Description

  • Die vorliegende Erfindung betrifft eine Vorrichtung zum Unterdrucksetzen, insbesondere zur Anwendung bei beim Ersatz des Hüftgelenk-Femoralschaftes. Die Vorrichtung zum Unterdrucksetzen ist auch für andere Prothesenanwendungen mit Zementierung anwendbar.
  • Ein Femoralknochen wird für das Einsetzen einer Hüftgelenlschaft-Prothese vorbereitet, indem der Innenhohlraum des Femoralknochens unter Verwendung eines Räumwerkzeuges oder einer Raspel aufgeweitet wird. Der geräumte Hohlraum wird entweder vor oder nach der Einführung der Hüftgelenlschaft-Prothese mit Knochenzement gefüllt.
  • Bei dem Verfahren, bei welchem die Hüftgelenlschaft-Prothese nach dem Einfüllen von Zement in den geräumten Hohlraum eingeführt wird, ist es wichtig, den Schaft im vorbereiten Femoralhohlraum zu zentrieren und den Druck des die Hüftgelenlschaft-Prothese umgebenden Zements aufrecht zu erhalten. Die Zentrierung der Hüftgelenlschaft-Prothese sowie das Unterdrucksetzen des Zements haben in der Hüftgelenkersatz-Chirurgie zu einer langen Lebensdauer des Schaftes geführt.
  • Die Veröffentlichungen EP-A-0 393 425, WO-A-96/38104 und DE-A-195 18 391 beschreiben jeweils Vorrichtungen zum Unterdrucksetzen unter Verwendung eines elastischen Materials zum Abdichten des Knochenhohlraumes.
  • Entsprechend der vorliegenden Erfindung ist eine Vorrichtung zum Unterdrucksetzen in Form einer Manschette vorgesehen, welche eine Öffnung zur Aufnahme eines Prothesenteiles umschließt, wobei die Manschette von einem sich in radialer Richtung erstreckenden Flansch umgeben ist. Der Flansch kann über der Außenfläche des Knochens sowie rund um den Knochen, in welchen das Prothesenteil eingeführt wird, niedergedrückt werden.
  • Vorzugsweise erstreckt sich der flexible Flansch in radialer Richtung von der hohlen Manschette aus derart, daß die Manschette ein Oberteil über dem flexiblen Flansch und ein Unterteil unter dem flexiblen Flansch aufweist.
  • Das Oberteil und das Unterteil können allgemein U-förmige Teile sein. Vorzugsweise ist das Innenprofil des Oberteiles derart ausgebildet, daß es dem Prothesenteil entspricht.
  • Vorzugsweise weisen das Oberteil und das Unterteil Dichtlippen auf, in welche der flexible Flansch dicht eingefügt ist.
  • Das Außenprofil des Unterteiles kann derart geformt sein, daß es einem Knochenhohlraum entspricht, in welchen das Prothesenteil eingesetzt werden soll.
  • Das Oberteil kann wahlweise eine an das U-förmige Element angrenzende Schulter aufweisen, welche beim Gebrauch auf dem Eingang zum Knochenhohlraum, in welchem das Prothesenteil (10) anzuordnen ist, aufliegt.
  • Vorzugsweise ist der flexible Flansch durchsichtig, um Sicht bei der Positionierung zu ermöglichen. Vorzugsweise ist das Prothesenteil eine Hüftgelenkschaft-Prothese und der Knochenhohlraum ist ein Femoralkanal.
  • Vorzugsweise sind das Oberteil und das Unterteil aus Silikongummi geformt. Die Dichtlippen des Oberteiles und des Unterteiles sind etwa 3 Millimeter dick.
  • Vorzugsweise ist der flexible Flansch zwischen 0,5 Millimeter und 1 Millimeter dick. Vorzugsweise hat der flexible Flansch einen Durchmesser von etwa 10 Zentimeter.
  • Die Vorrichtung zum Unterdrucksetzen kann in verschiedenen Größen geliefert werden, um sie an den Knochenhohlraum anzupassen.
  • Eine Ausführungsform der Vorrichtung zum Unterdrucksetzen entsprechend der vorliegenden Erfindung soll nun ausschließlich beispielhaft unter Bezugnahme auf die beigefügten Zeichnungen beschrieben werden, wobei diese darstellen:
  • 1 ist eine Schnittdarstellung der Vorrichtung zum Unterdrucksetzen entsprechend der vorliegenden Erfindung in situ zwischen einer Hüftgelenkschaft-Prothese und einem Femoralkanal.
  • 2 ist eine perspektivische Ansicht der Oberseite der Vorrichtung zum Unterdrucksetzen entsprechend der vorliegenden Erfindung.
  • 3 ist eine perspektivische Ansicht der Unterseite der Vorrichtung zum Unterdrucksetzen von 2.
  • Die 4a, 4b und 4c sind eine Draufsicht, eine Seitenansicht und eine Unteransicht der Vorrichtung zum Unterdrucksetzen von Zement von 2.
  • Die 5a, 5b und 5c sind eine Draufsicht, eine Seitenansicht und eine Unteransicht der Vorrichtung zum Unterdrucksetzen von Zement von 2.
  • Bezug nehmend auf die Zeichnungen ist eine Vorrichtung 1 zum Unterdrucksetzen mit einer Oberseite 2 und einer Unterseite 3 vorgesehen. Die Vorrichtung 1 zum Unterdrucksetzen hat eine Manschette in Gestalt eines Körpers 6 sowie einen dieses radial umgebenden flexiblen Flansch 8.
  • Die vorliegende Erfindung soll nun im Zusammenhang mit einer Hüftgelenkschaft-Prothese zum Einsetzen in einen Femoralkanal mittels eines Zementmantels beschrieben werden. Die Vorrichtung 1 zum Unterdrucksetzen ist jedoch für jede Form zementierter Prothesenteile anwendbar.
  • 1 zeigt die Position der Vorrichtung 1 zum Unterdrucksetzen an der Öffnung eines Femoralhohlraumes 14. Der umgebende Femur 12 ist geräumt worden, um den Hohlraum 14 in demselben zu vergrößern. Der Hohlraum 14 wird vor dem Einsetzen eines Femoralschaftes 10 mit Zement gefüllt. Die Vorrichtung 1 zum Unterdrucksetzen sitzt auf der Femurlippe 18 am Eingang des Hohlraumes 14 auf. Der Hüftgelenkschaft 10 wird durch eine zentrale Öffnung 50 im Teil 6 der Vorrichtung 1 zum Unterdrucksetzen eingeführt.
  • Die Vorrichtung 1 zum Unterdrucksetzen hat ein Oberteil 20 des Teiles 6, wobei das Oberteil 20 ein allgemein U-förmiges Teil 21 aufweist. Das U-förmige Teil 21 hat eine Oberseite 22 mit Stützwänden 24. Die Wände 24 sind im allgemeinen senkrecht mit Ausnahme der offenen Enden 54 des U-förmigen Teiles 21, die zu ihrer Basis hin unter einem Winkel zur Oberseite 22 verlaufen. Das Oberteil 20 weist auch eine Dichtlippe 28 auf. welche das U-förmige Teil 21 umgibt, wobei die Dichtlippe 28 am gebogenen Teil des U-förmigen Teiles 21 eine größere Breite hat als an den offenen Enden 54.
  • Die Wände 24 und die ebene Oberseite 22 des U-förmigen Teiles 21 sind durch eine geneigte Fläche 26 verbunden.
  • Die Innenwände 44 des Oberteiles 20 des Teiles 6 haben ein Profil, das entsprechend des Außenform des Hüftgelenkschaftes 10 gekrümmt ist. Die Innenwände 44 begrenzen die zentrale Öffnung 50 im Teil 6.
  • Der Flansch 8 erstreckt sich von der Dichtlippe 28 in radialer Richtung, um rund um das Teil 6 einen flexiblen Fortsatz zu bilden.
  • Das Teil 6 hat ein Unterteil 30 mit einem U-förmigen Teil 31 mit dünneren Abmessungen als das U-förmige Teil 21 des Oberteiles 20.
  • Das U-förmige Teil 31 des Unterteiles 30 ist von einer Außenlippe 38 mit einer gekrümmten Lippe 36, welche die Außenlippe 38 mit dem Grundkörper des U-förmigen Teiles 31 verbindet, umgeben.
  • Das U-förmige Teil 31 des Unterteiles 30 hat eine ebene Oberseite 32 mit Stützwänden 34. Die Wände 34 sind allgemein vertikal. Das gebogene Ende 46 des U-förmigen Teiles 31 hat eine schräge Oberseite 48, die unter einem Winkel von der ebenen Oberseite 32 zur umgebenden Außenlippe 38 ansteigt.
  • Die Schenkel 40 des U-förmigen Teiles 31 haben freie Enden 41 mit abgeschnittenen Bereichen 42, so daß die freien Enden 41 in der vom Oberteil 20 definierten U-Form abrupt bzw. vertikal abfallen.
  • Das Außenprofil des unteren Teiles 30 entspricht der Form des geräumten Hohlraumes 14 im Knochen. Die umgebende Außenlippe 38 bildet eine Schulter, welche auf der Öffnung 18 des Knochenhohlraumes 14 aufliegt.
  • Der Flansch 8 ist am Körper 6 zwischen der Außenlippe 38 des Unterteiles 30 und der oberen Dichtlippe 28 des Oberteiles 20 des Teiles 6 angebracht oder angeformt. Der Flansch 8 ist durchsichtig und gibt freie Sicht zur Positionierung der Vorrichtung 1 zum Unterdrucksetzen.
  • Der Flansch 8 hat einen Durchmesser von etwa 10 Zentimeter und ist 0,5 mm bis 1 mm dick. Die Dichtlippe 28 und die Außenlippe 38 sind etwa 3 mm dick.
  • Das Teil 6 ist bei diesem Beispiel aus Silikongummi geformt und wird in sechs Größen entsprechend den unterschiedlichen Größen des Femoralknochen bereit gehalten.
  • Bei der Anwendung wird die Vorrichtung 1 zum Unterdrucksetzen am Eingang 18 zu einem zementgefüllten Femoralhohlraum 14 positioniert, und der Hüftgelenkschaft 10 wird durch die zentrale Öffnung 50 im Teil 6 der Vorrichtung 1 zum Unterdrucksetzen eingeführt. Der Hüftgelenkschaft 10 wird eingeführt, bis er in der Zentralöffnung 50 richtig positioniert ist. Der durchsichtige flexible Flansch 8 hält den Druck im Zement im Hohlraum 14 aufrecht. Der Flansch 8 wird durch die Hand des Chirurgen rund um den Femur 12 niedergedrückt, wenn der Hüftgelenkschaft 10 eingeführt wird.
  • Abwandlungen und Verbesserungen können an der vorgenannten Ausführungsform vorgenommen werden, ohne den Schutzumfang der vorliegenden Erfindung zu verlassen.

Claims (14)

  1. Vorrichtung (1) zum Unterdrucksetzen in Form einer Manschette, welche eine Öffnung zur Aufnahme eines Prothesenteiles (10) zum Einsetzen in einen Knochenhohlraum (14) umschließt, wobei die Manschette von einem sich in radialer Richtung erstreckenden Flansch (8) umgeben ist, dadurch gekennzeichnet, daß der Flansch (8) aus flexiblem Material hergestellt ist, so daß er rund um den Knochen, in welchen das Prothesenteil eingeführt wird, niedergedrückt werden kann.
  2. Vorrichtung zum Unterdrucksetzen nach Anspruch 1, bei welcher sich der flexible Flansch (8) in radialer Richtung von der hohlen Manschette aus erstreckt derart, daß die Manschette ein Oberteil (20) über dem flexiblen Flansch und ein Unterteil (30) unter dem flexiblen Flansch aufweist.
  3. Vorrichtung zum Unterdrucksetzen nach Anspruch 2, bei welcher das Oberteil (20) und das Unterteil (30) allgemein U-förmige Teile (21, 31) sind.
  4. Vorrichtung zum Unterdrucksetzen nach Anspruch 2 oder 3, bei welcher das Innenprofil des Oberteiles (20) derart ausgebildet ist, daß es dem Prothesenteil (10) entspricht.
  5. Vorrichtung zum Unterdrucksetzen nach einem der Ansprüche 2 bis 4, bei welcher das Oberteil (20) und das Unterteil (30) Dichtlippen (28) aufweisen, in welche der flexible Flansch dicht eingefügt ist.
  6. Vorrichtung zum Unterdrucksetzen nach einem der Ansprüche 2 bis 5, bei welcher das Außenprofil des Unterteiles (30) derart geformt ist, daß es einem Knochenhohlraum (14) entspricht, in welchen das Prothesenteil eingesetzt werden soll.
  7. Vorrichtung zum Unterdrucksetzen nach Anspruch 3, bei welcher das Oberteil (20) eine an das U-förmige Element (21) angrenzende Schulter aufweist, welche beim Gebrauch auf dem Eingang zum Knochenhohlraum (14), in welchem das Prothesenteil (10) anzuordnen ist, aufliegt.
  8. Vorrichtung zum Unterdrucksetzen nach einem der bisherigen Ansprüche, bei welcher der flexible Flansch (8) durchsichtig ist, um Sicht bei der Positionierung zu ermöglichen.
  9. Vorrichtung zum Unterdrucksetzen nach einem der bisherigen Ansprüche, bei welcher das Prothesenteil (10) eine Hüftgelenkschaft-Prothese und der Knochenhohlraum (14) ein Femurkanal ist.
  10. Vorrichtung zum Unterdrucksetzen nach einem der Ansprüche 2 bis 9, bei welcher das Oberteil (20) und das Unterteil (30) aus Silikongummi geformt sind.
  11. Vorrichtung zum Unterdrucksetzen nach einem der Ansprüche 5 bis 10, bei welcher die Dichtlippen (28) des Oberteiles (20) und des Unterteiles (30) etwa 3 Millimeter dick sind.
  12. Vorrichtung zum Unterdrucksetzen nach einem der bisherigen Ansprüche, bei welcher der flexible Flansch (8) zwischen 0,5 Millimeter und 1 Millimeter dick ist.
  13. Vorrichtung zum Unterdrucksetzen nach einem der bisherigen Ansprüche, bei welcher der flexible Flansch (8) einen Durchmesser von etwa 10 Zentimeter hat.
  14. Vorrichtung zum Unterdrucksetzen nach einem der bisherigen Ansprüche in Kombination mit einem Prothesenteil (10).
