Description

Gegenwärtiger Stand der Technik
Aussackungen der Körperhauptschlagader und der Beckengefäße können neuerdings ohne Ope­ ration behandelt werden. Hierzu wird eine röhrenförmige Gefäßstütze in die Aorta in zusammen­ gefalteten Zustand mit Dacron überzogen in die Aorta eingebracht. Nach Zurückziehen eines Applikationskatheters entfaltet sich die rohrförmige Prothese entweder aufgrund der Elastizität des verwendeten Materials z. B. (Stahl) oder aufgrund der Erwärmung auf Körperwärme bei ei­ nem gedächtnistragenden Metall, z. B. (Nitinol) und verankert sich mit kleinen Wiederhäkchen an der Wand der Aorta, um auf diese Weise eine Aussackung oder Verletzung der Aortenwand zu überbrücken und einer Ruptur der ausgestülpten Wand vorzubeugen. Der Nachteil dieser Ge­ fäßstützen gegenwärtig ist, daß aufgrund ihrer großen Cirkumferenz auch im zusammengefalteten Zustand eine Einbringung in die Körperschlagader nur über eine operative Freilegung der Bein­ schlagader möglich ist, darüber hinaus ist der maximale Durchmesser dieser Systeme gegenwärtig auf 26 mm begrenzt. Der in dem Patentanspruch unter Punkt 1-4 genannten Erfindung liegt zu­ grunde, die maximale Cirkumferenz des einzubringenden prothetischen Materials dadurch zu ver­ kleinern, daß beide Beinarterien zur Applikation von zwei kleineren, halbkreisförmigen Gefäß­ stützen benutzt werden, die sich im Bereich des Applikationsortes zu einer größeren Gefäßstütze vereinigen. Dadurch kann die maximale Größe der komprimierten Gefäßstütze verkleinert wer­ den, und es kann darauf verzichtet werden, die Beinarterie chirurgisch freizulegen, da Gefäßstützen mit einem Außendurchmesser von 4 bis maximal 6 mm auch ohne chirurgische Freilegung der Beinarterien allein durch Punktion der Gefäße vorgebracht werden können.
Darüber hinaus besteht häufig das Problem, daß Aussackungen der unteren Aorta häufig in die Beckenarterien übergreifen, weshalb sogenannte Ypsilon förmige (hosenartig ausgefertigte) Ge­ fäßstützen implantiert werden müssen. Auch diesen Gefäßstützen liegt das o.g. Problem zu­ grunde, nämlich das Problem der Größe im zusammengefalteten Zustand, darüber hinaus haben diese Gefäßstützen noch das Problem, daß das sogenannte zweite Bein in die Gefäßstütze einge­ setzt werden muß, welches ein technisch schwieriger Vorgang ist (siehe Abb. 1). In der unter 1-4 genannten Erfindung ist das Einsetzen eines zweiten Beines nicht notwendig, da sich die beiden Halbschalen, die von beiden Beinarterien aus eingesetzt werden, in der Aorta vereinigen und somit zwei Blutstromüberbrückungen in beide Beine hinein darstellen. Die Halbkreise sollen in verschiedenen Längen und Durchmessern entsprechend der unterschiedlichen Anatomie verfügbar sein.
Zur Aufnahme von Herzklappenprothesen, die perkutan bislang nicht implantiert werden können, aufgrund der unter anderem großen Zirkumferenz des Stützapparates, auch im zusammengefalte­ ten Zustand, können die Halbkreisstützprothesen ebenfalls, dann allerdings ohne Überzug, aber mit einer in jedem Halbkreis eingebrachten Ventilprothese ebenfalls verwendet werden.
Ausführungsbeispiele der Erfindung sind in den Zeichnungen 2a bis 2c dargestellt.

Claims (6)

  1. Symmetrisch geteilte halbkreisförmige Gefäßstütze zur Überbrückung von Aussackungen der großen Körperhauptschlagader (Aorta) und/oder der Beckengefäße dadurch gekennzeichnet,
  2. 1. daß die Gefäßstütze nur halbschalenförmig ausgebildet wird, wobei an den der Halbschalen abgewandten Seiten Verankerungshaken angebracht sind.
  3. 2. Vorrichtung nach Anspruch 1, dadurch gekennzeichnet, daß den halbschalenförmigen Gefäßstützen trichterförmige, kreisförmige Röhren angesetzt sind, die zur Verankerung in den Beckengefäßen dienen.
  4. 3. Vorrichtung nach Anspruch 1 dadurch gekennzeichnet, daß die im Anspruch genannte Gefäßstütze in Halbkreisradien von 10-20 mm Durchmesser und in Längen von 60-120 mm gefertigt wird.
  5. 4. Vorrichtung nach Anspruch 1 dadurch gekennzeichnet, daß die im Anspruch 1 genannte Gefäßstütze mit Kunststoffen überzogen wird (z. B. Dacron).
  6. 5. Vorrichtung nach Anspruch 1 dadurch gekennzeichnet, daß die im Anspruch 1 genannte halbschalenförmige Gefäßstütze biologische oder mechanische Herzklappen trägt zur Einbringung dieser Gefäßstütze in die Position der Aortenklappenebene.
