Description

Die Erfindung betrifft die Verwendung von Dimethylpolysi­ loxan (Dimeticon) zur Behandlung und Prophylaxe von Helico­ bacter pylori (H. pylori)-assoziierten Krankheitsbildern.
Handelsübliche, Dimeticon enthaltende Produkte wie z. B. sab simplex® und Lefax® (vgl. Rote Liste 1993), dienen zur Be­ handlung von Flatulenz bzw. werden zur Vorbereitung der Sonographie eingesetzt.
Aus der WO 90/07930 ist die Verwendung von Dimethylpolysi­ loxan, insbesondere in Verbindung mit Kieselgel, zur Behand­ lung von Erkrankungen des Ösophagus, des Magens und des Duodenums, wie Ösophagitis, Ulcera (ventriculi und duodeni) und Gastritiden bekannt. Diese Druckschrift beschreibt die Fähigkeit von Dimeticon nach oraler Verabreichung im Ösophagus sowie im Magen und Duodenum eine schützende Schicht zu bilden und aufrechtzuerhalten.
Seit neuerem ist bekannt, daß ein Befall des Gastrointesti­ naltraktes mit dem gram-negativen Erreger H. pylori ein pathogenetischer Faktor bei einigen Formen der chronischen Gastritis sowie bei der Ulcusentwicklung (Ulcus ventriculi und duodeni) ist.
Des weiteren wird die durch H. pylori induzierte Gastritis als Präkanzerose angesehen.
Die Therapie von H. pylori-assoziierten Erkrankungen, wie der chronischen Gastritis, Ulcus ventriculi, Ulcus duodeni sowie Magenkrebs, ist unbefriedigend ohne die Eradikation des phatogenetischen Auslösers. Es werden vorwiegend Antazida, H2-Rezeptorenblocker, wie Ranitidin oder Muskarin- Rezeptorblocker eingesetzt, hauptsächlich in Kombination mit Antibiotika bzw. Antitumormittel.
Derartige Maßnahmen ermöglichen jedoch lediglich eine symptomatische Therapie, oder sind mit erheblichen Nebenwir­ kungen verbunden.
Es besteht somit ein Bedarf an einer kausalen Therapie von H. pylori-assoziierten Erkrankungen, die nicht mit den übli­ chen Nebenwirkungen verbunden ist.
Es wurde unerwarteterweise gefunden, daß Dimeticon wirksam zur Behandlung und Prophylaxe von H. pylori-assoziierten Er­ krankungen verwendet werden kann, in dem es den H-pylori eradifiziert.
Die Erfindung betrifft somit die Verwendung von Dimeticon zur Behandlung von H. pylori-assoziierten Erkrankungen, ins­ besondere zur Behandlung von Gastritis, Ulcus ventriculi, Ulcus duodeni sowie Magenkrebs.
In einer randomisierten klinischen Vergleichsstudie konnte festgestellt werden, daß Dimeticon in einer Dosis von 80 mg viermal täglich über 4 Wochen bei Probanden mit der Diagnose Ulcus ventriculi die Besiedlung von H. pylori signifikant vermindert. Ein entsprechendes Ergebnis konnte mit Ranitidin 300 mg abends über 4 Wochen nicht erzielt werden.
Neben der endoskopischen Untersuchung des Magens wurde die Besiedlung mit H. pylori mit Hilfe einer zusätzlichen Biopsie der Antrumschleimhaut überprüft. Die Ergebnisse die­ ser Studie ist in nachstehender Tabelle zusammengefaßt.

Die vorstehenden Daten belegen eindeutig, daß die Verabrei­ chung von Dimeticon zu einer signifikanten bzw. vollständi­ gen Reduktion der H. pylori-Besiedlung führt.
Erfindungsgemäß kann Dimeticon entweder allein oder auch in Verbindung mit weiteren Wirkstoffen, wie Antazida, H2- Blocker, Antitumormittel oder auch Antibiotika, wie Amoxi­ cillin oder Metronidazol verwendet werden.

Claims (5)

1. Verwendung von Dimeticon zur Behandlung und Prophylaxe von Helicobacter pylori-assoziierten Erkrankungen.
2. Verwendung gemäß Anspruch 1 zur Behandlung und Prophy­ laxe von chronischer Gastritis.
3. Verwendung gemäß Anspruch 1 zur Behandlung und Prophy­ laxe von Ulcus ventriculi.
4. Verwendung gemäß Anspruch 1 zur Behandlung und Prophy­ laxe von Ulcus duodeni.
5. Verwendung gemäß Anspruch 1 zur Behandlung und Prophy­ laxe von Magenkrebs.
