Claims (13)
1. Fremgangsmåde til diagnose, forudsigelse, prognose og/eller overvågning af hyper-volemi i et subjekt, omfattende: (i) måling af mængden af MCAM i prøven fra subjektet, hvor prøven er blod, serum eller plasma; (ii) sammenligning af mængden af MCAM målt under (i) med en referenceværdi for mængden af MCAM, hvilken referenceværdi repræsenterer en euvolemisk værdi, hvor en forøget mængde af MCAM indikerer at subjektet er overfyldt eller indikerer en ringe prognose for den nævnte overfyldning af subjektet.
2. Fremgangsmåde ifølge krav 1, fortrinsvis under forløbet af en medicinsk behandling af subjektet, omfattende: (i) måling af mængden af MCAM i prøver fra subjektet fra to eller flere efter hinanden følgende tidspunkter; (ii) sammenligning af mængden af MCAM imellem prøverne som målt under (i); (iii) fastlæggelse af en afvigelse eller ingen afvigelse i mængden af MCAM imellem prøverne, som sammenlignet under (ii); (iv) tilknytning af denne fastlæggelse af afvigelse eller ingen afvigelse til en ændring i hypervolemi i subjektet imellem de to eller flere efter hinanden følgende tidspunkter.
3. Fremgangsmåde ifølge ethvert af kravene 1 til 2, hvor hypvervolemien er kendetegnet ved en vilkårlig enkelt eller flere eller alle blandt forøget vaskulær fyldningsvolumen eller-tryk i subjektet og/eller en vægtforøgelse på grund af væskeophobning i subjektet.
4. Fremgangsmåde til bestemmelse af om et subjekt har behov for eller ikke har behov for en terapi til behandling af hypervolemi, omfattende: (i) måling af mængden af MCAM i prøven fra subjektet, hvor prøven er blod, serum eller plasma; (ii) sammenligning af mængden af MCAM målt under (i) med en referenceværdi for mængden af MCAM, hvilken referenceværdi repræsenterer en euvolemisk værdi, hvor en forøget mængde af MCAM indikerer, at subjektet har behov for en terapi til behandling af hypervolemi.
5. Fremgangsmåde ifølge krav 4, hvor terapien omfatter indgivelse af exogene og/eller endogene diuretika og/eller ultrafiltrering for fjernelse af salte og tilsvarende fluid fra kredsløbet.
6. Fremgangsmåde ifølge ethvert af kravene 1 til 5, hvor subjektet modtager eller har modtaget en antidiabetesbehandling, fortrinsvis en eller flere insulinsensitizere, mere foretrukkent en eller flere agonister af peroxisome proliferatoraktiverede receptor gamma (PPAR-gamma), endnu mere foretrukkent en eller flere thiazolidinedioner (glita-zoner), såsom rosiglitazon og/eller pioglitazon.
7. Fremgangsmåde ifølge ethvert af kravene 1 til 6, hvor hypervolemien er forårsaget af systolisk dysfunktion.
8. Fremgangsmåde ifølge krav 7, hvor den systoliske dysfunktion er kendetegnet ved en reduceret venstre ventrikulær ejektionsfraktion (LVEF), fortrinsvis hvor den nævnte LVEF er mindre end 55% eller mindre end 50% eller mindre end 45%, og/eller ved forøget hjertefyldningstryk.
9. Fremgangsmåde ifølge ethvert af kravene 1 til 8, omfattende måling af tilstedeværelsen eller fraværet og/eller mængden af en eller flere yderligere biomarkører, som er nyttige til diagnose, forudsigelse, prognose og/eller overvågning af hypervolemi i prøven fra subjektet.
10. Fremgangsmåde ifølge krav 9, hvor den yderligere biomarkør er valgt fra gruppen bestående af atrial natriuretisk peptid (ANP), proANP, midter-regional del af pro-ANP (MR-proANP), B-type natriuretisk peptid (BNP), pro-B-type natriuretisk peptid (proBNP), amino terminal pro-B-type natriuretisk peptid (NTproBNP), Cystatin C, NGAL, Albumin, γ-GT, NAG, A1M, B1M, creatinin, ureum, vasopressin, copeptin, aldosteron, angiotensin, ACE og fragmenter eller forløbere for enhver af disse.
11. Fremgangsmåde ifølge ethvert af kravene 1 til 10, hvor mængden af MCAM og/eller tilstedeværelsen eller fraværet og/eller mængden af en eller flere yderligere biomarkører måles under anvendelse af henholdsvis et bindemiddel, som er i stand til specifikt at binde til MCAM og/eller til fragmenter deraf, og et bindemiddel, som er i stand til specifikt at binde til den ene eller flere yderligere biomarkører.
12. Fremgangsmåde ifølge ethvert af kravene 1 til 11, hvor mængden af MCAM og/eller tilstedeværelsen eller fraværet og/eller mængden af den ene eller flere yderligere biomarkører måles under anvendelse af en immunoassay teknologi, eller under anvendelse af en massespektrometri-analysefremgangsmåde eller under anvendelse af en kromatografifremgangsmåde, eller under anvendelse af en kombination af disse fremgangsmåder.
13. Fremgangsmåde ifølge ethvert af kravene 1 til 12, hvor prøven er plasma, og hvor den plasmacirkulerende form af MCAM detekteres.
