Description

Die Erfindung betrifft eine Wirbelsäulenstützvorrichtung, die es erlaubt, eine intersoma­ tische Arthrodese zu verwirklichen.
Bei einer Verletzung einer Zwischenwirbelscheibe ist es erforderlich, diese abzutragen und, um ihr Fehlen zu beheben, eine intersomatische Arthrodese zu realisieren, d. h. die Wirbelkörper zweier dieser Scheibe benachbarten Wirbel zu vereinigen, und dabei die durch die Abtragung entstandene Lücke zu schließen.
Bei den heute verwendeten Techniken bestehen die häufigsten darin, entweder eine Platte, die durch äußere Verschraubung mit zwei Wirbelkörpern verbunden ist, oder eine zwischen den beiden Wirbelkörpern angeordnete Wirbelzwischenplatte aus porösem Po­ lyäthylen zu verwenden. In beiden Fällen wird ein Transplantat oder ein Knochensubsti­ tut zwischen den beiden Wirbelkörpern eingeschoben, um ihre Verbindung zu verwirkli­ chen.
Die verwendeten Vorrichtungen weisen jedoch Nachteile auf, wobei der Hauptnachteil der der Verwendung von Schrauben ist, die dazu neigen, sich zu lösen.
Diesen Nachteil haben alle Prothesen gemeinsam, aber er kann äußerst schwerwiegende Folgen haben, wenn es sich um einen empfindlichen Bereich, wie es Wirbelsäule ist, han­ delt.
Außerdem kann die Verwendung von Polyäthylen insbesondere aufgrund von Teilchen, die dazu neigen, sich von der Platte zu lösen, pathologische Folgen haben.
Andererseits erfordert es die in der Verwendung einer Zwischenwirbelplatte bestehende Technik, während des chirurgischen Eingriffs eine Vielzahl von Platten unterschiedlicher Größe und Dicke bereitzustellen.
Aufgabe der vorliegenden Erfindung ist es, eine Wirbelsäulenstütze zu schaffen, die dazu bestimmt ist, eine intersomatische Arthrodese zu verwirklichen, die ein Beheben dieser diversen Nachteile ermöglicht.
Die erfindungsgemäße Wirbelsäulenstützvorrichtung ist im wesentlichen dadurch ge­ kennzeichnet, daß sie aus einer Metallplatte gebildet ist, vorzugsweise auf der Grundlage von Titan, flach und langgestreckt, in ihrem Mittelbereich verbreitert ist, deren Kanten mit Ausnahme im Mittelbereich ein Sägezahnprofil haben, und daß die Platte gebogen ist, um einen offenen Ring zu bilden.
Nach einem weiteren Merkmal der erfindungsgemäßen Vorrichtung sind die auf den Kanten an den Enden angeordneten Zähne zum Mittelbereich der Stütze ausgerichtet, so daß die beiden Enden des Rings die Form von Greifern haben.
Nach einem anderen weiteren Merkmal der erfindungsgemäßen Vorrichtung weist der Mittelbereich der Stütze eine Bohrung mit Innengewinde auf, die die Anbringung einer Platzhaltevorrichtung erlaubt.
Bei einer bevorzugten Ausführungsform der erfindungsgemäßen Vorrichtung weist diese auf der einen und anderen Seite des Mittelbereichs und senkrecht dazu zwei scharfe und gezahnte Spitzen auf, die im rechten Winkel zur Krümmungskante der Stütze gebogen sind.
Bei einer anderen Ausführungsform der erfindungsgemäßen Vorrichtung weist diese auf der einen und anderen Seite des Mittelbereichs und senkrecht dazu zwei Befestigungs­ klammern auf, von denen jede an ihrem Ende eine Öffnung aufweist, die das Durchfüh­ ren einer selbsthemmenden Schraube erlaubt.
Die erfindungsgemäße Wirbelsäulenstützvorrichtung ist dazu bestimmt, zwischen den Wirbelkörpern zweier zu verbindender Wirbel angeordnet zu werden, wobei die Zähne, die vorzugsweise auf der Kortikale anliegen, ihr Verschieben verhindern, während ein im Innern angeordnetes Knochentransplantat die Verbindung der Wirbelkörper ermöglicht.
Die Vorteile und Merkmale der vorliegenden Erfindung ergeben sich klarer aus der fol­ genden Beschreibung, die sich auf die beigefügten Zeichnungen bezieht, wobei sie eine nicht einschränkende Ausführungsform darstellen.

Bezugnehmend auf Fig. 1, 2 und 3 kann man sehen, daß eine erfindungsgemäße Wirbel­ säulenstützvorrichtung eine flache und langgestreckte Platte 1 aufweist, deren Mittelbe­ reich 2 größer als die sich verjüngenden Endbereiche 3 und 4 ist und die so gebogen ist, daß sie einen offenen flachen Ring bildet.
Die Kanten der Endbereiche 3 und 4 sind gezahnt, sie haben zum Mittelbereich 2 gerich­ tete Zähne 30 und 40, so daß die Endbereiche 3 und 4 die Form von Greifern haben.
Der Mittelbereich 2 weist eine mit einem Innengewinde versehene Bohrung 20 auf, die ein Anpassen eines nicht gezeigten Einbaugeräts, das im wesentlichen aus einem Schaft besteht, der an einem Ende ein Außengewinde aufweist, erlaubt.
Das Anbringen einer erfindungsgemäßen Wirbelsäulenstützvorrichtung wird auf folgende Weise durchgeführt: Nach der Abtragung der Zwischenwirbelscheibe und der sie begren­ zenden Knorpelplatten werden die Wirbelkörper auseinandergespreizt, und die ein Kno­ chenimplantat beinhaltende Stütze wird zwischen den Körpern eingefügt. Vorher werden die Endbereiche 3 und 4 der Stütze, deren Höhe in Abhängigkeit von dem auszuglei­ chenden Zwischenraum zwischen den Wirbeln gewählt wird, je nach der Größe der Wir­ belkörper mehr oder weniger aufgespreizt. Nach Lockerung des auf die Wirbel ausgeüb­ ten Drucks kommen die Zähen 30 und 40 in Eingriff mit der Kortikale der Wirbelkörper, wobei ihre Greiferform das Verschieben der Stütze verhindert.
Das Baukastenprinzip der Größe der Stützen erlaubt es aufgrund der Möglichkeit, die Endbereiche 3 und 4 aufzuspreizen oder zusammenzubringen, die Anzahl der bei einem Eingriff erforderlichen Stützen zu begrenzen, wobei die Stützen nur in der Höhe variie­ ren.
Bezugnehmend auf Fig. 4 kann man nun sehen, daß die erfindungsgemäße Wirbelsäu­ lenstütze bei einer zweiten Ausführungsform zu beiden Seiten des Mittelbereichs 2 zwei scharfe und gezahnte Spitzen 5 und 6 aufweist, die im rechten Winkel zu der Seite der Endbereiche 3 und 4 gebogen sind.
Die Spitzen 5 und 6 sind dazu bestimmt, unter Krafteinwirkung in die Wirbelkörper hin­ eingedrückt zu werden, die vorher mit Hilfe einer Bohrschablone eingestochen wurden, was das Fixieren des adäquaten Abstands zwischen den Wirbelkörpern erlaubt.
Die Spitzen 5 und 6 vervollständigen die Fixierung der Stütze und ermöglichen insbe­ sondere ein Beibehalten des Abstands der Wirbelkörper.
Es ist anzumerken, daß die Spitzen aus federelastischem Metall derart hergestellt werden können, daß sich bei Körpertemperatur ein leichtes Zusammenziehen einstellt.
Gemäß einer anderen nicht gezeigten Ausführungsform kann die Stütze zwei an der ei­ nen und anderen Seite des Mittelbereichs angeordnete Befestigungsklammern aufweisen, die an ihrem Ende jeweils eine Öffnung aufweisen, die das Durchführen selbsthemmender Schrauben erlaubt, die, wenn sie einmal mit den Öffnungen in Eingriff stehen, nicht mehr herausgezogen werden können.

Claims (6)

1. Wirbelsäulenstützvorrichtung zur Verwirklichung einer intersomatischen Arthrode­ se, dadurch gekennzeichnet, daß sie aus einer flachen und langgestreckten Me­ tallplatte (1) hergestellt ist, die in ihrem Mittelbereich (2) verbreitert ist und deren Kanten mit Ausnahme der Fläche des Mittelbereichs Zähne (30, 40) aufweist, und daß die Platte (1) so gebogen ist, daß sie einen offenen Ring zu bildet.
2. Vorrichtung nach Anspruch 1, dadurch gekennzeichnet, daß die Zähne (30, 40) so auf den Mittelbereich der Stütze gerichtet sind, daß die Enden des Rings die Form von Greifern bilden.
3. Vorrichtung nach Anspruch i oder Anspruch 2, dadurch gekennzeichnet, daß der Mittelbereich (2) der Stütze eine mit einem Innengewinde versehene Bohrung (20) aufweist, die die Verbindung mit einem Einbauwerkzeug erlaubt.
4. Vorrichtung nach einem der vorhergehenden Ansprüche, dadurch gekennzeich­ net, daß sie an der einen und der anderen Seite des Mittelbereichs (2) und senk­ recht dazu zwei scharfe und gezahnte Spitzen (5, 6) aufweist, die im rechten Win­ kel zur Seite, zu der die Platte (1) gewölbt ist, gebogen sind und die dazu bestimmt sind, unter Krafteinwirkung derart in die Wirbelkörper eingedrückt zu werden, daß der Zwischenraum zwischen den Wirbelkörpern beibehalten wird.
5. Vorrichtung nach Anspruch 4, dadurch gekennzeichnet, daß die Spitzen (5, 6) aus federelastischem Metall hergestellt sind.
6. Vorrichtung nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß sie auf der einen und der anderen Seite des Mittelbereichs (2) und senkrecht dazu zwei Befestigungsklammern aufweist, die an ihrem Ende jeweils eine Öffnung aufwei­ sen, die das Durchführen selbsthemmender Schrauben erlaubt, die in einen Wirbel­ körper einschraubbar sind.
