Abstract

Die vorliegende Erfindung betrifft ein Kit aus einem wirkstoffhaltigen Pflaster zur transdermalen Abgabe eines pharmazeutischen Wirkstoffs und einem davon separierten Hautirritationen wenigstens reduzierenden Mittel.

Kit aus einem gegebenenfalls wirkstoffhaltigen Pflaster und einem Hautirritationen verhindernden Mittel
Die vorliegende Erfindung betrifft ein Kit aus einem wirkstoffhaltigen Pflaster zur transdermalen Abgabe eines pharmazeutischen Wirkstoffs und einem davon separierten Hautirritationen wenigstens reduzierenden Mittel.
Es ist bekannt, dass der intensive Kontakt von Pflastern mit der Haut, insbesondere trockener oder vorgeschädigter Haut, zu Hautrötungen, Pusteln, Juckreiz oder anderen Zeichen der Hautirritation führen kann*. Diese Hautreaktionen können bei bis zu 30% der Anwender von Pflastern in unterschiedlicher Stärke auftreten. Eine geringere Akzeptanz dieser Produkte ist die Folge, wodurch es zu einem vorzeitigen Abbruch der Therapie kommen kann.
Es stellte sich daher die Aufgabe, diesen Nachteil des Standes der Technik weitgehend zu vermeiden und damit eine größere Akzeptanz in der Anwendung von Pflastern zu erreichen.
Diese Aufgabe wurde durch die Bereitstellung eines Kits aus einem wirkstoffhaltigen Pflaster zur transdermalen Abgabe eines pharmazeutischen Wirkstoffs und einem davon separierten Hautirritationen wenigstens reduzierenden Mittel gelöst.
Das Hautirritationen wenigstens reduzierende, vorzugsweise hautverträgliche Mittel, liegt in einer auf die Haut auftragbaren Form und vom Pflaster separat verpackt vor. Dies hat den Vorteil, dass es für den Anwender des Pflasters unmittelbar vor dessen Applikation möglich ist, durch Auftragen des Mittels sofort Hautirritationen zu
BESTÄTIGUNGSKOP/E verhindern, und zwar bevor diese verursacht werden und nicht erst darauf warten zu müssen, dass, wenn überhaupt, durch Diffusion des Mittels aus Schichten des Pflasters die Hautirritationen reduziert werden.
Darüber hinaus ist es dem Anwender eines Pflasters auch möglich, wenn notwendig, während der Applikation das Mittel nachzudosieren, ohne dass die Therapie abgebrochen werden muss.
Dabei kann das Mittel gemäß Kit schon portionsweise gemäß dem Verbrauch einzeln verpackt sein oder aus einem geeigneten Behälter bei der Applikation auf die Haut portioniert werden.
Die jeweilige Portionsmenge des Mittels wird dafür vorzugsweise so gewählt, dass die Klebewirkung des Pflasters dadurch nicht oder nur geringfügig beeinflusst wird. Vorzugsweise wird das Mittel mit Hilfe eines Applikators auf die Haut aufgetragen. Dabei wird die Menge des Mittels vorzugsweise individuell nach der Größe des Pflasters bestimmt.
Das Hautirritationen wenigstens reduzierende Mittel kann in einem Behälter, wie beispielsweise in einem Druckbehälter, einem Blister, einer Ampulle, einem Beutel, einer Flasche, einer Dose, einem Tiegel, einer Kruke oder einer Tube abgefüllt sein, von wo es auf die Haut von Menschen oder Tieren aufgetragen wird.
Um das Auftragen des Mittels für den Anwender zu vereinfachen, kann der Behälter mit einem Applikator versehen sein, der Teil des Behälters, mit dem Behälter lösbar verbunden oder lose mitverpackt ist. Als Applikator eignen sich vorzugsweise gegebenenfalls sterile Spatel, Wattestäbchen, eine Tropfpipette, ein Pinsel, eine Düse mit Dosieπtentil, oder ein Roll-On Auftrags-System.
Vorzugsweise wird erfindungsgemäß als Behälter ein Druckbehälter verwendet, der mit einem Spender und mit einem bekannten, vorzugsweise umweltfreundlichen, Treibmittel ausgerüstet ist.
Das Hautirritationen wenigstens reduzierende Mittel kann in fester, flüssiger oder halbfester Form, beispielsweise als Emulsion, Mikroemulsion, Creme, Salbe, Paste, Lotion, Gel, Puder, Pulver oder Stick, vorliegen.
Bevorzugt liegt das Hautirritationen wenigstens reduzierende Mittel in flüssiger oder halbfester Form vor. Als halbfeste Formen sind Gele oder Cremen und als flüssige Formen sind Lotionen besonders bevorzugt. Dabei kann das Mittel, sei es in flüssiger oder halbfester Zubereitung, eine einzige (wässrige oder ölige) Phase umfassen oder in einer (wässrigen oder öligen) Suspension und/oder einer Emulsion vorliegen. Es kann beispielsweise in einer W/O-, einer W/O/W-, einer O/W- Emulsion vorliegen, wobei die ölige Phase aus Silikonen und/oder Silikonderivaten bestehen kann.
Das Hautirritationen wenigstens reduzierende Mittel enthält vorzugsweise eine Verbindung bzw. Substanz ausgewählt aus der Gruppe umfassend Paraffine, Silanole, Silikone, Silikon-Derivate, Mono-, Di- oder Polyalkohole, natürliche oder künstliche Lipide, natürliche oder künstliche Wachse, natürliche oder künstliche Fette, Fettsäuren und/ oder Fettalkohole, natürliche oder künstliche Öle, natürliche oder künstliche Polymere, Stärken, Proteine, Vitamine, Verbindungen mit entzündungsvorbeugenden oder antiphlogistischen Eigenschaften, Verbindungen zur Verhinderung des Wachstums von entzündungsverursachenden Mikroorganismen, Verbindungen mit anästhetisierenden Eigenschaften, Verbindungen wirksam als Radikalfänger, Enzyme, pflanzliche Extrakte, Konservierungsmittel und deren Mischungen aus wenigstens zwei Verbindungen einer Klasse oder wenigstens zwei Verbindungen unterschiedlicher Klassen. Sofern Glyzerin als hautverträgliche, insbesondere hautglättende, Verbindung eingesetzt wird, wird diese nur in Kombination mit wenigstens einer weiteren Verbindung aus einer verschiedenen Klasse, vorzugsweise wenigstens einer Verbindung der nachstehend aufgeführten, bevorzugt zum Einsatz kommenden, Hautirritationen wenigstens reduzierenden Verbindungen, verwendet.
Der Begriff „Hautirritationen wenigstens reduzierende Verbindung" umfasst erfindungsgemäß daher auch Kombinationen der aufgeführten Verbindungen, die die angestrebte Wirkung zeigen.
Als Silikon-Derivate eignen sich vorzugsweise gegebenenfalls substituierte Polysiloxane, die gegebenenfalls mit Acrylatpolymeren gemischt sind.
Als Verbindungen mit entzündungsvorbeugenden oder antiphlogistischen Eigenschaften eignen sich vorzugsweise Allantoin, Dexpanthenol, Bisabolol, Chamazulen, Aescin, bas. Aluminiumacetat-tartrat, Zinkoxid, Gerbstoffe, Melatonin, Perubalsam, Bismutgallat, deren Derivate und/oder Salze, Corticoide, wie vorzugsweise Hydrocortison, Betamethason, Fluocinolonacetonid, Fluocinonid, Prednisolon, Methylprednisolon, Triamcinolon, Flumetason, Clobetasol, Flupredniden, Alclometason, Prednicarbat, Mometason, Fluticason, Halcinonid, Clocortolon, Diflucortolon, Desoximetason und/oder deren Derivate sowie Antihistaminica, wie vorzugsweise Diphenhydramin, Dimetinden, Isoprenalin, Clemastin, Bamipin, deren Derivate und/oder Salze.
Als Verbindungen zur Verhinderung des Wachstums von entzündungsverursachenden Mikroorganismen eignen sich vorzugsweise Benzalkoniumchlorid wie z. B. Benzethoniumchlorid, Methylhydroxybenzoat, Propylhydroxybenzoat, Chlorhexidin, Dequaliniumchlorid, Clioquinol, Sorbinsäure, deren Derivate und/oder Salze, Antiseptica, wie vorzugsweise Povidon-Jod, Jodoform, Thymol, Tyrothricin, Chlorocresol, Salicylsäure, Ethacridin oder Polidocanol, deren Derivate und/oder Salze, und Antiinfektiva, wie vorzugsweise Framycetin, Neomycin, Gentamicin, Nystatin, Erythromycin, Tetracyclin, Chlortetracyclin, Oxytetracyclin, Fusidinsäure, Metronidazol, Bacitracin-Zink, Miconazol, Amphotericin B, deren Derivate und/oder Salze.
Als Verbindungen mit anästhetisierenden Eigenschaften eignen sich vorzugsweise Benzocain, Lidocain, Tetracain, Prilocain, Mepivacain, deren Derivate und/oder Salze.
Als Vitamine eignen sich vorzugsweise Vitamin A Derivate, vorzugsweise Retinol- Acetat oder -Palmitat, Vitamin B Derivate, Vitamin C Derivate, wie z. B. das entsprechende Palmitat, Vitamin D Derivate, vorzugsweise Colecaliciferon oder
Vitamin E Derivate, vorzugsweise α-Tocopherolacetat.
Als Enzyme eignen sich vorzugsweise Superoxid-Dismutase oder -Katalase. Als pflanzliche Extrakte eignen sich vorzugsweise Extrakte aus Pflanzen wie z. B. Aloe Vera, Arnika, Basilikum, Schlehe (Lat.: Prunus spinosa), große Klette (Lat.: Arctium lappa), Ringelblume (Lat.: Calendula officinalis), Kamelie (Lat.: Camellia oleifera), Muskatellersalbei (Lat.: Salvia clarea), Kamille (Lat.: Matricaria chamomilla), Beinwell (Lat.: Symphytum officinale), Sonnenhut (Lat.: Echinacea angustifolia), Gurke (Lat.: Cucumis Sativus), Augentrost (Lat.: Euphrasia officinalis), Ginseng, grüner Tee, Lavendel, Kamille (Lat.: Chamomilla recutita und Matricaria chamomilla), Pfefferminze (Lat.: Mentha piperita), Beifuß, Muskat, Hafer (Lat.: Avena sativa), Sandelbaum, Florsafran (Lat.: Carthamus tinctorius), Soja, Teebaum (Lat.: Melaleuca altemifolia), Vetivergras (Lat.: Vetiveria zizanioides), Veilchen, Süßholz (Lat.: Glycyrrhiza glabra) und/oder Zaubemuss (Lat.: Hamamelis).
Von den genannten Verbindungen eignen sich vorzugsweise solche Verbindungen, die auch eine hautglättende Eigenschaft haben. So eignen sich auch als Hautirritationen wenigstens reduzierende Verbindung wenigstens eine hautglättende Verbindung bzw. Substanz aus der Gruppe umfassend Glycerin, Chitosan, Hydroxypropylmethylcellulose, Cetearyl Octanoat, Vitamin E, Kokosfett, Erdnußöl, Sojaöl und Butyrospermum Parkii (Shea Butter), wobei hydrophile Verbindungen, wie Glyzerin, nur in Kombination mit einer der vorstehend genannten, Hautirritationen reduzierenden Verbindungen, eingesetzt werden.
Besonders bevorzugt ist wenigstens eine hautverträgliche Verbindung ausgewählt aus der Gruppe umfassend polymere Verbindungen, vorzugsweise fluorierte Polyether, besonders bevorzugt Polyperfluormethylisopropylether, oder Silikon- Derivate; Verbindungen mit entzündungsvorbeugenden oder antiphlogistischen Eigenschaften, vorzugsweise Corticoide oder Antihistaminica; Verbindungen zur Verhinderung des Wachstums von entzündungsverursachenden Mikroorganismen, vorzugsweise Antiseptica oder Antiinfektiva, und Verbindungen wirksam als Radikalfänger, vorzugsweise N-Acylethanolamin, geeignet.
Die wässrige Phase des Hautirritationen wenigstens reduzierenden Mittels kann ein Viskositätserhöhende Mittel, ausgewählt aus der Gruppe umfassend Cellulose, Cellulose-Derivate wie Methylcellulose, Hydroxymethylcellulose, Hydroxyethylcellulose, Hydroxypropylcellulose, Hydroxypropylmethylcellulose, mikrokristalliner Cellulose mit 11 Gew.-% Carboxymethylcellulose-Natrium (Avicel® RC 591), und Carboxymethylcellulose-Natrium (Blanose®, CMC-Na C300P®, Frimulsion BLC-5®, Tylose C300 P®), Polyacπylsäure (Carbopol® 980 NF, Carbopol® 981 ), Johannisbrotkernmehl (Cesagum® LA-200, Cesagum® LID/150, Cesagum® LN- 1), Pectine, vorzugsweise aus Citrusfrüchten oder Äpfeln (Cesapectin® HM Medium Rapid Set), Wachsmaisstärke (C*Gel 04201®), Natriumalginat (Frimulsion ALG (E401 )®) Guarkernmehl (Frimulsion BM®, Polygum 26/1-75®), lota-Carrageen (Frimulsion D021®), Karaya Gummi, Gellangummi (Kelcogel F®, Kelcogel LT100®), Galaktomannan (Meyprogat 150 ®), Tarakernmehl (Polygum 43/1®), Propylenglykoalginat (Protanal-Ester SD-LB®), Natrium-Hyaluronat, Tragant, Taragummi (Vidogum SP 200®), fermentiertes Polysaccharid- Welan Gum (K1A96) und Xanthane wie Xanthan-Gummi (Xantural 180®), enthalten. Die in Klammern angegebenen Bezeichnungen sind Beispiele für Handelsnamen, unter denen die jeweiligen Mittel am Markt geführt werden.
Das Hautirritationen wenigstens reduzierende Mittel kann ein Olverdickungsmittel wie hydrophobe Siliciumoxid (z. B. Aerosil®) enthalten. Das Hautirritationen wenigstens reduzierende Mittel kann einen Emulgator enthalten. Als Emulgatoren sind anionische Tenside wie Natriumdodecylsulfat, Natriumcetylstearylsulfat und Natriumdioctylsulfosuccinat, amphotere Tenside wie Lecithin, oder nichtionische Tenside, z.B. Fettalkohole und Sterole wie Cetylalkohol, Stearylalkohol, Cetylstearylalkohol und Cholesterol, Sorbitanfettsäureester wie Sorbitanmonooleat, Sorbitanmonopalmitat, Sorbitanmonostearat, Sorbitantrioleat, Sorbitantristearat und Sorbitanmonolaurat, Polyoxyethylen-Sorbitanfettsäureester wie Polyoxyethylen-20-monolaurat, Polyoxyethylen-20-monopalmitat Polyoxyethylen-20- monostearat und Polyoxyethylen-20-monooleat, Polyoxyethylen-Fettsäureglyceride wie Macrogol-1500-glyceroltriricinoleat, Macrogol-Glycerolhydroxystearat, Macrogol- 1000-glycerolmonoIaurat, Macrogol-1000-glycerolmonostearat und Macrogol-1000- glycerolmonooleat, Polyoxyethylen-Fettsäureester wie Macrogolstearat 400, Polyoxyl 40 stearate und Polyoxyl 50 stearate, Polyethylen-Fettalkoholetherwie Polyoxyl 23 laurylether, Polyoxyl 20 certostearylether und Polyoxyl 10 oleylether, Glycerolfettsäureester wie Glycerolmonostearat und makromolekulare Tenside wie Poloxamer geeignet.
Als Emulgatoren sind Siliconemulgatoren besonders geeignet.
Das Hautirritationen wenigstens reduzierende Mittel kann als Komponente ein Hautbefeuchtungsmittel, beispielsweise aus der Gruppe umfassend Glycerin, Chitosan, Fucogel, Propylenglycol, Dipropylenglycol, Butylenglycol, Mannitol, Milchsäure, Natriumpyrrolidoncarbonsäure, Hyaluronsäure, Salze der angegebenen Säuren sowie Glycin, Harnstoff und Salze von Metallen der ersten und zweiten Hauptgruppe enthalten. Besonders bevorzugt sind Glycerin, Milchsäure, Butylenglycol, Harnstoff und Hyaluronsäure.
Das Hautirritationen wenigstens reduzierende Mittel kann ein Konservierungsmittel enthalten. Als Konservierungsmittel eignen sich alle üblichen Konservierungsmittel. Es können Konservierungsmittel unter anderem aus der Gruppe umfassend Alkoholen (z.B. Chlorbutanol, Phenylethylalkohol oder Benzylalkohol), Säuren (z.B. Sorbinsäure, Benzoesäure oder Borsäure), PHB-Ester (z.B. Parabene), Phenolderivate (z.B. Phenol, Kresol oder Chlorkresol), Quartäre Verbindungen bzw. Quatemäre Ammoniumverbindungen (Quats) (z.B. Benzalkoniumchlorid), Organo- Hg-Verbindungen (z.B. Thiomersal, Phenylmercurinitrat oder Phenylmercuriborat) und Guanide (z.B. Chlorhexidin oder Chlorhexidinacetat) sein. Es können auch Mischungen von mindestens zwei Konservierungsmitteln eingesetzt werden.
Das Hautirritationen wenigstens reduzierende Mittel kann weiterhin Emulgatoren, Stabilisatoren, Antioxidationsmittel, Bakterizide, Parfüme, Antischaumverbindungen, Farbstoffe, Pigmente, Schaumstabilisatoren und/oder Elektrolyte enthalten.
Das wirkstoffhaltige erfindungsgemäße Pflaster kann nach dem Reservoir- oder Matrix- System aufgebaut sein (Bauer K. H., Frömming K.-H., Führer C, Pharmazeutische Technologie, Seiten 381-383; Müller R. H., Hildebrand G. E., Pharmazeutische Technologie: Moderne Arzneiformen, Kapitel 8).
Gemäß dem Matrix-System kann das wirkstoffhaltige Pflaster vorzugsweise eine Trägerschicht, eine wirkstoffhaltige Schicht, eine Klebeschicht und eine entfernbare Schutzfolie aufweisen. Die wirkstoffhaltige Schicht kann gleichzeitig auch die Klebeschicht sein, in dem der Wirkstoff gelöst und/oder dispergiert in einer Matrix zusammen mit dem Klebstoff vorliegt.
Vorzugsweise liegen die Klebeschicht und die wirkstoffhaltige Schicht als getrennte Schichten vor, wobei die Klebeschicht flächendeckend, partiell oder ringförmig auf der wirkstoffhaltigen Schicht aufgebracht sein kann.
Wenn das erfindungsgemäße Pflaster nach dem Reservoir-System aufgebaut ist, kann die Klebeschicht flächendeckend auf oder ringförmig um die Reservoir- Membran des Reservoir-Systems aufgebracht sein.
Sofern das erfindungsgemäße Pflaster keine separate wirkstoffhaltige Schicht aufweist, kann die Klebeschicht flächendeckend oder partiell, z. B. in Segmenten, mit und ohne wirkstoffhaltige Areale auf der Trägerschicht aufgebracht sein.
Als Klebstoffe zur Herstellung der Klebeschicht des Pflasters werden vorzugsweise druckempfindliche Klebemittel („pressure-sensitive adhesives") eingesetzt. Beispielsweise eignen sich dafür Polymere wie Polyacrylate, Polyvinylether, Polyisobutylene (PIB), Styrol/Isopren- oder Butadien-/Styrol Copolymere oder Polyisopren Kautschuke. Weiterhin eignen sich Silikon-Klebstoffe, wie z. B. gegebenenfalls vernetzte Polydimethylsiloxane. Ferner sind Harze wie z. B. Ester von Glycinen, Glycerin oder Pentaerythrol, oder Kohlenwasserstoff-Harze wie Polyterpene geeignet. Klebstoffe auf Acrylatbasis werden durch Polymerisation von Acrylaten, Methacrylaten, Alkylacrylaten und/oder Alkylmethacrylaten, mit
gegebenenfalls weiteren α, ß ungesättigten Monomeren wie Acrylamid, Dimethylacrylamid, Dimethylaminoethylacrylat, Hydroxyethylacrylat, Hydroxypropylacrylat, Methoxyethylacrylat, Methoxyethylmethacrylat, Acrylnitril und/oder Vinylacetat, hergestellt.
Die Klebeschicht kann zusätzlich Hilfsstoffe wie Weichmacher, z. B. Phthalate wie Dibutylphthalate, Mineralöle, Ester von Zitronensäure, oder Ester von Glycerin, Hautdurchdringungsverstärker, Füllstoffe (wie Zinkoxid oder Silika), Vernetzer, Konservierungsmittel und/oder lipophile Lösungsmittel enthalten.
Die Trägerschicht bzw. Deckschicht des Pflasters ist vorzugsweise für die in der wirkstoffhaltigen Schicht und in der Klebeschicht enthaltenen Stoffen, insbesonders für den gegebenenfalls vorhandenen Wirkstoff undurchlässig und inert und kann aus Polymeren wie Polyester, z. B. Polyethylenphthalat, Polyolefinen, wie Polyethylenen, Polypropylenen oder Polybutylenen, Polycarbonaten, Polyethylenoxiden, Polyterephthalaten wie Polyethylenterephthalaten, Polyurethanen, Polystyrolen, Polyamiden, Polyimiden, Polyvinylacetaten, Polyvinylchloriden und/oder Polyvinylidenchloriden, Copolymeren wie Acrylonitril/Butadien/Styrol Copolymeren enthaltend Papierfasern, Textilfasern und/oder deren Mischungen bestehen, die bei Bedarf metallisiert oder pigmentiert sein können. Die Trägerschicht kann auch aus einer Kombination aus Metallfolie und Polymerschicht bestehen. Das erfindungsgemäße Pflaster eignet sich für die transdermale Verabreichung eines jeden systemisch, d. h. transdermal wirksamen, pharmazeutischen Wirkstoff. Vorzugsweise eignet sich das erfindungsgemäße Pflaster zur transdermalen (systemischen) Abgabe von wenigstens einem pharmazeutischen Wirkstoff aus der Gruppe umfassend Analgetica, Lokalanästhetica, Hormone, Kontrazeptiva, Impfstoffe, Immunmodulatoren, Antiallergica, Antihistaminica, Cardiaca, Antihypertonica, Psychopharmaca, Antirheumatica und Enzyme. Besonders bevorzugt eignet es sich zur transdermalen Applikation von Opioiden, wie z. B. Buprenorphin, Tentanyl oder Morphin.
Die jeweilige Dosierung des transdermal zu verabreichenden Wirkstoffes ist dem Fachmann bekannt und wird u. a. von der Applikationsdauer beeinflusst.
Eine wirkstoffhaltige Matrix-Schicht des erfindungsgemäßen Pflasters bzw. Kits kann neben dem vorzugsweise pharmazeutischen Wirkstoff auch matrixbildende Polymere, Weichmacher, Permeationsförderer, lipophile Lösungsvermittler, Vernetzer, Konservierungsmittel, Emulgatoren, Konservierungsmittel, Verdickungsmittel und/oder übliche Reservoir-Membransystem- bzw. Reservoirpflaster-Hilfsmittel neben dem gegebenenfalls vorhandenen Klebstoff enthalten.
Als matrixbildende Polymere können üblicherweise filmbildende Polymere wie z. B. Hydroxypropylcellulose, Carboxymethylcellulose, Polyethylene, chlorierte Polyethylene, Polypropylene, Polyuretane, Polycarbonate, Polyacrylsäureester, Polyacrylate, Polymethacrylate, Polyvinylalkohole, Polyvinylchloride, Polyvinylidenchlor.de, Polyvinylpyrrolidone, Polyethylentherephthalate, Polytetrafluoroethylene, Ethylen/Propylen Copolymere, Ethylen/Ethylacrylat Copolymere, Ethylen/Vinylacetat Copolymere, Ethylen/Vinylalkohol Copolymere, Ethylen/Vinyloxyethanol Copolymere, Vinylchlorid/Vinylacetat Copolymere, Vinylpyrrolidon/Ethylen/Vinylacetat Copolymere, Kautschuke, gummi-artige synthetische Homo-, Co- oder Blockpolymere, Silikone, Silikon-Derivate wie Polysiloxan/Polymethacrylat Copolymere, Cellulose-Derivate wie Ethylcellulose oder Celluloseether und/oder deren Mischungen eingesetzt werden. Wenn die wirkstoffhaltige Schicht gleichzeitig die Klebeschicht ist, enthält sie vorzugsweise neben wenigstens einem der aufgezählten Polymere zumindest einen der vorstehend aufgeführten Klebstoffe.
Als lipophile Lösungsvermittler können N-methyl-2-pyrrolidon, Laurylpyrrolidon, Triethanolamin, Triacetin, Diethylenglykol-monoethylether, Derivate von Fettsäuren oder Fettalkoholen verwendet werden.
Wenn das wirkstoffhaltige Pflaster nach dem Reservoir-System aufgebaut ist, kann die Reservoir-Membran aus inerten Polymeren wie z. B. Polyethylenen, Polypropylenen, Polyvinylacetaten, Polyamiden, Ethylen/Vinylacetat Copolymeren und/oder Silikonen aufgebaut sein. Durch die Reservoir-Membran kann aus dem Reservoir eine kontrollierte Freisetzung des Wirkstoffes erzielt werden.
Die wirkstoffhaltige Matrix bzw. das wirkstoffhaltige Reservoir kann auch ein Lösungsmittel, wie z. B. Wasser, Ethanol, 1-Propanol, Isopropanol, einen niedermolekularen, mehrwertigen Alkohol, beispielsweise Propylenglycol oder Glycerin, oder einen Ester, wie Isopropylmyristat, oder deren Mischungen enthalten. Als Konservierungsmittel für wirkstoffhaltige Matrix bzw. das wirkstoffhaltige Reservoir können Antioxidantien, wie Vitamin E, Butylhydroxytoluol, Butylhydroxyanisol, Ascorbinsäure, Ascorbylpalmitat, und/oder Chelatbildner, wie Dinatriumethylendiamintetraessigsäure, Kalium- oder Natriumeitrat verwendet werden.
Die wirkstoffhaltige Matrix bzw. das wirkstoffhaltige Reservoir kann auch bekannte, übliche Permeationsförderer enthalten.
Das Pflaster kann auch in der wirkstoffhaltigen Matrix bzw. in dem wirkstoffhaltigen Reservoir Viskositätserhöhende Mittel wie z.B. Cellulose-Derivate oder natürliche oder synthetische Kautschuke enthalten. Vorzugsweise liegt der pharmazeutische Wirkstoff in dem erfindungsgemäßen Pflaster in einer Matrix-Schicht vor.
Die Wirkstofffreisetzung aus dem Pflaster ist vorzugsweise kontrolliert.
Das Pflaster des erfindungsgemäßen Kits kann in einer oder mehreren der Schichten auch wenigstens einen Weichmacher ausgewählt aus der Gruppe umfassend langkettige Alkohole wie Dodecanol, Undecanol, Octanol, Ester von Carbonsäuren mit polyethoxylierten Alkoholen, Diester von aliphatischen Dicarbonsäuren wie Adipinsäure, und mittelkettige Triglyceride von Caprylsäure und/oder Caprinsäure, Kokosfett, mehrwertige Alkohole wie z.B. 1 ,2 - Propandiol, Ester von mehrwertigen Alkoholen wie Glycerin mit Lävulinsäure oder Caprylsäure, und veretherte mehrwertige Alkohole enthalten. Die Schutzfolie kann aus Polyethylen, Polyester, Polyethylenterephthalat, Polypropylen, Polysiloxan, Polyvinylchlorid oder Polyurethan und gegebenenfalls aus behandelten Papierfasern, wie z. B. Zellophan, bestehen und gegebenenfalls eine Silikon-, Fluorsilikon- oder Fluorcarbonbeschichtung aufweisen.
Die Herstellung des Pflasters erfolgt nach den bekannten Herstellungsverfahren für Pflastern mit Verfahrensschritten wie Laminieren, Stanzen, Delaminieren, Abwickeln, Schneiden, Wiederaufwickeln, Montieren oder Dosieren (vgl. Verpackungs- Rundschau 4/2002, 83-84).
Das erfindungsgemäße Pflaster kann bei Mensch und Tier appliziert werden, wobei Hautirritationen, hervorgerufen durch eine Pflasterapplikation, weitgehend vermieden werden.
Bei der Anwendung des erfindungsgemäßen Kits soll wenn möglich zuerst die Hautstelle gereinigt (gewaschen und getrocknet) werden. Danach soll das Hautirritationen wenigstens reduzierende, hautverträgliche Mittel dünn, d. h. mit einer
Schichtdicke < 5 μin, vorzugsweise zwischen 0,5 und 2 μm, aufgetragen werden.
Nach dem Auftragen des Hautirritationen wenigstens reduzierenden, hautverträglichen Mittels wird anschließend das Pflaster durch die Ausübung von leichtem Druck auf die Haut geklebt. Beispiele
Beispiel 1
Zum Testen eines erfindungsgemäßen Kits im Vergleich zu der Anwendung nur eines Pflasters wurde folgende Untersuchung durchgeführt:
5 Testpersonen klebten jeweils ein 2 x 2 cm großes Matrix-Pflaster, das 0,5 Gew.% Capsaicin enthält, auf eine nicht behaarte Stelle des Unterarms, einmal nachdem diese Stelle mit einer Lösung eines Gemisches aus Hexamethylendisiloxan und Acrylatpolymer in Ethanol (Feststoffanteil « 2 Gew.%, Handelsname Cavilon® von
3M Health care, MN, USA) besprüht und ein Film nach dem Verdunsten flüchtiger Anteile mit einer Dicke von « 1 μm entstanden war und einmal ohne vorherigen
Auftrag eines die Hautirritation verhindernden Mittels auf die Haut.
Nach 3 Tagen wurden von den Testpersonen die Pflaster entfernt und die gegebenenfalls vorhandenen Hautirritationen nach der nachstehenden Bewertungsskala unmittelbar nach dem Entfernen bzw. nach weiteren 24 Stunden bewertet. Die Testergebnisse sind in der Tabelle 1 eingetragen. Tabelle 1

Das Testergebnis bestätigt, dass das Auftragen einer Hautirritationen verhindernden Verbindung auf die Haut vor dem Aufkleben des Pflasters diese Irritationen tatsächlich weitgehend verhindert.

Claims

Patentansprüche
1. Kit aus einem wirkstoffhaltigen Pflaster zur transdermalen Abgabe eines pharmazeutischen Wirkstoffs und einem davon separierten Hautirritationen wenigstens reduzierenden Mittel.
2. Kit nach Anspruch 1 , dadurch gekennzeichnet, dass das Mittel bereits in einer auf die Haut auftragbaren Form vorliegt.
3. Kit gemäß Anspruch 1 oder 2, dadurch gekennzeichnet, dass das Mittel portionsweise, jeweils einzeln verpackt vorliegt.
4. Kit gemäß einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass das Mittel in einem Behälter vorliegt.
5. Kit gemäß Anspruch 4 dadurch gekennzeichnet, dass das Mittel in einem Behälter mit einem Applikator vorliegt.
6. Kit gemäß Anspruch 4 oder 5, dadurch gekennzeichnet, dass der Behälter ein Druckbehälter, ein Blister, eine Ampulle, ein Beutel, eine Flasche, eine Dose, ein Tiegel, eine Kruke oder eine Tube ist.
7. Kit gemäß einem der Ansprüche 1 bis 6, dadurch gekennzeichnet, dass das Mittel in halbfester Form, vorzugsweise als Gel oder Creme vorliegt.
8. Kit gemäß einem der Ansprüche 1 bis 6, dadurch gekennzeichnet, dass das Mittel in flüssiger Form, vorzugsweise als Lotion vorliegt.
9. Kit gemäß einem der Ansprüche 1 bis 8, dadurch gekennzeichnet, dass das Mittel die Klebewirkung des Pflasters nicht oder nur geringfügig beeinflusst.
10. Kit gemäß einem der Ansprüche 1 bis 9, dadurch gekennzeichnet, dass das Mittel wenigstens eine Verbindung ausgewählt aus der Gruppe umfassend
Paraffine, Silanole, Silikone, Silikon-Derivate, Mono-, Di- oder Polyalkohole, natürliche oder künstliche Lipide, natürliche oder künstliche Wachse, natürliche oder künstliche Fette, Fettsäuren und/ oder Fettalkohole, natürliche oder künstliche Öle, natürliche oder künstliche Polymere, Stärken, Proteine, Vitamine, Verbindungen mit entzündungsvorbeugenden oder antiphlogistischen Eigenschaften, Verbindungen zur Verhinderung des Wachstums von entzündungsverursachenden Mikroorganismen, Verbindungen mit anästhetisierenden Eigenschaften, Verbindungen wirksam als Radikalfänger, Enzyme, pflanzliche Extrakte und Konservierungsmittel und deren Mischung aus wenigstens zwei Verbindungen einer Klasse oder wenigstens zwei Verbindungen unterschiedlicher Klassen
enthält.
11. Kit einem der Ansprüche 1 bis 10, dadurch gekennzeichnet, dass das Mittel wenigstens eine Verbindung ausgewählt aus der Gruppe umfassend fluorierte Polyether, Silikon-Derivate, Cortidoide, Antihistaminica, Antiseptica, Antiinfektiva und N-Acylethanolamin
enthält.
12. Kit gemäß einem der Ansprüche 1 bis 11 , dadurch gekennzeichnet, dass das Pflaster wenigstens einen Wirkstoff ausgewählt aus der Gruppe umfassend
Analgetica, vorzugsweise Opioide, Lokalanästhetica, Hormone, Kontrazeptiva, Impfstoffe, Immunmodulatoren, Antiallergica, Antihistaminica, Cardiaca, Antihypertonica, Psychopharmaca, Antirheumatica und Enzyme
enthält.
13. Kit gemäß Anspruch 12, dadurch gekennzeichnet, dass das Opioid Buprenorphin ist.
