Abstract
Ein therapeutisches System zur topischen und transmucosalen Applikation wenigstens eines Pflege- oder Wirkstoffs auf bzw. durch die Nasenschleimhaut ist dadurch gekennzeichnet, dass es der Naseninnenwand anpassbar und halbrund bis röhren- oder kegelförmig ausgebildet ist und eine den Pflege- oder Wirkstoff in Form einer homogenen Lösung oder einer Dispersion enthaltende, im Kontakt mit der Nasenschleimhaut verformbare aktive Schicht und zumindest eine weitere, die aktive Schicht aufnehmende, der Nasenschleimhaut abgewandte Tragschicht aufweist.
Description
Therapeutisches System zur topischen oder transmucosalen Applikation wenigstens eines Pflege- oder Wirkstoffs auf bzw. durch die Nasenschleimhaut sowie Verfahren zur Applikation des Systems
Die Erfindung betrifft ein therapeutisches System zur topischen oder transmucosalen Applikation wenigstens eines Pflege- oder Wirkstoffs auf bzw. durch die Nasenschleimhaut.
Weiterhin betrifft die Erfindung ein Verfahren zur Applikation des Systems.
Die Applikation von Pflege- und Wirkstoffen in die Naseninnenwand durch Sprays, Salben oder Tropfen ist bekannt. Diese Darreichungsform hat jedoch den Nachteil, daß die zu applizierenden Wirk- und Pflegestoffe mengenmäßig nicht dosierbar sind, ebensowenig wie ihre Verweilzeit auf der Nasenschleimhaut. Vielfach haften solche Stoffe nur sehr kurze Zeit auf der Nasenschleimhaut, sie tendieren infolge nicht angepaßter Viskosität zum raschen Herauslaufen aus der Nase und somit zu unkontrollierbarer Verweilzeit des Wirkstoffs auf der Nasenschleimhaut und erlauben somit keine kontrollierbare, nachhaltige Wirkung der Wirk- und Pflegestoffe.
Ausgehend hiervon liegt der Erfindung die Aufgabe zugrunde, ein therapeutisches System zur topischen oder transmucosalen Applikation wenigstens eines Pflege- oder Wirkstoffs auf bzw. durch die Nasenschleimhaut sowie ein Verfahren zur Applikation des Systems anzugeben, welches die vorgenannten Nachteile vermeidet und eine zufriedenstellende sowie kontrollierbare Wirkung der Wirk- und Pflegestoffe nach Applikation auf bzw. durch die Nasenschleimhaut ermöglicht und gegenüber bekannten buccalen Systemen zur Anwendung auf der Mundschleimhaut wesentliche Vorteile bietet.
Die Lösung der gestellten Aufgabe gelingt bei einem therapeutischen System der im Oberbegriff von Anspruch 1 genannten Art mit der Erfindung dadurch, daß es der Form der Naseninnenwand anpaßbar und halbrund bis röhren- oder kegelförmig ausgebildet ist und eine den Pflege- oder Wirkstoff in Form einer homogenen Lösung, Suspension oder Dispersion enthaltende, im Kontakt mit der Nasenschleimhaut verformbare aktive Schicht und gegebenenfalls mindestens eine weitere, die aktive Schicht aufnehmende, der Nasenschleimhaut abgewandte Tragschicht aufweist.
Die transmucosale Darreichung über die Nasenschleimhaut bietet gegenüber einer Applikation bekannter buccaler Systeme auf der Mundschleimhaut erhebliche Vorteile, insbesondere beim Tragekomfort, weil in die Mundschleimhaut geklebte Systeme dort ein unangenehmes Fremdkörpergefühl erzeugen. Dies führt dann unwillkürlich immer wieder zum Versuch, solche Systeme mit der Zunge zu entfernen. Sie stören auch bei der Aufnahme von Nahrung und Getränken. Außerdem sind sie dem Einfluß verschiedener Flüssigkeiten von Getränken, aber auch dem Speichel ausgesetzt, was zu einer Reihe unerwünschter chemischer Reaktionen führen kann. Auch ist die buccale Applikation mancher Wirkstoffe durch ihren unangenehmen bis widerwärtigen Geschmack erschwert bzw. unmöglich. Als Beispiel sei das besonders schlecht schmeckende Lobelin erwähnt. Darüber hinaus ist die nutzbare Oberfläche solcher buccaler Systeme sehr begrenzt, weil sich nur wenige Mundpartien mit einigermaßen fester oder glatter Oberfläche, z.B. die Partien unter- bzw. oberhalb der Zahnhälse, dafür eignen.
Demgegenüber ist das Fremdkörpergefühl eines Nasenschleimhautpflasters wesentlich geringer. Es ist auch nicht der Einwirkung der Zunge ausgesetzt, und die einzige Flüssigkeit, mit der es in Kontakt kommen kann, ist die Tränenflüssigkeit bzw. das Nasenschleimhautsekret. Die nutzbare
Fläche ist erheblich größer, weil die Naseninnenwände relativ fest sind.
Eine Ausgestaltung des therapeutischen Systems sieht vor, daß die aktive Schicht eine die Abgaberate des Wirkstoffs kontrollierende Zubereitung aus wenigstens einem Polymer ist. Geeignete Polymere sind dem Fachmann bekannt. Weiterhin sieht die Erfindung vor, daß die aktive Schicht derart haftklebend ausgerüstet ist, daß sie zumindest in einem ersten Zeitabschnitt nach der Applikation an der Mucosa haftet, jedoch infolge ihrer Löslichkeit in Flüssigkeit danach durch Sekretionsvorgänge der Mucosa ihre Haftung verliert.
Eine andere Variation sieht vor, daß die aktive und/oder die Tragschicht mittels Feuchtigkeit der Nasenschleimhaut in kontrollierbarer Weise aufquellbar sind und dabei infolge Verlustes ihres Zusammenhalts von der Nasenschleimhaut resorbierbar sind.
Sowohl die aktive Schicht als auch die Tragschicht können so beschaffen sein, daß durch Sekretionsvorgänge der Nasenschleimhaut im Lauf der Verweilzeit diese ihre Konsistenz kontrollierbar verlieren und dabei so weich und biegsam werden, daß sie beim Naseputzen ausgeschnaubt werden können.
Die Tragschicht für Wirk- oder Hilfsstoffe ist in einer Ausführungsform undurchlässig. Damit wird verhindert, daß Wirk- oder Hilfsstoffe unkontrolliert durch die Tragschicht hindurch diffundieren und damit unkontrollierbar verloren gehen.
Ähnlich wie bei bekannten transdermalen therapeutischen Systemen (TTS) kann bei dem therapeutischen System nach der Erfindung vorgesehen sein, daß die aktive Schicht vor der Applikation mit einer für Wirkstoff oder Hilfsstoff undurchlässigen Schutzschicht abgedeckt ist. Damit ist gewährleistet, daß das fertig präparierte therapeutische Sy-
stem zwischen Herstellung und Applikation keine stoffliche Veränderungen erfährt.
Wegen der erforderlichen Flexibilität des therapeutischen Systems zur Applikation innerhalb der Nase ist vorgesehen, daß die Tragschicht eine Folie mit einer Dicke zwischen 40 und 150 um und bevorzugt von 100 um und die aktive Schicht ein mucoadhäsives Polymergemisch mit einer Schichtdicke zwischen 100 und 400 μm, und bevorzugt von 200 um ist.
Mit Vorteil sieht eine Ausgestaltung eines therapeutischen Systems vor, daß die aktive Schicht wenigstens eine pflegende Substanz wie beispielsweise Meersalz oder Emser Salz enthält. Weiterhin kann die aktive Schicht als pflegende Substanzen wenigstens ein ätherisches Öl pflanzlichen Ursprungs enthalten.
Es kann von der Maßnahme Gebrauch gemacht sein, daß die aktive Schicht arzneilich wirksame Bestandteile enthält, wie Chlorphenaminhydrogenmaleat und/oder Phenylpropanolaminhy- drochlorid etc., sowie als Hilfsstoffe z.B. Saccharose, Maisstärke, weißen Ton, Talkum, Gelatine, hochdisperses Si- liciumdioxid, Wachs, Glycerolstearat und Farbstoffe.
Auch kann das therapeutische System mit Vorteil in der aktiven Schicht Wirkstoffe zur Abschwellung der Schleimhaut wie Oxymetazolin oder Wirkstoffe zur Bekämpfung von Allergien und/oder Schnupfen wie Menthol oder Eukalyptus enthalten.
Mit Vorteil im Vergleich zu anderen Applikationsarten kann für das therapeutische System nach der Erfindung vorgesehen sein, daß die aktive Schicht Wirkstoffe enthält, die nicht oder nicht befriedigend durch normale Haut applizierbar sind oder z.B. wie Lobelin infolge schlechten Geschmacks zur Abgabe über die Mundschleimhaut ungeeignet sind.
Ein Verfahren zur Applikation des Systems entsprechend der Erfindung unter sanftem Anpressdruck von innen gegen das System und damit gegen die Nasenschleinhaut ist gekennzeichnet durch die Verwendung eines Applikators in Form eines kleinen Stabes oder Röhrchens mit einem das System aufnehmenden und der Naseninnenformung angepaßten freien Ende. Sehr vorteilhaft ist auch die Verwendung eines am Ende aufblasbaren Applikators zur Aufweitung des Systems nach Einführen in den Nasenraum. Damit gelingt es, das therapeutische System unter Nutzung seiner elastischen bzw. plastischen Verformbarkeit praktisch vollflächig mit seiner aktiven Schicht an die Nasenwände anzupassen und mit sanftem Druck anzudrücken.
Die Erfindung ist unkompliziert und zweckmäßig und löst infolgedessen in optimaler Weise die eingangs gestellte Aufgabe.
Claims
1. Therapeutisches System zur topischen und transmucosalen Applikation wenigstens eines Pflege- oder Wirkstoffs auf bzw. durch die Nasenschleimhaut, dadurch gekennzeichnet, daß es der Naseninnenwand anpassbar und halbrund bis röhren- oder kegelförmig ausgebildet ist und eine den Pflege- oder Wirkstoff in Form einer homogenen Lösung oder einer Dispersion enthaltende, im Kontakt mit der Nasenschleimhaut verformbare aktive Schicht und zumindest eine weitere, die aktive Schicht aufnehmende, der Nasenschleimhaut abgewandte Tragschicht aufweist.
2. Therapeutisches System nach Anspruch 1, dadurch gekennzeichnet, daß die aktive Schicht eine die Abgaberate des Wirkstoffs kontrollierende Zubereitung auf Basis wenigstens eines Polymers ist.

Therapeutisches System nach Anspruch 1 oder 2, dadurch gekennzeichnet. daß die aktive Schicht derart haftklebend ausgerüstet ist, daß sie zumindest in einem ersten Zeitabschnitt nach der Applikation an der Mucosa haftet, jedoch infolge ihrer Löslichkeit in Flüssigkeit danach durch Sekretionsvorgänge der Mucosa ihre Haftung verliert.
4. Therapeutisches System nach einem oder mehreren der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß die aktive und/oder die Tragschicht mittels Feuchtigkeit der Nasenschleimhaut in kontrollierter Weise aufquellbar und dabei infolge Verlustes ihres Zusammenhalts von der Nasenschleimhaut resorbierbar ist/sind.
5. Therapeutisches System nach einem oder mehreren der Ansprüche 1 bis 4, dadurch gekennzeichnet, daß die aktive Schicht und/oder die Tragschicht durch Sekretionsvorgänge der Nasenschleimhaut im Laufe der Zeit kontrollierbar ihre
Konsistenz verlieren und dabei so weich und biegsam werden, daß sie beim Naseputzen ausgeschnaubt werden.
6. Therapeutisches System nach einem oder mehreren der Ansprüche 1 bis 5, dadurch gekennzeichnet, daß die Tragschicht für Wirk- oder Hilfsstoffe undurchlässig ist.
7. Therapeutisches System nach einem oder mehreren der Ansprüche 1 bis 6, dadurch gekennzeichnet, daß die aktive Schicht vor der Applikation mit einer für Wirkstoff undurchlässigen Schutzschicht abgedeckt ist.
8. Therapeutisches System nach einem oder mehreren der Ansprüche 1 bis 7, dadurch gekennzeichnet, daß die Tragschicht eine Folie mit einer Dicke zwischen 40 μm und
150 μm und die aktive Schicht ein mucosadhäsives Polymergemisch mit einer Schichtdicke zwischen 100 μm und 400 μm und bevorzugt von 200 μm, ist.
9. Therapeutisches System nach einem oder mehreren der Ansprüche 1 bis 8, dadurch gekennzeichnet, daß die aktive Schicht wenigstens eine pflegende Substanz wie Meersalz oder Emser Salz enthält.
10. Therapeutisches System nach einem oder mehreren der Ansprüche 1 bis 9, dadurch gekennzeichnet, daß die aktive Schicht als pflegende Substanzen wenigstens ein ätherisches Öl pflanzlichen Ursprungs enthält.

Therapeutisches System nach einem oder mehreren der Ansprüche 1 bis 10, dadurch gekennzeichnet, daß die aktive Schicht arzneilich wirksame Bestandteile enthält wie Chlor- phenaminhydrogenmaleat und/oder Phenylpropanolaminhydro- chlorid etc., sowie Hilfsstoffe wie Saccharose, Maisstärke, weißen Ton, Talkum, Gelatine, hochdisperses Siliciumdioxid, Wachs, Glycerolstearat und Farbstoffe.
12. Therapeutisches System nach einem oder mehreren der Ansprüche 1 bis 11, dadurch gekennzeichnet, daß die aktive Schicht Wirkstoffe zur Abschwellung der Schleimhaut wie Oxymetazolin oder Wirkstoffe zur Bekämpfung von Allergien und/oder Schnupfen wie Menthol oder Eukalyptus enthalten.
13. Therapeutisches System nach einem oder mehreren der Ansprüche 1 bis 12, dadurch gekennzeichnet, daß die aktive Schicht Wirkstoffe enthält, die nicht oder nicht befriedigend durch normale Haut applizierbar sind oder infolge schlechten Geschmacks zu Abgabe über die Mundschleimhaut ungeeignet sind.
14. Verfahren zur Applikation des Systems entsprechend den vorhergehenden Ansprüchen unter sanftem Anpressdruck von innen gegen das System und damit gegen die Nasenschleimhaut, gekennzeichnet durch die Verwendung eines Applikators in Form eines kleinen Stabes oder Röhrchens mit einem das System aufnehmenden und der Naseninnenformung angepaßten freien Ende.
15. Verfahren nach Anspruch 14, gekennzeichnet durch die Verwendung eines am Ende aufblasbaren Applikators zur Auf- weitung des Systems nach Einführen in den Nasenraum.
