Description
Die Erfindung betrifft ein keramische Implantat für eine zcrr.cntfreie Implantation mit einer inneren
öffnung zur paßgenauen Verbindung unmittelbar mit einem Knochenende. Ein solches Implantat kann zum
Zwecke der statischen Defektüberbrückung oder als Einsatz eines künstlichen Gelenkes dienen.
Die Erfindung geht von der Erkenntnis aus, daß Metall-Implantate zwar grundsätzlich möglich sind,
aber Keramik wegen ihrer besseren Verträglichkeit mit dem lebenden Organismus besser ist. Insbesondere bei
hochreinem Aluminiumoxid treten unter Einwirkung der Körperflüssigkeit keine Korrosionserscheinungen
auf. Insbesondere in der DE-OS 23 18 459 wird ein künstlicher Gelenkersatz aus gesinterter Oxidkeramik
beschrieben. Der Vorschlag besteht darin, einen Gelenkkopf mit einer inneren Öffnung formschlüssig
auf einen bearbeiteten natürlichen Oberschenkelknochen aufzubringen. Um die Verbindung zwischen
Knochen und Kugelkopf aus Oxidkeramik möglichst fest zu gestalten, sind Haltemittel in Form von
Widerhaken oder eine poröse Oberfläche an der Stelle des Implantats vorgesehen, die mit dem Knochen direkt
im Eingriff stehen. Des weiteren wird nach einer bevorzugten Ausführungsform der Gelenkkopf mit
einem Innenvierkant versehen und auf dem eckig aufgefrästen Schenkelkopf aufgesteckt Der Nachteil
eines solchen Implantates besteht darin, daß es nur einsetzbar ist, wenn der Femurkopf relativ gut erhalten
ist In der Praxis ergibt sich auch die Schwierigkeit, daß unter den operativen Bedingungen der Femurkopf sich
nur schlecht zu einem prismatischen Polygon bearbeiten läßt Die auftretende große Toleranz läßt dabei keinen
allzufesten Sitz des Implantates zu. Außerdem besteht die Gefahr der Knochenfraktur im Schenkelhalsbereich
durch die unspezifische Belastung, insbesondere beim Aufdrehen des Implantates mit dem selbstschneidenden
Gewinde treten erhebliche Kräfte auf. Auch ist ein solches Implantat für die Verbindung von zwei
Knochenresektionsenden nicht verwendbar.
Die derzeitige gebräuchliche Art für die Einpflanzung einer Endoprothese ist das Einzementieren eines
Schaftes in den Röhrenknochen. Der Knochenzement verursacht aber durch die exothermen Reaktionen und
durch spätere auftretende chemische Snahnrodukte irreversible Veränderungen des angrenzenden Gewebes,
was zu Entzündungen und Lockerungen des Implantates führen kann. Eine Abhilfe kann hier die
zementfreie Implantation bringen, wie sie beispielsweise aus der DE-AS 20 59 381 bekannt ist. Eine solche
zweiteilige Hüftgt'enkprothese besteht aus einem Kugelkopf und einem Schaftteil, wobei zur Schaffung
einer spielfreien Einheit zwischen den beiden Teilen eine konische Bohrung im Kugelkopf und ein entsprechend
geformter Zapfen verwendet wird. Ein solches zweiteiliges Implantat ermöglicht zwar eine paßgenaue
Verbindung zwischen zwei Implantatteile, aber durch das Einführen des Schaftes in den Markraum des
Röhrenknochens treten erhebliche mechanische und biologische Veränderungen auf, da vom Markraum aus
die wesentlichen Versorgungsgefäße über den spongiosen Bereich zu der corticalen Peripherie des Knochens
führen. Erschwerend in mechanischer Hinsicht kommt bei dieser intrameduHären Befestigung hinzu, daß der
Markraum einen verhältnismäßig geringen Durchmesser besitzt und somit Krafteinwirkungen auf den Schaft
direkt auf die Knochenwandungen übertragen werden. Diese Krafteinwirkungen sind um so größer, je kurzer
der in den Markraum eingeführte Schaft des Implantates ist.
Ausgehend von diesem Stand der Technik liegt der Erfindung die Aufgabe zugrunde, ein ein- oder
mehrteiliges keramisches Implantat für eine zementfreie Implantation als Knochen- oder Gelenkersatz zu
schaffen, wobei der Markraum nicht in Mitleidenschaft gezogen wird, der insbesondere wichtig für die
Versorgung der Resektionsenden ist. Ferner wird gleichzeitig für Röhrenknochenfrakturen ein hülsenförmiges
Stützelement gesucht.
Die Aufgabe wird erfindungsgemäß dadurch gelöst, daß die innere Öffnung des Implantates für eine
selbsthcmmende Verbindung mit dem entsprechend bearbeiteten Knochenresektionsende kegelförmig ausgebildet
ist.
Weitere Merkmale der Erfindung gehen aus den Unteransprüchen und den zeichnerisch dargestellten
Ausführungsbcispielen hervor. Es zeigt
Fig. I zwei einseitig mit Innenkegel ausgebildete
Muffen und einer Hülse auf Knochenstümpfen im Querschnitt,
. Fig. la einen vergrößerten Ausschnitt zwischen Innenteil der keramischen Muffe und dem Knochen-
stumpf,
Fig,2 dreiteiliges Implantat, bestehend aus Muffe,
Schaft und Kegelteil für einen Oberarmknochen im Querschnitt und
Fig,3 einteiliges Implantat für ein Hüftgelenk mit
Trochanterbefestigung.
Die Vorteile der einzelnen Ausführungsformen werden zusammen mit der Beschreibung erläutert
Grundsätzlich ist vorauszuschicken, daß eingehende Untersuchungen gezeigt haben, daß mit einer Muffenlänge
von mindestens dem l,5fachen Durchmesser des einzuführenden bearbeiteten Knochenresektionsendes
bereits eine Festigkeit erreicht wird, die im Mittel etwa 75% der natürlichen Knochenfestigkeit unmittelbar
nach der Operation erreicht wird. Wird dabei die innere Muffenlänge mit dem Wert /=1, der mittlere
Durchmesser der Muffe mit d und der Quotient von Ud = h bezeichnet, dann kann der Verlauf der
dimensionslosen Bruchbiegekraft Fre/ folgendermaßen

Abb. 1 zeigt den Verlauf dieser Funktion Frci(h) und
deren .erste Ableitung F°rci(h)\u grafischer Darstellung.
Daraus ist ersichtlich, daß/im Intervall 1
<Λ<2 der Optimalbereich der Anordnung in bezug auf eine
möglichst kurze Fassungslänge und damit kurze Strecke der biologischen und mechanischen Beschädigung des
lebenden Knochens eine möglichst hohe und mechanische Festigkeit erreicht ist. Die A b b. 2 zeigt weiterhin
eine grafische Darstellung des Ergebnisses von Festigkeitsmessungen mit der Muffe am Oberarmknochen, aus
der ersichtlich ist, daß mit einem Quotienten h = 1,5 im Mittel Frei(h)= 80% der natürlichen Festigkeit des
Knochens mit der Implantatverbindung erreicht wurden.
Ein weiterer Vorteil der Muffe besteht darin, wie aus Abb.3 hervorgeht, daß durch den großen Kreisringquerschnitt
über die extramedulläre Befestigung einerseits die spezifische mechanische Belastung der
Knochenoberfläche gering gehalten werden kann und damit unfer Anwendung des Wolfschen Transformationsgesetzes
eine Kompressionsbelastung für den Knochen gewählt werden kann, die nicht zur Resorption,
sondern zu einer Stimulation des Knochenwachstums führt und andererseits es gestattet, in werkstoffgerechter
Weise ein großes Widerstandsmoment des Implantafes im Gegensatz zu der intramedullären
Schaftbefestigung zu erzielen. Was bedeutet, daß bei einer vorgegebenen zulässigen Materialspannung für
die extramedulläi-e aufgesetzte Muffe ein wesentlich
höheres Biegemoment als für den intrameduiiär befestigten Schaft zugelassen werden kann. Wenn z. B.
bei einem Röhrenknochen mit einem Markraumdurchtnesser dm = 3a, die mittlere Corticalissiärke a und die
Wandstärke der Muffe wiederum a beträgt, dann verhalten sich die Widerstandsmomente der Muffe und
einen intrameduiiär anbringbaren Schaftes wie 9:1, d. h., bei einer gleichen gegebenen zulässigen Materialspannung
für beide Ausführungen kann mit der Muffe das Neunfache des Biegemoments der intramedullären
Schaftbefestigung üoertragen werden.
In Fig. I ist ein Verbindungselement für Röhrcnknochcnfrakttiren
dargestellt. Die Heiden Knochenslümpfe
1 werden dabei mit einem speziell entwickelten Konusfräser so bearbeitet, daß ein paßgenauer Kegelstumpf
vorhanden ist Als Anhaltspunkt für du.· kegelförmige öffnung 24 der Muffe U werden die
ursprünglichen Knochenabmessungen herangezogen. Dabei ist zu beachten, daß bei dünnen Corticalisstärken
der hohe Schlankheitsgrad genutzt wird. Nach dem Abfräsen werden die einzelnen Muffen 11 von Hand auf
den Knochenstumpf 1 aufgesteckt und durch eine kleine Drehbefestigung fest aufgezogen. Die beiden Muffen
der kegelförmigen öffnungen 24 werden mit einer Hülse 17 ausgerichtet Die endgültige Fixation aller
Keramikteile erfolgt dann durch Einspritzung von Polymethylmethacrylat in die Injektions- und Entlüftungskanäle
12. Die Oberfläche des Innenkegels der Muffe 11 ist in Fig. la erkennbar. Hier sind speziell
geformte Rillen 32 eingearbeitet, die zur Aufnahme der neu gebildeten Knochensubstanz dienen.
Aus der F i g. 2 ist die Verbindung eines Röhrenknochenresektionsendes
mit einem Implantatteil zu sehen, wobei an Stelle der reduzierten iinochenlänge ein
Schaftteil bzw. ein Distanzstück 26 mit Kugelkopf 27
einzusetzen ist Das Distanzkstück 26 wird mit Polymethylmethacrylat in den zylindrischen Teil 25 der
Muffe 11 eingeklebt wobei die Muffe 11 durch den Boden 23 in den konusförmigen Teil 24 und in den
zylindrischen Teil 25 unterteilt wird. Der zylindrische Teil 25 sowie das Ende des Distanzstückes 26 sind mit
Ringnuten 15 für die Aufnahme von Polymethylmethacrylat versehen. Außerdem ist der geschliffene
Kugelkopf 27 teilweise hohl ausgebildet und auf einen Zapfen 6. gesetzt, der entsprechende Nuten 15 ebenfalls
enthält, um ein Verdrehen des Kugelkopfes 27 bei Beanspruchung zu vermeiden. Das Implantat wird dann
in die Pfanne des Schultergelenkes gelegt und schichtweise zugenäht, wobei die Sehnen- oder
Muskelteile an bestimmten Stellen, nämlich den Bohrungen 3J angebunden bzw. befestigt werden
können.
Eine Besonderheit des erfindungsgemäßen keramischen Implantates mit einer kegelförmigen Öffnung ist
aus Fig.3 zu entnehmen. Bei diesem einteiligen Implantat für eine Hüftgelenkendoprothese wird
zusätzlich mit chirurgischen Schrauben das Trochanterteil 29 am Implantat angeschraubt. Grundsätzlich kann
aber die Primärfixierung des keramischen Implantates durch Schrauben 28 erfolgen. Hierzu sind Bohrungen 33
mit unterschiedlichen Durchmesser in der Keramik notwendig. Außerdem befindet sich eine Nut 34 in der
Bohrung, um als Sicherung gegen Verdrehung zu wirken. In diese Bohrung wird die Schraube 28
eingesetzt und mit Kunststoff bzw. Polymethylmeihacrylat ausgegossen. Nach der Polymerisation kann die
Schraube herausgedreht werden. Dabei besitzt das
entstandene Kunststoffgewinde die Eigenschaft, daß es sehr spezifisch an das Gewindeprofil der Schraube
angepaßt ist und sich nur mit einem gewissen Widerstand herausdrehen läßt. Diese Eigenschaft dient
gleichzeitig als Schraubensicherung. In jeder Stellung bleibt die Schraube fest in ihrem Sitz, auch dann, wenn
sie durch Beanspruchung bewegt wird. Es hat sich auch
als günstig erwiesen, die Trochanter-Majer-Aüfsitzfläche
29 mit Zungen 35 für die Aufnahme neu gebildeter Knochensubstanz zu versehen.
Die mit der Erfindung erzielten Vorteile bestehen insbesondere darin, daß man die Implantate, wie bei den
üblichen Metallschaften bekannt ist, nicht mehr in den Markraum des Knochens einschlagen oder durch
Knochenzement einkitten muß. Die Muffe dient als Trag- und Verbindungselement und soll ein Aufsplittern
des Knochens bei Beanspruchung verhindern. Durch die geruijc Paßform /wischen der Kegelhülse des keramischen
Implantats und dem Resektionsende des Knochens kommt es zu einer günstigen Kraftübertragung,
da eine flächenhafte Krafteinwirkung stattfindet, obwohl dieser durch das Abfräsen am Ende relativ
schwach ausgebildet ist. Ferner entstehen durch die Bearbeitung des Resektionsendes infolge seines unregelmäßigen
Querschnittes sogenannte asymmetrische Zungen am Auslauf der Kegelfläche, die ganz besonders
geeignet sind, die Ausbildung von Knochensubstanz zwischen Implantat und Knochen zu fördern.

Claims (8)
1. Keramisches Implantat für eine zementfreie Implantatation mit einer inneren öffnung zur
paßgenauen Verbindung unmittelbar mit einem Knochenende, dadurch gekennzeichnet,
daß die innere öffnung (24) des Implantats (U) für eine selbsthemmende Verbindung mit dem entsprechend
bearbeiteten Knochenresektionsende (1) kegelförmig ausgebildet ist
2. Implantat nach Anspruch 1, dadurch gekennzeichnet,
daß das Implantat (11) muffenartig ausgebildet ist
3. Implantat nach den Ansprüchen 1 und 2, dadurch gekennzeichnet, daß zur Verbindung zweier
Knochenresektionsenden (1) auf der Gegenseite der inneren öffnung (24) eine gleichartige öffnung
vorgesehen ist
4. ImpkJtat nach den Ansprüchen 1 und 2,
dadurch gekennzeichnet, daß zur Verbindung eines Knochenresektionsendes (1) mit einem anderen
Implantat (25,26,27) auf der Gegenseite der inneren öffnung (24) eine entsprechend andere Verbindung
vorgesehen ist
5. Anordnung nach Anspruch 2, dadurch gekennzeichnet, daß das Implantat (11) zweiteilig ausgeführt
ist und eine Hülse (17) zur Verbindung der beiden Teile aufschiebbar ist
6. Implantat nach einem der vorangehenden jo Ansprüche, risdurch gekennzeichnet, daß die innere
Öffnung (24) ein verrundetes Gewinde (32) aufweist.
7. Implantat nach einen· der vorangehenden Ansprüche, dadurch gekennzeichnet, daß die innere
Öffnung (24) eine oder mehrere Längsnuten j>
aufweist.
8. Implantat nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß chirurgische
Schrauben (28) zur Sicherung der Verbindung mit dem Knochenresektionsende (1) angebracht
sind.
