Abstract
Cette bandelette comprend plusieurs traits (7, 8) de graduation perpendiculaires au premier grand côté (2) et à distance les uns des autres suivant la direction parallèle au premier grand côté, respectivement deux premier et second trous (6) de graduation étant associés à chaque trait de graduation, et deux premiers et seconds trous (9) de suture sont ménagés dans un deuxième tronçon (1) de la bandelette.
Description
Bandelette prothésique ajustable La chirurgie de l'obésité morbide dite chirurgie bariatrique est devenue une discipline à part entière de la chirurgie digestive. L'inefficacité à long terme des différentes approches thérapeutiques (diététiques, médicamenteuses, psychologiques) ainsi que l'aspect micro-invasif apporté par la technique laparoscopique en sont les causes principales. A l'heure actuelle, plusieurs procédures chirurgicales sont utilisées, agissant soit par mécanisme restrictif (gastroplastie verticale calibrée, gastroplastie horizontale calibrée ajustable), ou mixtes, c'est-à-dire associant une restriction de la capacité gastrique à un shunt digestif intestinal provoquant une malabsorption (dérivation gastrique, Inversion duodénale). Certains problèmes techniques sont communs à toutes ces procédures. L'érosion de la paroi gastrique par les différents matériaux prothétiques mis en place pour tenter de calibrer les poches gastriques créées pour provoquer un processus de restriction est l'une des préoccupations principales des chirurgiens. Cette complication n'est pas nouvelle, puisque dans les années 80, le Dr. Angelchik avait développé une prothèse en silicone placée au niveau du cardia dans le but de lutter contre le reflux gastro-œsophagien. L'efficacité était intéressante, cependant des migrations de la prothèse en position intragastrique étaient décrites dans de nombreux cas (1 1 %) amenant les chirurgiens à délaisser cette technique. D'autres matériaux comme le MARLEX (polypropylène) ont été utilisés pour le calibrage de la poche gastrique de la GVC. Le même phénomène a été noté par les chirurgiens, avec des taux de 24 % de migration intragastrique dans la série de Mac Lean 1993. L'utilisation de silicone sous forme d'anneau en « Silastic » (marque de fabrique) par Fobi pour calibrer l'intervention de dérivation gastrique au niveau de la poche gastrique, a aussi cet inconvénient. L'anneau gastrique ajustable silicone est la technique la plus utilisée aujourd'hui en Europe. La complication érosive à jusqu'à ce jour été peu prise en compte, mais des publications récentes présentent cependant des résultats inquiétants avec des taux d'érosions allant jusqu'à 10 %, ainsi que des conséquences graves pouvant aller jusqu'au décès du patient (occlusion, hémorragie).
. Les explications concernant la physiopathologie de cette complication sont diverses : • La réalisation de microtraumatismes de la paroi gastrique lors de la mise en place de l'anneau sont évoqués et doivent faire préférer une dissection à vue de la partie postérieure de l'estomac, plutôt qu'une dissection à l'aveugle pouvant provoquer une lésion non diagnostiquée. • Une mise en place trop proche de l'œsophage, qui ne présente pas de tunique séreuse a été aussi imaginée. • Une érosion chronique par les parties rigides des anneaux (notamment l'arête du treillis de dacron du SAGB) est évoquée par Zimmerman. L'invention vise une bandelette qui pallie les inconvénients ci- dessus en diminuant le risque d'érosion et qui permet au chirurgien d'effectuer un ajustage précis per-opératoire ou post-opératoire précoce. On y parvient par une bandelette souple inextensible, ayant un grand côté et une médiatrice imaginaire du premier grand côté, en une matière à résorbabilité supérieure à deux ans et pouvant être colonisée par des fibroblastes, caractérisée en ce que sur une même face et sur un premier tronçon de la bandelette se trouvant d'un premier côté de la médiatrice de la bandelette sont apposés plusieurs traits de graduation perpendiculaires au premier grand côté et à distance les uns des autres suivant la direction parallèle au premier grand côté, respectivement deux premier et second trous de graduation étant associés à chaque trait de graduation, et deux premiers et seconds trous de suture sont ménagés dans un deuxième tronçon de la bandelette se trouvant d'un deuxième côté de la médiatrice. Par trait, on entend tout repère visuel. Grâce au choix de la matière peu rigide et grâce à la possibilité d'ajuster l'anneau formé par la bandelette par les traits de graduation, on obtient une bandelette qui présente un faible risque d'érosion et qui est ajustable en per-opératoire et surtout à post-opératoire précoce, par section des fils d'attache de la bandelette, réajustage et mise en place de deux nouveaux fils de fermeture. La matière à résorbabilité supérieure à deux ans et pouvant être colonisée par des fibroblastes est de préférence du collagène d'origine
animale, notamment d'origine porcine, avec une préférence pour du collagène dermique acellulaire ayant ses fibres d'élastine. On peut se procurer une bande de collagène de ce genre sous la marque "Pelvicol" auprès de la demanderesse 164 avenue J. Kessel portik P4 78960 Voisins le Bretonneux. II s'agit d'un implant d'origine porcine constitué de collagène dermique acellulaire avec ses fibres d'élastine. C'est donc une hétérogreffe qui se présente sous une forme plate imbibée de sérum physiologique. Cet implant permet d'avoir une bonne qualité de suture et il est colonisé par les fibroblastes et revascularisé. On peut utiliser aussi une matrice collagénique extracellulaire de sous-muqueuse d'intestin de porc connue sous le nom de Stratadis et que l'on peut se procurer au laboratoire Cook Le Levant 2 rue du Nouveau Bercy 94227 Charenton. De préférence, il est apposé sur la même face de la bandelette au moins un premier trait supplémentaire de graduation sur le premier tronçon à une première distance donnée de l'un des premiers traits et parallèlement à celui-ci et, de préférence, il est apposé sur la même face de la bandelette plusieurs premiers traits supplémentaires de graduation sur le premier tronçon à la distance donnée les uns des autres et parallèlement au premier trait. On peut ainsi modifier en post-opératoire précoce le diamètre de l'anneau formé par la bandelette si cela est nécessaire. On facilite encore cette opération s'il est apposé sur la même face de la bandelette un deuxième trait supplémentaire de graduation sur le deuxième tronçon à une deuxième distance donnée de l'un des premiers traits et parallèlement à celui-ci. De préférence, les premières et les deuxièmes distances sont égales, ce qui facilite les opérations d'ajustement. Pour faciliter la pose de la bandelette, il est prévu deux trous tangents à chacun des premiers traits de graduation. Les trous peuvent avoir un diamètre de 1 à 3 mm. Ils permettent le passage d'un fil de suture pour fermer la bandelette. De préférence, chaque trou sur un premier trait de graduation est aligné parallèlement au grand côté avec un trou tangent à l'autre premier trait de graduation. De préférence, les trous sont à une distance entre eux égale à leur distance aux grands côtés. Les traits peuvent être apposés à l'encre. Par trait, on entend tout repère visible. Suivant un mode de réalisation particulièrement adapté à l'anatomie, la longueur du tronçon principal est de 5 cm et la distance entre
les traits sur un même tronçon est de 1 cm. La bandelette peut avoir une largeur comprise entre 1 ,5 et 3 cm et de préférence la largeur est de 2 cm. Elle peut avoir notamment une épaisseur de 0,5 à 5 mm. On utilise la bandelette de la manière suivante. On entoure l'extrémité distale de la poche gastrique d'une bandelette suivant l'invention que l'on serre en un anneau en passant des fils de suture dans deux trous en coïncidence. On réalise ainsi une calibration de la poche gastrique de la gastroplastie verticale calibrée ou toute intervention de chirurgie bariatrique nécessitant une calibration. Suivant une autre possibilité, on court-circuite l'estomac en reliant directement la poche gastrique à une partie de l'intestin et on place une bandelette suivant l'invention recourbée en un anneau sur cette liaison, de préférence sur la poche gastrique. Un contrôle post-opératoire vérifiant la bonne qualité du calibrage est réalisé en période post-opératoire précoce par opacification oesogastrique. Si le résultat n'est pas satisfaisant, on peut réopérer le patient et on ajuste le diamètre de la bandelette, en utilisant les traits supplémentaires de graduation encore visibles et alors que la bandelette peut encore être redétachée et réajustée. La figure unique du dessin annexé illustre l'invention. La bandelette représentée à la figure est en Pelvicol. Elle est constituée d'un deuxième tronçon 1 d'une longueur de 5 cm et d'une largeur de 2 cm. Le tronçon 1 a deux grands côtés 2, 3 et un petit côté 4. La bandelette comprend un premier tronçon 5 d'extrémité respectivement d'une largeur de 2 cm, et d'une longueur de 5 cm. Sur le tronçon 5, sont ménagés deux trous 6A, 6B d'un diamètre de 2 mm tangents à un premier trait 7 de graduation qui est apposé sur une face de la bandelette perpendiculairement aux grands côtés 2, 3. Les trous 6 sont disposés à égale distance des grands côtés 2, 3. Le trait 7 adjacent au tronçon 1 est sur la médiatrice du grand côté 2. Toujours sur la même face qui est destinée à être à l'extérieur éloignée du viscère lors de la pose, il est apposé sur le tronçon 5 quatre traits 8 supplémentaires de graduation équidistants de 1 cm les uns des autres, le premier d'entre eux étant à 1 cm du premier trait 7 de graduation, d'une épaisseur de 0,2 à 1 mm. Sur le deuxième tronçon 1 sont percés deux trous 9A, 9B de suture, les trous 9A et 6A d'une part et 9B et 6B d'autre part sont alignés respectivement suivant deux lignes imaginaires parallèles au grand côté 2.
. Grâce à ces traits 7, et trous 6, on peut adapter la dimension de la prothèse aux conditions anatomiques de dissection et à la taille de la poche gastrique qui est créée. Les complications à titre d'érosion sont moindres et le coût de l'opération est moindre.
Claims
REVENDICATIONS 1. Bandelette souple, inextensible, ayant un grand côté (2) et une médiatrice imaginaire du premier grand côté (2) , en une matière à résorbabilité supérieure à deux ans et pouvant être colonisée par des fibroblastes, caractérisée en ce que sur une même face et sur un premier tronçon (5) de la bandelette se trouvant d'un premier côté de la médiatrice de la bandelette sont apposés plusieurs traits (7, 8) de graduation perpendiculaires au premier grand côté (2) et à distance les uns des autres suivant la direction parallèle au premier grand côté, respectivement deux premier et second trous (6) de graduation étant associés à chaque trait de graduation, et deux premiers et seconds trous (9) de suture sont ménagés dans un deuxième tronçon (1) de la bandelette se trouvant d'un deuxième côté de la médiatrice.
2. Bandelette suivant la revendication 1 , caractérisée en ce que les premiers et seconds trous (6) de graduation associés à un même trait (7) de graduation sont à une même distance, le premier du premier grand côté (2) , le second du grand côté (3) opposé au premier grand côté (2).
3. Bandelette suivant l'une des revendications précédentes, caractérisée en ce que les premiers trous (6A) de graduation sont alignés suivant une direction parallèle au premier grand côté (2).
4. Bandelette suivant la revendication précédente, caractérisée en ce que les second trous (6B) de graduation sont alignés suivant une direction parallèle au premier grand côté (2).
5. Bandelette suivant l'une des revendications précédentes, caractérisée en ce que le premier trou (9A) de suture est aligné avec les premiers trous (6A) de graduation, suivant une direction parallèle au premier grand côté (2).
6. Bandelette suivant la revendication précédente, caractérisée en ce que le second trou (9A) de suture est aligné avec les seconds trous
(6B) de graduation, suivant une direction parallèle au premier grand côté (2).
7. Bandelette suivant l'une des revendications précédentes, caractérisée en ce que les traits de graduation sont équidistants.
8. Bandelette suivant l'une des revendications précédentes, caractérisée en ce que les trous de graduation sont tangents à leur trait.
9. Bandelette suivant Tune des revendications précédentes, caractérisée en ce que le premier trou de suture et le premier trou de graduation qui en est le plus proche est de 5 cm, et la distance entre eux des traits de graduation est de 1 cm.
10. Bandelette suivant l'une des revendications précédentes, caractérisée en ce qu'elle comporte au moins 3 traits.
11. Bandelette suivant l'une des revendications précédentes, caractérisée en ce que la largeur du trait est de 0,2 à 1 mm.
12. Bandelette suivant l'une des revendications précédentes, caractérisée en ce que le trait occupe sensiblement toute la largeur de la bandelette.
13. Procédé de traitement de l'obésité, dans lequel on entoure l'extrémité distale de la poche gastrique, ou une liaison entre la poche gastrique et l'intestin, d'une bandelette suivant l'une des revendications précédentes, et on serre la bandelette en un anneau en passant un fil de suture dans un trou de graduation et un trou de suture en coïncidence.
