Der zylindrische Teil 25 sowie das Ende des Distanzstückes 26 sind mit Eingnuten JlJj für die Aufnahme von Polymethylmethaerylat zur festigkeitsmäßig ausreichenden Verbindung versehen. Außerdem ist der geschliffene Kugelkopf 27 teilweise hohl ausgebildet und auf einem Zapfen j6 aufgesetzt, der entsprechende Nuten 15 enthält, um ein Verdrehen des Kugelkopfes 27 bei Beanspruchung zu vermeiden. Auch hier werden wieder die beiden Teile mit Polymethylmethacrylat vergossen. Sodann wird das Implantat in die Pfanne des Schultergelenkes gelegt und schichtweise zugenäht, wobei Sehnen oder Muskelteile an bestimmten Stellen, nämlich den Bohrungen 31, angebunden bzw. befestigt sind.
Eine Besonderheit des erfinderischen Implantates ist aus Fig. 3 zu entnehmen. Bei diesem einteiligen Implantat für.eine Hüftgelenk-Endoprothese wird zusätzlich mit chirurgischen Schrauben das Troehanter-Teil 29_ am Implantat angeschraubt. Hierzu sind Bohrungen 33. mit unterschiedlichen Durchmesser in der Keramik notwendig. Außerdem befindet sich eine Nut 3.4, in der Bohrung, um als Sicherung gegen Verdrehung zu wirken. In diese Bohrung wird die Schraube hineingesetzt und mit Kunststoff, z. B. Polymethylmethacrylat ausgegossen. Nach der Polymerisation kann die Schraube herausgedreht werden. Dabei besitzt das entstandene Kunststoffgewinde die Eigenschaft, daß es sehr spezifisch an das Gewindeprofil der Schraube angepaßt ist und sich nur mit einem gewissen Widerstand herausdrehen läßt. Diese Eigenschaft dient gleichzeitig als Schraubensicherung. In jeder Stellung bleibt die Schraube fest in ihrem Sitz, auch dann, wenn sie durch Beanspruchungen bewegt wird.

Es hat sich auch als günstig erwiesen, die Trochanter-Majer-Aufsitzfläche 29 mit Zungen 3_5. ^ur äie Aufnahme neu gebildeter Knochensubstanz zu versehen. Die Gesamtheit der Zungenprofile gestattet dann kein Ablösen mehr der natürlichen Knochenpartien. Dies konnte durch vorläufige histologische Untersuchungen auch bewiesen werden, da diese Rillen 32 und Zungen 35 ein Einwachsen der Knochensubstanz fördern.
Die mit der Erfindung erzielten Vorteile bestehen insbesondere darin, daß man die Implantate, wie es bei den üblichen Metall-Endoprothese bekannt ist, nicht mehr in den Markraum des Knochens schlagen oder durch Knochen zement einkitten muß. Die Muffe dient als Trag- und Verbindungselement und soll nicht wie früher ein Aufsplittern des Knochens durch Beanspruchung der eingesetzten Schraube verhindern. Durch die genaue Paßform zwischen der Kegelhülse des Implantates und dem Knochen kommt es zu einer günstigen Kräfteübertragung, da eine flächenhafte Krafteinwirkung auf den ganzen Knochen-Kegelstumpf wirkt, obwohl dieser durch das Abfräsen am Ende schwach ausgebildet ist. Durch die Bearbeitung des Knochenstumpfes entstehen infolge seines unregelmäßigen Querschnittes sogenannte asymmetrische Zungen am Auslauf der Kegelfläche, die ganz besonders geeignet sind, die Ausbildung von Knochensubstanz zwischen Implantat und Knochen zu fördern. Als wichtigster Vorteil dürfte sich aber der Gesichtspunkt erweisen, daß das Knochenmark erhalten bleibt und so dem Knochen die wichtigsten Versorgungswege für das Wachstum weitgehend erhalten bleiben.

Die Erfindung ist aber nicht auf die dargestellten und beschriebenen Ausführungsformen und konstruktiven Modifikationen beschränkt, vielmehr können Einzelheiten abgeändert ,und den jeweiligen Verhältnissen angepaßt werden, ohne vom Grundsystem des Erfindungsgedankens, das Verbindungsproblem zwischen Knochen und Implantat zu lösen, abzuweichen. So ist es gut denkbar, daß man die erfinderischen Einzelteile des Implantates nicht nur für Hüft- und Arm-Endoprothesen einsetzt, sondern die Muffe allgemein als Verbindungselement für alle infrage kommenden Skeletteile benutzt.

    l./Ein- oder mehrteiliges Implantat als dauerhafte Verbindung von Knochenresektionsenden ohne daß Knochenzement verwendet wird und der für die Yersorgung des Resektionsendes wichtige Markraum in Mitleidenschaft gezogen wird gemäß der Hauptpatent— anmeldung P 2 305 329*^-35 unter Verwendung einer fixierenden Halterung, dadurch gekennzeichnet, daß der intramedulläre Schaftteil der Endoprothese weggelassen und eine außen in geeigneter Weise über den Knochen greifende Muffe (ll) paßgenau auf den mechanisch bearbeiteten Teil des Knochenstumpfes (l) aufgesetzt ist.
  2. 2. Ein- oder mehrteiliges keramisches Implantat nach Anspruch 1, dadurch gekennzeichnet, daß der Innenteil der Muffe 11 ein- oder beidseitig als Kegelstumpf (24) geformt ist.
  3. 3· Ein- oder mehrteiliges keramisches Implantat nach Anspruch 1, dadurch gekennzeichnet, daß die eine Seite des Innenteiles der Muffe (ll) als Zylinder (25) ausgebildet ist, um ein Distanzstüek (26) mit oder ohne Gelenkkopf (2?). aufzunehmen.

  4. 4. Ein- oder mehrteiliges keramisches Implantat nach Anspruch 1, dadurch gekennzeichnet, daß eine Verbindung zu den Knochenstümpfen (l) durch zwei Muffen (ll) mit Boden (23) und jeweils einseitigem Kegelstumpf (24) mittels einer Überschubklemme (l7) erreicht ist.
  5. 5. Ein- oder mehrteiliges keramisches Implantat nach Anspruch 1, dadurch gekennzeichnet, daß die Muffe ("11) auf ihrer Innenseite ein verrundetes Profil besitzt, wobei die Steigung und die Gewindetiefe ein Zahlenverhältnis von 1 - 20 fo haben.
  6. 6. Ein- oder mehrteiliges keramisches Implantat nach Anspruch 1, dadurch gekennzeichnet, daß der Konuswinkel der Muffe (ll) zwischen 1 : 10 bis 1 : 6 liegt.
  7. 7. Ein- oder mehrteiliges keramisches Implantat nach Anspruch 1, dadurch gekennzeichnet, daß das Innengewinde der Muffe (ll) durch eine oder mehrere achsiale Längsnuten geschnitten ist.
  8. 8. Ein- oder mehrteiliges keramisches Implantat nach Anspruch 1, dadurch gekennzeichnet, daß bei den Einzelteilen verschiedene große Muffen (ll), Distanzstücke (26) und Kugelköpfe (27) einsetzbar sind.
  9. 9. Ein- oder mehrteiliges keramisches Implantat nach Anspruch 1, dadurch gekennzeichnet, daß chirurgische Schrauben (28) zur primären Sicherung der Verbindung mit dem Knochenstumpf (l) und der Muffe (ll) angebracht sind.

  10. 10. Ein- oder mehrteiliges keramisches Implantat nach Anspruch 1-5» dadurch gekennzeichnet, daß zur Befestigung von Knochenteilen (29) mit dem Implantat chirurgische Schrauben (28) verwendet sind, deren Muttergewinde aus Kunststoff (30) besteht, um ein Lockern bei Beanspruchung zu verhindern.
  11. 11. Ein- oder mehrteiliges keramisches Implantat nach Anspruch 1-6, dadurch gekennzeichnet, daß an der Muffe (ll), den Distanzstücken (26) und den Kugelköpfen (27) entsprechende Bohrungen (31) angebracht sind, um Sehnen oder Muskelteile anzubinden.
