- HINTERGRUND DER ERFINDUNG
- 1. Gebiet der Erfindung
- Die Erfindung betrifft orthopädische Vorrichtungen und insbesondere Knochenplatten.
- 2. Beschreibung der verwandten Technik
- Eine orthopädische Knochenplatte wird an einem Knochen angebracht und dient dazu, unterschiedliche Knochen relativ zueinander abzustützen oder Stücke eines einzelnen Knochens während des Heilungsprozesses abzustützen. Normalerweise wird eine Knochenplatte so hergestellt, daß sie der Form eines Knochens nahekommt, an dem sie anzubringen ist. Die Form einer Knochenplatte kann auf statistischer Analyse einer verbreiteten Größe und Form eines speziellen Knochens beruhen. Da aber die tatsächliche Größe und Form eines Knochens von einem Patienten zum anderen variieren kann, ist es häufig notwendig, die Knochenplatte beim Eingriff manuell zu verformen, damit sie richtig paßt. Man kann die Knochenplatte am Knochen plazieren, auf Abweichung untersuchen, vom Knochen abnehmen und mit manuellen Biegetechniken verformen. Danach wird die Knochenplatte am Knochen plaziert und einer erneuten Sichtprüfung unterzogen, um etwaige weitere notwendige Einstellungen zu bestimmen. Während des Eingriffs kann dieses Verfahren einige Zeit beanspruchen.
- In der Technik besteht Bedarf an einer Knochenplatte, die leicht und schnell zu installieren ist und sich an die Form des Knochens leicht anpaßt, an dem sie anzubringen ist.
- Ein Beispiel für eine bekannte Zusammensetzung zum Vereinigen von Knochenfrakturen ist in der GB-A-952842 beschrieben, die Epoxidharzzusammensetzungen beschreibt. Die offenbarten Schienen oder Verstärkungsplatten werden vor dem Anbringen am Knochen vorgegossen und vorgehärtet.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Die Erfindung stellt einen orthopädischen Knochenplattenvorläufer bereit, der einen flexiblen Behälter mit mehreren Löchern und ein im Behälter enthaltenes härtbares Polymer aufweist.
- In einer Form weist die Erfindung auf: einen orthopädischen Knochenplattenvorläufer mit einem flexiblen Behälter mit mehreren Durchgangslöchern; und einem härtbaren Polymer im Behälter und ferner mit einer Strukturstütze mindestens teilweise im vollen Behälter. Der Knochenplattenvorläufer wird an einem Knochen angebracht bzw. befestigt, indem der Behälter am Knochen plaziert wird; der Behälter mit mehreren Befestigungselementen am Knochen befestigt wird, die sich durch den Behälter erstrecken; ein Polymer in den Behälter eingespritzt wird; und das Polymer im Behälter gehärtet wird.
- Ein Vorteil der Erfindung ist, daß die orthopädische Knochenplatte an die Form des Knochens anpaßbar ist, an dem sie angebracht wird.
- Ein weiterer Vorteil ist, daß eine Strukturstütze im Behälter positioniert sein kann, um der Knochenplatte verstärkte Struktursteifigkeit zu verleihen.
- KURZE BESCHREIBUNG DER ZEICHNUNGEN
- Diese sowie weitere Merkmale und Vorteile der Erfindung, die Art und Weise ihrer Realisierung sowie ein besseres Verständnis der Erfindung gehen aus der nachfolgenden Beschreibung von Ausführungsformen der Erfindung im Zusammenhang mit den beigefügten Zeichnungen deutlicher hervor. Es zeigen:

Die exemplarischen Darstellungen hierin veranschaulichen nur eine bevorzugte Ausführungsform der Erfindung in einer Form, und solche exemplarischen Darstellungen sind als keinerlei Einschränkung der Erfindung aufzufassen.
- NÄHERE BESCHREIBUNG DER ERFINDUNG
- In den Zeichnungen und insbesondere
1 bis3 ist eine Ausführungsform einer orthopädischen Knochenplatte der Erfindung dargestellt, die an Wirbeln12 angebracht wird und dazu dient, die Wirbel in einer Wirbelsäule zu fixieren. - Allgemein verfügt die orthopädische Knochenplatte über einen flexiblen Beutel
16 , eine darin angeordnete Strukturstütze18 sowie ein hochfestes Polymer, das die Strukturstütze18 umgibt und im Beutel16 angeordnet ist. Der Beutel16 ist allgemein H-förmig und so konfiguriert, daß er drei nebeneinanderliegende Wirbel12 relativ zueinander fixiert. Der Beutel16 hat ein erstes Paar Löcher22 , ein zweites Paar Löcher24 und ein drittes Paar Löcher26 . Jedes Paar Löcher22 ,24 und26 ist einem jeweiligen Wirbel12 zugeordnet und ermöglicht den Durchgang eines Befestigungselements, z. B. einer Knochenschraube28 , um die orthopädische Knochenplatte an den drei zugeordneten Wirbeln12 anzubringen. - Der Beutel
16 kann aus jedem geeigneten Material hergestellt sein. Darstellungsgemäß ist der Beutel16 ein konturierter Beutel mit einer spezifischen H-Form in der Ausführungsform gemäß1 bis3 . Allerdings kann das Material, aus dem der Beutel16 aufgebaut ist, Expansion unter Druck in gewissem Maß ermöglichen. Außerdem kann der Beutel16 aus einem porösen oder nicht porösen Material aufgebaut sein, wodurch ihn mindestens ein Teil des darin angeordneten Polymers20 durchdringen oder durchsickern und den Beutel16 mit einem benachbarten Knochen verkleben kann. Das poröse Material würde für die Entlüftung sorgen, wenn das Polymer in den Beutel eingespritzt wird. - Die Strukturstütze
18 hat die Form einer metallischen, hohlen Einspritzröhre, die im Beutel16 angeordnet ist. Die Einspritzröhre18 verfügt über einen sich vom Beutel16 erstreckenden Einlaßanschluß30 und vier Auslaßanschlüsse, die jeweils den Schenkeln des H-förmigen Beutels16 zugeordnet sind. Ein Füllschlauch34 ist am Einlaßanschluß30 an seinem einem Ende und an einer Druckquelle für Polymer (nicht gezeigt) an seinem entgegengesetzten Ende angebracht. Das Polymer20 wird unter Druck durch den Füllschlauch34 und die Einspritzröhre18 in das Innere des Beutels16 eingespritzt, um so den Beutel16 mit einem Sollfülldruck zu füllen. - Das Polymer
20 wird im Beutel16 gehärtet, um eine im wesentlichen steife orthopädische Knochenplatte zu bilden. Das Polymer20 ist ein hochfestes Polymer, z. B. PMMA, das bei Einwirkung von Energie, z. B. Wärmeenergie, Lichtenergie oder Röntgenenergie, oder durch Zugabe eines chemischen Katalysators härtbar ist. Ist der Beutel16 als poröser Beutel aufgebaut, durchdringt ihn das Polymer20 mindestens teilweise und kann so ausgewählt sein, daß es biologisch resorbierbar ist. - Während des Eingriffs wird die Wirbelsäule posterior freigelegt, um Zugang zu den drei nebeneinanderliegenden Wirbeln
12 zu haben. Die orthopädische Knochenplatte wird über den Wirbeln12 gemäß1 und2 plaziert. Die Lage von Führungslöchern36 an jedem Wirbel12 wird markiert, und sie werden in den Wirbeln12 gebohrt. Bei der Bildung der Führungslöcher36 kann die orthopädische Platte an den Wirbeln12 plaziert bleiben oder von den Wirbeln12 abgenommen sein. Danach werden die Knochenschrauben28 durch jedes jeweilige Loch22 im Beutel16 geführt und in jedes entsprechende Füh rungsloch36 eingeschraubt. Anschließend wird das Polymer20 durch den Füllschlauch34 und die Einspritzröhre18 unter Druck in den Beutel16 eingespritzt. Danach wird das Polymer20 im Beutel16 gehärtet, entweder durch Einwirkung von Energie, z. B. Wärmeenergie, Lichtenergie oder Röntgenenergie, oder durch die Zugabe eines chemischen Katalysators vor oder während des Einspritzverfahrens. Im Anschluß daran wird der Füllschlauch34 vom Einlaßanschluß30 gelöst oder abgeschnitten. Schließlich wird der Einschnitt über der Wirbelsäule geschlossen. - In
4 ist eine weitere Ausführungsform einer orthopädischen Knochenplatte40 der Erfindung gezeigt. Die orthopädische Knochenplatte40 ähnelt der orthopädischen Knochenplatte gemäß1 bis3 in dem Sinn, daß sie auch zur Fixation nebeneinanderliegender Wirbel12 relativ zueinander dient. Die orthopädische Knochenplatte40 weist einen flexiblen Beutel42 und ein Paar Strukturstützen in Form eines Paars Einspritzröhren44 darin auf. Jede Einspritzröhre44 ist in einem entsprechenden Vorsprung46 angeordnet, der sich vom Beutel42 erstreckt. Ein Paar Füllschläuche48 ist jeweils an den Einspritzröhren44 angebracht. - Während des Eingriffs wird die Wirbelsäule posterior freigelegt, und die orthopädische Knochenplatte
40 wird benachbart zu einem Paar Wirbel12 plaziert, die relativ zueinander zu fixieren sind. Führungslöcher50 werden markiert und in jedem Wirbel12 gebohrt. Danach wird die orthopädische Knochenplatte40 mit jedem Wirbel12 so verbunden, daß sich jeder Vorsprung46 in ein entsprechendes Führungsloch50 erstreckt. Anschließend wird ein hochfestes Polymer52 unter Druck in die orthopädische Knochenplatte40 eingespritzt. Der Beutel42 ist vorzugsweise als poröser Beutel aufgebaut, wodurch ihn das Polymer52 durchdringen und dadurch jeden Vorsprung46 im entsprechenden Führungsloch50 verkleben kann. Danach wird das Polymer52 durch Einwirkung von Energie, z. B. Wärmeenergie, Lichtenergie oder Röntgenenergie, oder durch die Zugabe eines chemischen Katalysators gehärtet. - In
5 ist eine weitere Ausführungsform einer orthopädischen Knochenplatte60 der Erfindung gezeigt. Die ortho pädische Knochenplatte60 verfügt über eine Einspritzröhre64 , Vorsprünge66 und Füllschläuche68 ähnlich wie in der Ausführungsform der orthopädischen Knochenplatte40 gemäß4 . Der Hauptunterschied zwischen der orthopädischen Knochenplatte60 und der orthopädischen Knochenplatte40 ist, daß ein Beutel62 einen akkordeonförmigen Zwischenverbindungsabschnitt70 aufweist, durch den der Abstand und die Winkelorientierung zwischen den Vorsprüngen66 je nach genauer Plazierungslage und Orientierung von Führungslöchern72 in den Wirbeln12 etwas variieren kann. - In
6 bis9 ist eine weitere Ausführungsform einer orthopädischen Knochenplatte80 der Erfindung gezeigt, die an einem Knochen in Form einer proximalen Tibia82 angebracht wird. Die Tibia82 hat eine Gelenkauflagefläche84 , die nicht richtig zu einer gepaarten Gelenkauflagefläche eines distalen Femurs ausgerichtet war. Folglich wird ein Knochenkeil in einem Verfahren entfernt, das in der Praxis als Keilosteotomie der Tibia bekannt ist. Nach Entfernung des Keils werden die Knochenflächen auf bekannte Weise in Kontakt gebracht. Die orthopädische Knochenplatte80 wird an der Tibia82 mit Knochenschrauben90 angebracht, die Löcher92 und einen Beutel94 durchlaufen und in Führungslöchern96 in der Tibia82 eingeschraubt aufgenommen werden. Eine Strukturstütze98 im Beutel94 weist einen Einlaßanschluß100 auf, wodurch ein Polymer104 unter Druck durch Auslaßanschlüsse102 im Beutel94 eingespritzt werden kann. Danach wird das Polymer104 im Beutel94 gehärtet. - Angebracht wird die Knochenplatte am Knochen durch Plazieren eines Beutels am Knochen;
Befestigen des Beutels am Knochen mit Hilfe mehrerer Befestigungselemente, die sich durch den Beutel erstrecken;
Einspritzen eines Polymers in den Beutel; und
Härten des Polymers im Beutel. Zweckmäßig kann der Einspritzschritt das Einspritzen des Polymers unter Druck in den Beutel aufweisen. Vorteilhaft kann der Befestigungsschritt vor dem Einspritzschritt erfolgen. Zweckmäßig kann der Härtungsschritt nach dem Einspritzschritt durchgeführt werden. - Der Beutel kann ferner eine Strukturstütze mindestens teilweise im Beutel aufweisen. Zweckmäßig kann die Strukturverstärkung eine hohle Einspritzröhre aufweisen, wobei der Einspritzschritt mit Hilfe der Einspritzröhre durchgeführt wird. Vorteilhaft kann die hohle Einspritzröhre Metall aufweisen.
- Die Einspritzröhre kann auch einen Füllschlauch aufweisen, der an der Einspritzröhre angebracht ist, und nach dem Einspritzschritt kann ein weiterer Schritt des Abschneidens des Füllschlauchs durchgeführt werden.
- Zweckmäßig kann der Beutel einen flexiblen Beutel aufweisen.
- Angebracht wird die Knochenplatte am Knochen durch
Plazieren eines Beutels an einem Knochen;
Befestigen des Beutels am Knochen;
Einspritzen eines Polymers in den Beutel; und
Härten des Polymers im Beutel.
Claims (22)
- Orthopädischer Knochenplattenvorläufer, der aufweist: einen flexiblen Behälter (
16 ,42 ,94 ) mit mehreren Durchgangslöchern (22 ,24 ,26 ,92 ); und ein härtbares Polymer (20 ,52 ,104 ), das im Behälter (16 ,42 ,94 ) enthalten ist. - Orthopädischer Knochenplattenvorläufer nach Anspruch 1, ferner mit einer Strukturstütze (
18 ,44 ,64 ,98 ) mindestens teilweise im Behälter (16 ,42 ,94 ). - Orthopädischer Knochenplattenvorläufer nach Anspruch 2, wobei die Strukturstütze (
18 ,44 ,64 ,98 ) eine hohle Einspritzröhre (18 ,44 ,64 ,98 ) zum Einspritzen des Polymers (20 ,52 ,104 ) aufweist. - Orthopädischer Knochenplattenvorläufer nach Anspruch 3, wobei die hohle Einspritzröhre (
18 ,44 ,64 ,98 ) Metall aufweist. - Orthopädischer Knochenplattenvorläufer nach Anspruch 3 oder 4, wobei die hohle Einspritzröhre (
18 ,102 ) allgemein H-förmig ist. - Orthopädischer Knochenplattenvorläufer nach einem der vorstehenden Ansprüche, wobei die Strukturverstärkung (

20 ,52 ,104 ) mit Wärmeenergie, Lichtenergie, Röntgenenergie oder einem chemischen Katalysator härtbar ist. - Orthopädischer Knochenplattenvorläufer nach einem der vorstehenden Ansprüche, wobei das Polymer (
20 ,52 ,104 ) ein biologisch resorbierbares Polymer aufweist. - Orthopädischer Knochenplattenvorläufer nach einem der Ansprüche 1 bis 11, wobei das Polymer (
20 ,52 ,104 ) Polymethylmethacrylat aufweist. - Orthopädischer Knochenplattenvorläufer nach einem der vorstehenden Ansprüche, wobei der Behälter (

40 ,60 ). mit einem flexiblen Behälter (42 ,62 ), mindestens einer Strukturstütze (44 ,64 ) mindestens teilweise im flexiblen Behälter und einem härtbaren Polymer im Behälter, das die mindestens eine Strukturstütze (44 ,64 ) umgibt, wobei der flexible Behälter mindestens einen Vorsprung (46 ) aufweist, wobei der Vorsprung (46 ,66 ) eine entsprechende Strukturstütze hat, die sich mindestens teilweise darin erstreckt. - Orthopädischer Knochenplattenvorläufer nach Anspruch 18, wobei jeder Vorsprung ein Polymerbefestigungselement zum Anbringen am Knochen bildet.
- Ausstattungssatz für einen orthopädischen Knochenplattenvorläufer, der aufweist: i) einen Behälter (

20 ,52 ,104 ) vor einem Eingriff zur Herstellung eines Vorläufers nach einem der Ansprüche 1 bis 19 oder eines Ausstattungssatzes nach Anspruch 20 oder 21 zur Fertigung einer orthopädischen Knochenplatte zum Gebrauch beim Abstützen unterschiedlicher Knochen relativ zueinander oder beim Abstützen von Stücken eines einzelnen Knochens während des Heilungsprozesses.
