Description
- Die Erfindung betrifft ein künstliches Wirbelband, das über Pedikelschrauben angebracht ist und zwei benachbarte Wirbel verfestigt.
- Die Erfindung eignet sich insbesondere für ein künstliches Wirbelband, das dazu vorgesehen ist, jeweils zwischen Lendenwirbel angeordnet zu werden, so wie zwischen dem letzten Lendenwirbel und dem Kreuzbein, insbesondere zwischen dem vierten und dem fünften Lendenwirbel (L4 und L5) oder zwischen dem fünften Lendenwirbel und dem ersten Kreuzbeinwirbel (L5 und S1).
- Die Erfindung betrifft insbesondere ein künstliches Wirbelband in Modulbauweise, das dazu bestimmt ist, aufeinanderfolgend an Lendenwirbeln angebracht zu werden, wobei diese in Art Treppenstufen übereinanderliegen.
- Das künstliche Wirbelband, das dazu vorgesehen ist, mittels Pedikelschrauben an zwei Wirbeln der Wirbelsäule befestigt zu werden, ist dadurch gekennzeichnet, daß es wie folgt aufgebaut ist, nämlich:
- - aus einer ersten und einer zweiten hohlen Hülse in der allgemeinen Form eines Diabolo, von denen jede folgendes aufeinanderfolgend enthält, nämlich:
- eine konkave Aufnahme, die dazu bestimmt ist, den Kopf einer der Pedikelbefestigungsschraube aufzunehmen;
- einen eine Einschnürung aufweisenden Körper;
- einen zur konkaven Aufnahme komplementären konvexen Anguß, der auf den Wirbel ausgerichtet ist;
- - sowie aus dem genannten künstlichen Band aus einem flexiblen textilen Material, gebildet aus:
- einer ersten, längsverlaufenden Wicklung, die in Form einer Acht um die Einschnürungen der beiden Hülsen angeordnet ist;
- sowie einer zweiten, querverlaufenden Wicklung, die in aneinanderliegenden Windungen um die erste Wicklung herum angeordnet ist.
- Die Erfindung und die sich daraus ergebenden Vorteile werden an Hand des Ausführungsbeispiels, das aus den beiliegenden Zeichnungen hervorgeht, besser verstanden.
- Die Figuren 1 und 2 zeigen Schnittdarstellungen eines erfindungsgemäßen künstlichen Wirbelbandes, und zwar eine geschnittene Seitenansicht und eine Draufsicht.
- Die Figuren 3 und 4 zeigen kennzeichnende Hülsen der Erfindung, und zwar im Längsschnitt und in Draufsicht.
- Figur 5 zeigt eine Reihe von drei künstlichen Wirbebändern, die erfindungsgemäß an Ort und Stelle gebracht sind.
- Erfindungsgemäß (siehe Figuren 3 und 4) weist das Band eine erste (1) und eine zweite hohle Hülse (10) aus Metall (Edelstahl, Titanlegierung, Chrom-Kobalt-Stahl) in der allgemeinen Form eines Diabolo auf. Jede der beiden Hülsen (1, 10) weist auf der Außenseite (2), eine konkave Aufnahme (3) auf, die dazu vorgesehen ist, den Kopf von hier nicht dargestellten Befestigungsschrauben aufzunehmen. Eine solche Schraube, beispielsweise nach Muller oder nach Maconnor, ist dazu bestimmt, die Hülse auf den Gelenkfortsätzen (siehe Figur 5) zu befestigen. Diese Corticalisschrauben durchqueren die Pediculi, um in dem Wirbelkörper zum Liegen zu kommen. In einer Variante kann die konkave Aufnahme (3) eine weitere identische folgende Hülse aufnehmen, und zwar derart, daß diese die Rolle eines Ringes spielt, der ein seitliches Verschieben verhindert.
- Diese Seite (2) ist mit einem Körper (4) verbunden, in dem eine mittige Einschnürung (5) ausgespart ist, die an ihrem Rand mit einem konvexen Anguß (6) versehen ist, der komplementär zur konkaven Aufnahme (3) ist. Dieser konvexe Anguß (6) ist, wie bereits erwähnt, entweder dazu vorgesehen, daß er sich in den Gelenkfortsatz einfügt, wobei dort zuvor eine kleine Einsenkung bewerkstelligt wurde, die die Rolle eines Ringes gegen seitliches Verschieben bildet, oder um in einer konkaven Aufnahme (3) einer benachbarten Hülse aufgenommen zu werden (siehe Figur 5). Ein mittiger Durchlaß (7) ermöglicht den Durchtritt der Befestigungsschraube. Um Verletzungen zu vermeiden und die Gelenkigkeit des Zusammenbaus zu erleichtern, sind die verschiedenen Kanten der Hülse abgerundet.
- Entsprechend einem weiteren Kennzeichen der Erfindung weist das Band das erwähnte künstliche Band aus einem flexiblen textilen Material auf, das durch eine erste, längsverlaufende Wicklung (20) gebildet wird, die in Form einer Acht zwischen bzw. um die Einschnürungen (5) und (15) von zwei aufeinanderfolgenden Hülsen (1, 10) angeordnet ist. Diese erste, längsverlaufende Wicklung (20) ist über ihren Umfang von einer zweiten, querverlaufenden Wicklung (25) überdeckt, die in aneinanderliegenden Windungen um diese erste, längsverlaufende Wicklung (20) angeordnet ist. Die erste, längsverlaufende Wicklung (20) verleiht dem Zusammenbau eine Zugfestigkeit. Im Gegensatz dazu verhält sich die zweite, querverlaufende Wicklung (25) wie eine Ausfütterung und verleiht dem Band eine Steifigkeit gegenüber Zusammendrücken.
- In der Praxis bestehen sowohl die erste, längsverlaufende Wicklung (20) als auch die zweite, querverlaufende Wicklung (25) aus einem Flechtwerk an Multifilamentfäden aus schrumpfbarem Polyester. Daher kann durch ein einfaches Erwärmen in einem Trockenofen ein Schrumpfen der Fasern (beispielsweise um 12%) verursacht werden, was dem Zusammenbau seine Steifigkeit und sein gewünschtes Maß gibt. Vorzugsweise ist der Querschnitt des Flechtwerkes, das die erste Wicklung (20) bildet, unter dem des zweiten äußeren Flechtwerkes (25), und zwar um ein Überlappen der aneinanderliegenden Windungen der zweiten Wicklung (25) zu verhindern.
- In Figur 5 ist dargestellt, wie ein erfindungsgemäßes Band zum Verstärken einer Wirbelsäule, die in ihrer Gesamtheit durch die Bezugsziffer (30) bezeichnet ist, an Ort und Stelle gebracht werden kann, in der die Bezugsziffern (31, 33, 35, 37) Wirbel bezeichnen, beispielsweise (L4, L5 und S1) und die Bezugsziffern (32, 34, 36) die Bandscheiben bezeichnen. Der erste Zusammenbau (1, 25, 10) wird mit hier nicht dargestellten Pedikelschrauben an den Gelenkfortsätzen befestigt, indem sie die Pediculi durchqueren, um im Wirbelkörper (31, 33) zum Liegen zu kommen. Der zweite Zusammenbau (1', 25', 10') verstärkt die folgenden Wirbel (33, 35), die untere Hülse (10) des ersten Zusammenbaus arbeitet mit der ersten Hülse (1') des folgenden Zusammenbaus, wie das in der Figur dargestellt ist, zusammen. Auf die gleiche Art und Weise arbeitet der dritte Zusammenbau (20", 25", 10") mit dem zweiten Zusammenbau zusammen, um die Wirbel (35, 37) zu verstärken. Man erkennt, daß die männlichen (6) und weiblichen (3) halbkugelförmigen Abschnitte der Hülsen ineinander verschachtelt sind, wodurch ein kompakter Zusammenbau gebildet wird, der eine geringfügige kniegelenkartige Spielanpassung ermöglicht. Gleichermaßen ermöglicht die Einschnürung (5), die die erste, längsverlaufende Wicklung (20) aufnimmt, aufgrund der Form eines Diabolo, diese erste Wicklung (20) in Translationsrichtung unbeweglich zu machen, ohne diese zu ermüden, was Ermüdungsbrüche verhindert (auf lange Zeit im Laufe des Tragens gesehen).
- Bei der in Figur 5 dargestellten Variante kann eine charakteristische Hülse mit einer Distanzhülse (40) zusammenarbeiten mit gleichen Ausmaßen und gleicher Höhe wie die charakteristische Hülse (10").
- Die Steifigkeit des erfindungsgemmßen Bandes ist, was sowohl Zug als auch Druck betrifft, beachtlich.
- Bei einer praktischen Ausführungsform wurden gute Resultate mit Hülsen erhalten, deren Durchmesser und Länge bei etwa zehn Millimetern liegt, die von einem Durchlaß (7) mit etwa sieben Millimeter durchdrungen sind, und bei denen die Krümmungsradien der konkaven Aufnahme (3) und des konvexen Angusses (6) etwa fünf Millimeter betragen, wodurch es möglich ist, Schrauben nach Muller von 4,5 mm Durchmesser oder nach Maconnor mit 5 mm Durchmesser aufzunehmen. Die erste, längsverlaufende Wicklung (20) wird durch sechzig Umläufe in Form einer Acht eines schrumpfbaren Polyesterfadens hoher Zugfestigkeit mit 1000 Denier gebildet. Diese erste Wicklung (20) wird anschließend mit einem Flechtwerk von aneinanderliegenden Windungen von drei Millimetern Durchmesser bedeckt, das aus gleichen schrumpfbaren Polyesterfäden hoher Zugfestigkeit mit 1000 Denier von sechs Spindeln ausgehend hergestellt wurde, wobei eine zweite Wicklung (25) gebildet wird. Das äußere Netzwerk (25), das die zweite Wicklung bildet, ist geschlossen um die innere erste Wicklung (20) gewickelt, und zwar derart, daß durch deren äußere Enden die Schlaufen der ersten Wicklung geschlossen sind. Ist dieser Zusammenbau bewerkstelligt, so wird er auf an sich bekannte Weise mit Wärme behandelt, um ein Schrumpfen der Polyesterfäden hervorzurufen. Dies verleiht dem Zusammenbau die gewünschte Zugfestigkeit mit einer geringen Dehnbarkeit, bis hin zur Öffnung des hohlen Kopfes der ersten Wicklung, die in der Tat praktisch nicht dehnbar ist.
- Der Abstand zwischen zwei Hülsen (1, 10) ein und desselben Zusammenbaus ist bausteinartig erweiterbar, beispielsweise fünfundzwanzig, dreißig, fünfunddreißig Millimeter.
- Unter diesen Bedingungen wurde beobachtet, daß die maximale Amplitude zwischen der Verlängerung des Bandes bei Kyphose und dem Zusammendrücken bei Lordose etwa zwei Millimeter beträgt.
- Das künstliche Wirbelband entsprechend der vorliegenden Erfindung weist gegenüber den bislang bekannten Lösungen zahlreiche Vorteile auf. Dabei kann man folgendes erwähnen:
- - die feste Pedikel-Implantation, und wobei diese nach einer vollständigen Laminektomie möglich ist;
- - die Steifigkeit, die die Pedikelbefestigung mit sich bringt, verbunden mit einer Zug- und Drucksteifigkeit, begrenzt die Kyphose-Lordose zwischen zwei Wirbeln und dadurch ein Verschieben der Bandscheiben;
- die Möglichkeit von bausteinartigen Implantaten erlaubt, beliebig die Zahl der zu befestigenden Zwischenwirbelstufen zu erweitern, wobei die für jedes Modul vorgeschlagenen unterschiedlichen Größen sich jeder anatomischen Situation anpassen können.
Claims (5)
1. Künstliches Wirbelband, das dazu vorgesehen ist, mittels
Pedikelschrauben an zwei Wirbeln (31, 33) der Wirbelsäule
(30) befestigt zu werden, dadurch gekennzeichnet, daß es
wie folgt aufgebaut ist, nämlich
- aus einer ersten (1, 1', 1") und einer zweiten (10,
10', 10") hohlen Hülse in der allgemeinen Form eines
Diabolo, von der jede folgendes aufeinanderfolgend enthält:
eine konkave Aufnahme (3), die dazu vorgesehen
ist, den Kopf einer der
Pedikelbefestigungsschrauben aufzunehmen;
einen eine Einschnürung (5) aufweisenden Körper
(4);
einen zur konkaven Aufnahme (3) komplementären
konvexen Anguß (6), der auf den Wirbel (31
- 37) ausgerichtet ist;
- sowie aus dem genannten künstlichen Band aus einem
flexiblen textilen Material, gebildet aus:
einer ersten, längsverlaufenden Wicklung (20),
die in Form einer Acht um die Einschnürungen
(5, 15) der beiden Hülsen (1, 10) angeordnet
ist;
sowie einer zweiten, querverlaufenden Wicklung
(25), die in aneinanderliegenden Windungen um
die erste Wicklung (20) herum angeordnet ist.
2. Künstliches Wirbelband nach Anspruch 1, dadurch
gekennzeichnet, daß die Hülsen (1, 10) aus Metall bestehen.
3. Künstliches Wirbelband nach Anspruch 1, dadurch
gekennzeichnet, daß die einstückigen Hülsen (1, 10) aus Edelstahl,
einer Titanlegierung oder einem Chrom-Kobalt-Stahl bestehen.
4. Künstliches Wirbelband nach Anspruch 1, dadurch
gekennzeichnet, daß die innere, längsverlaufende erste Wicklung
(20) und die äußere, querverlaufende zweite Wicklung (25)
aus einem Multifilamentflechtwerk aus geschrumpften
Polyesterfasern bestehen.
5. Künstliches Wirbelband nach einem der Ansprüche 1 bis 4,
dadurch gekennzeichnet, daß der Zusammenbau außerdem eine
Distanzhülse (40) aufweist, die sich einerseits auf dem
Wirbel (37) und andererseits auf dem konvexen Anguß (16")
an der Hülse (10") abstützt.
