技术领域
本发明涉及大麻素(cannabinoids)的组合用于治疗关节炎的疼痛、炎症和/或疾病缓和的应用。优选地,大麻素选自大麻二酚(CBD)或次大麻二酚(cannabidivarin,CBDV)和Δ-9-四氢大麻酚(THC)或四氢次大麻酚(tetrahydrocannabinovarin,THCV)。更优选地,大麻素是以确定的小于或等于19∶1重量比组合的CBD或CBDV和THC或THCV。
背景技术
关节炎是痛苦的关节病症。该疾病有不同的类型,但都可以引起关节的疼痛和炎症并经常具有实质性的变性。一些关节炎最常见的类型是骨关节炎和类风湿性关节炎。
在超过50岁的人中,10个人中有约8个人的关节受到骨关节炎疾病的影响。骨关节炎是由于随着时间关节软骨变薄和不平所引起的,在一些情况下其能够完全磨损。除了关节的磨损,关节囊能够变得更厚,结果使产生的滑液的量增加。导致引起关节肿胀。骨刺也可能在受影响的区域生长,引起受影响组织的炎症。骨关节炎能够涉及身体的所有关节,但是最常发现于指、膝、髋和脊柱。
类风湿性关节炎是全身性疾病,其能够影响全身,并且是最常见的关节炎形式之一。它的特征在于关节滑膜的炎症,其从而引起该区域的疼痛、僵硬、热、发红和肿胀。指和手的小关节受到的影响最严重,但是所述病症能够扩散涉及到腕、肘、肩和其它关节。当炎性细胞释放能消化骨和软骨的酶时,发炎的关节滑膜也能够侵入和破坏骨和软骨。发炎的关节可以失去它的形状和匹配,导致疼痛和运动丧失。它通常是慢性的并且能够不时的突然发作。
除了受影响关节经受的疼痛和炎症以外,类风湿性关节炎能够引起食欲减退和体重减轻、嗜睡、肌肉和和腱疼痛、发烧、皮肤下肿块(类风湿性节结)和严重的眼部炎症。还有很多并发症,包括贫血、心包炎、脉管炎和雷诺现象(Raynaud’s phenomenon)。
类风湿性关节炎的发病原因仍然是不清楚的。然而,已知类风湿性关节炎是一种自身免疫疾病。身体的自然免疫系统正常工作,导致免疫系统攻击健康的关节组织并引起炎症和随后的关节损伤。这种疾病在一些人中由于感染而可以被激发,所述的这些人具有该疾病的遗传倾向,其促进免疫系统形成抗原和抗体免疫复合物的损害性聚集体。
在疾病的早期,人们通常注意到全身疲劳、触痛、僵硬和隐痛(aching)。疼痛和肿胀可以发生在身体两侧的相同关节并且通常起始于手或脚。类风湿性关节炎影响腕和很多手关节,但通常不影响与指甲最接近的关节(拇指除外)。类风湿性关节炎也可以影响肘、肩、颈、膝、髋和踝。它趋向于持续较长时间,一段时间后,发炎的关节可能被损害。
类风湿性关节炎的治疗限制于通过休息、用夹板夹住发炎关节、物理疗法和应用消炎去痛药物来控制炎症和缓解疼痛。治疗方法集中于缓解疼痛、减少炎症、停止或减慢关节损害和改善患者健康。
当前提供给类风湿性关节炎患者的药物可以被分成两组:
1.对症类药物,比如非甾体消炎药(NSAIDs)和阿斯匹林、止痛剂和皮质类固醇。这些药物帮助减轻关节疼痛、僵硬和肿胀。对症类药物可以与对病类(disease-modifying)抗风湿药物结合使用。
2.对病类抗风湿药物(DMARDs),包括低剂量的甲氨蝶呤、来氟洛米、D-青霉胺、柳氮磺吡啶、金疗法、米诺环素、硫唑嘌呤、羟化氯喹(和其它抗疟药)、环孢霉素和生物剂(biologic agents)。
除了药物疗法,最经常涉及的治疗方法包括锻炼、休息、关节保护及物理和职业治疗的一些组合。对于严重损伤和疼痛的关节,可以选择外科手术。均衡的休息和锻炼能够帮助保存能量和维持关节一定范围的运动和使用。
大麻作为药物的应用已经知道很长时间了,在19世纪期间大麻制剂被推荐作为催眠镇静剂,用于治疗癔症、精神错乱、癫痫症、神经性失眠、偏头痛、疼痛和痛经。
直到最近,对患者施用大麻只能通过在乙醇中煎煮而得的大麻制剂,随后其可以被吞咽,或者通过患者吸入干燥植物材料烟中的大麻蒸气。最近的方法已经致力于寻找向患者施用大麻素的新方式,其包括那些方法,它们绕开胃以及肝的相关首过效应,所述首过效应能去除高达90%的所吸收活性物质,所述方法还避免了患者必须将不健康的焦油和相关致癌物质吸入到肺中。
这些给药形式包括施用大麻素到舌下或口腔粘膜,经蒸汽化或雾化吸入大麻素蒸汽,灌肠剂或固体给药形式比如凝胶、胶囊、片剂、软锭剂(pastilles)和糖锭剂(lozenges)。
在1988年,为确定各种大麻素和大麻素前体的止痛和消炎活性,进行了一项研究。发现口服施用CBD对抑制PBQ诱导的小鼠痛苦挣扎最有效。发现THC和CBN对减轻痛觉和炎症的效用最小(Formukong et al.,1988)。
Holdcroft等人已经显示大麻素具有止痛和可能的消炎性质。给地中海热的患者施用50mg THC导致非常明显的降低患者所需止痛剂的量(Holdcroft et al.,1997a)。
相同作者接下来发表了一项研究,其中分析口服施用大麻油。含有5.75%THC、4.73%CBD和2.42%CBN的胶囊被施用于患有家族性地中海热的患者。在三周的积极治疗期间,减少了患者必需的解脱药物(吗啡)的量。(Holdcroft et al.,1997b)。所测量的炎性标记物没有变化。
Feldmann等人(国际专利申请WO 99/52524)在早先已经显示,纯CBD可以被用来治疗炎性疾病,比如类风湿性关节炎或Crohn’s病。炎性疾病涉及几种组分如白介素、TNF-α和一氧化氮之间的复杂相互作用。Feldmann等人所提供的数据描述了CBD对TNF-α和一氧化氮产生的抑制作用。这种大麻素在胶原诱导关节炎的小鼠模型中也显示出以剂量依赖方式抑制关节炎的作用。
有相当多的文献涉及大麻成分的免疫调节作用,Klein(Klein,1998)对这些文献进行了综述。
不同比例的大麻素比如THC或CBD或它们的丙基变体、四氢次大麻酚(THCV)和次大麻二酚(CBDV)在治疗不同疾病和病症中的应用已经在申请人早先的英国专利申请GB2377633中被描述。
已经有报道特定比率的THC和CBD或THCV和CBDV可用于治疗或管理特定疾病或医学状况。下表详细列举了这些方面中的一些。

含有具体的、比率确定的大麻素的制剂可以从纯的、合成的大麻素或从得自大麻类植物的提取物并组合药用载体和赋形剂进行配制。
当前可用的药物疗法治疗关节炎的主要缺点是,患者为了治疗疾病的症状比如疼痛和相关炎症经常必须接受一组药物,患者同时还必须接受改变病因(disease-modifying)的药物。
当前没有已知的药物被用来治疗疼痛和炎症症状并且同时又起改变病因的抗风湿药物的作用。
令人惊奇的是,已经发现含有大致等量大麻素δ-9-四氢大麻酚(THC)和大麻二酚(CBD)的基于大麻的药物提取物可以用于既改变类风湿性关节炎的病因并且又治疗由该疾病引起的疼痛和炎症的症状。
使用本发明所描述药物的一个重要的好处是可以用同一药物既治疗疾病又治疗疾病的症状。这有很多好处,包括对患者更大程度的灵活性,因为它们只需坚持更低严格的药物治疗方案,患者也可以经受更少的副作用,因为更少有组合药物治疗之间的潜在有害的相互作用。
发明内容
根据本发明的第一方面,提供大麻素x和y组合在制造用于治疗关节炎的药剂中的应用,其中x∶y的重量比率小于或等于19∶1,且在x和y组合中x选自大麻二酚(CBD)和次大麻二酚(CBDV)而y选自δ-9-四氢大麻酚(THC)或四氢次大麻酚(THCV)。
优选地,关节炎治疗是对骨关节炎或类风湿性关节炎的治疗。
本发明的一个实施方案提供大麻素的组合用于治疗关节炎的一种或多种症状比如疼痛、炎症或睡眠不足。优选的是,提供一种大麻素的组合用于改变关节炎的病因。更优选的是,提供一种大麻素组合用于治疗一种或多种症状并改变关节炎的病因。
在一个实施方案中,大麻素x∶y的比率小于或等于19∶1,更优选x∶y的比率是以2的整倍数从小于或等于17∶1至小于或等于3∶1。更优选地,x∶y的比率是以0.25的整倍数从小于或等于2.5∶1至小于或等于1.25∶1。最优选地,x∶y的比率基本为1∶1,尤其是0.93∶1。
大麻素的优选组合包括CBD∶THC、CBDV∶THCV、CBDV∶THC和CBD∶THCV。作为替代,包含CBD、CBDV、THC和THCV的组合可以被使用。
本发明进一步的实施方案提供用作药物制剂的大麻素组合,其以凝胶、片剂、液体、胶囊或用于汽化的给药形式被包装。更优选作为药物制剂的大麻素组合被包装用来舌下或口腔给药,优选作为舌下或口腔喷雾剂。有益的是,该药物制剂进一步包含一种或多种载体溶剂。优选的载体溶剂是乙醇和/或丙二醇。更优选乙醇对丙二醇的比率为4∶1-1∶4。仍然更优选比率为1∶1。
优选地剂量被配制成患者能够滴定其剂量。剂量范围优选为每种大麻素5-25mg,更优选每种大麻素10-20mg,优选每种大麻素12-14mg,仍然更优选每种大麻素12.5-13.5mg。
施用大麻素组合比如THC和CBD可以是同时向患者施用,其中大麻素包含于同一制剂中。大麻素也可以在分开的时间施用,例如,可以在施用含THC的制剂之前的固定时间向患者施用含CBD的制剂,由此缓解THC的已知可用CBD改善的一些副作用,或反之亦然。如果需要,两种大麻素也可以连续施用给患者。
优选的是,本发明提供一种大麻素组合,其作为一种或多种基于大麻的药物提取物(CBME/s)。在一个实施方案中,通过超临界或亚临界CO2提取来生产CBME/s。在另一个实施方案中,通过与加热气体共挥发从植物材料提取来生产CBME/s。优选的是,CBME/s含有在植物材料中天然存在的所有大麻素。作为替代,合成的或高纯的大麻素分离物可以被使用。
根据本发明的第二方面,提供一种治疗具有关节炎的对象的方法,其包括给对象施用大麻素组合x和y,其中x选自大麻二酚(CBD)或次大麻二酚(CBDV)而y选自δ-9-四氢大麻酚(THC)或四氢次大麻酚(THCV),其中x∶y的重量比率小于或等于19∶1。

具体实施方式
基于大麻的药物提取物(CBME)的制备依照实施例1所概述而制备,其含有基本等量的大麻素THC和CBD,其被施用于患有类风湿性关节炎的患者,作为次级病症所述患者还具有疼痛。施用这种大麻素组合有可能减轻由类风湿性关节炎引起的疼痛和炎症,但是没有预料到的是,含有约等量THC和CBD的这种基于大麻的药物提取物对类风湿性关节炎患者中也产生了改变病因的作用。

实施例1:
制备基于大麻的药物提取物(CBME)
参照公开于WO 02/064109的方法(实施例15)生产和制备药用大麻。获得的植物材料按以下的流程图进行加工。高THC或高CBD的基于大麻的药物提取物的制造工艺被描述。

通过在减压下薄膜蒸发从滤出物中去除乙醇
获得的提取物被称为基于大麻的药物提取物,并按照美国食品药品管理局对工业植物药物制品的指南(the US Food and Drug Administration Guidance for IndustryBotanical Drug Products)也被分类为植物药物质(Botanical Drug Substance)。
CBME中大麻素的量可以参考公开于WO 02/064109的方法,通过HPLC测量来准确评估(实施例16)。
用CO2萃取高CBD药用大麻植物生产的含CBD的CBME的HPLC色谱图的实例被显示于图1。用CO2萃取高THC药用大麻植物生产的含THC的CBME的HPLC色谱图的实例被显示于图2。含相当比率THC和CBD的CBME的HPLC色谱图的实例被显示于图3。
本发明已经参考THC和CBD举例说明,然而对本领域的技术人员来说由于THC和THCV之间以及CBD和CBDV之间的药理学相似性,很显然除THC和CBD之外另加大麻素THCV和CBDV或用它们代替可以获得相似的结果。
实施例2:
通过在类风湿性关节炎人患者中进行临床试验来评估基于大麻的药物提取物的功效
为了评估基于大麻的药物提取物对类风湿性关节炎中疼痛的功效,进行了为期7周的多中心、双盲、随机、平行分组研究。基于大麻的药物提取物在乙醇∶丙二醇(50∶50)的赋形剂中含有浓度为27mg/ml的δ-9-四氢大麻酚(THC)和浓度为25mg/ml的大麻二酚(CBD)。基于大麻的药物提取物在泵动作喷雾中提供,其中每次动作递送100μl喷雾,其含THC(2.7mg)和CBD(2.5mg)。
研究对象被随机分别相同地施以基于大麻的药物提取物或安慰剂。在乙醇∶丙二醇(50∶50)赋形剂中,安慰剂与活性制剂在外观、气味和味道方面相当,但不含有活性成分。而且安慰剂也在崩动作喷雾中提供,其中每次动作递送100μl喷雾。
在第一次访问时筛选合格的患者,并在此时进行基线评估。两周后患者回来进行第二次访问,并在此时被随机分到两组中的一组。将研究药物仅作为晚间剂量施用并要求患者对其施用剂量进行滴定直至他们获得最佳功效。
对药物进行两周的滴定后,患者回来做第三次访问,此时患者要确定他们在研究的余下三周中要摄入的剂量。
每个患者所摄入的药物剂量不同,但是都在THC和CBD各自5-25mg范围内,大部分患者接受各自10-20mg的THC和CBD。每个患者滴定的平均剂量为13.5mg THC和12.5mg CBD。
用研究药物5周后,患者回来进行第四次访问。并在此阶段重复全部基线评估。
功效评价被认为是本研究的一部分。每位患者每日在自我评价日记卡上记录休息和活动时的晨痛、晨僵和睡眠质量。在第一次和第四次访问时完成简单格式的McGill问卷调查以比较疼痛强度的变化,目前的疼痛强度,目前疼痛以及变化的总体印象。
在第一次和第四次访问时,从28关节计数(28 joint count)、红细胞沉降速率和总体疾病活动指数计算出疾病活动指数(Disease Activity Score)。
为了判断任何变化,在第一次和第四次访问时记录对使用复苏痛觉丧失(rescueanalgesia)、不良反应、血液化学和生命体征的评估。
结果:
从该研究中收集到的一些数据描述如下。
施用含浓度为27mg/ml的THC和浓度为25mg/ml的CBD的基于大麻的药物提取物时,对类风湿性关节炎患者休息时晨痛的比较
如上所述对基于大麻的药物提取物的功效进行评估并且每日通过自我评估记录休息时晨痛的程度。比较数据并进行统计分析。患者评估休息时晨痛的数值范围在0(无疼痛)-10(极度疼痛)量表范围。表1和2对结果进行了举例说明。

当向具有与类风湿性关节炎相关疼痛的患者提供时,使用大约等量的大麻素THC和CBD的混合物,导致休息时晨痛降低。被提供给等量THC和CBD混合物的患者所经历的睡眠质量也显示出改善。被提供了药物的患者也经历了疼痛的降低,正如问卷调查的记录所示。所有这些中最有意义的是药物对患者疾病活动评分的影响。
疾病活动评分是一种用来测定患者所经历的类风湿性关节炎程度的方法。它涉及测定28个不同关节是如何肿胀和疼痛。验血也被用作疾病活动评分的一部分以测量红细胞沉降速率。该速率是一种实验室方法用于确定急性期对炎症的反应。基于病人感觉的综合疾病活动评分也对计算全面数值有贡献。综合评分大于3.7被认为是高的。
本发明发现使用约1∶1组合的THC和CBD能够降低类风湿性关节炎患者的疾病活动评分,该发现的显著性很大。
