Abstract
Die vorliegende Erfindung beschreibt eine Wundabdeckung und ein Verfahren zur Erhöhung der NO Konzentration in einer Wunde durch eine Wundabdeckung aufweisend Fasern in die ein Anteil an partikulären Mineralien und/oder Karbiden eingebettet sind.

Die vorliegende Erfindung beschreibt eine Wundabdeckung und ein Verfahren zur Erhöhung der NO Konzentration in einer Wunde durch eine Wundabdeckung aufweisend Fasern in die ein Anteil an partikulären Mineralien und/oder Karbiden eingebettet sind.
Pflaster sind medizinische Produkte, die auf einer Wunde appliziert werden und diese verschließen. Hauptziel eines Pflasters ist das Eindringen von Schmutz und Bakterien in den Körper zu unterbinden, sowie das natürliche Wundheilungsprozess zu unterstützen.
Eine Wundabdeckung, im Volksmund auch weitläufig Pflaster genannt, besteht üblicherweise aus einem Träger der eine Wundauflage trägt, die die Wundkontaktschicht ausbildet. Der Träger wird in der Regel mit Hilfe einer aufgetragenen hautverträglichen Klebmasse auf der Körperoberfläche befestigt, wobei die Wundkontaktschicht über der abzudeckenden Wunde zu liegen kommt. Ebenso ist es möglich die Wundabdeckung nicht selbstklebend zu gestalten und durch Verbände auf/über der Wunde zu fixieren.
Die Wundkontaktschicht ist in der Regel die Stelle einer Wundabdeckung, die mit der Wunde interagiert. Sie ist der Teil (Oberflächenberiech) der Wundauflage die der Wunde zugewandt ist. Ihre Aufgabe ist zu verhindern, dass zum Beispiel Fasern aus der Wundauflage in die Wunde eindringen oder der T räger mit der Wunde verklebt. Die Wundkontaktschicht kann aus unterschiedlichen Materialien bestehen, gewöhnlich werden aber Polyethylen-, Polylaktid-, Viskose-Vliese verwendet.
Aufgrund des direkten Austausches mit dem Körper, ist es vorteilhaft, wenn die Wundkontaktschicht eine hohe Biokompatibilität aufweist, da so mögliche zusätzliche Entzündungsprozesse und allergische Reaktionen vermieden werden können, was zu einem besseren Wundheilungsprozess führt.
Es gibt zahlreiche Begriffe für Biokompatibilität. Im Rahmen dieser Erfindung ist unter Biokompatibilität zu verstehen: Die Fähigkeit eines Materials durch seine gewünschte Funktion (in diesem Fall den Wundheilungsprozess zu unterstützen) zu erfüllen, ohne unerwünschte lokale oder systemische Wirkungen hervorzurufen.
Die Biokompatibilität eines Stoffes hängt nicht nur von dessen intrinsischen Materialeigenschaften (Hydrophilie, Ladung, Mechanische Eigenschaften, usw.) ab, sondern
auch von der Umgebung (Einsatzort) und gewünschte Funktion. Eine Möglichkeit, die Biokompatibilität eines Materials zu verbessern, besteht darin dessen Oberfläche zu ändern.
Antimikrobielle Wirkstoffe sind dem Fachmann seit langem bekannt. Im Grunde genommen wirken alle Stoffe ab einer gewissen Konzentration zellschädigend und damit auch gegen Mikroben (z.B. Bakterien, Pilze), also antimikrobiell. Antimikrobielle Wrkstoffe zeichnen sich dadurch aus, dass die zellschädigende Wrkung gegenüber Mikroben höher ist als gegenüber den Zellen eines höheren Organismus (Mensch, Säugetier, Fisch, usw.).
Ein für die Wundheilung sehr interessante Substanz, die nicht nur die Wundheilung fördert, sondern auch noch antimikrobiellen Eigenschaften aufweist, ist Stickstoffmonoxid (NO).
NO ist dafür bekannt positiv zur Wundheilung über durch verschiedene Wrkmechanismen beizutragen:
NO fördert die Angiogenese: Die Blutgefäße werden deren Neubildung wird gefördert. NO wirkt Antiinflammatorisch: Das Immunsystem wird stimuliert.
NO wirkt Antimikrobiell: Zum Beispiel gegen Pseudomonas aeruginosa, Acinetobacter baumannii, Escherichia coli, Klebsiella pneumoniae und Staphylococcus aureus (auch MRSA Spezies).
Durch die Erhöhung der NO Produktion in der Wunde, wird auch die Durchblutung der Wunde erhöht, wodurch der Sauerstoff Gehalt der Wunde zunimmt und die Wundheilung effektiv beschleunigt wird (Referenz: https://doi.org/1Q.1Q16/sQQ76-6879f 05)96048-5. httos://doi . oro/10.1038/s41598- 5 und htps://doi.Org/10.1016/i.niox.2QQ9.10.0Q6)
Pflaster Systeme, die NO freisetzen und dadurch zur Wundheilung beitragen sind bereits aus WO2014188174A bekannt. Hierbei setzt das Pflastersystem durch RedOx Reaktionen gebildetes NO in die Wunde frei, wodurch es zu einem kostspieligen und komplexen Produktaufbau kommt.
Natürliche Mineralien und Carbide sind dafür bekannt elektromagnetische Strahlen zu reflektieren, zu absorbieren, zu emittieren, zum Interagieren von Wellenlängen zu führen (Verschieben, Verstärkung, Auslöschung der Wellenlängen) sowie zu polarisieren. Besonders bevorzugt sind Mineralien und Karbide in Form von Partikeln weil diese eine besonders hohe Oberflächenenergie aufweisen. Besonders interessant wirkt sich die Form und Größe der Partikel aus, da diese die Wrksamkeit effektiv beeinflussen können zum Beispiel durch den anisotropischen Einfluss der Partikelform.
Zu den bekanntesten natürlich vorkommenden Mineralien zählen, SiC, Si02, CaC2, Ti02, Zr02 sowie AI203.
Aufgabe der Erfindung ist es, eine Kostengünstige Wundabdeckung zur Verfügung zu stellen, die durch Ausnutzung von Umgebungsstrahlung, insbesondere Licht und die Körperwärme, den NO-Gehalt in der Wunde erhöhen kann.
Gelöst wird die Aufgabe durch eine Wundabdeckung aufweisend eine Trägerschicht, eine Wundauflage, die auf der Trägerschicht fixiert ist und mit ihrer freien Fläche die Wundkontaktschicht ausbildet,
- wobei die Wundauflage einen Gehalt an synthetischen Fasern aufweist,
- wobei in die synthetischen Fasern einen Anteil an partikulären Mineralien und/oder Karbiden eingebettet ist,
- wobei die Fasern zu 90 - 99 Gew.-% bezogen auf das Gesamtgewicht der synthetischen Fasern aus einem Polymer oder Mischungen von Polymeren aus der Gruppe Acrylate, Acetate, Elastane (insbes. Lycra, Spandex), Polyester, Polyamide (insbesondere Nylon), Viskose (insbesondere Rayon), PU, PE, PET, PP, Cellulose oder semi-Cellulose Materialien bestehen,
- wobei der Gehalt an Mineralien und/oder Karbiden 1 - 10 Gew.-% bezogen auf das Gesamtgewicht der synthetischen Fasern beträgt und
- wobei die maximale Partikelgröße der Mineralien und/oder Karbide kleiner als 1 ,5 mm ist und gegebenenfalls eine auf der Trägerschicht aufgetragene Klebmasse zur Fixierung der Wundauflage auf dem Träger und/oder Fixierung der Wundabdeckung auf der Haut.
Die vorliegende Erfindung bietet den Vorteil das die eigene Körperwärme genutzt wird, um die NO Produktion innerhalb der Wunde zu erhöhen.
In den Wunden wird NO üblicherweise durch Makrophagen und Fibroblasten enzymatisch produziert. Dabei kann aus der Guanidin Gruppe in Arginin in einem oxidativen Prozess NO hergestellt werden. Eine alternative zu diesem biologisch internen Prozess, ist die Herstellung von NO durch Abstrahlung. Hierfür werden Nitrit reiche Verbindungen auf der Haut photolytisch in NO umgewandelt. Dies ist auch ein natürlicher Prozess und kann durch UV-A Strahlung erzeugt werden, nichtsdestotrotz kann das Ablichten von Wunden mit UV kontraproduktiv sein, da sie oft zu Narbenbildungen führen können.
Um dieses Problem zu umgehen und gleichzeitig die positiven Eigenschaften von NO zu nutzen, können erfindungsgemäße Wundabdeckungen genutzt werden, um die Eigenproduktion des NO in der überdeckten Wunde zu fördern.
Dafür können erfindungsgemäße Wundabdeckungen genutzt werden, aufweisend einen Gehalt an Fasern, die die Absorption und Reflektion von elektromagnetischer Strahlung ermöglichen. Solche Fasern sind aus WO 2020150493 A1 bekannt. Dadurch werden die UV- A Strahlen in andere Wellenlängen umgewandelt und ersetzt durch Wellenlängen im NIR oder FIR Bereich, die die Körpereigene NO Produktion fördern.
Die Dotierung der in einer erfindungsgemäßen Wundabdeckung eingesetzten Wundauflage mit Mineralien und/oder Karbiden, insbesondere mit SiC, Si02, CaC2, Ti02, Zr02 und/oder AI203, erfolgt in einer anderen Ausgestaltung der Erfindung über eine Einbettung in die Zwischenräume der Wundauflage, in Form von einer Lackschicht die auf die Wundauflage aufgetragen wurde oder durch Einarbeitung in das jeweilige Material der Wundkontaktschicht.
Gegenstand einerweiteren Erfindung ist ein Verfahren zur Erhöhung der NO Konzentration in einer Wunde durch eine Wundabdeckung aufweisend:
Eine Trägerschicht,
Eine Wundauflage, die auf der Trägerschicht fixiert ist und mit ihrer freien Fläche die Wundkontaktschicht ausbildet,
Wobei die Wundauflage einen Gehalt an SiC, Si02, CaC2, Ti02, Zr02 und/oder AI203 aufweist und
Gegebenenfalls eine auf der T rägerschicht aufgetragene Klebmasse wobei die Wunde mit der Wundabdeckung abgedeckt wird.
Die Wundauflage in einer erfindungsgemäßen Wundabdeckung weist vorzugsweise eine spezielle Wundkontaktschicht auf, die sich Bioabbaubarkeit sowie hoher Biokompatibilität auszeichnet und/oder eine Wundkontaktschicht mit entzündungshemmenden Eigenschaften aufweist und/oder eine Wundkontaktschicht mit bakteriostatischen Eigenschaften aufweist.
Erfindungsgemäß vorteilhaft ist Trägersystem was zugleich atmungsaktiv ist, aber auch als eine semi-okklusive Barriere die feuchte Wundheilung ermöglicht.
Eine erfindungsgemäße Wundauflage die die Wundheilung fördert, verursacht durch physikalische Prozesse ein günstiges Milieu für die Wundheilung, steigert die Durchblutung des Wundumfeldes, erhöht den Sauerstoffgehalt der Wunde durch die Freisetzung oder Produktion von Stickstoffmonoxid unter Nutzung von (Körper-)Wärmestrahlung
Die NO Freisetzung innerhalb einer Wunde sorgt für eine bessere Durchblutung dieser wodurch eine schnellere Heilung begünstigt wird.
Als Trägermaterial eignen sich grundsätzlich alle starren, flexiblen und elastischen Flächengebilde aus synthetischen und natürlichen Rohstoffen.
Beispielhaft seien Textilien wie Gewebe, Gewirke, Gelege oder Vliese, weiter Netze, Folien, Schäume und Laminate aus den vorstehenden Materialien sowie Papiere aufgeführt.
Bevorzugt ist das Trägermaterial eine Folie, ein Gewebe oder ein Vlies, besonders bevorzugt mit einer Dicke von 20 bis 200 pm, insbesondere mit einem Flächengewicht von 20 bis 200
Grundsätzlich kommen sowohl natürliche als auch synthetische Trägermaterialien in Betracht. Bevorzugt ist das Trägermaterial ausgewählt aus der Gruppe bestehend aus Baumwolle, Viskose, Polypropylen, Polyestern, Polyamiden, PET, PVC, Polyethylen, Polyurethanen, Silikonen und Polymilchsäure. Das Trägermaterial ist bevorzugt ausgewählt aus der Gruppe bestehend aus Baumwolle, Viskose, Polyestern, Polyethylen und Polyurethanen. g/m2.
Besonders bevorzugt ist das Trägermaterial eine Polyurethanfolie, eine Polyethylenfolie, ein Polyester-Vlies, ein Gewebe aus Viskose, Polyester oder Baumwolle oder ein Gewebe aus beliebigen Gemischen von Viskose, Polyester und/oder Baumwolle.
Das Trägermaterial weist bevorzugt eine luft- und wasserdampfdurchlässige, aber wasserundurchlässigen Klebmasseschicht mit einer Dicke von ca. 10 bis 200 pm auf.
Bevorzugt sind Trägermaterialien, die nach Applikation der Klebmasse so eingesetzt werden können, dass sie die Anforderungen an eine funktionsgerechte Hautauflage erfüllen.
Bevorzugt können diese Materialien vor- bzw. nachbehandelt werden. Gängige Vorbehandlungen sind Plasma- bzw. Coronabehandlung und Hydrophobieren; geläufige Nachbehandlungen sind Kalandern, Tempern, Kaschieren, Stanzen, Eindecken und Sterilisieren.
Die Wundauflage kann ein Substrat zur Aufnahme von Wundexsudat umfassen.
Geeignete Materialien für Wundauflagen sind Viskosefasern, diese können aus Cellulose, Bambus und ähnliche Materialien bestehen. Weitere geeignete Polymere für die Herstellung von Wundauflagen sind, PLA (Polylactid), PHA (Polyhydroxyalkanoat) sowie PP (Polypropylen).
In den Wundauflagen können auch unterschiedliche verkapselte Wirkstoffe eingelagert werden, wobei die Wirkstoffe die Wundheilung zusätzlich fördern. Insbesondere antimikrobielle Wirkstoffe, Wundheilungsfördernde Wirkstoffe, Schmerzlindernde Wirkstoffe sind vorteilhaft.
Bevorzugt werden die in erfindungsgemäßen Wundabdeckungen eingesetzten synthetischen Fasern durch gängige Herstellungsverfahren (Extrusion, Spinnen) hergestellt, wozu das Faserhauptmaterial in geeignete ionischen Lösungsmitteln gelöst wird und diese Lösung mit Mineralien und/oder Karbiden versetzt wird. Diese entstehenden Suspensionen werden Versponnen oder Extrudiert, wobei die erfindungsgemäß einzusetzende synthetische Faser erhalten wird.
Diese synthetischen Fasern können zur erfindungsgemäßen Wundauflage als Gewebe oder Faserflor (insbesondere Vlies) verarbeitet werden.
Claims
1. Wundabdeckung aufweisend eine Trägerschicht, eine Wundauflage, die auf der Trägerschicht fixiert ist und mit ihrer freien Fläche die Wundkontaktschicht ausbildet,
- wobei die Wundauflage einen Gehalt an synthetischen Fasern aufweist,
- wobei in die synthetischen Fasern einen Anteil an partikulären Mineralien und/oder Karbiden eingebettet ist,
- wobei die Fasern zu 90 - 99 Gew.-% bezogen auf das Gesamtgewicht der synthetischen Fasern aus einem Polymer oder Mischungen von Polymeren aus der Gruppe Acrylate, Acetate, Elastane (insbes. Lycra, Spandex), Polyester, Polyamide (insbesondere Nylon), Viskose (insbesondere Rayon), PU, PE, PET, PP, Cellulose oder semi-Cellulose Materialien bestehen,
- wobei der Gehalt an Mineralien und/oder Karbiden 1 - 10 Gew.-% bezogen auf das Gesamtgewicht der synthetischenFasern beträgt und
- wobei die maximale Partikelgröße der Mineralien und/oder Karbide kleiner als 1 ,5 mm ist und gegebenenfalls eine auf der Trägerschicht aufgetragene Klebmasse zur Fixierung der Wundauflage auf dem Träger und/oder Fixierung der Wundabdeckung auf der Haut.
2. Verfahren zur Erhöhung der NO Konzentration in einer Wunde durch eine Wundabdeckung aufweisend: eine Trägerschicht, eine Wundauflage, die auf der Trägerschicht fixiert ist und mit ihrer freien Fläche die Wundkontaktschicht ausbildet,
- wobei die Wundauflage einen Gehalt an synthetischen Fasern aufweist,
- wobei in die synthetischen Fasern einen Anteil an partikulären Mineralien und/oder Karbiden eingebettet ist,
- wobei die Fasern zu 90 - 99 Gew.-% bezogen auf das Gesamtgewicht der synthetischen Fasern aus einem Polymer oder Mischungen von Polymeren aus der Gruppe Acrylate, Acetate, Elastane (insbes. Lycra, Spandex), Polyester, Polyamide (insbesondere Nylon), Viskose (insbesondere Rayon), PU, PE, PET, PP, Cellulose oder semi-Cellulose Materialien bestehen,
- wobei der Gehalt an Mineralien und/oder Karbiden 1 - 10 Gew.-% bezogen auf das Gesamtgewicht der synthetischenFasern beträgt und
- wobei die maximale Partikelgröße der Mineralien und/oder Karbide kleiner als 1 ,5 mm ist und gegebenenfalls eine auf der Trägerschicht aufgetragene Klebmasse zur Fixierung der Wundauflage auf dem Träger und/oder Fixierung der Wundabdeckung auf der Haut, wobei die Wunde mit der Wundabdeckung abgedeckt wird.
3. Wundauflage nach Anspruch 1 oder Verfahren nach Anspruch 2, dadurch gekennzeichnet, dass die Cellulose oder semi-Cellulose der synthetischen Fasern aus nachwachsenden Rohstoffen, insbesondere Bambus, Soya, Holz oder Zucker, hergestellt wurden.
4. Wundauflage nach mindestens einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die Fasern ein oder mehrere natürliche aufweisen, insbesondere Mineralien und/oder Karbide aus der Gruppe SiC, S1O2, CaC2, T1O2, ZrÖ2 sowie AI2O3.
