Definitions
- the present invention relates to a dental implant for a prosthetic restoration.
- An implant root is implanted in the jawbone of the patient.
- the implant root has a thread on the outside, which is inserted self-tapping or displacing into the bone.
- Various systems are known in the art.
- the implant neck rises above the implant root. This is located at the junction of the bone to the gums (gingiva) and usually has a smooth or slightly grooved surface.
- the implant head is arranged, which carries the dental restoration.
- the implant head which carries the dental restoration.
- implants are basically made of metal or ceramic. There are single-part and multi-part implants that are used depending on the situation. From the three-part structure described, the following conditions arise:
- the surface design may be relevant to the duration of the healing. For example, a surface roughness leads to a possibly improved attachment of osteoblasts.
- the driving force with which the implant root is screwed into the bone is important because bone qualities can vary greatly from patient to patient.
- the insertion force for the secondary part for example implant neck or implant head, must also be taken into account.
- the shape of the implant root is conical, but also cylindrically shaped implant roots are used.
- the implant neck area is characterized by the shape and the exit profile of the gingiva. After inserting the implant root is the
- the gingiva is placed over the site in which the implant was inserted, whereas in the case of the open implant
- Gingiva protrudes is arranged and healed. This is also critical
- the implant head is extremely relevant to the patient, as he is largely responsible for the appearance of the prosthetic restoration.
- requirements placed on the integration of the implant in the area of the gingiva in terms of material, color, shape and physiological compatibility.
- the exposed areas of the prosthetic restoration must be easy to clean in order to achieve the longest possible service life of the prosthetic restoration.
- connection of the two parts can not be created gap-free, which in turn is e.g. can lead to problems with oral hygiene.
- DE 20 2006 002 232 U1 discloses a hybrid implant with a screw part and a prosthetic part.
- the said parts can be constructed of different ceramic materials or plastic. Indications of advantages of the use of ceramics for the production of the screw part and prosthetic part, let alone their hardness, are not given.
- US 2005/0136378 A1 relates to an implant system for simplified use and insertion comprising an implant and a prosthetic part.
- the prosthetic part contains a so-called “abutment” and a collar-like device.
- the technical problem underlying the invention was, inter alia, to provide a dental implant that meets the requirements mentioned in the prior art and also ensures improved aesthetics over a long period of wear.
- the dental implant should enable the design of an "emergence profile" to allow for optimal oral hygiene, even with bone retardation during the implantation of the implant.
- a dental implant for a prosthetic restoration with a region I, which is designed for permanent insertion into a jawbone and a subsequent connection area II, which carries the prosthetic restoration and is characterized in that
- the connecting region II is prefabricated, comprising at least one ceramic region 3, which has a core region 3a and is constructed at least one outer region 4, the core region 3a is at least partially enclosed by the outer region 4, the at least one outer area 4 is an editable material for preparation for a prosthetic restoration,
- the ceramic of the ceramic core region 3 has a greater hardness relative to the outer region 4, and the material of the outer region 4 a natural one
- Enamel has corresponding or lower hardness.
- the implant neck mentioned at the beginning can be regarded as a region between the region I and the connection region II, wherein the beginning of the connection region II can protrude into the gingival region, but this need not be so.
- the dental implant according to the invention allows, unlike previously known dental implants, an individual design of the implant head.
- the implant head which will carry the later prosthetic restoration in an embellished form, is to be worked with the tools usually used by the dentist, without any adverse effects on the patient comes.
- dental implants consisting of very hard materials, for example zirconium oxide ceramics
- the processing of the implant head is very difficult to realize, because the processing, due to the necessary intensity, is extremely uncomfortable for the patient and, moreover, the risk due to the vibration and grinding forces the implant suffers damage in the root area, possibly leading to total loss of the implant.
- the implant according to the invention can be easily processed after insertion or ingrowth and prepared for optimal dental care.
- the region I and the connection region II of the dental implant are formed in one piece.
- connection region II has a Einschraub Bü at the upper end of the dental implant. It is known e.g. internal and external interfaces of different geometries, e.g. Hexagon, Octagon.
- the region I and the connection region II of the dental implant according to the invention are designed in several parts.
- the multi-parting allows the user a covered healing as well as additional degrees of freedom in case of any necessary angular correction of the implant abutment.
- the region I can have a device for receiving the connection region II
- the connection region II can have a device for receiving the region I.
- connection area rotatable and / or pivotable.
- Predefined modification options or freely selectable options are possible.
- a measure that allows to mechanically change the relative position of the implant and the structure for example by a detent element which provides locking devices at certain angular intervals.
- the locking element can allow a 1 degree detent. This allows twisting of the connection area II in fine gradation.
- freely selectable possibilities of change can be realized, eg by free rotation of the connection area II with fixation of the optimal position, eg by gluing.
- the transition can be discrete or fluent.
- the hardness of the ceramic of the ceramic core region is typically 1000 to 2500 HV 10, in particular 1000 to 1500 HV 10.
- the hardness of the material of the ceramic or glass outer area is typically 250 to 900 HV 1, in particular 500 to 700 HV 1. If the outside area is formed from plastics, the hardness is selected accordingly, e.g. Shore D hardness 38 to 104, in particular 67 - 91.
- the hardness is given as Shore-D hardness (DIN 53505 and DIN 7868). Concordance sites of different hardnesses are e.g. available at Wikipedia.
- the size and extent of the outer area depends in particular on the diameter of the implant and can be tailored to the size of the gap to be supplied.
- the color of the ceramic of the dental implant can be tooth-colored according to known color systems, eg according to VITA SYSTEM 3D-MASTER, VITAPAN classical.
- the outer region may extend into a region which is covered by the gingiva after insertion and healing of the dental implant.
- the ceramic of the ceramic core portion of the dental implant of the invention is made of suitable, high strength ceramic materials, e.g. Nitrides, carbides, oxides, in particular zirconium oxide, aluminum oxide or combinations thereof.
- the exterior material of the dental implant according to the invention is made of ceramic, glass, plastic or combinations thereof, e.g. Silicate ceramic, glass ceramic, glass, oxide ceramics, infiltration ceramics or combinations thereof. Particular mention should be made of so-called resin-infiltrated ceramics, as they are e.g. in WO-A-02/076907. Reference is made expressly to WO-A-02/076907.
- the outer region can be connected to the ceramic core region in a form-fitting, cloth-like or non-positive manner.
- FIGS. 1 and 2 schematically indicate the areas removed by mechanical processing. The remaining area of the outer area 4 then serves to attach the outer veneer.
- FIG. 1a shows a typical one-piece implant with an editable implant head.
- the implant root is made of a ceramic material with high hardness, in particular a Vickers hardness 1000-2500 HV 10 and protrudes into the interior of the outer area 4, as Figure 1b and 2b illustrate.
- the outer region 4 consists of a material, for example of a ceramic, with a lower Vickers hardness, which corresponds for example to the hardness or lower hardness of the natural tooth enamel.
- Typical Vickers hardnesses that are considered here are 500-700 HV 1 or when using plastics 67 - 91 Shore-D hardness.
- the outer region 4 encloses the core region 3a, in particular completely, in the region of the transition between the implant root and the implant neck.
- the outer region 4 can be connected to the ceramic core region 3a in a form-fitting, cloth-like or non-positive manner, for example by latching, gluing or friction.
- connection region II head
- a screw-in aid 15 which may be formed, for example, as a hexagon, wherein other geometries are also possible.
- the screw-in aid 16 in the form of an internal hexagon is formed in FIG. 2a at the upper end of the implant head.
- other embodiments are possible and known in the art.
- FIGS. 1c and 2c schematically show the implant head after grinding by means of conventional grinding devices, which are usually present in the dental practice, wherein the implant head was prepared as a stump for receiving a veneer.
- the gray tinted areas 6 have been removed during the grinding process.
- the stumps 4a obtained in the preparation are hatched and show an idealized representation.
- the dental technician can now create working models using known impression techniques and then build the appropriate prosthetic restoration, which is then fitted after completion.
- FIG. 3 relates to an embodiment of a two-part ceramic implant according to the present invention.
- FIG. 3a shows a ceramic dental implant which can be lowered in the bone region.
- the embodiment in FIG. 3a has an outer region 4 made of a material, in particular ceramic with the previously mentioned degrees of hardness which are the same as or lower than the natural enamel, ie about 500-700 HV 1.
- the embodiment according to FIG in particular central blind holes 5 as means for receiving the connection area II.
- the connection region II has an extension 10, which may be conical, for example.
- the blind bore 5 is formed complementary, so that the head part 2 can be positively inserted into the blind bore 5.
- a channel 20 is shown by way of example, which passes through the entire head part 2, preferably vertically in the middle. After insertion of the head part in the root part located in the bone, an adhesive can be introduced through the channel 20 and thus a cohesive connection can be established.
