背景技术
单核细胞、巨噬细胞、树突状细胞在机体的免疫防疫系统中是极为重要的,通过其呈递抗原和调节T淋巴细胞功能的能力,在诱导获得性免疫应答中发挥重要作用。
单核细胞是来源于髓样干细胞的单个核巨噬白细胞。单核细胞在血流中循环,然后迁移进入组织,在组织中它们成熟为巨噬细胞。单核细胞和巨噬细胞是免疫过程中的第一道防线。单个核巨噬细胞在适应性免疫应答的识别和激活期作为辅助细胞发挥功能。它们的主要功能是以能被T淋巴细胞识别的方式呈递抗原,以及产生分泌型膜蛋白作为T细胞激活的次级信号。有的单个核巨噬细胞可以分化为树突状细胞,其在诱导T淋巴细胞对蛋白抗原的应答中发挥重要作用。

已经显示,许多化合物,例如,集落刺激因子和某些细胞因子都能调节单个核巨噬细胞的发育。例如,粒细胞-巨噬细胞集落刺激因子(GM-CSF)是一种细胞因子,它能诱导许多免疫活性细胞群的分化、增殖和激活。GM-CSF促进细胞介导免疫的发生,近来研究表明,GM-CSF作用中的关键事件是诱导单核细胞分化为树突状细胞,其为有效的抗原呈递细胞。(Bell,D.等.,Adv Immunol,72:255-324(1999);Avigan,D.,Blood Rev.,13:51-64(2000))。
由于这些化合物例如GM-CSF作为免疫佐剂的效力,特别的兴趣集中到将它们用于克服与癌症和慢性感染相关的免疫应答低下(综述见Lawson,D.,和Kirkwood,J.M.,J.Clin.Oncol.,18:1603-1605(2000))。例如,有人提出用GM-CSF进行治疗以保护对恶性黑色素瘤的复发和致死具有高度风险的相当大的一部分病人。在Spitlier及其同事的研究中,GM-CSF治疗的黑色素瘤患者(处于疾病III期或IV期)的半数生存时间与相应的对照组患者相比,增加了三倍(Spitlier等.,J.Clin.Oncol.,18:1614-1621)。
尽管有这些结果,仍需对用单核细胞促成熟疗法治疗赘生性疾病和慢性感染进行改进。
发明内容
本公开发明涉及到促单核细胞成熟的组合物和方法。在一个实施方案中,提供了促单核细胞成熟的一种方法。将包括一种活性氧(ROS)抑制剂或清除剂的组合物和一种单核细胞促成熟剂共同给药,针对大多数的单核细胞以促进单核细胞成熟。单核细胞促成熟剂包括白介素-1(IL-1)、粒细胞-巨噬细胞集落刺激因子(GM-CSF)、白介素-3(IL-3)、白介素-4(IL-4)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、粒细胞集落刺激因子(G-CSF)、巨噬细胞集落刺激因子(M-CS F)、白介素-12(IL-12)、白介素-15(IL-15)或白介素-18(IL-18)。单核细胞促成熟剂可单独给药或与疫苗、多种抗原或佐剂联合给药。ROS抑制剂可包括组胺和/或组胺相关化合物,例如盐酸组胺、磷酸组胺和组胺受体激动剂。过氧化氢或ROS清除剂,例如过氧化氢酶、谷胱甘肽过氧化物酶或抗坏血酸过氧化物酶也可与单核细胞促成熟剂联合给药以促进单核细胞发育成为树突状细胞。
有利的是,促进单核细胞成熟的方法通过体内给予一种ROS抑制剂和至少一种单核细胞促成熟剂得到了实现。单核细胞促成熟剂和ROS抑制剂或清除剂的给药可同时进行。另一可选择方案是,ROS抑制剂的给药可在给予单核细胞促成熟剂后的24小时内完成。
本发明的另一个方面,R0S抑制剂或清除剂以大约0.1~20mg/天的剂量给药。优选地,ROS抑制剂或清除剂以大约0.5~8mg/天的剂量给药。更优选地,ROS抑制剂或清除剂的剂量为大约1~6mg/天。有利的是,单核细胞促成熟剂以大约500~大约1,000,000U/公斤/天的剂量给药;更优选地,该剂量为大约1,000-大约500,000U/公斤/天;甚至更优选地,单核细胞促成熟剂的给药剂量为大约3,000-大约200,000U/公斤/天。

此外,慢性感染也可由寄生虫引起。寄生虫可包括利什曼虫、弓形体、锥虫、疟原虫、血吸虫或脑胞内原虫等种类。
同样地,提供了一种药物组合物,其包括一种能有效促进单核细胞成熟的化合物和一种ROS抑制剂或清除剂,两者结合入一个单一的制药学可接受的递送体中。能有效促进单核细胞成熟的化合物包括IL-1,GM-CSF,IL-3,IL-4,IL-6,TNF-α,G-CSF,M-CSF,IL-12,IL-15或IL-18。优选地,能有效促进单核细胞成熟的化合物为GM-CSF。
在另一个实施方案中,公开了制造药物组合物的方法,该药物组合物包括以制药学可接受的方式提供一种ROS抑制剂或清除剂和至少一种单核细胞促成熟剂。优选地,单核细胞促成熟剂包括IL-1,GM-CSF,IL-3,IL-4,IL-6,TNF-α,G-CSF,M-CSF,IL-12,IL-15或IL-18。更优选地,单核细胞促成熟剂为GM-CSF。
优选实施方案详述
本公开发明涉及促进单核细胞成熟的组合物和方法。给予一种或多种单核细胞促成熟剂,以加强处于多种疾病状态研究对象的免疫系统,这些疾病状态包括癌症和多种感染性疾病。这里公开的组合物和方法促进单核细胞成熟为树突状细胞,它们属于哺乳动物细胞免疫系统最有效的抗原呈递细胞。因此,这里公开的组合物和方法对于治疗多种疾病状态是有用的,在这些疾病状态中,树突状细胞的抗原呈递和其后由宿主免疫系统的清除参与了疾病的解除。

CD86(原来的B7-2)是树突状细胞的一个细胞表面标记物(Kiertscher和Roth,J.,Leukocyte Biol.,59:208-218)。CD86也是细胞免疫发育的一种重要辅助刺激分子(Gieseler等.,Dev.Immunol.,6:25-39(1998);Slavic等.,Immunol.Res.,19:1-24(1999);Croft等.,Crit.Rev.Immunol.,17:89-118(1997))。代表性的,在单核细胞和其它抗原呈递细胞上的CD86与CD28相互作用,后者是表达在T细胞上的一种抗原(Slavik,J.M.等.,Immunol.Res.,19:1-24(1999))。这种CD28/CD86相互作用为T细胞提供活化信号,放大并维持抗原和T细胞受体相互作用诱导的T细胞活化{McCoy,K.D.等.,Immunol.Cell Biol.,77:1-10}。
本公开发明部分是基于,抑制或清除ROS的化合物能够促进单核细胞成熟这一令人惊讶的发现。组胺保护T细胞和NK细胞不受单核细胞和巨噬细胞诱导的氧化抑制。通过这种机制,组胺使淋巴细胞最大程度激活,以对活化的细胞因子作出应答,例如:IL-2或α-IFN。在III期临床试验中,对患有例如恶性黑色素瘤的患者,盐酸组胺与IL-2同时给药。最初结果表明,在正规的白介素治疗中加入组胺显著提高有肝脏转移的黑色素瘤患者的生存率。
ROS抑制剂或清除剂在体外增强CD86抗原在人单核细胞上的表达,表明组胺和组胺相关化合物促进从单核细胞生成树突状细胞。因此,本发明的一个实施方案提供了ROS产生和释放抑制剂及ROS清除剂的给药,以增强某些促进单核细胞成熟的化合物的作用。这里所用的“单核细胞促成熟剂”包括所有能促进单核细胞成熟为树突状细胞的物质。这些物质包括,但不限于,某些细胞因子,例如,白介素-1(IL-1)、粒细胞-巨噬细胞集落刺激因子(GM-CSF)、白介素-3(IL-3)、白介素-4(IL-4)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、粒细胞集落刺激因子(G-CSF)、巨噬细胞集落刺激因子(M-CSF)、白介素-12(IL-12)、白介素-15(IL-15)和白介素-18(IL-18)。
单独给药降低ROS量的化合物,或与GM-CSF或其它单核细胞促成熟剂联合给药,能够协同作用刺激单核细胞成熟为树突状细胞。单核细胞成熟,在宿主防御系统防止在体内出现肿瘤性疾病和慢性感染中发挥重要作用。因此,单独给药ROS清除剂或抑制细胞内ROS产生和释放的化合物、或与其它单核细胞促成熟剂联合给药能有效治疗多种疾病状态,包括但不限于,癌症和慢性感染,如由持久的病毒或寄生虫感染引起。
因此,本发明的一个方面包括促进单核细胞成熟的组合物和方法。在一个实施方案中,单核细胞促成熟剂与ROS抑制剂或清除剂联合给药。单核细胞促成熟剂是能够促进单核细胞向树突状细胞发育和分化的任何一种化合物。合适的单核细胞促成熟剂包括但不限于,IL-1,GM-CSF,IL-3,IL-4,IL-6,TNF-α,G-CSF,M-CSF,IL-12,IL-15,和IL-18。优选地,单核细胞促成熟剂为GM-CSF。ROS产生和释放抑制剂和ROS清除剂也可被视为单核细胞促成熟剂,因为给予这些化合物可以提高单核细胞成熟效率。

本公开发明的方法和组合物进一步涉及与单核细胞促成熟剂共同给予ROS清除剂以促进单核细胞发育为树突状细胞,从而促进抗原呈递和其后的某些靶细胞清除。众所周知的ROS清除剂包括过氧化氢酶,谷胱甘肽过氧化物酶和抗坏血酸过氧化物酶。此外,已知维生素A、E和C有清除剂活性。矿物例如硒、锰也是有效的ROS清除剂。本公开发明意欲包括已列出的化合物和有类似ROS抑制剂活性的化合物。
组胺或组胺相关化合物和单核细胞促成熟剂可单独给药促进单核细胞成熟,或与多种药物、疫苗、抗原或佐剂联合给药从而对多种疾病状态提供有效治疗。
单核细胞促成熟剂的给药
给予一种或多种单核细胞促成熟剂以促进单核细胞发育为树突状细胞,同时给予上述讨论的抑制剂或清除剂,这可通过本领域技术人员所熟知的多种方法在体外或体内完成。当体内给予本公开发明的组合物时,合适的给药方法包括通过静脉、腹膜内或肌肉注射进行胃肠外给药。单核细胞促成熟剂和ROS清除剂可分别给药或作为一个单一组合物给药。当分别给药时,涉及到单核细胞促成熟剂是先给予还是后给予。
此处公开的化合物可经水给药,水中可含有或不含表面活性剂如羟丙基纤维素。也涉及到分散液,如使用甘油、液态聚乙二醇和油类。制品中也可加入抗菌剂。可注射制品可包括无菌水溶液或分散液和在使用前可溶解或悬浮于无菌溶液环境中的粉剂。递送剂,例如溶剂或分散介质包含水、多羟基醇、植物油和相似的物质,都可以加入到此处描述的化合物中。涂料例如卵磷脂和表面活性剂可用于维持组合物合适的流动性。可加入等渗剂如蔗糖或氯化钠,以及用于延缓活性化合物吸收的物质,如单硬脂酸铝和明胶。无菌可注射溶液根据本领域技术人员所熟知的方法进行制备,在贮存和/或使用前可经过滤。无菌粉剂可从溶液或悬浮液中经真空抽干或冻干。本发明也涉及缓释制剂。本发明中用于组合物的任何物质在所用剂量下为制药可接受且完全无毒。

应了解的是,化合物可单剂量给药或更优选地,延长给药时间。典型地,治疗可进行一段时间,上至约一周、一个月或甚至一年以上的时间。在一个实施方案中,可给予三年或更长时间的治疗。在有些情况下,治疗可停止并在一段时间后重新开始。日剂量可以单剂量给药,或分为多个剂量,特别是如果观察到有副作用时。此外,化合物可以单组合物给药,或分别给药,若为分别给药,化合物应在同一天给予,这样单核细胞促成熟剂的活化作用得以增强。

对用单核细胞促成熟剂和ROS抑制剂或清除剂联合治疗患有癌症或慢性感染的患者的方法进行了描述。癌症是由单个异常细胞的子代细胞进展性的,不受控制的生长所引起。这里所用的术语“癌症”包括赘生性疾病、赘生性细胞、肿瘤、肿瘤细胞、恶性肿瘤和任何转化细胞,包括实体肿瘤和弥散性的赘生性疾病。从历史的观点上说,癌细胞通常被认为能够逃避免疫系统的监测和清除,因为癌细胞含有与机体其它非癌细胞相同的遗传物质。癌细胞与机体健康细胞的遗传一致性或相似性被设想为导致了从健康细胞中识别癌细胞的困难,因此,免疫系统不能作出有效的免疫应答,正如癌细胞在体内持续存在所证实的那样。
相反的,许多肿瘤相关抗原(TuAAs)被描述为能够,且的确可以激发免疫应答。许多研究描述过肿瘤浸润性淋巴细胞,它能在体外杀死呈递来源于不同TuAAs的肽的靶细胞。在某些实施方案中,给予受治疗者本公开发明的组合物以治疗癌症。已知的单核细胞促成熟剂与组胺或组胺相关化合物联合给药可以促进单核细胞成熟为树突状细胞。结果,更多的抗原呈递细胞呈递TuAAs给细胞毒T淋巴细胞。细胞毒T淋巴细胞可特异地攻击并杀死肿瘤细胞。因此,增加能将TuAAs呈递于其细胞表面的树突状细胞数目将有益于癌症的治疗。

这里所描述的方法可单独使用或与其它抗癌治疗联合使用,这由医师来决定。例如,特别涉及到ROS抑制剂或ROS清除剂和至少一种单核细胞促成熟剂与癌疫苗、佐剂和/或抗癌药物联合给药。
同样涉及到治疗慢性感染的方法。慢性感染通常由能逃逸宿主免疫系统监视和其后清除的病原感染所致。这些病原的例子包括,但不限于某些病毒、原虫或其它生物体。
持续存在的病原体利用广泛的多种机制以在宿主机体内形成慢性感染。共有的特点是减少或改变抗原的表达。病毒抗原的呈递开始于病毒抗原被蛋白酶体消化为肽。蛋白酶体将蛋白消化为肽后,一些肽被输载到内质网的I类复合体中,并转运到细胞表面。在细胞表面I类-肽复合物被位于细胞毒T细胞表面的T细胞受体识别,然后感染的细胞被杀死。
某些病毒和其它细胞内寄生虫通过下调T细胞有效识别感染细胞必需的宿主分子的表达,能逃避T细胞的识别。慢性感染,例如由腺病毒、巨细胞病毒、Epstein-Barr病毒、单纯疱疹病毒1、单纯疱疹病毒2、人疱疹病毒6、水痘-带状疱疹病毒、乙型肝炎病毒、丁型肝炎病毒、乳头瘤病毒、细小病毒B19、多瘤病毒BK、多瘤病毒JC、丙型肝炎病毒、麻疹病毒、风疹病毒、人类免疫缺陷病毒(HIV)、人T细胞白血病毒I或人T细胞白血病毒II引起,持续存在于宿主体内是由于宿主不能作出有效的细胞毒T细胞应答来对抗这些病毒。同样的,许多寄生虫,例如利什曼虫、弓形体、锥虫、疟原虫、血吸虫或脑胞内原虫等种类持续存在于宿主体内。不限于某一特殊理论,猜测这种持续存在可归因于不理想的识别和其后的宿主免疫系统进行的病原清除,也就是,细胞毒T淋巴细胞不能识别和杀死感染的靶细胞。另外一种解释是,当巨噬细胞在制造ROS时,巨噬细胞不能呈递抗原,因为ROS会杀伤T细胞。组胺和组胺相关化合物可作为一个转换器,以这种方式降低ROS的量。因此,观察到树突状细胞表面CD86增加,预示由树突状细胞进行的抗原呈递增加。
这里所描述的,先前在本领域未被承认,通过给予有需要的对象促成熟剂,例如组胺,可能能够增加单核细胞成熟为树突状细胞的数量和效率。这种给药通过抗原呈递细胞起到增加病毒抗原呈递的作用。通过促进单核细胞发育为树突状细胞以进行抗原呈递,更多的抗原呈递细胞将在其细胞表面展示慢性病毒感染相关的抗原。细胞毒T细胞的受体将与展示在抗原呈递细胞表面的抗原结合,结果慢性感染的细胞将被细胞毒T淋巴细胞杀死。
因此,公开了治疗患有慢性感染的对象的方法。慢性感染可由病毒或寄生虫,如上描述种类,所引起。在确定患有慢性感染的对象后,以制药可接受形式给予对象ROS抑制剂或清除剂,如组胺或组胺相关化合物。在一个实施方案中,至少一种单核细胞促成熟剂也以制药可接受形式给予对象。在另外一个实施方案中,给予多种单核细胞促成熟剂。至少一种单核细胞促成熟剂和ROS抑制剂或清除剂可分别或作为一个单组合物给药。当单独给药时,涉及到单核细胞促成熟剂是先给药还是后给药。合适的单核细胞促成熟剂包括IL-1,GM-CSF,IL-3,IL-4,IL-6,TNF-α,G-CSF,M-CSF,IL-12,IL-15和IL-18。优选地,单核细胞促成熟剂为GM-CSF。上面描述的治疗慢性感染的方法可单独使用或与其它的抗病毒或抗寄生虫治疗相结合,这由有经验的专业人员决定。例如,进行单核细胞促成熟剂和ROS抑制剂或清除剂的给药时,可一同给予抗病毒疫苗或多种佐剂,本领域技术人员都知道其可刺激对象的免疫系统。
下面的实施例讲授了此处公开的,通过与单核细胞促成熟剂一同给予ROS抑制剂或清除剂,以促进单核细胞成熟为树突状细胞。实施例仅用于阐述本发明,不应该看作是对此处公开的本发明范围的限制。下面描述的治疗方法可通过本领域技术人员所熟知的经验性技术进行优化。此外,本领域技术人员可以使用下列实施例描述的原则来实践此处公开发明的全部范围。
实施例1
用组胺和GM-CSF处理单核细胞
分离单核细胞。在瑞典Sahigren′s医院Gteborg血液中心,从四个健康的献血者获取外周静脉血,并新鲜制备为白细胞压积物(leukopacks)。血液(65ml)与92.5ml Iscove′s培养液、35ml6%的右旋糖苷(Pharmacia,Uppsala,Sweden)和7.5ml ACD(Baxter,Deerfield,Illinois)混合。室温孵育15min后,将上清小心铺到Ficoll-Hypaque(Lymphoprep,Nyegaard,Norway)上。380g离心15min后在分界面收集单核细胞,用PBS洗两遍,然后重悬于含10%人AB+血清的Iscove′s培养液中。
用逆流离心淘洗(CCE)技术来进一步从单个核细胞中富集单核细胞,该技术在Hellstrand,K.等.,J.Immunol.,153:4940-4947(1994)中进行了详细描述。简单地说,单个核细胞重悬于淘洗缓冲液中,其为在NaCl缓冲液中含0.05%BSA和0.015%EDTA,然后放入到Beckman J2-21超速离心机中,用JE-6B转子2100rpm进行离心。在18ml/min流速下获得含有大于90%单核细胞的成分(表现在其CD14抗原的表达上)。
处理和细胞染色。单核细胞(3×105细胞/ml,总体积为1ml)在24孔板中,37℃孵育过夜(16hrs)。在孵育过程中,单核细胞用终浓度为10μM的盐酸组胺(购自Sigma,St.Louis,USA)和/或终浓度为0.15μg/ml的GM-CSF(Leukomax,Schering Plough,Stockholm,Sweden)处理。孵育完成后,将细胞进行离心,CD14(一种泛单核细胞抗原)或CD86(一种树突状细胞标记)的单克隆抗体(购自Becton Dickinson,Stockholm,Sweden)加入到离心沉淀块中进行染色,按Hellstrand,K.等.,J.Immunol 153:4940-4947(1994)中的描述。漂洗后,对细胞进行分析,以了解细胞表面表达的CD14和CD86。
结果
与GM-CSF过夜孵育后,平均88%的单核细胞表达CD86。用GM-CSF+组胺处理的单核细胞相应的数值为97%。
组胺加强GM-CSF诱导的CD86抗原表达强度的上调。明确的是,细胞表面表达CD86的强度(反映了每一个细胞表达了多少CD86分子)经组胺处理后适度加强,组胺和GM-CSF的联合,导致CD86的表达较单独用组胺或单独用GM-CSF要高(见下表1)。组胺+GM-CSF处理单核细胞,其CD86的表达较所有实验中用GM-CSF单独处理的要高。用组胺+GM-CSF处理的单核细胞,细胞表面表达的CD86中值为单独用GM-CSF处理相应单核细胞的352%(平均值)/366%(中间值)。

用GM-CSF处理的单核细胞和用GM-CSF+组胺处理的单核细胞,其细胞表面表达的CD14是相似的,表明用GM-CSF+组胺处理后CD86表达的增强不是抗体吸收非特异性增加的结果。
人们发现组胺加强GM-CSF诱导的CD86辅助刺激分子在人单核细胞中的表达。组胺上调单核细胞HLA-DR的表达,其在树突状细胞上特征性高表达。组胺和单核细胞促成熟剂如GM-CSF共同给药增强了单核细胞促成熟剂的活性。组胺和单核细胞促成熟剂共给药引起CD86的可检测到的增加,其为树突状细胞表面标记物。此外,组胺诱导单核细胞和巨噬细胞上重要的辅助刺激分子上调。组胺增强GM-CSF促进单核细胞成熟为树突状细胞的效应。给予组胺,通过促进从单核细胞发育为专职的抗原呈递树突状细胞,与适应性免疫具有显著的相关性。
实施例2
给予盐酸组胺以保护NK细胞、T细胞和NK淋巴细胞防止氧化诱导
的凋亡
氧化应激,例如:活性氧(ROS)引起的毒性,被认为导致在恶性肿瘤和慢性病毒感染的部位造成了免疫抑制状态。位于或邻近肿瘤处的淋巴细胞显示出氧化损伤的表现,包括较高程度的凋亡和跨膜信号转导缺陷。肿瘤生长部位的氧化应激,可假定以邻近的吞噬细胞如单核细胞/巨噬细胞(MO)或噬中性粒细胞(GR)产生的ROS为传递媒介。组胺,巨噬细胞中的一种ROS产生抑制剂,目前被用作淋巴细胞激活细胞因子如IL-2和IFN-α的辅剂,以增强细胞因子效率。临床试验结果表明患有肝脏转移性黑色素瘤和患有慢性丙型肝炎的患者得益于组胺治疗,表明组胺能在肝脏组织中选择性发挥功效。
在人肝脏中,NK细胞以及NK/T细胞比在外周血中更丰富。因此,比较了三种类型的人淋巴细胞CD3+T细胞,CD3-/56+NK细胞,和CD3+/56+NK/T细胞,在体外对氧化诱导凋亡的敏感性。在与自体同源的MO或GR孵育后,或用过氧化氢,一种活性氧,处理后,所有的三种细胞类型都出现凋亡。因而,在淋巴细胞与MO的比率为1∶1时,35±5%的NK细胞,和76±7%的NK/T细胞凋亡。在淋巴细胞与GR的比率为1∶1时,相应的凋亡率为21±4%(T细胞),30±7%(NK细胞),和66±8%(NK/T细胞)。所有的数值都是平均值±s.e.m(n=15-30个献血者)。所有细胞类型的凋亡都被组胺显著抑制(p<0.001)。NK/T细胞和NK细胞对氧化诱导凋亡的较高敏感性在实验中得到证实,凋亡被外源性的过氧化氢所诱导,过氧化氢浓度为25μM时,35±5%的T细胞、65±7%的NK细胞,和78±7%的NK/T细胞(n=8)凋亡。我们推断,肝脏型的淋巴细胞,特别是NK/T细胞,对氧化应激特别敏感。因此,由于肝浸润性淋巴细胞对于氧化诱导的凋亡具有较高的敏感性,认为抗氧化剂如组胺在肝脏肿瘤或慢性感染中更为有效。
治疗慢性病毒感染
病毒和其它胞内寄生虫感染的成功消退需要宿主免疫系统识别感染细胞为“非己”的,并作出有效的免疫应答以抑制其生长和有传染性物质的播散。由于病毒感染宿主自身的细胞,因此对感染细胞的识别及其后的消灭是较为困难的。许多病毒和细胞内寄生虫持续存在于宿主体内是由于宿主免疫系统不能将感染的细胞识别为“非己”的,从而不能作出针对消除感染细胞的有效的细胞毒T细胞应答。本发明此处公开的组合物和方法可用于治疗慢性病毒感染。ROS抑制剂或清除剂与单核细胞促成熟剂共给药将导致对病毒感染免疫应答的增加。
实施例3
治疗生殖器疱疹
病毒中的疱疹病毒家族,例如,包含了属于十分重要的人类已知病原体。疱疹感染周期性发作且极端疼痛。不适的程度主要归因于,疱疹病毒在宿主的三叉神经节处于静息状态,当疾病发作时通过神经到达皮肤表面。
患有生殖器疱疹的女性研究对象,给予包含浓度为制剂重量的0.7%的磷酸组胺和浓度为制剂重量的0.15%的GM-CSF的药膏。使用涂药器将药膏注入到阴道中,治疗那里的疱疹性病患。药膏每天使用三次,用五天。组合物起到促进单核细胞成熟,从而发动宿主免疫系统去攻击病毒感染的细胞。无不适反应,且治疗根除了病毒感染。
实施例4
治疗水痘
配制含有减毒水痘病毒、占疫苗重量0.025%的GM-CSF和占疫苗重量1.0%的组胺的疫苗,对研究对象进行肠胃外给药。组胺起增强作为单核细胞促成熟剂的GM-CSF活性的作用。宿主作出免疫应答以对抗水痘病毒。
实施例5
治疗HIV
患有HIV感染的研究对象,静脉给予含有有效剂量的蛋白酶抑制剂、制药可接受形式的组胺受体激动剂和制药有效形式的G-CSF的组合物。组合物促进单核细胞成熟,从而发动宿主免疫系统攻击病毒感染细胞。观察到呈递HIV抗原的树突状细胞数量增加。组胺受体激动剂促进单核细胞成熟为树突状细胞。
治疗慢性寄生虫感染
持续存在于宿主体内致慢性感染的病原体的例子包括原生动物,例如利什曼虫、弓形体、锥虫、疟原虫(疟疾的致病原)、血吸虫和脑胞内原虫。这些生物体在全世界引起大量的健康风险。例如疟疾,是一种全球性的破坏性疾病,每年它感染超过4亿的人并使3百万人致死。
实施例6
治疗疟疾感染
患有疟疾的研究对象经胃肠外按100,000U/kg/天的剂量给予GM-CSF和6mg/天的剂量给予5-羟色胺。组合物起促进单核细胞成熟的作用。从而发动宿主的免疫系统清除感染了疟原虫的细胞。特别地,5-羟色胺增强G-CSF的单核细胞促成熟活性。观察到呈递疟原虫相关抗原的树突状细胞数目增加。T细胞识别疟原虫属抗原为外源物,攻击并杀死细胞。感染细胞的数目下降。
治疗赘生性疾病
每年诊断为癌症病例的数目在美国大约为150万,在全球超过500万。恶性肿瘤,例如白血病、癌、淋巴瘤、星形细胞瘤、肉瘤、神经胶质瘤、视网膜母细胞瘤、黑色素瘤、Wilm′s瘤、膀胱癌、乳腺癌、结肠癌、胰腺癌、前列腺癌、肺癌、肝癌、胃癌、子宫颈癌、睾丸癌、肾细胞癌和脑癌,引起显著的健康风险,如不治疗会导致死亡。
为患有癌症的患者提供了很多种治疗方式,包括外科切除、化疗、放射治疗和骨髓移植。虽然多种传统的癌症治疗通常能有效地阻碍肿瘤生长,但正常细胞常常也受到危害。因而,传统的癌症治疗伴有许多副作用。
最近,攻克癌症的努力集中在动员患者免疫系统清除展示肿瘤相关抗原的肿瘤细胞,从而避免传统癌症治疗的副作用。促进单核细胞成熟为抗原呈递的树突状细胞增强了宿主抗肿瘤细胞的免疫应答。
组胺和组胺相关化合物如盐酸组胺显示出能保护T细胞和NK细胞对抗单核细胞(特异地,巨噬细胞)诱导的氧化抑制。通过这种机制,在对活化的细胞因子如IL-2或IFN-α作出应答时,组胺使淋巴细胞的激活最优化。在正在进行的临床试验中,对患有转移性黑色素瘤、肾细胞癌、急性髓性白血病和丙型肝炎的患者,盐酸组胺单独用药和与IL-2联合给药或再与IFN-α三者联合给药。结果表明在正规的IL-2治疗中加入组胺显著提高了有肝转移的黑色素瘤患者的生存率。GM-CSF诱导单核细胞分化为树突状细胞,其为有效的抗原呈递细胞。GM-CSF的这些特性是在转移性黑色素瘤中将GM-CSF用作免疫佐剂的重要背景。
实施例7
治疗转移性黑色素瘤
给予患有肝脏转移性黑色素瘤的女性研究对象200,000U/kg/天的G-CSF和10mg/天剂量的磷酸组胺。给予的药剂起到促进单核细胞成熟的作用,从而使得患者免疫系统能够攻击肝脏中的癌细胞。观察到肿瘤块变小。
实施例8
治疗前列腺癌
给予患有前列腺癌的男性患者300,000U/kg/天的GM-CSF、100,000U/kg/天的IL-12和4mg/天的盐酸组胺。给予的药剂起到促进单核细胞成熟的作用,从而使得患者免疫系统能够攻击肿瘤细胞。观察到肿瘤块变小。
前面的描述详述了本发明的某些实施方案。然而应知道的是,不管文中前述的内容有多详细,本发明可以多种方式来实施。也如前述,应该注意的是,在描述本发明的某一特征或方面时使用特殊术语不应该被认为是这些术语在这里被重新定义,从而被限制到包括与该术语相关的本发明特性或方面的任何具体特征。本发明的范围因此应该被认为与附属的权利要求及其任何等价物相一致。
Claims (38)
1.一种促进单核细胞成熟的方法,包含给予包括单核细胞的细胞群体以至少一种单核细胞促成熟剂和至少一种ROS产生和释放抑制剂或ROS清除剂。
2.权利要求1中的方法,其中所述单核细胞促成熟剂从由IL-1,GM-CSF,IL-3,IL-6,TNF-α,G-CSF,M-CSF,IL-4,IL-12,IL-15和IL-18组成的组中选择。
3.权利要求1中的方法,其中所述ROS抑制剂或清除剂和所述的至少一种单核细胞促成熟剂进行体内给药。
4.权利要求1中的方法,其中所述的ROS抑制剂或清除剂和所述的至少一种单核细胞促成熟剂的给药是同时进行的。
5.权利要求1中的方法,其中所述的ROS抑制剂或清除剂的给药是在所述的至少一种单核细胞促成熟剂给药后的24小时内进行。
6.权利要求1中的方法,其中所述的至少一种单核细胞促成熟剂给药剂量为大约500-大约1,000,000U/kg/天。
7.权利要求1中的方法,其中所述的至少一种单核细胞促成熟剂给药剂量为大约3,000-大约200,000U/kg/天。
8.权利要求1中的方法,其中所述的ROS抑制剂或清除剂给药剂量为大约0.1-大约20mg/天。
9.一种增加单核细胞促成熟剂活性的方法,其中包括:
a)以制药可接受形式给予细胞群体ROS抑制剂或清除剂;和
b)给予单核细胞促成熟剂,其中所述单核细胞促成熟剂的单核细胞成熟效应被增强。
10.权利要求9中的方法,其中所述单核细胞促成熟剂为一种细胞因子。
11.权利要求10中的方法,其中细胞因子是从由IL-1,GM-CSF,IL-3,IL-4,IL-6,TNF-α,G-CSF,M-CSF,IL-12,IL-15和IL-18组成的组中选出。
12.权利要求10中的方法,其中细胞因子为GM-CSF。
13.权利要求10中的方法,其中细胞因子给药剂量为大约500-大约1,000,000U/kg/天。
14.权利要求10中的方法,其中细胞因子给药剂量为大约3,000-大约200,000U/kg/天。
15.权利要求9中的方法,其中所述的ROS抑制剂或清除剂的给药和所述的至少一种单核细胞促成熟剂的给药是同时进行的。
16.权利要求9中的方法,其中所述的ROS抑制剂或清除剂的给药是在所述的至少一种单核细胞促成熟剂给药后的24小时内进行。
17.含有ROS抑制剂或清除剂和至少一种单核细胞促成熟剂的组合物用于制备治疗癌症的药剂的用途,所述治疗包括:
a)确定患有癌症的对象;
b)以制药可接受的形式给予所述的ROS抑制剂或清除剂;以及
c)给予所述的至少一种单核细胞促成熟剂。
18.权利要求17的用途,其中所述的至少一种单核细胞促成熟剂是从由IL-1,GM-CSF,IL-3,IL-4,IL-6,TNF-α,G-CSF,M-CSF,IL-12,IL-15和IL-18组成的组中选出的一种细胞因子。
19.权利要求17的用途,其中所述的至少一种单核细胞促成熟剂的给药剂量为大约500-大约1,000,000U/kg/天。
20.权利要求17的用途,其中所述的至少一种单核细胞促成熟剂的给药剂量为大约3,000-大约200,000U/kg/天。
21.权利要求17的用途,其中所述的至少一种单核细胞促成熟剂和所述的ROS抑制剂或清除剂的给药同时进行。
22.权利要求17的用途,其中所述的至少一种单核细胞促成熟剂的给药是在所述的ROS抑制剂或清除剂给药后的24小时内进行。
23.含有ROS抑制剂或清除剂和至少一种单核细胞促成熟剂的组合物用于制备治疗慢性感染的药剂的用途,其中所述治疗包括:
a)确定患有慢性感染的对象;
b)以制药可接受形式给予一种ROS抑制剂或清除剂;以及
c)给予至少一种单核细胞促成熟剂,其中所述的单核细胞促成熟剂促进单核细胞的成熟。
24.权利要求23的用途,其中所述的至少一种单核细胞促成熟剂是从由IL-1,GM-CSF,IL-3,IL-4,IL-6,TNF-α,G-CSF,M-CSF,IL-12,IL-15和IL-18组成的组中选出的一种细胞因子。
25.权利要求23的用途,其中所述慢性感染由病毒引起。
26.权利要求25的用途,其中病毒是从由腺病毒、巨细胞病毒、埃-巴二氏病毒、单纯疱疹病毒1、单纯疱疹病毒2、人疱疹病毒6、水痘-带状疱疹病毒、乙型肝炎病毒、丁型肝炎病毒、乳头瘤病毒、细小病毒B19、多瘤病毒BK、多瘤病毒JC、丙型肝炎病毒、麻疹病毒、风疹病毒、人类免疫缺陷病毒(HIV)、人T细胞白血病毒I和人T细胞白血病毒II组成的组中选出的。
27.权利要求23的用途,其中所述慢性感染由寄生虫引起。
28.权利要求27的用途,其中所述寄生虫是从由利什曼虫、弓形体、锥虫、疟原虫、血吸虫和脑胞内原虫组成的组中选出的。
29.权利要求23的用途,其中所述的至少一种单核细胞促成熟剂和所述的ROS抑制剂或清除剂的给药同时进行。
30.权利要求23的用途,其中所述的至少一种单核细胞促成熟剂的给药是在所述的ROS抑制剂或清除剂给药后的24小时内进行。
31.一种药物组合物,其中含有:一种能有效促进单核细胞成熟的化合物和一种ROS抑制剂或清除剂,并组合到一种制药可接受的载体中。
32.权利要求31中的药物组合物,其中所述的能有效促进单核细胞成熟的化合物是从由IL-1,GM-CSF,IL-3,IL-4,IL-6,TNF-α,G-CSF,M-CSF,IL-12,IL-15和IL-18组成的组中选出的。
33.权利要求31中的药物组合物,其中所述的能有效促进单核细胞成熟的化合物是GM-CSF。
34.权利要求31中的药物组合物,其中所述的ROS抑制剂或清除剂是从由组胺、盐酸组胺、磷酸组胺、其它组胺盐、酯、前体药物、组胺受体激动剂、5-羟色胺、5-羟色胺激动剂和内源性组胺释放化合物组成的组中选出的。
35.权利要求31中的药物组合物,其中所述的ROS清除剂是从由过氧化氢酶、谷胱甘肽过氧化物酶、抗坏血酸过氧化物酶、维生素A、维生素E或维生素C组成的组中选出的。
36.一种制备药物组合物的方法,包括以制药可接受方式提供一种ROS抑制剂或清除剂和至少一种单核细胞促成熟化合物。
37.权利要求36中的方法,其中所述至少一种单核细胞促成熟化合物是从由IL-1,GM-CSF,IL-3,IL-4,IL-6,TNF-α,G-CSF,M-CSF,IL-12,IL-15和IL-18组成的组中选出的。
38.权利要求36中的方法,其中所述单核细胞促成熟化合物为GM-CSF。
