Description
- Die vorliegende Erfindung liegt in den technischen Gebieten der Chirurgie, der Medizin, der Gewebetechnologie, der Biologie, der Biomaterialien, der Polymere und der Biochemie. Es handelt sich dabei um ein Produkt für die Reparatur von Knorpelverletzungen mit und ohne den Einsatz von in vivo gezüchteten Chondrozyten.
- Es wurde sehr gut dokumentiert, daß ein verletzter artikulärer Knorpel nur eine begrenzte Fähigkeit zur Selbstreparatur aufweist. Da ein artikulärer Knorpel relativ avaskulär und aneural ist, führt ein Verlust des Oberflächengewebes zu einer dauerhaft narbigen Stelle. Verletzungen, bei denen der subchondrale Knochen gebrochen ist, der eine größere vaskuläre Versorgung aufweist, reagiert mit einer Entzündung/Reparatur, wobei sich die geschädigte Stelle mit Faserknorpelgewebe füllt (Convery et al., 1972). In jedem Fall ist die Funktion verschlechtert und chronische Schmerzen sind die übliche Prognose, da sich die biochemischen und die biomechanischen Eigenschaften des Knorpels geändert haben. Laufende Behandlungsprotokolle verlangen nach einem chirurgischen Eingriff (z. B. einer Ausschabungsarthroplastik, einer Exzision und einem Ausbohren, einem Ausschneiden von artikulärem Knorpel und einem arthroskopischen Abschaben) und führt meistens wieder zu einer ungenügenden Reparatur. Die Langzeitmorbidität, wie die Degeneration zu arthritischen Zuständen, tritt oft bei Patienten mit chronischen Knorpelproblemen auf.
- Nichtsdestoweniger hat ein artikulärer Knorpel theoretisch einige Eigenfähigkeit, nach einer Verletzung zu heilen. Beispielsweise sind Chondrozyten zu einer Replikation in der Lage, wenn sie enzymatisch aus der Knorpelmatrix isoliert werden (Grande et al., 1989). Es wurde vorgeschlagen, daß eine Knorpelreparatur begonnen werden kann durch entweder eine Replikation von Chondrozyten in den dem Defekt benachbarten Bereichen oder durch Metaplasie von Chondrozyten aus anderen Bindegewebsstammzellen innerhalb der Gelenkkapsel, z. B. aus dem Synovium und dem Subchondralknochen (Sokoloff, 1978). Ausgehend von dieser Möglichkeit sind die Forschungen bezüglich eines Autotransplantats oder Allotransplantatgewebes und analogen Geweben zur Heilung von Knorpelverletzungen fortgeschritten.
- Es wurden Techniken für den Einsatz von autologem Gewebe, wie die Transplantation von 1) osteochondralem Implantat (DePalma et al., 1963), 2) Chondrozyten (Grande et al., 1989), 3) Periost (Homminga et al., 1990), und 4) demineralisiertem Knochen (Dahlberg und Kreicbergs, 1991), entwickelt. Diese Techniken wurden benutzt, um ganze Gelenke oder Teile hiervon zu transplantieren, mit unterschiedlichen Ergebnissen. Beispielsweise versuchte eine Reihe von Forschern, Knorpeldefekte unter Einsatz von aus epiphysären Platten isolierten Chondrozyten sowie von artikulären Zellen zu heilen, und zwar mit der Hypothese, daß diese Zellen wegen ihres erhöhten Metabolismus (Itay et al., 1987) eine größere Erfolgschance hätten. Von klinischen Studien unter Einsatz von gezüchteten Zellen wurden hervorragende Ergebnisse berichtet, die nach zwei bis vier Jahren nach der Operation eine deutliche Abnahme des Schmerzes und eine Wiederherstellung der normalen Funktion zeigten (Iloika et al., 1990; Ilomminga et al., 1990).
- Andere Untersuchungen benutzten eine Kombination aus Materialien und autologem Gewebe, um Knorpeldefekte wirksam zu reparieren, z. B. 1) demineralisierten Knochen mit Perichondrium (Billings et al., 1990), 2) Polymilchsäurematrices und periostale Implantate (von Schroeder et al., 1991), und 3) bioresorbierbare Netze und Chondrozyten (Freed et al., 1993). Obwohl diese Versuche ein Reparaturgewebe ergaben, das mehr einem normalen Knorpel als entweder den ungefüllten oder den mit Materialien allein gefüllten Stellen ähnelte, war es offensichtlich, daß wieder eine erhebliche Menge an Faserknorpelbildung eintrat.
- In den US-Patenten Nr. 4,505,266 und 4,458,678 berichten Yannas et al. in Spalte 11, daß verschiedene "Arten von Fasergittern für den Einsatz als temporäre prosthetische Vorrichtungen innerhalb der meisten Körperregionen, z. B. der Haut, der Blutgefäfle, der Knochen, des Bindegewebes, des kontraktilen Gewebes und der kontraktilen Organe geeignet sein können. Solche Gitter ergeben ein strukturelles System, in dem eigentlich jede Art von Zellen wachsen, wandern und proliferieren kann. Sie können chirurgisch eigentlich in jede Körperregion eingesetzt werden, und wenn sie in entsprechender Weise mit der geeigneten Art oder den geeigneten Arten von Zellen beimpft werden, können sie die Regenerierung von neuem Gewebe ermöglichen. Wenn beispielsweise ein Patient unter einer Schädigung oder einer Erkrankung eines Organs leidet, kann das Entfernen eines Teils des Organs nötig sein. An der Stelle, die durch das Entfernen des Teils des Organs entstanden ist, kann ein Fasergitter eingesetzt werden. Wenn eine ausreichende Anzahl von gesunden Zellen von einem anderen Teil jenes Organs oder von einem verträglichen Spender mittels der Methoden der vorliegenden Erfindung in das Gitter ein geimpft wird, kann es möglich sein, die Erholung und Regenerierung des Organs stark zu fördern."
- Das US-Patent Nr. 4,846,835 berichtet, daß Chondrozyten, die in einer dreidimensionalen Collagenmatrix gewachsen sind, das Heilen von artikulären Knorpelverletzungen, bei denen die subchondrale Platte nicht gebrochen ist, verbessern können.
- Auch die
US 5,306,311 beschreibt eine prosthetische artikuläre Knorpelvorrichtung, die mittels einer Presspassungsmethode eingesetzt wird. Die Vorrichtung ist aus bioverträglichen und mindestens teilweise bioresorbierbaren Fasern hergestellt. Die Vorrichtung kann mittels eines Klebstoffs an ihrer Stelle an dem Knochen gehalten werden. - In bei Kaninchen durchgeführten Versuchen, in denen die Lehren von Yannas und Grande nachgearbeitet wurden, wurden gezüchtete Chondrozyten in eine dreidimensionale Collagenmatrix eingeimpft, und die beimpfte Matrix wurde in eine chirurgisch erzeugte artikuläre Knorpelverletzung implantiert. Im Hinblick auf jene Lehren wurde überraschenderweise gefunden, daß zusätzlich zur Anwesenheit des gewünschten hyalinartigen Knorpels eine deutliche Menge von unerwünschtem Faserknorpel gebildet wurde, anscheinend durch Fibroblasten, die aus der subchondralen Platte in die Matrix gewandert sind. Somit zeigen diese Versuche, daß weder Yannas noch Grande ein Verfahren zur Bildung eines hochwertigen hyalinartigen Knorpels lehren, der für eine Reparatur von Defekten in artikulärem Knorpel geeignet wäre, da sie keine Maßnahme vorsehen, im Hinblick auf unerwünschte Arten von Zellen, welche von dem umgebenden Gewebe aus die Matrix infiltrieren können, eine Auswahl zu treffen. Bei der vorliegenden Erfindung haben wir einen neuen Weg gefunden, um das Wachstum des gewünschten hyalinartigen Knorpels auszurichten, und vermeiden so die Schwierigkeiten des Standes der Technik.
- Dementsprechend ist es eine allgemeine Aufgabe der vorliegenden Erfindung, ein mehrstufiges Collagenimplantat zur Reparatur von Knorpelverletzungen bereitzustellen, welches die Nachteile des Standes der Technik überwindet.
- Eine weitere Aufgabe dieser Erfindung ist es, ein mehrstufiges Collagenimplantat zur Reparatur von Knorpelverletzungen anzugeben, das wirksam und sicher ist.
- Eine andere Aufgabe dieser Erfindung ist es, ein mehrstufiges Collagenimplantat zur Reparatur von Knorpelverletzungen zu schaffen, das resorbierbar ist.
- Defekte in artikulärem Knorpel können durch Verwendung einer Regenerierungsschablone geheilt werden, die durch Kombinieren eines porösen Collagenschwamms (einer "Collagenmatrix") mit einer dichten Collagenmembran hergestellt worden ist. Die dichte Collagenmembran wird mit einer Porengröße von weniger als 1 μm (Mikrometer) gebildet und mit einem nichtzytotoxischen Mittel vernetzt, um die Festigkeit zu erhöhen und die Resorptionszeit zu verlängern. Wegen dieser dichten Collagenmembran kann die Erfindung bei Defekten in voller Dicke, einschließlich jenen, die durch die subchondrale Platte hindurchgehen, angewandt werden. Die dichte Collagenmembran wird auf die Oberfläche des Knorpeldefekts aufgesetzt, um eine Zellenwanderung von der subchondralen Platte und den Gefäßen aus zu verhindern. Die Collagenmembran erlaubt eine Bewegung und einen Austausch von Fluiden, Nährstoffen, Cytokinen und anderen Faktoren, die für eine Knorpelregenerierung nötig sind. Die Collagenmatrix wird auf der Oberseite der dichten Collagenmembran angeordnet. Die Collagenmatrix hat eine Porengröße zwischen 50 und 200 Mikrometer und erlaubt das Anbringen von Zellen, insbesondere von Chondrozyten.
- Andere Aufgaben und viele begleitende Merkmale der vorliegenden Erfindung werden leicht ersichtlich, da diese durch Bezugnahme auf die folgende detaillierte Beschreibung. besser zu verstehen ist, wenn eine Betrachtung in Verbindung mit den beigefügten Zeichnungen erfolgt. Darin zeigt

12 geheilt werden, die durch Kombinieren eines porösen Collagenschwamms ("Collagenmatrix")13 mit einer dichten Collagenmembran14 gebildet worden ist. - Die dichte Collagenmembran
14 wird mit einer Porengröße von weniger als einem Mikrometer hergestellt und mit einem nichtzytotoxischen Mittel vernetzt, um die Festigkeit zu erhöhen und die Resorptionszeit zu verlängern. Wegen dieser dichten Collagenmembran12 kann die Schablone12 bei Defekten4 , die sich über die gesamte Dicke erstrecken, verwendet werden, einschließlich jener, die durch die subchondrale Platte9 hindurchgehen. Die dichte Collagenmembran14 wird auf die Oberfläche des Knorpeldefekts4 aufgelegt, um zu verhindern, daß eine Zellenwanderung von der subchondralen Platte9 und dem Gefäßsystem ausgeht. Die Collagenmembran14 erlaubt eine Bewegung und einen Austausch von Fluiden, Nährstoffen, Cytokinen und anderen Faktoren, die für eine Knorpelregenerierung nötig sind. - Es wurde eine Collagenschablone
12 entwickelt, die eine Befestigung und ein Wachstum von Zellen, insbesondere von Chondrozyten6 , ermöglicht. In-vitro-Untersuchungen wurden benutzt, um die optimale Porengröße der porösen Collagenmatrixkomponente13 der Schablone12 zu bestimmen. Die Collagenmatrix13 kann verwendet werden, um Chondrozyten6 in vitro zu immobilisieren und das nachfolgende Zellwachstum zu unterstützen. Die Zellenzahl kann dann in vitro vergrößert werden, und die Collagenmatrix13 kann benutzt werden, um die Zellen zu der Reparaturstelle4 zu transportieren und sie im Anschluß an die Implantation an ihrer Stelle zu halten. - Die Collagenmatrixkomponente
13 der Schablone12 wurde entwickelt, um ein Festhalten und ein Wachstum der Zellen, insbesondere der Chondrozyten6 , zu ermöglichen. Es wurden in-vitro-Untersuchungen zur Bestimmung der optimalen Porengröße der porösen Collagenmatrixkomponente13 der Schablone12 benutzt. Die Collagenmatrix13 kann verwendet werden, um Chondrozyten6 in vitro zu immobilisieren und das nachfolgende Zellwachstum zu unterstützen. Die Zellenzahl kann dann in vitro erhöht werden, und die Collagenmatrix kann verwendet werden, um die Zellen an die Reparaturstelle zu transportieren und sie im Anschluss an die Implantation an ihrer Stelle zu halten. - Frühere Untersuchungen haben gezeigt, daß die Porengröße der Collagenmatrix
13 durch Verändern des pH-Werts der Dispersion, der Collagenkonzentration und des Lyophilisierungszyklus (Gefrierzeit, Temperaturbereich und Zykluszeit (Dillion et al. 1986)) gesteuert werden kann. Siehe auch US-Patent Nr. 4,522,753. Die Collagenmatrices wurden auch gemäß ihrer Durchlässigkeit gegenüber kugelförmigen Makromolekülen charakterisiert. Beispielsweise wurde gefunden, daß eine Porenstruktur von etwa 15 Mikrometer Moleküle ausschließt, die größer als 106 Daltons sind; eine dichte Collagenmembran hatte einen Molekulargewichtsausschluß von 7 × 104 Daltons (Li, 1987). Man ließ Chondrozyten an Collagenmatrices des Typs I mit verschiedener Porenstruktur wachsen, um die Wirkung der durchschnittlichen Matrixporengröße auf die Geschwindigkeit des Zellwachstums zu bestimmen. Es wurde gefunden, daß die Porenstruktur die Geschwindigkeit des Zellwachstums nach 12 Tagen nicht beeinflußt. Jedoch war die Infiltration von Chondrozyten bei durchschnittlichen Porengrößen von über 100 Mikrometer größer. Eine parallele Untersuchung unter Einsatz von Fibroblasten zeigte ähnliche Ergebnisse des Zellwachstums. Es ist wich tig, zu bemerken, daß die Wachstumsgeschwindigkeit von Fibroblasten auf der dichten Collagenmembran etwa die gleiche wie im Fall einer porösen Matrix war, daß aber das wandern von Zellen durch die Membran hindurch ausgeschlossen wurde (Pachence et al., 1991). - Die dichte Collagenmembran
14 kann vor der Zellenzüchtung oder vor der Implantation an der Collagenmatrix13 befestigt werden, und zwar unter Benutzung 1) von bioresorbierbaren Nähten oder 2) einer Verschmelzungstechnik, die erfordert, daß während der Bildung die dichte Collagenmembran14 in die Collagenmatrix13 incorporiert wird. - Es wurde durch eine Reihe von in-vivo-Untersuchungen gezeigt, daß die Schablone
12 mit und ohne Zusatz von Chondrozyten6 das Heilen von chirurgisch induzierten, in voller Dicke vorliegenden Defekten in einem Kaninchenmodell mit einer Knorpelschädigung fördert. Die mit Chondrozyten beimpften Schablonen haben durch die Anwendung von histologischen, biochemischen und mechanischen Analysen von wiedergewonnenen Stellen von Implantatgeweben gezeigt, daß sie ein Reparaturgewebe ergeben, das ein hyaliner Knorpel zu sein scheint. - Wie in den
1 bis3 gezeigt wird, kommt der Orientierung der Schablone12 in dem Knorpeldefekt4 eine fundamentale Bedeutung zu, um ein erfolgreiches Ergebnis zu erzielen. Die dichte Schicht14 wird "nach unten" in den Defekt4 eingesetzt und berührt den Knochen3 , und die poröse Schicht13 liegt in der Ebene des natürlichen Knorpels. Es wurde experimentell gezeigt, daß die dichte Schicht14 die Bildung von Fibroknorpel inhibiert. Die Dicken der Komponenten der Schablone12 können in Abhängigkeit von den Umständen des Einsatzes variieren. Beispiels weise kann die Dicke der dichten Collagenmembran im Bereich von 50 bis 200 Mikrometer oder mehr um die Dicke der porösen Collagenmatrix im Bereich von 0,5 bis 8 Millimeter oder mehr liegen. - Die Methoden der chirurgischen Fixierung für ein Knorpelreparaturmaterial
12 sind wichtig, da eine Bewegung des Gelenks das Implantat vor der Heilung verschieben kann. Bei der vorliegenden Erfindung wird eine Befestigungsmethode angewandt, um die Collagenmatrix13 und die dichte Collagenmembran14 an ihrer Stelle zu halten. Die Methode besteht im Verankern von Nahtfäden10 durch die subchondrale Platte9 in das knochenartige Gewebe3 hinein, wobei mindestens zwei Fäden10 aus der Oberfläche heraustreten. Die verankerten Nahtfäden10 werden dann durch das Collagenimplantat12 an seinen vier Viertelkreisen hindurchgezogen und werden so dazu benutzt, das Knorpelreparaturmaterial12 an der Wundstelle4 zu befestigen. Wie in4 gezeigt wird, wird in einigen Fällen ein Stück von einem autologen Periost15 über der Oberseite der Collagenmatrix13 angeordnet und auch durch die verankerten Nahtfäden10 befestigt. - Beispiel 1
- Herstellung und Eigenschaften der porösen Matrix
- Unter Anwendung der Standardmethoden, wie sie im US-Patent Nr. 5,206,028 beschrieben sind, wird eine Collagenmatrix hergestellt. Die Matrix hat eine mittlere Porenstruktur zwischen 50 und 200 Mikrometer, vorzugsweise von 150 Mikrometer. Collagen des Typs I wird in einer 0,5%-igen Milchsäurelösung mit einer endgültigen Collagenkonzentration von 0,7 Gew.-% dispergiert. Die Collagendispersion wird durch einen Filter aus rostfreiem Stahl mit einer Größe von 100 mesh gedrückt und dann bis zu einer Dicke von 4 mm in Schalen gegossen. Die Dispersion wird zwei Stunden bei –35°C gefroren, bevor ein Vakuum angelegt wird. Die gefrorene Dispersion wird dann während 48 Stunden unter einem Vakuum von 50 Micron lyophilisiert, wobei die Temperatur auf 20°C ansteigt. Die Lagertemperatur wird nachfolgend während weiterer 24 Stunden auf 25°C erhöht, um den Zyklus zu vervollständigen. Die Collagenmatrix wird unter Einsatz eines nichtzytotoxischen Mittels oder durch Anwendung einer vorher beschriebenen physikalischen Methode (Weadock et al., 1983) vernetzt. Beispielsweise kann die Collagenmatrix verdampftem Formaldehyd (5%-ige Lösung) ausgesetzt werden. Das auf diese Weise erfolgende Vernetzen ermöglicht es, daß das Implantat für vier bis acht Wochen intakt bleibt.
- Beispiel 2
- Herstellung und Eigenschaften der dichten Collagenmembran
- Gemäß der im US-Patent Nr. 5,206,028 angegebenen Verfahrensweise wird eine dichte Collagenmembran hergestellt. Eine poröse Matrix mit einer Dicke von 4 bis 10 mm wird unter Einsatz einer feuchtigkeitsgesteuerten Kammer bei einer relativen Luftfeuchtigkeit von 80% während 60 Minuten bei 25°C hydratisiert. Das feuchte Collagenmaterial wird zwischen zwei Teflon-Platten bis zu einer Dicke von weniger als 0,2 mm gepresst. Das gepresste Material wird dann in einer Lösung von 0,5% Formaldehyd und 1% Natriumbicarbonat bei pH 8 während 60 Minuten vernetzt. Die vernetzte Membran wird dann gründlich mit Wasser gespült und dann über Nacht unter ähnlichen Bedingungen wie im Beispiel 1 gefriergetrocknet, mit der Ausnahme, daß die Zeit für das Gefriertrocknen etwa 48 Stunden beträgt. Die dichte Collagen membran weist einen inneren Aufbau aus dicht gepackten Fasern auf, die in einer Mehrschichtenstruktur miteinander verflochten sind. Die Collagenmembran erlaubt eine Diffusion von Molekülen mit einem Molekulargewicht von mindestens 105, erlaubt aber kein Eindringen von Fibroblasten.
- Beispiel 3
- Verwendung einer nicht mit Zellen beimpften Matrix bei Knorpeldefekten
-
- 1. Herstellen einer chirurgisch definierten Stelle, etwas kleiner als die Größe des Matriximplantats. Die Tiefe soll etwa die gleiche Größe wie der Collagenmatrix-Zellen-Verbund aufweisen. Die chirurgisch vorbereitete Stelle soll durch die subchondrale Platte hindurchgehen (d. h., ein blutendes Bett).
- 2. Setzen eines Ankers, der zwei daran befestigte resorbierbare
Nahtfäden
aufweist, in das Zentrum der chirurgisch vorbereiteten Stelle (
2 ). Es stehen vier Fäden zur Verfügung, einer in jedem Viertelkreis. - 3. Anbringen einer dichten Collagenmembran (Porenstruktur kleiner
als ein Mikrometer) auf der Collagenmatrix unter Verwendung von
resorbierbaren Nahtfäden,
wie Vicryl 7-0 (
3 ). - 4. Anordnen der Schablone mit der dichten Collagenmembran am Boden der chirurgisch vorbereiteten Stelle, Befestigen der Schablone an ihrer Stelle mit den Nahtfäden durch Hindurchziehen derselben durch die Schablone.
- 5. Anordnen eines schützenden
Teils aus tibialem Periost über
der Matrix. Das Periost muß in
folgender Weise über
dem Implantat ausgerichtet werden. Die Kambiumschicht wird zum Implantat
hin orientiert (nach unten in den Defekt hinein), und die Faserschicht
ist zu der artikulären
Oberfläche
hin gerichtet (aufwärts
in den Raum der Verbindung hinein). Durchziehen des Nahtfadens durch
das Periost und Festbinden der Fäden
an der Oberseite der Schutzplatte (
4 ). - Beispiel 4
- Verwendung einer mit Zellen beimpften Matrix bei Knorpeldefekten
-
- 1. Herrichten einer autologen Gewebsprobe, die Knorpel- oder Vorläuferzellen enthält.
- 2. Entfernen der extrazellularen Matrix von der Gewebsprobe und anschließendes Isolieren von Zellen unter Anwendung von Standardmethoden.
- 3. Vermehren der Zellen in einer Kultur.
- 4. Die dichte Collagenmembran wird an der Collagenmatrix mittels bioresorbierbarer Nahtfäden, wie Vicryl 7-0, befestigt.
- 5. Hinzufügen von Zellen zu der porendefinierten Collagenmatrix, so daß Zellen durch die Matrix hindurchdringen. Dies kann durch Aufbringen der Zellsuspension auf die Matrix und dann vorsichtiges Anlegen eines Vakuums unter der Matrix geschehen.
- 6. Züchten der Collagenschablone/Zellen-Verbunds während einer Woche oder mehr.
- 7. Herstellen einer chirurgisch definierten Stelle, die etwas kleiner ist als die Größe des Implantats. Die Tiefe sollte etwa die gleiche Größe wie der Collagenschablone/Zellen-Verbund haben. Die chirurgisch vorbereitete Stelle sollte durch die subchondrale Platte hindurchgehen (d. h., ein blutendes Bett).
- 8. Einsetzen eines Ankers mit zwei daran befestigten resorbierbaren
Nahtfäden
in das Zentrum der chirurgisch vorbereiteten Stelle (
2 ). Es stehen vier Fäden zur Verfügung, einer in jedem Viertelkreis. - 9. Anordnen des Collagenschablone/Zellen-Verbunds auf dem Boden
der chirurgisch vorbereiteten Stelle, wobei die dichte Collagenmembrankomponente
in Kontakt mit dem Boden steht, und Einfädeln der Nahtfäden in die
Membran (
3 ). - 10. Anordnen einer Schutzplatte aus entweder klarem Collagen
oder einem Stück
tibialem Periost über
der Matrixkomponente der Schablone. Das Stück aus klarem Collagen ist
ein luftgetrockneter Film mit einer Dicke von 50 bis 200 Mikrometer.
Der Film wird durch Trocknen einer einprozentigen Collagendispersion
in einer haftungsfreien Schale hergestellt. Wenn man Periost verwendet,
wird es über
dem Implantat wie folgt ausgerichtet: Die Kambiumschicht wird zum
Implantat hin orientiert (nach unten in den Defekt hinein), und die
faserige Schicht wird zur artikulären Oberfläche hin ausgerichtet (nach
oben in den Raum der Verbindung hinein). Durchziehen des Nahtfadens
durch die Matrix und die Collagenplatte oder das Periost, Binden
der Nahtfäden über der
Oberseite der Schutzplatte (

Claims (8)
- Schablone (
12 ) zur Reparatur von Knorpeldefekten, die zur Regenerierung eines hyalinartigen Knorpels führt, mit a) einer ersten Schicht mit einer porösen Collagenmatrix (13 ), die eine Porengröße von 50 bis 200 Mikrometer aufweist, welche das Einwachsen von Zellen erlaubt, gekennzeichnet durch b) eine weitere, unter der ersten Schicht befestigten zweiten Schicht mit einer dichten Collagenmembran (14 ), die eine Porengröße von weniger als 1 Mikrometer aufweist und durch ein nichtzyto-toxisches Mittel vernetzt ist, um die Festigkeit zu erhöhen und die Resorptionszeit zu verlängern, damit eine Sperre gegen eine Bewegung von Zellen aus der subchondralen Platte (9 ) geschaffen wird, wobei die Membran (14 ) ausreichend durchlässig ist, um den Durchtritt von Fluiden, Nährstoffen, Zytokinen und anderen für das Heilen nötigen endogenen Faktoren zu erlauben. - Schablone (
12 ) nach Anspruch 1, worin die dichte Collagenmembranschicht (14 ) eine Dicke im Bereich von 50 bis 200 Mikrometer aufweist. - Schablone (
12 ) nach Anspruch 1 oder 2, worin die poröse Collagenmatrixschicht (13 ) eine Dicke im Bereich von 0,5 bis 8 Millimeter aufweist. - Schablone (
12 ) nach einem der vorstehenden Ansprüche, worin die dichte Collagenmembranschicht (14 ) mittels einer resorbierbaren Naht an der Collagenmatrixschicht (13 ) befestigt ist. - Schablone (
12 ) nach einem der Ansprüche 1 bis 3, worin die dichte Collagenmembran (14 ) in die Collagenmatrix (13 ) eingefügt ist. - Schablone (
12 ) nach einem der vorstehenden Ansprüche, worin Chondrozytzellen (6 ) in der porösen Collagenmatrix (13 ) immobilisiert sind. - Schablone (
12 ) nach einem der vorstehenden Ansprüche, die ferner an der Oberseite der Collagenmatrixschicht (13 ) ein autologes Periost (15 ) aufweist. - Schablone (
12 ) nach einem der Ansprüche 1 bis 6, die ferner an der Oberseite der Collagenmatrixschicht (13 ) einen Collagenfilm aufweist.
