Abstract
Description
- Die Erfindung bezieht sich auf ein Ultraschall-Gerät für Sektorabtastung,mit einem Applikatorgehäuse für ein Ultraschall-Sende-/Empfangssystem und zugehörigen Antriebs- und Steuermitteln, wobei da's Ultraschall-Sende-/Empfangssystem aus einem schwenkbaren Ultraschallkopf mit wenigstens einem Wandlerelement besteht, der um eine Schwenkachse innerhalb eines vorgebbaren Winkelbereiches periodisch schwenkbar ist.
- Das Sektor-Scan-Verfahren wird insbesondere für kardiologische Untersuchungen angewendet, da zwischen den-Rippen einerseits interkostal) und oberhalb des Schlüsselbeins andererseits (suprasternal) ein akustisches Fenster besteht, über das Ultraschall eingestrahlt werden kann. Sektor-Scanner sind in einer Vielzahl mit unterschiedlichen technischen Ausbildungen bekannt. Dabei geht die technische Entwicklung dahin, möglichst kleine und gut handhabbare Applikatoren zu entwickeln.
- Die Erzeugung eines Sektor-Scans kann mechanisch oder elektronisch erfolgen. Im ersten Fall wird ein Ultraschallkopf mit einem Empfangs/Wandler-System periodisch hin und her verschwenkt, während im zweiten Fall eine elektronische Strahlablenkung nach dem Prinzip des "phased array" den entsprechenden Scan-Bereich liefert. Bei Scannern mit mechanisch bewegtem Ultraschallkopf kann der Antrieb für das Verkippen des Ultraschallkopfes beispielsweise über einen Exzenter mit zugehöriger Schubkurbel erfolgen. Ebensogut kann der Schallkopf als Drehwalze mit mehreren am Umfang angebrachten Einzelschwingern ausgebildet sein, welche Einzelschwinger bei Rotation der Walze zyklisch aufeinanderfolgend ein- und wieder ausgeschaltet werden. Schließlich besteht auch die Möglichkeit eines elektromagnetischen Antriebes, beispielsweise nach dem Prinzip eines Drehspulinstrumentes oder auch eines Drehstrom- bzw. Wechselstrommotors. Bei letzterem Antrieb werden also über elektrodynamische Felder entsprechend steuerbare Kräfte erzeugt, die den drehbar gelagerten Ultraschallkopf beeinflussen.
- Wie bereits erwähnt, verläuft die Entwicklung bei Sektor-Scannern dahingehend, möglichst kleine und gut handhabbare Applikatoren zu schaffen. Noch nicht befriedigend ist bei den vorbekannten Applikatoren, daß durch die mechanische Bewegung des Ultraschallkopfes unvermeidbare Trägheitskräfte im Applikaor auftreten. Man hat sich teilweise bereits bemüht, derartige Trägheitskräfte durch entgegengesetzt laufende Motoren zu kompensieren. Daneben ist beim Stand der Technik unbefriedigend, daß jeder Ultraschallkopf auf eine spezifische Ultraschall-Frequenz ausgelegt ist. Bei der Notwendigkeit anderer Ultraschall-Frequenzen muß daher üblicherweise der gesamte Applikator ausgetauscht werden.
- Aufgabe der Erfindung ist es daher, ein Ultraschall-Gerät für Sektor-Abtastung zu schaffen, das applikationsfreundlich ist und für den Anwender bei der Handhabung Vorteile gegenüber den vorbekannten Sektor-Scannern bietet.
- Die Aufgabe wird erfindungsgemäß dadurch gelöst, daß mittels eines einzigen Antriebs zwei in Applikationsstellung einander gegenüberstehende Ultraschallköpfe, deren Wandlerelemente voneinander abweisen, in der Form antreibbar sind, daß bei einer Scan-Bewegung mit Ultraschall-Abstrahlung des einen Schallkopfes der andere Schallkopf eine gegenläufige Schwenkbewegung zwecks Kompensation mechanischer Kräfte aus-' führt.
- Mit der Erfindung ist also in vorteilhafter Weise einerseits eine Kompensation der bei mechanisch angetriebenen Sektor-Scannern unvermeidbar auftretenden Trägheitskräfte erreicht; andererseits weist das Ultraschall-Gerät gleichzeitig zwei Ultraschallköpfe mit unterschiedlichen Arbeitsfrequenzen auf, welche bei Bedarf leicht in die Applikationslage gebracht werden können. Z.B. kann eine Untersuchungsperson den ApplikatormLt einem Führungsteil nach Art eines Stiftes führen und so genau positionieren. Der jeweils zu applizierende Ultraschallkopf ist dabei in allen Richtungen bewegbar und bei kardiologischen Untersuchungen gut applizierbar. Die neue Raumform der Applikatorgehäuse mit Führungsteil ermöglicht durch Verschwenken des Führungsteils wahlweise eine ausgezeichnete Handhabung bei Applikation im Interkostal- oder auch im Suprasternalraum. Letzteres ist mit den Geräten des Standes der Technik nur unter Schwierigkeiten möglich.
- Die Auslegung des Bewegungssystems in zweifacher Form wird vorzugsweise durch einen elektromagnetischen Antrieb realisiert. Durch Verwendung eines Doppelankers, entsprechender Wicklungen und darauf ausgelegter Magnetelemente auf den Ultraschallköpfen kann eine komplizierte Mechanik vermieden und somit der Applikator klein gehalten werden.
- Weitere Einzelheiten und Vorteile der Erfindung ergeben sich aus der nachfolgenden Figurenbeschreibung eines Ausführungsbeispieles anhand der Zeichnung. Es zeigen:
- Fig. 1 eine Seitenansicht eines erfindungsgemäßen Gerätes,
- Fig. 2 und 3 zwei Schnitte durch die Figur 1, welche zueinander senkrecht verlaufen,und
- Fig. 4 eine perspektivische Darstellung eines Ultraschallkopfes.
- In der Figur 1 kennzeichnet 1 das Applikatorgehäuse. Dieses ist in etwa zylinderförmig ausgebildet und weist an der Ober- und Unterseite jeweils Kappen 2 und 3 als in etwa symmetrische Kugelkalottenteile auf. Die Kalottenteile bestehen aus flexiblen, aber in sich stabilem Kunststoff, beispielsweise aus Polyäthylen oder Polypropylen. Sie schließen beide jeweils einen Scanner mit zugehöriger Flüssigkeitsvorlaufstrecke ab. Das Applikatorgehäuse 1 mit den beiden Scanner-Kappen 2 und 3 ist äußerst kompakt aufgebaut. Dadurch läßt sich der Applikator z.B, zwischen den Rippen eines Patienten applizieren. Es sind somit insbesondere Sektor-Scans für Schnittdarstellungen des Herzens erzeugbar.
- Das Applikatorgehäuse 1 wird in der Mitte beidseitig von einem Führungsteil 4 umschlossen. Am Führungsteil 4 ist proximal ein Anschluß für das Applikatorgehäuse 2 mit Drehachse 5 angebracht. Distal am Führungsteil 5 befindet sich ein Kabelanschluß 6 mit zugehörigem Betriebskabel 7. Durch die spezielle Verbindung des Führungsteils 4 mit dem Applikatorgehäuse 1 ist es möglich, wahlweise einen der beiden Ultraschallköpfe stiftförmig für eine interkostale Untersuchung oder mit abgekröpften Führungsteil für eine suprasternale Untersuchung zu verwenden.
- In den Schnittdarstellungen nach den Figuren 2 und 3 bedeuten 1 bis 7 die bereits oben erläuterten Bezugszeichen. Das Gehäuseteil 1 ist im wesentlichen aus Kunststoff gebildet und nimmt einen elektromagnetischen Antrieb mit Joch, Spule und Drehanker auf. Das Gehäuse 1 ist mittels einer Lagereinrichtung beidseitig im Führungsgehäuse 4 drehbar bzw. verkippbar gehaltert, so daß das Applikatorgehäuse 1 wahlweise mit einem der Scanner in die geeignete Applikationsstellung gebracht werden kann.
- Im einzelnen bedeuten 20 und 30 zwei Ultraschallköpfe, die im Gehäuse 1 jeweils um einen vorgebbaren Winkelbereich um Drehachsen A bzw. B verschwenkbar gelagert sind. Dabei sind am Ultraschallkopf 20 bzw. 30 jeweils zur Applikationsseite hin die Ultraschallwandler 21 bzw. 31 über Dämpfungskörper (nicht eingezeichnet) angebracht. Von den Ultraschallköpfen 20 bzw. 30 werden Drehanker 22 bzw. 32 gebildet, die rückseitig Permanentmagnetschichten 23 bzw. 33 aufweisen. Derartige Permanentmagnete sind beispielsweise auf der Basis von Seltenen Erden gebildet und sind schichtartig auf die Ankerteile 22 und 32 aufbringbar. Sie sind entsprechend jeweiligem Bedarf magnetisiert, beispielsweise in umfangseitiger Aufeinanderfolge von etwa 90° mit einem Nordpol, einem Südpol und wieder einem Nordpol. Applikationsseitig sind die Ultraschallköpfe 20 und 30 von den bereits erwähnten Kappen 2 und 3 abgeschlossen. Diese bilden gewissermaßen das Ankoppelsystem mit Membranen 24 bzw. 34, wobei zwischen den Wandlern und den Membranen ultraschalleitende Flüssigkeit, z.B. Wasser, als Vorlaufstrecke eingebracht ist.
- Zwischen den beiden schwenkbar gehalterten Ultraschallköpfen 20 und 30 ist ein Elektromagnet angebracht, mit welchem gleichzeitig die Bewegung beider Ultraschallköpfe aktiviert werden kann. Im einzelnen besteht der Elektromagnet aus aufeinandergeschichteten Transformator-Blechen 40, die derart ausgestanzt sind, daß sie endseitig jeweils Polschuhe'für die von den Ultraschallköpfen rückseitig gebildeten Ankeroberflächen ausformen. Insgesamt wird so ein Joch mit Ausnehmungen 41 und 42 gebildet, durch welche die Wicklungen einer Spule 43 verlaufen.
- Mit dem beschriebenen Elektromagneten läßt sich das geforderte, ,dynamisch veränderbare Magnetfeld er- .zeugen. Entsprechend der Polung wird demzufolge der Anker derart bewegt, daß bei sich änderndem Feld jeweils eine Schwenkbewegung ausgeführt wird. Dabei kann gleichzeitig einer der beiden Wandler 21 bzw. 31 für eine Ultraschall-Abstrahlung bzw. -empfang aktiviert werden.
- Die elektrischen Verdrahtungen sowie Signalverarbeitungseinheiten sind in den vorliegenden Figuren nicht dargestellt Es ist jedoch ersichtlich, daß im Führungsteil 4 genügend Raum für die Aufnahme derartiger Einheiten vorhanden ist. Dies führt dazu, daß das eigentliche Applikatorgehäuse 1 im erwünschten Maße äußerst kompakt gehalten werden kann.
- Aus der Schnittdarstellung nach Figur 3 ist weiterhin der konstruktive Aufbau des Elektromagneten mit Joch 40 und Spule 41 ersichtlich. Besonders deutlich wird hier der Aufbau des Ultraschallkopfes 20 als Anker mit Permanentmagnet: Auf der Drehachse 26 ist ein Steg 27 angebracht. Auf diesem befindet sich die Permanentmagnetschicht 23. Das Ankerteil 22 ist an die Form der Polschuhe des Jochs 40 angepaßt und befindet sich in einem schachtartigen Ausschnitt des eigentlichen Kunststoffgehäuses. Zur Steuerung weist das Ankerteil weiterhin seitlich eine Metallfahne 28 auf. Eine entsprechende Metallfahne 29 befindet sich weiterhin an dem inwandigen Gehäuseschacht. Es wird also gewissermaßen ein Drehkondensator gebildet, der zur Positionserfassung des Drehankers und damit auch für Steuerungszwecke verwendet werden kann.
- Der zweite Ultraschallkopf 30 ist bezüglich des Ankers und des kapazitiven Meßelementes ebenso aufgebaut. Aus der perspektivischen Darstellung der Figur 4 wird die Funktion des verschwenkbaren Ultraschallkopfes mit kapazitiver Meßelement weiter deutlich. Die Bezugszeichen für Wandlerelementschwenkachse und Permanentmagnetschicht wurden oben schon erläutert. In Ruhestellung liegen die beiden Metallfahnen 28 und 29 einander gegenüber, so daß ein Kondensator gebildet wird. Bei Verschwenken des Ultraschallkopfes ist die wirksame Fläche verändert, womit jede Position des Wandlers erfaßt werden kann.
- Wie aus der Beschreibung des Ausführungsbeispiels und der Zeichnung im einzelnen ersichtlich ist, eignet sich ein elektromagnetischer Antrieb besonders für einen kompakten Geräteaufbau im erfindungsgemäßen Sinne. Davon abgesehen kann aber ein Applikatorgehäuse mit zwei einander distal gegenüberliegenden Ultraschallköpfen auch mit einem mechanischen Antrieb ausgebildet sein. Die günstigen Eigenschaften bezüglich Applikation und Handhabbarkeit können dabei erhalten bleiben. Auch ein entsprechend aufgebauter Applikator mit mechanischem Antrieb fällt also unter die Erfindung.
