Description
- Diese Erfindung bezieht sich auf einen Satz von Instrumenten für die Wiederherstellung von beschädigtem Gewebe, das an oder auf der Oberfläche von Knochen vorliegt, und/oder zum Füllen von Kavitäten an der Oberfläche der Knochen oder in den Knochen (z.B. Condyli eines Knochengelenks) bei einem Tier, einschließlich eines Menschen.
- Im Folgenden wird Bezug auf die Reparatur von beschädigtem Knorpel genommen. Es sollte klar sein, dass das beschädigte Gewebe auch ein anderer Gewebetyp sein kann (z.B. Knochen, Haut), einschließlich beschädigter Oberflächen des Knochens selbst oder Defekte in Knochen selbst. Im Folgenden wird auch auf die Reparatur bzw. die Wiederherstellung des Knorpels von Kniegelenken Bezug genommen, und wiederum sollte einzusehen sein, dass die vorliegende Erfindung auf andere Körpergelenke und darüber hinaus auf andere Organe des Körpers angewendet werden kann, die aus Knochen oder Haut bestehen oder Knochen oder Haut enthalten.
- HINTERGRUND DER ERFINDUNG
- Defekte in der Gelenkfläche des Kniegelenks, speziell bei jungen aktiven Personen, stehen derzeit bei orthopädischen Chirurgen im Brennpunkt des Interesses. Eine Schädigung am Knorpel, der die Gelenke schützt, kann entweder aus einer physikalischen Verletzung (z.B. osteochondrale Fraktur, sekundäre Schädigung in Folge einer Kreuzbandverletzung) oder aus einer Erkrankung (z.B. Osteoarthritis, rheumatoide Arthritis, aseptische Nekrose, Osteochondritis dissecans) resultieren. Osteoarthritis resultiert aus einem allgemeinen Verschleiß und einer Ruptur von Gelenken und ist bei älteren Personen gängig. Rheumatoide Arthritis ist ein inflammatorischer Zustand, der in der Zerstörung des Knorpels resultiert. Es wird angenommen, dass sie wenigstens teilweise eine Autoimmunerkrankung ist, wobei die Betroffenen eine genetische Prädisposition für die Erkrankung haben. Die orthopädische Prävention/Wiederherstellung bzw. Reparatur von beschädigten Gelenken ist eine signifikante Belastung für den medizinischen Beruf, sowohl bezüglich der Kosten als auch des Zeitaufwands für die Behandlung der Patienten.
- Eine Arzneimittelintervention zur Verbesserung oder Prävention des Einsetzens von Knorpelverlust ist verfügbar, hat aber signifikante Nachteile.
- Als Alternative für eine Arzneimittelintervention ist unter Vermeidung unerwünschter Nebenwirkungen die orthopädische Chirurgie verfügbar, um Defekte wiederherzustellen bzw. zu reparieren, und eine Gelenkflächenschädigung zu verhindern, durch die es zu ernsten degenerativen Veränderungen im Gelenk kommt. Solche Änderungen können in der Notwendigkeit für einen vollständigen Knieersatz resultieren, der insbesondere bei jungen aktiven Personen mit einer langen Lebenserwartung unerwünscht ist. Wenn die Lebenszeit des Implantats kürzer als die des Patienten ist, kann ein Revisionseingriff erforderlich sein. Solche Revisionseingriffe sind im Hinblick auf die Unannehmlichkeiten für den Patienten vorzugsweise zu vermeiden. Darüber hinaus sind Implantatrevisionseingriffe sowohl lang als auch sehr kostspielig.
- Die Verwendung von chirurgischen Techniken zur Reparatur/zum Ersetzen von beschädigtem Gewebe in Gelenken erfordert oft die Entfernung und Spende von gesundem Gewebe, um das beschädigte oder erkrankte Gewebe zu ersetzen. Es gibt drei Quellen für Übertragungsgewebe, das bei der Gewebemanipulation dieses Typs eingesetzt wird:
- i) Autotransplantant: Gewebe wird aus einem Bereich des Patienten entfernt, von der Region, die wiederherzustellen ist, entfernt und zu der geschädigten Region transplantiert, um die Wiederherstellung bzw. Reparatur durchzuführen;
- ii) Allotransplantat: Gewebe wird von einem Spenderindividuum, zum Beispiel einem Leichnam, entfernt und in die beschädigte Region transplantiert und
- iii) Xenotransplantat: Gewebe wird aus einer anderen Tierart, zum Beispiel einem Schwein, entnommen und über dem beschädigen Bereich angeordnet.
- Autotransplantate können aufgrund der begrenzten Verfügbarkeit von geeignetem Gewebe und des dem Patienten während der Entfernung des Gewebes von einem anderen Teil des Körpers als dem beschädigten Bereich zugefügten Traumas problematisch sein. Allotransplantate sind durch die immunologische Reaktivität des Wirts, die Verfügbarkeit von geeignetem Spendergewebe und die Probleme der Übertragung von infektiösen Mitteln begrenzt. Xenotransplantate sind sogar noch problematischer, und zwar in Folge der schweren immunologischen Reaktivität.
- Verschiedene Techniken zur Knorpelreparatur bzw. Knorpelwiederherstellung sind in begrenzter derzeitiger Verwendung oder in der Entwicklung, allerdings allgemein offenbart. Das Osteochondral Autogenous Transplant System (OATS) von Arthrex Inc. ist wohl das am umfangsreichsten verwendete Verfahren. Osteochondrale Pfropfen werden von einem gesunden Spender entnommen und insbesondere von einer Stelle entnommen, die als „nicht-Gewicht-tragend" bezeichnet wird. Diese Pfropfen werden in die Stelle des chondralen Defekts transplantiert. Dieses Verfahren wird in erster Linie im Kniegelenk angewendet.
- Allerdings gibt es im Knie keine Spenderstellen mit Knorpel einer Dicke, die vergleichbar mit der der defizienten Stelle ist, die auch als „nicht-Gewicht-tragende" Bereiche beschrieben werden können. Sulcus terminalis, eine häufig verwendete Stelle zur Entnahme solcher Transplantate, ist in direktem Kontakt mit dem lateralen Miniskus in der Position der vollen Kniestreckung und ist daher eine Gewicht-tragende Stelle.
- Darüber hinaus kann die Entnahme eines großen osteochondralen Pfropfens aus dem Sulcus terminalis bewirken, dass der laterale Miniskus locker wird, und somit seine lasttragende Funktion verschlechtern. Als Resultat würden die gesamten tibiofemoralen Lasten auf den kleinen Bereich eines direkten Kontakts zwischen dem Femur und der Tibia übertragen. Die resultiertenden Spannungen könnten so hoch sein wie die, die nach Meniskektomie ver ursacht werden, und zwar mit ihren degenerativen nachfolgenden Änderungen beim Knorpel des tibialen Plateaus. Solche Änderungen wurden immer als Vorläufer für Osteoarthritis angesehen.
- Obgleich das OATS-Verfahren eine vernünftige Technik, einschließlich guter Instrumente, zum Transplantieren lebender Autotransplantate zur Wiederherstellung oder Reparatur von Defekten im Knorpel bereitstellt, involviert es die Einführung von potentiell schädigenden Wirkungen an anderen Stellen mit den ernsten oben diskutierten Nachteilen. Außerdem schafft die Entnahme eines Pfropfens aus einer Spenderstelle eine neue Schädigung in der Kniegelenkfläche. Aus diesem Grund wäre OATS für die Reparatur von großen Defekten nicht geeignet. Die Verwendung von OATS für kleine Reparaturen würde wahrscheinlich die Größenordnung des oben diskutierten Problems beschränken, wäre aber auch eine Grenze für die Indikation zur Verwendung dieser Technik.
- Die Technik, die als Autogenous Chondrocyte Implants (ACI) von Genzyme Inc. bekannt ist, ist ein vom Konzept her eleganter Ansatz, der zunehmend Popularität gewinnt, aber noch eine begrenzte Verwendung hat. Das Verfahren ist zur Reparatur bzw. Wiederherstellung von kleinen wie auch von großen unregelmäßigen Defekten bestimmt und wird in einer Zweistufenoperation durchgeführt. In der ersten Stufe werden Chondrozyten (Knorpelzellen) vom Patienten entnommen bzw. geerntet (harvested) und in Suspension kultiviert. In der zweiten Stufe des operativen Verfahrens wird ein Knorpelrest von der Reparaturstelle entfernt (cleared). Die Stelle wird dann mit einem Stück Periostgewebe bedeckt, das am Umfang des Reparaturbereichs angenäht wird. Die Chondrozyten werden dann in die Reparaturstelle unter Verwendung einer hypodermen Spritze injiziert, wobei das Periosteum mit der Nadel der Spritze punktiert wird. In einer Variation dieses Verfahrens wird das Periostgewebe in der ersten Stufe der Reparatur auf die Reparaturstelle bzw. Wiederherstellungsstelle gelegt, um sicherzustellen, dass zu der Zeit, zu der die Chondrozyten injiziert werden sollen, eine adäquate Abdichtung zwischen dem Gewebe und dem Perimeter des Knorpels gebildet ist. Es gibt in jeder Version des Verfahrens eine hohe Wahrscheinlichkeit, dass Chondrozyten durch das Loch der hypodermen Nadel entweichen.
- Ein weiteres Problem bei der zweiten Version des Verfahrens ist die Wahrscheinlichkeit, dass Gewebeadhäsionen zwischen dem Periostgewebe und dem Boden der Reparaturstelle bzw. Wiederherstellungsstelle auftreten.
- Dieses Verfahren hat keine etablierte Erfolgsrate und die Qualität des Knorpels an der Reparaturstelle ist fraglich. Wie bei dem OATS-Verfahren ist dieses Verfahren nicht minimal invasiv. Darüber hinaus ist es ein äußerst teures Verfahren. Es hat auch den Nachteil, dass es zwei chirurgische Arbeitsgänge erfordert, obgleich die erste Stufe weniger invasiv ist, da sie arthroskopisch durchgeführt werden kann.
- Ein Verfahren, das von Smith & Nephew vorgeschlagen wurde, involviert die Produktion von Knorpelscheiben, die durch allogene Chondrozytenkultur an einem absorbierbaren Tex tilgewebe gebildet werden. Die Scheiben werden im Labor wachsen gelassen, die allogenen Chondrozyten werden auf der Matrix eines Vliesnetzes aus biologisch absorbierbarem Material, typischerweise Polyglycolsäure, kultiviert. Wenn dieses Verfahren beendet ist, wird die Scheibe für eine Implantation an der Reparaturstelle geliefert.
- Ein Vorteil dieses Verfahrens ist, dass es keine Schädigung an einer intakten gesunden chondralen Stelle involviert, da das Verfahren allogene Quellen verwendet. Darüber hinaus wird das Verfahren in einer einstufigen Operation beendet.
- Die Scheiben können in verschiedenen Größen hergestellt werden, es muss aber eine Grenze bezüglich der Größe des Defekts bestehen, welcher mit einer lockeren Scheibe repariert werden kann, welche nur auf die Reparaturstelle gelegt wird. Das Implantat könnte sich frei im Gelenk bewegen. Es könnte unter dem Einfluss von Tangentialkräften faltig werden und könnte als Resultat vollständig beschädigt werden. Dieses Problem würde durch einen niedrigen Druckmodul des Materials verschlimmert werden.
- Ein weiterer Nachteil dieses Verfahrens ist, dass das Material, das ein Allotransplantat ist, die Gefahr einer Virusinfektion, zum Beispiel durch das HIV-Virus, birgt. Obgleich dies ein kleines Risiko ist, ist es ein inhärentes Problem bei einem Allotransplantat.
- Ein weiteres Problem, das mit diesem Transplantattyp zu erwarten ist, ist der Druckmodul des Materials. Er kann ziemlich niedrig sein und das Material könnte einer mechanischen Konditionierung bedürfen (ein zeitaufwändiges und teures Verfahren), um einen Modul zu erreichen, der mit dem von Knorpeln des umgebenden Bereichs kompatibel ist.
- Das Depuy-Knorpelreparatursystem ist eine Scheibe aus Vliesgewebe (non-woven fabric), hergestellt aus biologisch absorbierbaren Material, welches ein hartes Substrat ist, das es ermöglicht, dass das Implantat an dem Knochen befestigt wird. Die Form der Scheibe erlaubt eine Reparatur von beschädigten Bereichen unregelmäßiger Formen durch Verwendung einer Vielzahl von Scheiben in einem eng gepackten Array. Die Nachteile bei diesem System sind, dass die Verwendung von zu vielen benachbarten hexagonalen Scheiben in einer großen Schädigung am Knochensubstrat resultieren wird, und dass die Technik außerdem beachtliches Fachwissen erfordert und dass ihre Anwendung auch zeitaufwändig sein kann.
- Aus der
US-A-5,632,747 ,15 bis17 , ist bekannt, eine Reibvorrichtung bzw. einen Reamer bereitzustellen, umfassend ein Rohr40 mit einer Schnittkante an einem Ende und einen Stab220 , der nahe an einem Ende einen Zylinder44 angeordnet hat, und wobei angrenzend an den Zylinder der Stab mit einem Punkt ausgestattet ist, der bei Anwendung auf die Oberfläche des geschädigten Knochengewebes als Führung für die Reibvorrichtung wirkt. - Gemäß der Erfindung wird ein Satz von Instrumenten für die Reparatur bzw. Wiederherstellung von beschädigtem Knochengewebe an einem Knochenort bereitgestellt, umfassend:
eine Reibvorrichtung bzw. einen Reamer zum Bilden eines schmalen Einschnitts an oder auf der Oberfläche des Knochenorts, um beschädigtes Knochengewebe, das wiederhergestellt werden soll, zu umgeben, wobei die Vorrichtung umfasst: ein Rohr, das eine Schnittkante an einem Ende aufweist; einen Führungsstab und einen Führungszylinder, die im Inneren des Rohrs angebracht sind, um die Reibvorrichtung zu führen; eine Spitze an einem Ende des Stabs, um in das beschädigte Knochengewebe einzugreifen und dadurch den Ort des schmalen Einschnitts zu bestimmen, der durch die Schnittkante der Reibvorrichtung bzw. der Bohrvorrichtung bzw. des Reamers gebildet wird;
eine Knochengewebeentfernungsvorrichtung zum Entfernen von beschädigtem Knochengewebe, das von dem schmalen Einschnitt eingefasst ist, und wodurch ein Raum definiert wird, in welchen ein Polster (Pad) aus Ersatzmaterial eingesetzt werden kann;
dadurch gekennzeichnet, dass
der Satz von Instrumenten des Weiteren einen halbkreisförmigen Schutz, der in den schmalen Einschnitt eingreifen kann, um mit der Knochengewebeentfernungsvorrichtung zusammenzuarbeiten, und Verankerungsmittel umfasst, die Haltemittel an dem Knochenort verankern, um das Polster aus Ersatzmaterial an dem Ort zu halten, wobei die Verankerungsmittel eine rohrförmige Druckvorrichtung umfassen, die wirksam ist, um einen Teil der Haltemittel in den schmalen Einschnitt zu drücken, und dadurch das Polster aus Ersatzmaterial in Position zu halten. - Vorzugsweise ist die Gewebeentfernungsvorrichtung eine Schabevorrichtung.
- Alternativ kann die Gewebeentfernungsvorrichtung die Form einer Drahtbürste annehmen und ist vorzugsweise mit Führungsmitteln ausgestattet.
- Vorzugsweise nimmt das Haltemittel die Form eines dünnen Netzgewebes an, welches in den Einschnitt mittels der rohrförmigen Druckvorrichtung einführbar ist.
- Die rohrförmige Vorrichtung kann mit dem Haltemittel und dem Polster aus Ersatzmaterial vorbeladen werden, um die Aufbringung von Ersatzmaterial auf einen Bereich, aus dem beschädigtes Gewebe entfernt worden war, zu beschleunigen.
- Die rohrförmige Vorrichtung kann außerdem einen Haltering umfassen, der verschiebbar um die rohrförmige Vorrichtung angebracht ist, um das Haltemittel zu befestigen und das Aufbringen des Polsters aus Ersatzmaterial auf einen Bereich, der wiederhergestellt werden soll, zu vereinfachen.
- Die Gewebeentfernungsvorrichtung kann auch die Form eines Schnittkopfs annehmen, der drehbar auf einem Halteelement bzw. Trägerelement angebracht ist, wobei ein solches Element vorzugsweise die Form eines Schafts hat. Die unterste Fläche des Schnittkopfes kann im Wesentlichen flach sein.
- Alternativ kann die Gewebeentfernungsvorrichtung einen Kopf umfassen, der drehbar an einem Halteelement angebracht ist und mit wenigstens einem Vorsprung versehen ist.
- KURZE BESCHREIBUNG DER ZEICHNUNGEN

22 stellt ein Gerät zur Stimulation der Angiogenese an einer wiederherzustellenden Stelle dar. - DETAILLIERTE BESCHREIBUNG DER ERFINDUNG
- Die Erfindung wird nun anhand der beigefügten Zeichnungen und anhand von Beispielen näher beschrieben.
- Was
1 der beigefügten Zeichnung angeht, so ist ein Teil eines Kniegelenks1 , einschließlich Knochen3 mit darüber liegendem Knorpel5 , dargestellt. Das Verfahren der vorliegenden Erfindung involviert die Bildung eines ringförmigen Raums oder eines ringförmigen Einschnitts7 , der sich durch den Knorpel und in den Knochen erstreckt, der innerhalb des Knochens auf einer Ebene endet, die ein mehrfaches der Knorpeltiefe ist, zum Beispiel das 4- oder 5-fache der Tiefe des Knorpels ist. - Die Entfernung des beschädigten Knorpels aus dem Knochenbereich, der durch den Einschnitt
7 definiert wird, resultiert in einem Raum, in den ein kleines Stück oder ein Polster aus biokompatiblem Material9 lokalisiert wird. Polster9 hat eine solche Form und solche Abmessungen, dass es im Wesentlichen den gesamten Raum, der vorher durch Knorpel besetzt war, besetzt, und die Tiefe von Polster9 entspricht etwa der des umgebenden Knorpels5 . -
12 zeigt eine alternative Polsteranordnung. In diesem Beispiel bilden das Polster und die Haltefolie eine integrale Einheit, welche die Anbringung an einem wiederherzustellenden Bereich erleichtert. - Der in
1 dargestellte Einschnitt hat eine kreisförmige Form. Alternative Formen werden in Betracht gezogen.14 zum Beispiel zeigt eine polygonale Einschnittanordnung. Es wird klar sein, dass Polster aus Ersatzmaterial angepasst werden, um Unterschieden in der Einschnittsanordnung Rechnung zu tragen. - Polster
9 ist aus einem Vliesgewebe eines bioverstärkenden Materials hergestellt, welches entwickelt wurde, um eine Zellrekrutierung zu begünstigen, die das Vielfache derjenigen von unbehandeltem Material ist. - Polster
9 kann auch mit einer Rate biologisch absorbierbar sein, die so konzipiert ist, dass sie mit der Entwicklung einer neuen Knorpelschicht einhergeht, welche an dem darunter liegenden Knochen und dem umgebenden Knorpel5 befestigt wird. - Eine Gentherapie involviert die Übertragung und die stabile Insertion von neuem genetischen Material in Zellen für die therapeutische Behandlung. Stammzellen oder pluripotente Vorläuferzellen sind geeignete Targets für den Gentransfer, da die verschiedenen durch diese Zellen produzierten Nachkommenlinien das Fremdgen potentiell exprimieren werden.
- Einige Studien in der Gentherapie haben sich auf die Verwendung von hämatopoetischen Stammzellen konzentriert. Gentransfersysteme hoher Effizienz zur hämatopoetischen Progenitorzelltransformation wurden für eine Verwendung untersucht (Morrow, JF, 1976, Ann. NY Acad. Sci 265: 13; Salzar, W et al, 1981 in Organization and Expression of Globin Genes, AR Liss, Inc, New York, S. 313; Bernstein A 1985 in Genetic Engineering: Principles and Methods, Plenum Press, New York, S. 235; Dick JE et al 1986, Trends in Genetics 2: 165). Virale Vektorsysteme zeigen eine höhere Transformationseffizienz als DNA-vermittelte Gentransferverfahren (z.B. CaPO4-Präzipitation und DEAE-Dextran) und zeigen die Fähigkeit, übertragene Gene stabil in eine weite Vielzahl von Zelltypen einzubauen. Rekombinante Retrovi rusvektoren wurden in großem Umfang experimentell eingesetzt, um hämatopoetische Stammzellen und Progenitorzellen zu transduzieren.
- Gentransferverfahren umfassen Mikroinjektions-, Elektroporations-, Liposomen-, Chromosomtransfer- und Transfektionstechniken (Cline MJ 1985, supra). Salser et al verwendeten eine Kalziumpräzipitations-Transfektionstechnik, um das Gen für Methotrexat-resistente Dihydrofolatreduktase (DHFR) oder das Herpes-simplex-Virus-Thymidinkinase-Gen und ein humanes Globingen in murine hämatopoetische Stammzellen zu transferieren. Die In-vivo-Expression der DHFR- und Thymidinkinase-Gene in der Stammzellnachkommenschaft wurde bewiesen (Salser W et al, 1981 in Organization and Expression of Globin Genes, Alan R Liss, Inc, New York, S. 313–334).
- Als Alternative kann das prothetische Material
9 natürlicherweise mit Zellen aus dem Gelenk nach der Operation besät werden. - Es kann ein nützlicher Schritt in dem beschriebenen Knorpelwiederherstellungsverfahren bzw. -reparaturverfahren sein, das Vliespolster mit autologen Zellen von dem Patienten, zum Beispiel Chondrozyten, Fibroblasten, Stammzellprogenitorzellen von Chondrozyten oder Fibroblasten, zu beimpfen bzw. diese auszusäen. Die Quelle dieser würde die Restknorpel an der Defektstelle sein, der mit dem Drehinstrument entfernt wird. Eine Fraktion des Knorpelrestes wird gesunder Knorpel sein. Bei Entfernung dieses Restes wird vorgeschlagen, ihn mit Gewebeentfernungsvorrichtungen, die auf dem Fachgebiet bekannt sind, oder einem chemischen oder mechanischen Mittel, das wirksam gesunde Chondrozyten, Fibroblasten oder Stammzellen freisetzt, zu dezimieren. Ein nicht limitierendes Beispiel einer solchen Vorrichtung wäre ein Dounce-Homogenisator.
- Mit dem Zusatz des geeigneten Mediums zu dem dezimierten Knorpel, der von der Reparaturstelle entfernt wurde, wäre das Resultat eine Zellsuspension, in welcher das Reparaturpolster für einen Zeitraum eingeweicht werden kann, wonach das Pad bzw. Polster gemäß der Erfindung implantiert wird. Die gewebeentfernende Vorrichtung kann mit alternativem Gewebe, zum Beispiel Synovium, das dem Patienten entnommen worden war, verwendet werden, außer dass in diesem Fall die Zellen, die auf das Polster ausgesät werden, Synovialfibroblasten anstelle von Chondrozyten waren. Der Vorteil des obigen ist, dass autologe Zellen verwendet würden und daher vom Patienten nicht abgestoßen würden. Die Verwendung des Polsters (des Pads) stellt sicher, dass viele der Zellen an der Reparaturstelle bleiben. Es wird davon ausgegangen, dass die Zelle proliferieren, was dazu führt, dass das Gewebe induziert wird, das Pad in schnellerer Art zu füllen, als wenn das letztgenannte keine ausgesäten Zellen enthält.
- Sobald das Material
9 an der Stelle positioniert ist, von der das beschädigte Gewebe entfernt wurde, wird ein Stück eines dünnen Netzes/Maschengewebes oder Gewebes11 in der Position, die in1 dargestellt ist, lokalisiert. Das Netzgewebe11 erstreckt sich über das Polster9 und in den kreisförmigen Einschnitt7 , in dem es als Pushfit angeordnet ist. Somit ist Element11 ein Netz, das auch aus einem biologisch absorbierbaren Material hergestellt ist, wobei dieses wiederum so berechnet wurde, dass es mit einer Rate eliminiert ist, die mit dem Wachstum und der Fixierung des neuen Knorpels sowohl am Knochen als auch am umgebenden Knorpel kompatibel ist. Element11 kann auch ein Vliesgewebe aus einem Bioverstärkungsmaterial sein oder kann alternativ ein Gewebestück (zum Beispiel Periosteum, Synovium, Faszies, Retinaculum) sein. - Wenn Knorpel zu reparieren ist, kann Polster
9 in Form einer größeren Folie aus demselben Material geliefert werden, in welcher Ausschnitte verschiedener Größen und Formen mit Hilfe eines Laserschneidemittels oder eines anderen geeigneten Mittels (z.B. Stempel, Wasserstrahl) geformt wurden. Die gewählten Formen sind solche, die unter Verwendung von Standardinstrumenten einfach zu erzeugen oder zu schneiden sind. Ein kleiner Knorpeldefekt wird am besten unter Verwendung eines kreisförmigen Reamers bzw. Bohrers bzw. einer kreisförmigen Reibvorrichtung mit einer dünnen Wand, die Schneidezähne hat, repariert. Für eine unregelmäßige Form ist es bevorzugt, eine Vielzahl prothetischer Elemente zu verwenden, von denen jedes eine einfache Form hat, zum Beispiel ein Kreis, ein Halbmond oder ein Segment eines Kreises. Diese werden dicht gepackt, um den gesamten Reparaturbereich bzw. den gesamten Wiederherstellungsbereich zu bedecken. - In einer alternativen Anordnung wird eine Reparatur an dem beschädigten Bereich durch die Verwendung von wenigstens zwei konzentrisch gebohrten bzw. geriebenen Einschnitten bzw. Rillen durchgeführt, wie es in
10 dargestellt ist. Dies erfordert die Verwendung von zwei Ersatzelementen und wenigstens einer Haltefolie. Ein kreisförmiges Polster aus Ersatzmaterial wird auf den inneren konzentrischen Kreis aufgebracht und ein ringförmiges Polster wird auf den äußeren konzentrischen Kreis aufgebracht. Wenn diese an Ort und Stelle sind, wird jedes der Polster mit wenigstens einer Haltefolie befestigt. - Wie oben angegeben wurde, kann das prothetische Material einen Teil einer größeren Folie bilden, welche eine Bedeckung oder eine „Capping"-Schicht umfasst. Das Material selbst ist in der Form einer dünnen Lage aus Vliesgewebe aus einem geeigneten Gerüstmaterial, das eine ausreichende Festigkeit hat, um in den kreisförmigen Raum gedrückt zu werden, der vorher von dem beschädigten Gewebe besetzt war. Typischerweise besteht das Material aus statistisch angeordneten Fasern. In der oben beschriebenen Ausführungsform ist das Material ein bioabsorbierbares Material. In einer anderen Ausführungsform kann das Material allerdings ein nicht-abbaubares Material sein, das biologisch kompatibel ist und verstärkte Oberflächeneigenschaften hat, um so Gewebewachstum in das Material zu ziehen.
- Die prothetischen Polster bzw. Pads können in Form von Scheiben mit variierenden Dicken bereitgestellt werden, so dass das gewählte prothetische Gerüst eine zu dem angrenzenden Knorpel passende Dicke haben kann. Wie oben angegeben ist, kann es (das Polster) in einer Einlegeform mit Elementen geliefert werden, welche bei Bedarf von einem geeigneten Substrat, welches zum Beispiel aus Karton bestehen kann, abgezogen werden. Dementspre chend kann der Chirurg die geeigneten Elemente, einschließlich erster und anschließender Elemente, auswählen, um einen unregelmäßigen Defekt zu füllen.
- Das Material der prothetischen Elemente ist dasselbe wie das der Deckfolie. Die Struktur ist geschlungen oder statistisch und mit einem biokompatiblen Kleber an Stellen stabilisiert, wo die Filamente des Materials sich kreuzen, oder ist durch Verfilzen stabilisiert.
- Was
2 der beigefügten Zeichnungen angeht, so involviert das Operationsverfahren die Verwendung eines Reamers15 bzw. Reibwergzeugs15 , der/das in der Form eines Rohrs mit kreisförmigem Querschnitt ist, das an einem Ende eine gezahnte Kante17 hat. Das Reibwerkzeug bzw. der Reamer15 ist mit einem dünnen Stahlstab19 (z.B. Kirschner-Draht)19 ausgestattet, der nahe an einem Ende einen Zylinder21 angeordnet hat, welcher einen solchen Außendurchmesser hat, dass er innerhalb der Reibvorrichtung15 eine Schlichtpassung ist. Angrenzend an Zylinder21 hat der Stahlstab19 eine Spitze23 , die dem Stab19 und seinem assoziierten Zylinder21 ermöglicht, als Führung für die Reibvorrichtung15 zu wirken. Bei Verwendung ist die Spitze23 des Stahlstabs19 in der Mitte der Stelle lokalisiert, welche das beschädigte Knorpelgewebe umfasst. Auf den Stahlstab wird ein leichter Druck angewendet. Die Reibvorrichtung15 , die um den Stahlstab19 und den Zylinder21 lokalisiert ist, wird, während sie zum Beispiel mit einer elektrischen Bohrmaschine gedreht wird, einem relativ starken Druck unterworfen, um einen ringförmigen Einschnitt zu schneiden, der sich durch den Knorpel und in den Knochen erstreckt, wie es in1 gezeigt ist. -
3 der beigefügten Figuren zeigt die Position nach Verwendung der Reibvorrichtung bzw. Bohrvorrichtung bzw. des Reamers15 . Ein ringförmiger Einschnitt25 erstreckt sich durch den Knorpel27 und in den Knochen29 zu einer Tiefe, die ein Mehrfaches der Knorpeldicke ist. In diesem Fall umfasst der ringförmige Einschnitt25 die Defektstelle, und der umgebende Knorpel ist gesund.11 stellt einen Fall dar, in dem sowohl Knorpel wie auch Knochengewebe repariert werden. Vor der Aufbringung des Polsters wird Ersatzmaterial zu dem beschädigten Knochen gegeben. Die Reparatur von Knochengewebe kann. mit Knochen- (Feststoff oder pulverisiert), Korallen- oder synthetischem Knochenmaterial erfolgen. - Was die
4 und5 der beigefügten Zeichnungen angeht, so ist dort die Entfernung von Knorpel aus dem durch Einschnitt25 definierten Bereich dargestellt. Um diese Knorpelentfernung durchzuführen, wird ein metallischer Schutz31 mit halbkreisförmigen Querschnitt in den Einschnitt25 eingeführt, um den umgebenden gesunden Knorpel27 zu schützen. Eine Schabevorrichtung33 wird dann verwendet, um die Entfernung des Knorpels durchzuführen, indem bewirkt wird, dass dieses Werkzeug durch die Knorpelschicht penetriert, und dieser dann in Richtung zum Schutz bewegt wird. - Wie oben angegeben wurde, kann beschädigtes Gewebe durch die abrasive Verwendung einer Drahtbürste entfernt werden, wie es in
9 gezeigt ist. Die Verwendung einer Drahtbürste als Alternative zu der Schabevorrichtung, die in den4 und5 gezeigt ist, ist vorteilhaft, da es weniger wahrscheinlich ist, dass sie den darunter liegenden Knochen beschädigt. Die Bürste ist mit einem Führungsmittel ausgestattet, welches die Bewegung der Bürste auf den Bereich des beschädigten Gewebes beschränkt, wodurch eine unabsichtliche Beschädigung an umgebendem gesunden Gewebe verhindert wird. - Das in
17 gezeigte Gerät ist eine weitere Vorrichtung, die zur Entfernung von beschädigtem Gewebe verwendet werden kann.17 zeigt ein Gerät, dass einen Griff60 umfasst, der sich in einen Schaft61 erstreckt, an dem ein Schnittkopf62 , der eine Vielzahl von Schnittkanten63 umfasst, drehbar montiert ist. Alternativ kann der Schnittkopf drehbar an dem Griff montiert sein. - Bei Verwendung wird der Schneidevorgang erreicht, indem der Schaft gedreht wird, während der Kopf über die Knorpeloberfläche bewegt wird und während Druck auf den Schnittkopf angewendet wird. Der Kopf hat ein im Wesentlichen flaches Ende mit scharfkantigen Rillen, die gebildet werden, indem Löcher im Kopf in einer senkrechten Richtung zur Drehachse ausgebildet werden, und indem eine geeignete Menge an Material vom Ende maschinell entfernt wird.
- Dieses Instrument ist zum Schneiden in ein weiches Material, zum Beispiel Knorpel, besonders geeignet, insbesondere um ihn von dem darunter liegenden Knochen zu entfernen, wobei eine minimale Schädigung oder keine Schädigung an diesem letztgenannten verursacht wird. Was
21 angeht, so wölbt sich der Knorpel, der weich ist, wenn das Gerät gegen ihn gedrückt wird, innerhalb des Einschnitts und wird der Schneidwirkung der scharfen Kante des Einschnitts unterworfen. Das entfernte Material entweicht seitlich durch den Einschnitt, wie es in21 gezeigt ist. Das Ende des Instruments, das flach ist, gleitet über die harte und relativ unverformbare Oberfläche des darunter liegenden Knochens (ohne ihn zu schädigen), wenn das Instrument die Knochenoberfläche erreicht, nachdem es die Knorpelschicht entfernt hat. - In Fällen einer einzelnen Reparaturstelle wäre es vorteilhaft, eine kürzere Reibvorrichtung bzw. einen kürzeren Reamer zu verwenden, die/der bei Fertigstellung des Einschnitts in situ gelassen werden kann, um als Schutz für die Bürste während der Entfernung von Knorpel aus der Defektstelle zu wirken.
- Was
6 der beigefügten Zeichnungen angeht, so wird, sobald der beschädigte Knorpel entfernt wurde, ein prothetisches Polster35 mit einer Form, die geeignet ist, den vorher vom beschädigten Knorpel besetzen Raum auszufüllen, in diesem Raum lokalisiert. Ein weiteres Gerät in Form einer rohrförmigen Druckvorrichtung37 wird dann verwendet, um das prothetische Polster an dem Knochen29 zu verankern. Die rohrförmige Druckvorrichtung37 hat eine Wanddicke, die ausreichend dünn ist, damit sie in den Einschnitt25 gedrückt werden kann. Bevor dies erfolgt, wird eine kreisförmige Folie aus Netzgewebe39 (hergestellt aus Vliesgewebe) mit einem Durchmesser des Mehrfachen desjenigen des Polsters35 über das Polster gelegt, so dass es sich über den umgebenden gesunden Knorpel erstreckt. Druckvor richtung bzw. Stanzvorrichtung37 wird dann in den Einschnitt25 eingeführt, wobei sie den äußeren Teil des Netzgewebes39 mit sich trägt. Die Stanzvorrichtung37 wird weiter in den Einschnitt25 bewegt, bis der Außenrand des Netzgewebes39 vollständig in den Einschnitt25 gedrückt ist. Die Stanzvorrichtung35 wird dann entfernt, wobei das Netzgewebe39 in den Einschnitt25 geklemmt bleibt. Das Netzgewebe39 wird das Polster35 an Ort und Stelle halten, bis das Polster, das entweder aus prothetischem Material oder schließlich aus neuem Knorpel gebildet ist, selbst an dem darunter liegenden Knochen29 und dem umgebenden gesunden Knorpel27 befestigt ist. Das Deckfoliengewebe kann Löcher haben, um Knochen und Gewebe innerhalb des Einschnitts hindurchwachsen zu lassen, wodurch die Deckfolie weiter befestigt wird. -
13 zeigt eine alternative Verwendung der Druckvorrichtung37 . In dieser Ausführungsform ist die Druckvorrichtung37 mit einem Polster und einer Haltefolie vor Anwendung auf den Einschnitt beladen. Diese ist insbesondere für die integrale Polster/Haltefolie von12 geeignet und beschleunigt die Aufbringung und Retention des Pads an der Reparaturstelle. Die Haltefolie wird mit einem Ring in Position gehalten, welcher entlang der Stanzvorrichtung gleitet, wenn diese verwendet wird, um das Pad bzw. Polster in der Reparaturstelle zu implantieren und die Haltefolie in den kreisförmigen Einschnitt einzuführen. Durch Beladen der Stanzvorrichtung mit dem Implantat und der Haltefolie kann dieser dem Chirurgen in einer sterilen Packung geliefert werden, die, wenn sie vom Chirurgen geöffnet wurde, in einfacher Weise ohne Notwendigkeit für eine weitere Behandlung durch den Chirurgen verwendet werden kann. Dieses besondere Verpackungsverfahren würde ein Implantieren der Vorrichtung durch kleine Einschnitte, zum Beispiel solche, die bei arthroskopischen oder arthroskopisch unterstützten Eingriffen gemacht werden, weiter erleichtern. - Ein Verfahren, das die vorliegende Erfindung nutzt, kann in Verbindung mit einem Knorpeldefekt angewendet werden, welcher auf einen kleineren Bereich als den eines einzelnen kreisförmigen Polsters begrenzt ist. Wenn der Defekt groß und/oder unregelmäßig ist, kann er mit Hilfe einer Vielzahl von Polstern in Form von Kreisen, Ellipsen, Halbmonden oder anderen einfachen Formen behandelt werden. Wenn nicht-kreisförmige Polster in einer Position befestigt werden, kann eine Stanzvorrichtung verwendet werden, die im Schnitt teilkreisförmig ist, zum Beispiel halbkreisförmig, vierteilkreisförmig usw.

7 stellt die Verwendung eines einzelnen Polsters dar, welches einen größeren Bereich als die Gesamtheit des Defektbereichs hat (7A ).7B stellt die Verwendung eines kreisförmigen Polsters41 und eines angrenzenden halbmondförmigen Polsters43 dar.7C stellt die Verwendung eines elliptischen Polsters45 wie auch eines halbmondförmigen Polsters47 dar. In der Praxis wird der Chirurg wählen, welcher Kombination von Polstern auch immer den Defektbereich am wirksamsten bedeckt. - Folglich ist es machbar, einen wesentlichen Bereich eines Knieknochens mit einer neuen Oberfläche zu versehen, wenn dies erforderlich ist.
- Was
8 der beigefügten Zeichnungen angeht, so ist hier eine andere Ausführungsform dargestellt. In diesem Fall wird ein ganzer Knochenpfropfen51 , der den beschädigten Knorpel umfasst, vom Knochen entfernt. Ein prothetisches Polster53 (auf das dieselben Attribute von Polster9 , das vorher beschrieben wurde, Anwendung finden) wird auf den Knochenpfropfen an Stellen des beschädigten Knorpels gelegt und ein offenes Haltenetzgewebe45 wird um den ganzen Knochenpfropfen und das Polster angeordnet, wodurch das Polster53 an dem Knochenpfropfen51 befestigt wird. Der Knochenpfropfen wird dann erneut innerhalb des Knochens positioniert, wie es in8 gezeigt ist. Der ringförmige Raum, der zwischen den Knochenpfropfen und dem verbleibenden Knochen geschaffen wurde, wird dann durch das Haltenetz55 besetzt. Der Knochenpfropfen und der Wirtsknochen werden durch das Netz55 vereint. - Was die
16 und22 angeht, so sind Vorrichtungen dargestellt, die verwendet werden können, um die Angiogenese an einer Reparaturstelle bzw. Wiederherstellungsstelle zu stimulieren.16 zeigt einen zylindrischen Stab64 , der mit einer Vielzahl von Nadeln65 versehen ist, die verwendet werden können, um in die subchondrale Platte einzustechen, um die Angiogenese zu fördern.22 zeigt eine Vorrichtung, die in ihrer Struktur ähnlich der Vorrichtung in17 ist, bei der aber der Schnittkopf durch einen rotierenden Kopf66 ersetzt ist, der mit wenigstens einem Vorsprung67 ausgestattet ist, der an einer im Wesentlichen flachen Oberfläche68 befestigt ist. Typischerweise haben die Vorsprünge eine Höhe von etwa 1 mm. Die Anwendung des rotierenden Kopfes66 auf eine Gewebeoberfläche, die von beschädigtem Gewebe gereinigt worden war, erzeugt einen abrasiven Effekt auf den Knochen, um die Oberfläche einzukerben, wodurch die Angiogenese stimuliert wird. - Dem Fachmann wird klar sein, dass die entfernbare Natur der rotierbaren Köpfe
62 und66 insoweit vorteilhaft ist, dass Ersatzköpfe einfach und schnell ausgetauscht werden können. Der Griff60 oder der Schaft61 können so angepasst sein, dass neue, nicht gebrauchte Köpfe an dem oberen Teil des Geräts fixiert werden können, wodurch eine kompakte Aufbewahrung für die Köpfe bereitgestellt wird, welche auch die Köpfe vor einer physikalischen Beschädigung schützt.
Claims (11)
- Satz von Instrumenten für die Wiederherstellung von beschädigtem Knochengewebe an einem Knochenort (
3 ) umfassend: eine Reibvorrichtung bzw. einen Reamer (15 ) zum Bilden eines schmalen Einschnitts (7 ) an oder auf der Oberfläche des Knochenorts, um beschädigtes Knochengewebe, das wiederhergestellt werden soll, zu umgeben, wobei die Vorrichtung umfasst: ein Rohr, das eine Schnittkante (17 ) an einem Ende aufweist; einen Führungsstab (19 ) und einen Führungszylinder (21 ), die im Inneren der Rohrs angebracht sind, um die Reibvorrichtung (15 ) zu führen; eine Spitze (23 ) an einem Ende des Stabs (19 ), um in das beschädigte Knochengewebe einzugreifen und dadurch den Ort des schmalen Einschnitts (7 ) zu bestimmen, der durch die Schnittkante (17 ) der Reibvorrichtung (15 ) gebildet wird; eine Knochengewebeentfernungsvorrichtung (33 ,33a ,60 ) zum Entfernen von beschädigtem Knochengewebe, das von dem schmalen Einschnitt (7 ) eingefasst ist, und wodurch ein Raum definiert wird, in welchen ein Polster (9 ) aus Ersatzmaterials eingesetzt werden kann; dadurch gekennzeichnet, dass der Satz von Instrumenten des Weiteren einen halbkreisförmigen Schutz (31 ), der in den schmalen Einschnitt (7 ) eingreifen kann, um mit der Knochengewebeentfernungsvorrichtung (33 ,33a ,60 ) zusammenzuarbeiten, und Verankerungsmittel (37 ) umfasst, die Haltemittel (11 ) an dem Knochenort (3 ) verankern, um das Polster (9 ) aus Ersatzmaterial an dem Ort (3 ) zu halten, wobei die Verankerungsmittel (37 ) eine rohrförmige Druckvorrichtung umfassen, die wirksam ist, um einen Teil der Haltemittel (11 ) in den schmalen Einschnitt (7 ) zu drücken, und dadurch das Polster (9 ) aus Ersatzmaterial in Position zu halten. - Satz von Instrumenten nach Anspruch 1, dadurch gekennzeichnet, dass die Gewebenentfernungsvorrichtung eine Schabevorrichtung (
33 ,60 ) umfasst. - Satz von Instrumenten nach Anspruch 1, dadurch gekennzeichnet, dass die Gewebenentfernungsvorrichtung eine Drahtbürste

37 ) mit dem Haltemittel (11 ) und dem Polster aus Ersatzmaterial (9 ,35 ,53 ) vorbeladen ist, um das Aufbringen von Ersatzmaterial auf ein Gebiet, von welchem beschädigtes Gewebe entfernt wurde, zu beschleunigen. - Satz von Instrumenten nach Anspruch 5 oder 6, dadurch gekennzeichnet, dass die rohrförmige Vorrichtung (
37 ) des Weiteren einen Haltering umfasst, der verschiebbar um die rohrförmige Vorrichtung (37 ) angebracht ist, um das Haltemittel (11 ) zu befestigen und das Aufbringen des Polsters aus Ersatzmaterial (9 ,35 ,53 ) auf ein Gebiet, das wiederhergestellt werden soll, zu vereinfachen. - Satz von Instrumenten nach einem der Ansprüche 1–7, dadurch gekennzeichnet, dass die Gewebeentfernungsvorrichtung einen Schnittkopf (
62 ) umfasst, der drehbar auf einem Halteelement (61 ) angebracht ist. - Satz von Instrumenten nach Anspruch 8, dadurch gekennzeichnet, dass das Halteelement ein Schaft (
61 ) ist. - Satz von Instrumenten nach Anspruch 8 oder 9, dadurch gekennzeichnet, dass die unterste Fläche des Schnittkopfes (
