技术领域
本发明涉及中成药制剂，具体地说是一种用于治疗脂肪肝、高脂血等症的三七脂肝制剂及其制备方法。
背景技术
肝脏为人体最重要的重要器官之一，是人体最大的消化腺，具有分泌胆汁、储存肝糖原及解毒等重要机能。脂肪的消化、吸收等过程离不开肝脏，各种药物也需通过肝脏代谢，若肝脏功能发生障碍，则将影响整个机体健康。据专家介绍，部分脂肪肝患者可发展成肝纤维化，甚至肝硬化，继而导致生活质量下降和预期寿命缩短。因此，保护肝脏健康及对肝脏疾病进行有效治疗，一直都是医学工作者们致力于研究解决的重要课题，也为此，医学工作者们已为社会提供出了多种治疗肝脏疾病的药物，为减轻肝疾病患者的痛苦作出了重要贡献。然而，迄今为止，就治疗脂肪肝而言，却没有一种特效药，致使发病率不断增高，媒体常常引用的一组数据显示，近年来脂肪肝的发生率较5年前增加了50倍；在一些医院的健康体检中，脂肪肝检出率最高，达50％。已成为仅次于病毒性肝炎的第二大肝脏疾病。
发明内容
本发明的目的是提供一种能有效治疗脂肪肝、高脂血等症、停药后不反弹的三七脂肝制剂及其制备方法。
本发明的技术方案是：三七脂肝制剂由三七、莪术、菟丝子、菊花、白术、泽泻、白芍、荷叶、青皮、赤芍、云山楂等11种中药材制成。
11种中药材的重量份为：
三七20-30 莪术20-30 菟丝子20-30
菊花20-30 白术20-30 泽泻10-30
白芍10-30 荷叶20-30 青皮10-30
赤芍20-30 山楂20-40
三七脂肝制剂可制作成片剂、丸剂、胶囊剂和颗粒制等。
制备方法是：
①、制作A料，将三七、泽泻、白芍三种中药材混合，进行超微粉碎(约400目)得A料粉剂；
②、制作B料清膏：将莪术、菟丝子、菊花、白术、荷叶、青皮、赤芍、云山楂八种中药材混合后用水浸提取，浓缩浸取液至相对密度为1.10(80～85℃)，得B料清膏；
③、制备三七脂肝片剂：在A料粉剂中加入5％淀粉、3％糊精混合均匀后加入B料清膏搅拌均匀，制成颗粒，干燥，再加入总量的0.5％硬脂酸镁压片。
制备三七脂肝丸剂：在B料清膏中加入20％炼蜜混匀后与A料粉剂一起泛丸，干燥，打光。
制备三七脂肝胶囊剂：将A料与B料混合搅拌，干燥，粉碎过60～80目筛，分装成胶囊。
另外，三七脂肝制剂还可以制备成颗粒剂型，制备方法是：将11种中药材混合，粉碎粗粉后用水浸提取，浓缩浸取液至相对密度为1.10(80～85℃)的清膏，加入20％糖粉和5％糊精，混合均匀后制成颗粒。
中医认为脂肪肝是脾虚痰湿内阻，肝气失调之故，若使脾能健运则痰湿自消，肝气亦能畅调。结合脂肪肝的病因病机，应重用健脾助运化湿浊消积滞之方法；本发明组方中云山楂消食健胃，行气散瘀。泽泻有较强的利水渗湿作用，现代药理研究证明山楂、泽泻均具有显著的降血脂作用；白术健脾益气、潮湿利水，用于脾虚食少，泄泻便溏，现代药理研究白术有扩张血管作用，使血压下降，有抗血凝作用；三七、莪术有显著的活血化瘀，消肿止痛作用，可降低血液粘稠度，有助于改善高脂血症。青皮有疏肝破气、消积化滞、解郁理气作用；其余药味：白芍平肝止痛；菊花散风清热，平肝明目；赤芍清热凉血，散瘀止痛。菟丝子滋补肝肾。诸药合用，能起到良好的降脂改善肝功能之作用。

三七脂肝丸(本发明的制剂)用于治疗脂肪肝226例的临床疗效观察结果如下：
1、资料及方法
1.1诊断标准
观察对象(病例)为营养失调性脂肪肝，肝炎后脂肪肝，酒精性脂肪肝。
上述病例具有：
(1)B超检查诊断为脂肪肝(轻、中、重度)。
(2)血脂检测：TRLG、CHOL、LDL有两项以上异常者。
(3)肝功能正常或异常。
1.2一般临床资料
226例患者中男142例，女84例，年龄最小16岁，最大70岁，平均50.7岁。病程最短6个月，最长10年，平均4.2年。其中诊断为营养失调脂肪肝81例，肝炎后脂肪肝83例，酒精性脂肪肝62例。其中血清甘油三脂(TRIG)升高212例，血清胆固醇(CHOL)增高176例，血清低密度脂蛋白(LDL)增高70例。B超检查诊断为重度脂肪肝38例，中度脂肪肝105例，轻度脂肪肝83例，肝功能ALT异常136例。
1.3治疗方法
口服三七脂肝丸，一次6克，一天三次，3个月为一个疗程。
1.4观察指标
临床症状：胁肋胀满，腹胀，乏力等；
血脂：TRIG、CHOL、LDL；
肝功：ALT、ALB、A.G、TBil；
B超：肝；脾。
1.5疗效判断标准
临床基本治愈：血脂、肝功能恢复正常，B超显示脂肪肝消失；临床症状消失，追踪半年未见复发。
显效：血脂、肝功能恢复正常或明显改善。B超显示脂肪肝改善，临床症状消失或明显改善。
无效：血脂或肝功能无改变，B超显示脂肪肝无变化，临床症状改善不明显。

三七脂肝丸治疗后，肥胖患者体重均有不同程度的下降。

表2显示三七脂肝丸具有明显的降血脂作用，总有效率达93.67％，其中甘油脂有效率为96.23％。血清胆固醇有效率为92.62％，ALT复常率为79.41％，说明其具有较明显的保肝降酶作用。

表3显示，三七脂肝丸治疗脂肪肝B超检查结果，其消失率为30.09％，改善率为63.27％。

表4显示，三七脂肝丸治水肿脂肪肝226例，临床总有效率为89.83％，其中临床基本治愈率为31.42％，显效率为58.41％。
3、讨论
正常肝脂肪含量为肝重的3-5％，主要是甘油三脂和极低密度脂肪酸，若肝脏脂肪超过肝重的5％即为脂肪肝。随着人们生活水平的不断提高和生活方式的变化，脂肪肝的发病有逐年上升的趋势，因此防治脂肪肝的研究具有现实意义。
中医学和民族医药虽无脂肪肝的病名，但根据其病理病机可归属于“痞满”“积聚”等症，三七脂肝丸由三七、莪术等药物组成，具有活血降脂、健脾软肝之功效，临床治疗226例脂肪肝患者结果显示其治疗脂肪肝总有效率为89.83％，其中临床基本治愈率为31.42％，显效率为58.41％。同时治疗结果表明脂肝消丸具有明显的保肝降酶，降脂作用，血清甘油三脂(TRLG)复常率为38.21％，好转率为58.02％，血清胆固醇(CHOL)复常率为32.39％，好转率为60.23％，血清低密度脂蛋白(LDL)复常率为28.59％，好转度为60％，肥胖患者治疗后均有不同程度的体重下降，ALT复常率为79.41％。临床观察未发现有何不良反应。
三七脂肝丸是使用安全，且原料资料丰富的口服制剂，可作为治疗脂肪肝的选用药物。此外对脂肪肝治疗除药物外，还应指导患者调节饮食，并适当地参加体育锻炼，有利其康复和疗效的巩固。
具体实施方式
下面结合具体实施对本发明技术方案作进一步说明：
实施例1
制作三七脂肝制剂所用11种中药材的重量组份为(单位：克)
三七 20 莪术 25 菟丝子 25
菊花 20 白术 22 泽泻 10
白芍 18 荷叶 25 青皮 16
赤芍 25 云山楂 35
制备方法为：
①、制作A料粉剂：将三七、泽泻、白芍三种中药材混合，进行超微粉碎(约400目)得A料粉剂；
②、制作B料清膏：将莪术、菟丝子、菊花、白术、荷叶、青皮、赤芍、云山楂八种中药材混合后用水浸提取，浓缩浸取液至相对密度为1.10(80～85℃)，得B料清膏；
③、制备三七脂肝片剂：在A料粉剂中加入5％淀粉、3％糊精混合均匀后，加入B料清膏搅拌均匀，制成颗粒，干燥，再加入总量的0.5％硬脂酸镁压片。

制备方法与实施例1相同。
Claims (6)
1、一种三七脂肝制剂，其特征在于它是由以下重量份原料制成的药剂：
三七20-30 莪术20-30 菟丝子20-30
菊花20-30 白术20-30 泽泻10-30
白芍10-30 荷叶20-30 青皮10-30
赤芍20-30 云山楂20-40。
2、根据权利要求1所述的三七脂肝制剂，其特征在于：三七脂肝制剂可制成片剂、胶囊剂、丸剂或颗粒剂。
3、权利要求1所述的三七脂肝制剂的制备方法，其特征在于：按以下步骤制备得三七脂肝片剂：
①、制作A料粉剂：将三七、泽泻、白芍三种中药材混合，进行超微粉碎至400目，得A料粉剂；
②、制作B料清膏：将莪术、菟丝子、菊花、白术、荷叶、青皮、赤芍、云山楂八种中药材混合后用水浸提取，浓缩浸取液至相对密度在80～85℃时为1.10，得B料清膏；
③、制备三七脂肝片剂：在A料粉剂中加入5％淀粉、3％糊精混合均匀后加入B料清膏搅拌均匀，制成颗粒，干燥，再加入总量的0.5％硬脂酸镁压片。
4、权利要求1所述的三七脂肝制剂的制备方法，其特征在于：按以下步骤制备得三七脂肝丸剂：
①、制作A料粉剂：将三七、泽泻、白芍三种中药材混合，进行超微粉碎至400目，得A料粉剂；
②、制作B料清膏：将莪术、菟丝子、菊花、白术、荷叶、青皮、赤芍、云山楂八种中药材混合后用水浸提取，浓缩浸取液至相对密度在80～85℃时为1.10，得B料清膏；
③、在B料清膏中加入20％炼蜜混匀后与A料粉剂一起泛丸，干燥，打光，得三七脂肝丸剂。
5、权利要求1所述的三七脂肝制剂的制备方法，其特征在于：按以下步骤制备得三七脂肝胶囊剂：
①、制作A料粉剂：将三七、泽泻、白芍三种中药材混合，进行超微粉碎至400目，得A料粉剂；
②、制作B料清膏：将莪术、菟丝子、菊花、白术、荷叶、青皮、赤芍、云山楂八种中药材混合后用水浸提取，浓缩浸取液至相对密度在80～85℃时为1.10，得B料清膏；
③、将A料与B料混合搅拌，干燥，粉碎过60～80目筛，分装成胶囊，得三七脂肝胶囊剂。
6、权利要求1所述的三七脂肝制剂的制备方法，其特征在于：将11种中药材混合，粉碎成粗粉后用水浸提取，浓缩浸取液至相对密度80～85℃时为1.10的清膏，加入20％糖粉和5％糊精，混合均匀后制成颗粒剂。
