la necesidad de un
encaje, conectándose al pin una pierna artificial.
Los candidatos a este tipo de endoprótesis
son:

Útil en todos los pacientes amputados de nivel
femoral con capacidad de deambulación y sin contraindicaciones
óseas (p.e. osteoporosis severa) o por muñón corto, ya sean de
origen vascular (el grupo más numeroso) o
traumático/tumoral/infeccioso.

Fase
Útil en todos los pacientes a quienes se les
haya implantado la 1ª fase y tengan una capacidad de marcha y
funcional suficiente para andar por fuera de su domicilio (nivel K
3).
Con la endoprótesis femoral modular objeto de la
presente invención se consigue un mejor nivel de calidad de vida
para el paciente ya que consiste en la combinación del femoral y de
la desarticulación de rodilla, teniendo así las ventajas del
espacio que permite en el nivel femoral la colocación de la rodilla
protésica y de la comodidad del apoyo distal del muñón en los
cóndilos femorales y del brazo de palanca en el nivel de
desarticulación de rodilla, así como la conservación o mejora de la
calidad de densidad ósea del fémur debido a la estimulación del
mismo por medio de la prótesis.
El éxito en la deambulación del paciente
amputado depende, sobretodo de la buena adaptación del encaje por
un lado y del tratamiento rehabilitador por otro. Las fuerzas que
actúan en la interfase muñón - prótesis son una parte esencial del
éxito en la adaptación y alineación de la prótesis.
En resumen, la movilidad libre de la
articulación de la cadera, permite que la musculatura del muslo se
potencie más eficazmente que en el nivel femoral, y la
transferencia efectiva del peso en los cóndilos femorales permite
al paciente desarticulado de rodilla andar más cómodamente y con un
coste energético más favorable.
En los pacientes de nivel femoral la colocación
de una endoprótesis femoral con final en forma de cóndilos permite
el apoyo distal y disminuye la presión isquiática lo que permite
aprovechar al máximo las prestaciones del encaje sin molestias
proximales.
Esta endoprótesis está anclada dentro del canal
medular del fémur del paciente, pudiéndose colocar en la
intervención primaria (cuando se produce la amputación) o secundaria
(en una segunda intervención), según cada paciente. Al aumentar la
distancia del muñón de los amputados femorales, el área para
acomodar las fuerzas de estabilización
medio-laterales es mayor, por lo que las presiones
de contacto muñón - encaje son más bajas.
Todos los pacientes con un nivel de amputación
femoral son susceptibles para la colocación de este tipo de
endoprótesis, sea cual fuere su edad. Con la comodidad que
obtendría el paciente con el apoyo distal, podría utilizar la
prótesis durante más tiempo y con más calidad.
Por lo tanto el paciente obtiene las siguientes
mejoras respecto a lo existente hasta la fecha:
- Mejor control de la rodilla protésica al poder
apoyar distalmente, en relación con el nivel femoral que no apoya.
La consecuencia será la de tener mayor estabilidad y seguridad en
la marcha.
- Mejor control del muñón con lo que el paciente
es más estable en la marcha.
- Mejora de la marcha en Trendelenburg por el
control del encaje, con menor gasto energético en la marcha.
- Permite cambios en el encaje:
- -
- Posibilidad de realizar ventanas anterior y posterior que en la sedestación dan mayor confort al paciente.
- -
- Supresión de la presión en el isquion, por lo que el paciente puede deambular sin tanta molestia.
- -
- Estimulación ósea provocada por la prótesis que favorece la conservación de una buena densidad ósea y evita los problemas de fractura de cuellos de fémur derivada de la ausencia de cargas en este tipo de pacientes.
Para complementar la descripción que se está
realizando y con objeto de ayudar a una mejor comprensión de las
características del invento, se acompaña a la presente memoria
descriptiva, como parte integrante de la misma, una serie de figuras
en las cuales con carácter ilustrativo y no limitativo, se ha
representado lo siguiente:

En las figuras adjuntas se puede apreciar en
primer lugar un vástago proximal (1), el cual puede presentar
varias medidas, también un espaciador (2) con un sistema de
amortiguación e integración, un cilindro de bloqueo (3) y un
tornillo de cierre corto (4).
El vástago (1) proximal se conforma con un
cuerpo preferentemente ranurado (6) y está dotado de un orificio
roscado (5) donde se inserta el tornillo de cierre (4). Por otro
lado, el espaciador se acopla al cilindro alargador (3) de modo que
envuelve la unión de este cilindro y el vástago (1), y presenta una
serie de orificios (7).

- En la primera fase, el espaciador (2) está hecho de un polímero que tiene un módulo de elasticidad similar al del hueso.
- -
- En la zona de contacto con el frontal del fémur tiene unos orificios (7) que aseguran una buena osteointegración para su fijación segundaria y amortiguación del impacto.
- -
- El tapón de cierre (10) del mismo material que el espaciador, sirve para fijar el espaciador (2) al vástago (1) y proteger el ajuste cónico para la segunda fase.
- -
- Está provisto de un sistema de extracción para facilitar su retirada para la segunda fase.
- -
- Está fijado con un tornillo corto (4) en aleación de titanio al vástago.
El vástago (1) está hecho de aleación de titanio
para los diámetros pequeños y para diámetros más grandes está hecho
en PEEK que asegura una cierta elasticidad. Estos vástagos pueden
ser recubiertos de un material biointegrador y tener distintos
largos y diámetros según el caso.
Para la segunda fase, se utiliza un adaptador en
aleación de titanio (9) con terminal estándar de ortoprótesis y
lleva una ranura de fragilidad (8) para rotura controlada en caso
de accidente o sobrecarga.

- El prolongador (3) de aleación es de titanio de tamaño variable según caso, y une el adaptador al vástago (1)
- -
- El tornillo largo (11) preferentemente en titanio puro, (más plástico que la aleación de titanio) asegura la fijación del conjunto con la posibilidad de deformarse en caso de rotura del adaptador, pero sin romperse.
Su aplicación y funcionamiento es bien sencillo,
para ello, en la primera fase el montaje se hace fuera de campo y
se inserta el vástago (1) montado con el espaciador (2), el tapón
de cierre (10) y el tornillo de bloqueo corto (4). Esta es la
prótesis básica.
Si el usuario así lo desea, en una segunda fase
de este sistema, se hace una incisión en el muñón, se quita el
tornillo de bloqueo corto (4) y el tapón de cierre (10). La
modularidad se consigue con el tubo alargado (3) que se monta con
el adaptador con terminal estándar (9) para prótesis artificial y se
bloquea el conjunto con el tornillo largo (11).
Una vez descrita suficientemente la naturaleza
de la presente invención, así como una aplicación práctica de la
misma, sólo nos queda por añadir que, tanto su forma como los
materiales y ejecución de la misma, son susceptibles de
modificaciones, siempre y cuando no afecten de forma substancial a
las características que se reivindican a continuación.
Claims (5)
1. Endoprótesis femoral modular, empleada en
amputaciones femorales caracterizada porque, el conjunto se
compone de un vástago proximal (1) constituido por un cuerpo
ranurado (6) dotado longitudinalmente por uno de sus extremos de un
orificio roscado (5) al que se acopla el espaciador (2) con un
sistema de amortiguación e integración y se cierra el conjunto por
el otro extremo, bien mediante la inserción del tornillo corto de
cierre (4) con un tapón de cierre (10) extraible, o bien, con un
cilindro alargador modular de bloqueo (3) y un adaptador con
terminal estándar (9) sujetos por medio de un tornillo largo (11)
que se fija directamente al vástago (1) existente.
2. Endoprótesis femoral modular, según
reivindicación primera, caracterizada porque, el espaciador
(2) se acopla al cilindro (3) de modo que envuelve la unión de este
cilindro y el vástago (1).
3. Endoprótesis femoral modular, según
reivindicación primera y anterior, caracterizada porque, el
espaciador (2) presenta una serie de orificios (7) que favorecen la
osteointegración y la amortiguación.
4. Endoprótesis femoral modular, según
reivindicación primera, caracterizada porque, el tapón de
cierre (10) es del mismo polímero que el espaciador (2), y este
tapón fija este espaciador (2) al vástago (1) protegiendo el ajuste
cónico.
5. Endoprótesis femoral modular, según
reivindicación primera, caracterizada porque, el adaptador
en aleación de titanio (9) con terminal estándar de ortoprótesis
lleva una ranura de fragilidad (8) para rotura controlada.
