Description
Dispositivo de fijación de la columna
vertebral.
La presente invención hace referencia a un
sistema o dispositivo para fijar la columna vertebral que utiliza
unos elementos o piezas alargadas que se extienden a lo largo de la
longitud de la columna y se fijan a múltiples vértebras por medio de
unos elementos de fijación como los tornillos para huesos.
Históricamente, la corrección de los trastornos
en la columna vertebral y el tratamiento de las lesiones de la
médula espinal se realizaba por la vía posterior, es decir, por la
espalda del paciente. Inicialmente, la colocación de algún
instrumento por la vía anterior, es decir por delante y por el lado
del paciente no era lo preferido, ya que los cirujanos no estaban
familiarizados con este método y temían complicaciones serias como
algún tipo de lesión neurovascular o el compromiso de la médula
espinal. Sin embargo, a la vista de algunas dificultades dirigidas
hacia la corrección de las curvaturas
torácico-lumbares y escoliótico- lumbares por el
método posterior, los cirujanos buscaron formas anteriores de
corrección y estabilización. Una de esas técnicas fue desarrollada
por Dwyer en Australia durante los años sesenta, en la que se aplicó
al lado convexo de la curvatura escoliótica un elemento
grapa-tornillo. Los tornillos se conectaban mediante
un cable y la corrección se obtenía aplicando fuerzas compresoras a
cada nivel instrumentado. La compresión de la columna por vía
anterior producía unas fuerzas de tracción dentro del cable que en
cambio generaban un momento de flexión corrector en cada uno de los
niveles vertebrales.
Al éxito de Dwyer, le siguió muy de cerca otro
dispositivo por vía anterior. Además, los cirujanos empezaban a
reconocer que ciertos tratamientos de la columna vertebral tenían un
mejor acceso por vía anterior que por vía posterior. Los métodos por
vía anterior proporcionan un acceso directo al espacio
intervertebral para una liberación por vía anterior y la fusión
localizada entre los cuerpos de dos vértebras. Actualmente, las
indicaciones más frecuentes para la instrumentación por vía anterior
incluyen: escoliosis lumbar con elementos deficientes por vía
posterior; curvas torácico-lumbares con lordosis
extrema; escoliosis torácico-lumbar paralítica que
requiere tanto una fusión por vía anterior como posterior;
traumatismo de la columna torácico-lumbar, como las
fracturas por explosión; y condiciones degenerativas del cuerpo
vertebral. En caso de fracturas por explosión, se sabe que la
neurocompresión se produce por la vía anterior. Además, el
desbridamiento por vía anterior de los fragmentos aplastados resulta
ser frecuentemente un medio más eficaz de descompresión del conducto
vertebral o raquídeo, frente a las técnicas conocidas de acceso
posterior.
Desde la primera instrumentación de Dwyer, se han
desarrollado muchos sistemas de placa y varilla por vía anterior,
como los sistemas de Dunn, Kostuik-Harrington,
Zielke y Kaneda. Muchos de estos sistemas permiten la distensión o
separación dinámica de las vértebras seguida de una compresión
directa del hueso óseo implantado contenido en el espacio
intervertebral resecado y después de la descompresión de los
elementos neurales.
Muchos de estos sistemas de acceso anterior
pueden traer complicaciones. Algunos de los problemas más destacados
que se han producido implican fallos en los componentes de fijación,
y una incidencia a menudo elevada de pérdida de la reducción o
corrección. Muchas de las dificultades a este respecto pueden
hallarse en las vértebras instrumentadas al final del conjunto,
donde las cargas en la instrumentación son máximas. En algunos
casos, se ha descubierto que la adquisición bicortical de tornillos
del cuerpo vertebral garantiza una fijación más sólida en los
extremos del conjunto y protege contra un desplazamiento de los
tornillos. No obstante, queda la necesidad de un dispositivo de
acceso anterior que pueda proporcionar la corrección adecuada de las
deformidades de la columna y que pueda ser implantado fácilmente.
Además, el sistema debe garantizar una fijación fuerte que no se
deteriore con el tiempo, dando lugar a una pérdida de la
corrección.
En algunos casos, se ha descubierto que se
necesita una cirugía de revisión, incluso cuando se realiza la mejor
implantación quirúrgica posible de la instrumentación. Las
revisiones frecuentes de la instrumentación de la columna vertebral
incluyen la extensión de la instrumentación ya existente y la
sustitución de los implantes averiados. En los casos que implican
implantes vertebrales prematuros. A medida que el diseño del
implante es más sofisticado, se han desarrollado métodos de cirugía
de revisión más seguros para el paciente y no destructivos para los
implantes, en particular para aquellos implantes que deban
\hbox{quedarse.}
En los sistemas que utilizan tornillos para
huesos, las cirugías de revisión pueden comprometer de forma
significativa el cuerpo vertebral. Además, en determinados métodos
de tipo anterior donde la fijación más fuerte es esencial, los
procedimientos de revisión para sustituir los componentes averiados
comprometen necesariamente la nueva estructura.
A la vista de estas dificultades, existe la
necesidad de un sistema de fijación vertebral que se adapte
fácilmente a la cirugía de revisión. En particular, el sistema debe
ser apropiado para la adición o retirada de componentes mediante una
revisión sin sacrificar una estructura ya existente o bien eliminar
la posibilidad de implantar una estructura nueva más estable.
Además, existe la necesidad de técnicas de revisión que permitan la
retirada completa de una estructura una vez ha ocurrido la fusión,
de nuevo sin comprometer la columna o los implantes.
La US-A-4.987.892
describe un sistema de fijación vertebral en el que los tornillos
del pedículo están conectados a una varilla vertebral alargada
mediante una pinza articulada. El sistema incluye un perno de
apriete o fijación que une los tornillos a la pinza articulada.
De acuerdo con la presente invención, se ha
dispuesto un sistema para la fijación de la columna que comprende:
un elemento alargado configurado para extenderse a lo largo de una
parte de la longitud de la columna, que tiene un primer extremo
adyacente a una vértebra de la parte superior, hacia la cabeza, y un
segundo extremo adyacente a una vértebra coccígea, de la parte
inferior; que se caracteriza por que el sistema comprende además un
implante cilíndrico, dimensionado para situarse en el espacio
intervertebral adyacente a como mínimo una de dichas vértebras
superior e inferior. Dicho implante cilíndrico definirá una cámara
interior para retener el material de crecimiento óseo y una serie de
aberturas que se comunicarán con dicha cámara interior, teniendo
dicho implante una superficie posterior accesible desde fuera de
dicho espacio intervertebral cuando dicho implante cilíndrico está
allí dispuesto, teniendo además dicho implante un eje longitudinal
que se extenderá lejos de dicha superficie posterior; un elemento de
fijación conectado a dicha superficie posterior de dicho implante
cilíndrico, teniendo dicho elemento de fijación una parte superior
para acoplarse a dicho elemento alargado adyacente a como mínimo una
vértebra; y un elemento de conexión que fije rígidamente dicho
elemento de fijación a dicho elemento alargado en un punto de dicho
eje longitudinal de dicho implante cilíndrico.
El sistema de acuerdo con una realización o
modelo utiliza un implante cilíndrico de fusión roscado que se
coloca entre o bien roscado a las placas terminales de las vértebras
adyacentes. El implante puede rellenarse de hueso autógeno morcelado
para promover la fusión a través del implante y entre las placas
terminales vertebrales. Un extremo del implante de fusión se dispone
lateralmente y resulta accesible para la colocación de un elemento
de fijación. En un modelo o realización, el extremo expuesto tiene
una abertura roscada para recibir un puntal roscado del elemento de
fijación. De acuerdo con una característica de la presente
invención, el elemento de fijación incluye también una pieza
superior que se configura básicamente de un modo similar a la parte
superior o cabeza del tornillo engranado en las vértebras
intermedias. La parte superior define una ranura para recibir el
cuerpo de un montaje de perno de anilla, e incluye un modelo de
estrías radiales. Las piezas superiores de los elementos de fijación
del implante de fusión y los tornillos para hueso se engranan a la
varilla de la columna mediante un elemento de conexión de ángulo
variable que comprende un perno de anilla, una tuerca y una arandela
estriada. El elemento de fijación proporciona una ventaja
significativa ya que adapta un conocido implante de fusión para que
parezca y se comporte como un tornillo para hueso con una mayor
rigidez y fijación.
Otro objetivo consistirá en lograr un sistema que
pueda aprovecharse de las ventajas de un dispositivo de fusión
intervertebral que sirva como un ancla para el sistema. Otros
objetivos y ventajas significativas de la presente invención se
pondrán de manifiesto al considerar la siguiente descripción por
escrito y las figuras que la acompañan.
Figura 1 es una vista del aspecto anterior de una
parte de la columna, que muestra unos dispositivos de fijación
cilíndricos de acuerdo con la presente invención representados en un
cuadriculado parcial e implantados en un cuerpo vertebral y unos
espacios intervertebrales.
Figura 2 es una vista de la parte de la columna
que se muestra en la figura 1 con unos componentes adicionales
añadidos que permiten el engranaje de los elementos de fijación a
una varilla alargada.
Figura 3 es una vista de la parte de la columna
que aparece en la figura 2 junto con una varilla vertebral que se
extiende entre los tres segmentos de movimiento vertebral.
Figura 4 es una vista en perspectiva despiezada
de un tornillo hueco y de unos elementos engranados a la varilla,
tal como se utilizan en el modelo representado en la figura 3.
Figura 5 es una vista despiezada del instrumento
configurado para la extracción de un implante de fusión cilíndrico
dispuesto entre vértebras adyacentes, como parte de una técnica de
revisión de acuerdo con la presente invención.
Figura 6 es una vista de una parte de la columna
vertebral representada en la figura 1, que muestra el uso de la
instrumentación de la figura 5 para la extracción de un tornillo
hueco.
Figura 7 es una vista en perspectiva del
instrumento de extracción con el tornillo hueco engranado al
mismo.
Con la finalidad de promover una comprensión
mayor de los principios de la invención, se hace referencia ahora a
las realizaciones ilustradas en los dibujos y se utilizará un
lenguaje específico para describirlas. Sin embargo se entenderá que
no se pretende limitar la invención, y que se contemplan futuras
aplicaciones de los principios de la invención tal como son
contempladas por un experto en la materia.
En relación con la figura 1, se visualizan los
pasos iniciales de la técnica quirúrgica contemplados por la
presente invención. En particular, la invención contempla la
fijación anterior de diversos segmentos vertebrales, identificados
como las vértebras V_{1}-V_{4} y sus discos
adyacentes D_{1}-D_{4}. Esta instrumentación
anterior podría utilizarse, por ejemplo, para corregir un estado de
escoliosis lumbar seguido de la fusión de los niveles vertebrales
afectados. Inicialmente, los aspectos laterales de los cuerpos
vertebrales se ponen de manifiesto por medio de una exposición
torácico abdominal. Puede realizarse una discectomía en cada uno de
los espacios intervertebrales D_{1}-D_{4}, ya
que estos segmentos de movimiento se eliminarán mediante los
implantes de estabilización permanentes.
Utilizando calibradores se determina la distancia
de las cortezas o cortex de los cuerpos vertebrales intermedios para
evaluar la longitud del tornillo que se colocará lateralmente en la
vértebra. En este momento, la vértebra intermedia V_{2} se
accionará con un tornillo de ángulo variable 30. En la patente
americana nº 5.261.909 del Dr. Chester Sutterlin se ha descrito con
detalle un tornillo de un ángulo variable preferido. Esta patente
909 hace referencia a la descripción del tornillo 30 de ángulo
variable y a su engranaje a la varilla vertebral. Para los fines de
esta aplicación, el tornillo de ángulo variable 30 incluye una pieza
31 que se engrana al hueso o bien una espiga roscada que se
dimensiona para cubrir la distancia entre los cortex en los lados
del cuerpo vertebral V_{2}. El tornillo 30 incluye además una
pluralidad de estrías radiales 37 que se forman para la
interdigitación con una arandela correspondiente sobre la cual se
hace referencia a continuación.
En el modelo que se muestra, solamente se
instrumentan cuatro niveles vertebrales. Como consecuencia de ello,
la mayoría de espacios intervertebrales inferior y superior D_{1}
y D_{4} se instrumentan con un tornillo hueco que se configura
para aumentar el grado de fijación a las vértebras adyacentes. En
particular, el tornillo hueco incluye una primera pieza, del tipo de
un implante cilíndrico 10. Preferiblemente, este implante 10 es un
dispositivo de fusión que permite el crecimiento interno óseo para
la fusión y fijación permanente. De acuerdo con la realización
preferida de la presente invención, este implante cilíndrico 10 es
un implante vertebral roscado tal como se ha descrito en la patente
americana nº 5.015.247 del Dr. Gary Michelson. Se contemplan otros
dispositivos de fusión como un dispositivo de fusión localizado
entre los cuerpos de dos vértebras del Dr. Kozak descrito en la
patente americana Nº 5.397.364.
Este dispositivo Kozak y otros dispositivos de
fusión pueden ser modificados para su introducción lateral en el
espacio intervertebral para su implementación en la actual técnica
quirúrgica de la invención.
Para fines de la presente invención, el implante
cilíndrico 10 define una cámara interior hueca 12 que puede
rellenarse de material de injerto óseo o bien astillas de hueso. A
través del implante se definen una pluralidad de aberturas 14 que se
comunican con el interior hueco para permitir el crecimiento interno
del tejido. Un capuchón terminal roscado encierra o incluye un
extremo abierto del implante que permite que el implante se rellene
y retenga astillas de hueso en la cámara interior hueca 12 antes de
su implantación. La superficie exterior del implante 10 incluye las
roscas 16 engranadas al hueso formadas allí, que se han configurado
para roscarse a base de tornillos a las placas terminales de las
vértebras adyacentes. El implante cilíndrico 10 tiene una superficie
de la parte posterior que se abre lateralmente cuando el implante 10
se ensarta en el espacio intervertebral D_{1}. Esta superficie
posterior 18 define una abertura roscada 20. En la patente 247 se
recogen más detalles de la construcción de este implante cilíndrico
preferido.
El implante cilíndrico 10 puede implantarse entre
los dos pares de vértebras adyacentes de la forma que se indica en
la patente 247, cuya técnica se ha descrito en las columnas
9-10 y se ha incorporado haciendo referencia a ella.
Una vez se ha dispuesto la zona en el espacio intervertebral, puede
engranarse una varilla de inserción 23 que tenga un puntal roscado
25 a la abertura roscada 20, en la superficie posterior 18 del
implante. Un elemento guía 27 rodea de forma concéntrica la varilla
de inserción, y la varilla pasa específicamente a través del
orificio 29 en el elemento guía. La varilla de inserción 23 y el
elemento guía 17 se utilizarán para guiar cada uno de los implantes
cilíndricos 10 en sus respectivos espacios intervertebrales D_{1}
y D_{4}. De nuevo, en la patente 247 se revelan más detalles sobre
la forma de implantación del implante hueco 10. Se entiende que
podrían utilizarse otros implantes intervertebrales siempre que el
implante incluyera una abertura roscada en su superficie posterior,
como la abertura roscada 20 de la superficie posterior, o alguna
equivalente a ella. Por ejemplo, el dispositivo de fusión localizado
entre los cuerpos de dos vértebras de la patente 364 incorporado
antes, puede modificarse para incluir una abertura roscada similar
en una cara lateral del dispositivo.
Ahora respecto a la figura 2, los segmentos
instrumentados se ven con los implantes huecos 10 totalmente
dispuestos en los espacios intervertebrales D_{1} y D_{4}. De un
modo similar, el tornillo de ángulo variable 30 se inserta
completamente en el cuerpo vertebral V_{2}. Preferiblemente, el
tornillo 30 se asienta con el tornillo de cabeza embutida 33 contra
la superficie lateral de la vértebra.
En los extremos de anclaje opuestos de la
estructura, en los espacios intervertebrales D_{1} y D_{4}, se
engrana una pieza superior de fijación 40 a cada implante cilíndrico
10. En particular, la pieza superior de fijación 40 incluye una base
42 desde la cual se proyecta una espiga roscada 44. La espiga
roscada 44 se adapta para engranar la abertura roscada 20 en la
superficie posterior 18 del implante cilíndrico 10. La pieza
superior de fijación 40 incluye una pieza superior 46 que es
básicamente similar a la pieza superior 33 del tornillo 30 de ángulo
variable anteriormente descrito. En particular, la pieza superior 46
incluye una ranura 47 de perno de anilla y está dotada de unas
estrías radiales 48 e una superficie de la parte superior. Cada uno
de los implantes cilíndricos recibe una pieza superior de fijación
40 correspondiente.
Preferiblemente, las piezas superiores 46 de las
piezas de fijación 40 y la pieza superior 33 del tornillo 30 de
ángulo variable se mantienen en una línea recta desde los extremos
superior hasta el inferior de los segmentos instrumentados. Con el
método quirúrgico de la presente invención, no es necesario utilizar
técnicas de colocación de tornillos o de desrotación específica para
producir una lordosis en los segmentos instrumentados. En lugar de
ello, como se ha dispuesto en los pasos siguientes del procedimiento
reflejado en la figura 3, una varilla 50 se dobla con una curvatura
C para producir la lordosis lumbar o cifosis torácica en el plano
sagital. En general, la varilla 50 se doblará para adaptarse a la
curvatura escoliótica estando la varilla en el lado cónico de la
curvatura. Una vez se ha doblado la varilla, se ajusta a cada una de
las piezas superiores 33 y 46 por medio de una arandela estriada 52
y un elemento de conexión, preferiblemente en forma de un montaje
tipo perno de anilla. Tal como se muestra en la figura 3, el montaje
de perno de anilla incluye un cuerpo 43 que se extiende tanto a
través de la arandela estriada 52 como a través de las ranuras 35 y
47 en las respectivas piezas superiores del tornillo variable 33 y
46. Estos detalles de la estructura de perno de anilla se muestran
con mayor claridad en la figura 4. También se reconoce que este
mismo montaje de fijación se ha descrito en la patente 909.
Respecto a la figura 4, se observa que la
fijación de la pieza superior 46 de la pieza superior de fijación 40
se consigue cuando la cara estriada de la pieza superior mira hacia
la varilla 50. La arandela estriada 52 se dispone entre la varilla y
la cara estriada de la pieza superior 40, más particularmente con
sus estrías correspondientes entremezcladas con las estrías 48. Como
se comenta con mayor detalle en la patente 909, las estrías permiten
que la varilla 50 se oriente en una diversidad de ángulos en el
plano sagital relativo a la pieza superior de ajuste 40. Mientras
que las figuras 1-3 reflejan los tornillos de
fijación que se disponen en paralelo para mayor claridad, las piezas
superiores 33 y 46 se disponen orientadas en distintos ángulos en el
plano frontal, dependiendo de la curvatura vertebral corregida.
El montaje del perno de anillo en sí incluye un
cuerpo de perno de anillo 53 que incluye una abertura para recibir
la varilla vertebral 50. Una parte del cuerpo 53 se proyecta a
través de la arandela estriada 52. Como prolongación del cuerpo se
dispone una espiga roscada 54 que recibe una tuerca 55. Apretando la
tuerca 55 en la espiga 54, se pinza todo el conjunto, es decir la
varilla 50, la arandela estriada 52 y la pieza superior de fijación
40. Un dispositivo de perno de apriete similar se utiliza para
engranar el tornillo de ángulo variable 30 a la varilla espinal
50.
En esta etapa del procedimiento, cada uno de los
pernos de anillo se aprieta lo suficiente como para asentar la
varilla y los tornillos de cabeza, mientras que permiten que la
varilla gire en las aberturas de los montajes. Un extremo hexagonal
51 se dispone sobre la varilla 50 de manera que la varilla puede
girar mientras se mantiene ajustada al tornillo de ángulo variable
30 y a los tornillos huecos 10. El hacer girar la varilla de esta
forma hace que la curvatura escoliótica predefinida pase al plano
sagital para producir la lordosis requerida en los segmentos
vertebrales instrumentados. Una vez la varilla se ha girado a su
posición apropiada para producir la curvatura lordótica, la
descompresión o la compresión de los segmentos puede aplicarse para
corregir además cualquier deformidad en el plano frontal. Se ha
averiguado que instrumentar los cuerpos vertebrales mediante este
método anterior da lugar a una descompresión más completa y fiable
del conducto vertebral o raquídeo que con otros métodos, como una
posterior instrumentación. Una vez se han restaurado las alturas de
disco apropiadas y se ha conseguido la curvatura apropiada en los
planos frontal y sagital, los pernos de apriete se tensan para
pinzar firmemente la varilla 50 a cada uno de los elementos de
fijación acoplados a la columna.
De acuerdo con la realización preferida, la
varilla vertebral está descentrada o desviada del eje longitudinal
del implante cilíndrico (es decir, el eje se extiende a través de la
abertura roscada 20). Preferiblemente, la varilla vertebral se
coloca por detrás de las piezas superiores a las que se acopla la
varilla. Esta colocación permite que la espiga y la tuerca del perno
de apriete den la cara por el lado anterior a la cavidad abdominal y
lejos de las regiones más sensibles. (Debería destacarse que para
mayor claridad las piezas superiores representadas en las figuras
1-3 se han mostrado giradas 180º de esta orientación
preferida).
Preferiblemente, cada uno de los implantes huecos
10 se rellena de material de injerto óseo morcelado, como un auto
injerto. Un objetivo del uso del implante cilíndrico de abertura
hueca 10 y del material de injerto óseo consiste en inducir la
fusión a través de los espacios intervertebrales. Una vez se produce
la fusión, los implantes cilíndricos 10 que proporcionan el anclaje
en los extremos opuestos de la estructura se fijarán más rígidamente
a la columna que otras estructuras como los tornillos. Se piensa que
el uso del implante de fusión 10 en los segmentos vertebrales
intermedios no es esencial, ya que las cargas no son tan duras en
las piezas intermedias como en los extremos de los segmentos
vertebrales instrumentados.
En unos modelos específicos, el tornillo de
ángulo variable 30 puede ser un tornillo esponjoso de 5,5, 6,5 ó 7,5
mm. Los implantes cilíndricos 10 pueden tener un diámetro de rosca
exterior de entre 10 mm-24 mm. Además, pueden
utilizarse varillas de un diámetro de 6,4 ó 4,8 mm (1/4 ó
3-16 pulgadas), junto con un conjunto de pernos de
anillas del tamaño apropiado. Naturalmente, se entiende que la
longitud de la parte esponjosa roscada del tornillo de ángulo
variable, así como el diámetro de los implantes cilíndricos 10 se
determina por la geometría de los segmentos vertebrales
instrumentados de forma específica. Se observa que la actual técnica
quirúrgica de la invención se puede utilizar a todos los niveles de
la columna con unos implantes de tamaño apropiado.
Se ha descubierto que incluso con la mejor
planificación preoperatoria se requiere algún tipo de revisión del
sistema de fijación. Además, el fallo en el sistema antes que se
complete la fusión quizás necesite una revisión parcial o completa
del implante. En algunos casos, la revisión comporta separar la
varilla vertebral 50 y desenroscar cada uno de los tornillos
engranados a las vértebras. Con el actual método e instrumentación
por vía anterior o lateral de la invención, se dispondrá de una
técnica de revisión y de una instrumentación más refinada.
Utilizando el sistema actual, la retirada de la varilla es
extremadamente simple y se reduce meramente a la etapa de aflojado
de los pernos de apriete fijados a cada una de las piezas superiores
en los diversos segmentos vertebrales. El tornillo de ángulo
variable 30 se libera fácilmente desenroscando el tornillo del
cuerpo vertebral V_{2}. En los extremos inferior y superior del
conjunto, las piezas superiores de fijación 40 se desatornillan de
sus respectivos implantes cilíndricos 10. Hasta este punto, el
proceso de revisión es relativamente simple. Sin embargo, las
dificultades pueden aparecer al retirar los implantes cilíndricos 10
de sus respectivos espacios intervertebrales D_{1} y D_{4}. La
retirada de los implantes es particularmente más difícil si se ha
producido el crecimiento interno del hueso entre las placas
terminales vertebrales y el hueco anterior 12 del implante.
La retirada de estos implantes de fusión 10 es
una característica importante de la técnica de revisión nueva de la
presente invención y de los instrumentos de revisión 60 asociados a
ella. Los instrumentos 60 incluyen una varilla de alineación 62 que
tiene un extremo ensanchado 63 con un puntal roscado 64 que
sobresale del mismo. El puntal roscado 64 se ha configurado para
acoplar la abertura roscada 20 a la superficie exterior 18 del
implante cilíndrico 10. En el proceso de revisión, la varilla de
alineación 62 se engrana al implante cilíndrico por medio del puntal
roscado 64 y de una abertura roscada 20.
Con la varilla de alineación 62 así colocada, se
hace avanzar una trefina 70 por la varilla. La trefina 70 incluye un
extremo de corte tubular 71 que define una pluralidad de dientes de
corte afilado 72. Preferiblemente, los dientes de corte 72 presentan
unas configuraciones conocidas para cortar el hueso cortical a
medida que el extremo de corte 71 se hace girar en la dirección
apropiada. El extremo cortante 71 define además una pluralidad de
marcas de profundidad de los anillos 74 que pueden utilizarse para
determinar la profundidad de la trefina 70 en el espacio
intervertebral. La trefina 70 incluye un anillo metálico de rotación
76 en el extremo proximal de la trefina, que se dispone
concéntricamente alrededor del cuerpo tubular del extremo cortante
71. El extremo proximal de la trefina define además un saliente guía
78 que se configura para recibir una herramienta convencional o bien
una llave de tuercas apropiada para girar el extremo cortante.
Finalmente, el extremo más próximo de la trefina 70 incluye una
pinza de varilla de alineación 80 que se utiliza para pinzar el
extremo proximal de la varilla de alineación 62 recibida
concéntricamente en la trefina.
Respecto a la figura 6, puede verse que la
varilla de alineación 62 está engranada con el implante cilíndrico
10 dispuesto en el espacio intervertebral D_{4}. La trefina 70
avanza por la varilla de alineación 62 hasta que el borde cortante
72 entra en contacto con los cuerpos vertebrales V_{3} y V_{4}.
La rotación de la trefina hace que los dientes cortantes 72 perforen
un núcleo 84 de material óseo alrededor del implante cilíndrico 10.
El núcleo 84 se puede extraer junto con el implante 10 retirando
primero la trefina 70 y luego estirando la varilla de alineación
para retirar todo el conjunto. En la figura 7, el implante
cilíndrico aparece junto a la varilla de alineación 62 a medida que
todo el instrumento de revisión 60 se extrae de la zona
quirúrgica.
Las marcas de profundidad 74 se pueden utilizar
para limitar o determinar la profundidad de la inserción de la
trefina. En su función limitante de la profundidad, la profundidad
del extremo distal del implante cilíndrico ya se conoce y se
correlaciona con la de cada una de las marcas de profundidad. Luego
se hace avanzar la trefina en la vértebra hasta que la marca de
profundidad apropiada se alinea con el cuerpo vertebral. Las marcas
74 pueden utilizarse de forma interactiva cuando la inserción de la
trefina es controlada mirando de forma indirecta. Una vez se observa
que el borde cortante 72 de la trefina ha pasado el extremo del
implante cilíndrico, puede anotarse la marca de profundidad
apropiada. Una vez extraído el implante, la marca de profundidad
anotada se utiliza luego para determinar el tamaño apropiado del
tarugo de hueso que debe extraerse de otro lugar que se insertará de
nuevo en el espacio intervertebral.
Una vez el núcleo 84 del hueso y el implante
cilíndrico 10 hayan sido retirados del espacio intervertebral, en el
agujero se insertará un tarugo ajustado exactamente. La trefina 70
puede utilizarse para ahuecar hueso del paciente, como en la cresta
iliaca, que luego puede ser reinsertado usando la misma trefina. Una
vez retirada la trefina, las vértebras adyacentes se colapsarán
alrededor del tarugo óseo recién insertado. Este tarugo óseo
mantendrá la altura del disco y permitirá de nuevo la fusión en el
punto o zona de revisión. Además, una vez se produce la fusión, la
zona del tarugo óseo se refuerza y es capaz de soportar nuevos
implantes vertebrales.
La actual técnica de revisión tiene una ventaja
significativa ya que permite el uso de implantes de fusión
cilíndrica hueca en cualquier lugar del conjunto. Se ha observado
que el uso de los implantes de fusión aumenta la rigidez y fortaleza
de la fijación. Esta misma rigidez hace que las revisiones
quirúrgicas ordinarias sean difíciles o imposibles de llevar a cabo
sin dañar seriamente lo que de otro modo sería un cuerpo vertebral
sano. El uso de la trefina 70 según la presente técnica elimina este
riesgo y permite una extracción fácil y rápida del implante
cilíndrico roscado, incluso cuando se ha producido la fusión.
Mientras que la invención se ha ilustrado y
descrito con detalle en los dibujos y en la descripción de ellos,
todo ello se debe considerar ilustrativo pero no de carácter
limitativo, entendiéndose que solamente se han descrito y mostrado
los modelos preferidos y que se protegen todos los cambios y
modificaciones que se encuentren dentro del objetivo de la
invención, tal como definen las reivindicaciones siguientes. Por
ejemplo, en el modelo preferido de la técnica de instrumentación por
vía anterior se utiliza un implante de fusión cilíndrico hueco para
soportar las piezas superiores de fijación para su engranaje a una
varilla vertebral. Sin embargo, pueden utilizarse otros implantes de
diversas configuraciones que permitan el crecimiento óseo. Además,
mientras se muestra una varilla vertebral, pueden utilizarse otros
elementos de fijación longitudinales como placas vertebrales, para
llevar a cabo esta técnica.
Claims (12)
1. Un sistema para la fijación de la columna que
comprende: un elemento alargado(50) configurado para
extenderse a lo largo de una parte de la longitud de la columna,
teniendo dicho elemento(50) un primer extremo adyacente a una
vértebra superior hacia la cabeza, y un segundo extremo adyacente a
una vértebra inferior coccígea;
Que se caracteriza porque el sistema
comprende además un implante cilíndrico (10), de unas dimensiones
para ser colocado en el espacio intervertebral(D) adyacente a
como mínimo una de dichas vértebras superior e inferior (V), de
forma que dicho implante cilíndrico defina una cámara
interior(12) para retener el material de crecimiento óseo y
una serie de aberturas(14) que comunicarán con dicha cámara
interior, de forma que dicho implante tenga una superficie posterior
(18) accesible desde fuera de dicho espacio intervertebral cuando
dicho implante cilíndrico se coloque, de forma que dicho implante
tenga además un eje longitudinal que se vaya separando de dicha
superficie posterior;
Un elemento de fijación(40,42,44)
conectado a dicha superficie posterior de dicho implante cilíndrico,
de forma que dicho elemento tenga una parte superior(40) para
la fijación a dicho elemento alargado(50) adyacente a como
mínimo una vértebra(V); y
Un elemento de
conexión(52-55) que fije rígidamente dicho
elemento de fijación a dicho elemento alargado en caso de
descentrarse el eje longitudinal de dicho implante cilíndrico.
2. El sistema de fijación de la reivindicación 1,
donde dicho implante cilíndrico (10) define una abertura
roscada(20) en dicha superficie posterior(18); y dicho
elemento de fijación(40,42,44) incluye una espiga
roscada(44) para engranarse a dicho implante cilíndrico en
dicha abertura roscada.
3. El sistema de fijación de la reivindicación 1,
donde dicha parte superior(40) y dicho elemento de
conexión(52-55) tienen unas estrías o ranuras
mutuamente interdigitadas (48) que permiten una fijación rígida de
dicho implante cilíndrico(10) a dicho elemento
alargado(50) en ángulos variables.
4. El sistema de fijación de la reivindicación 1
que comprende además, una serie de tornillos para hueso(30)
engranados en el cuerpo vertebral de las correspondientes
vértebras(V), de forma que cada tornillo tenga una pieza
superior o cabeza(52) para recibir un elemento de conexión
que fijará rígidamente el tornillo(30) a dicho elemento
alargado(50).
5. El sistema de fijación de cualquiera de las
reivindicaciones anteriores, donde dicho implante(10) tenga
una superficie exterior con una rosca como mínimo(16).
6. El sistema de fijación de cualquiera de las
reivindicaciones anteriores, que comprende además un capuchón o
tapón terminal(15), configurado para engranar dicha
superficie posterior(18) de dicho implante(10).
7. El sistema de fijación de cualquiera de las
reivindicaciones anteriores, donde dicho elemento de
prolongación(50) consiste en una varilla, cable y placa.
8. El sistema de fijación de cualquiera de las
reivindicaciones anteriores, en combinación con un instrumento de
inserción(23), para insertar dicho implante(10) en la
columna.
9. El sistema de fijación de cualquiera de las
reivindicaciones anteriores, en combinación con una
trefina(70) para crear un agujero en la columna.
10. El sistema de fijación de cualquiera de las
reivindicaciones anteriores, en combinación con al menos uno de un
injerto y un tarugo para la inserción en la columna.
11. El sistema de fijación de cualquiera de las
reivindicaciones anteriores, en combinación con un material que
promueve el crecimiento óseo.
12. El sistema para la fijación de la
reivindicación 11, donde el material que promueve el crecimiento
óseo incluye hueso.
