Abstract

技术领域
本发明涉及一种体内施用设定量的金雀异黄素(genistein)以保持哺乳动物的血浆金雀异黄素浓度在设定范围内的方法。更具体的,本发明是关于一种控制血浆金雀异黄素浓度以刺激骨发生并同时通过避免激活过氧化物酶体增殖因子活化的受体γ(peroxisome proliferator-activatedreceptorγ,PPARγ)以预防脂肪生成的方法。
本发明特别适合于预防肥胖症以及有着骨组织丢失的疾病或情况(例如骨质疏松症、Paget病、肿瘤患者的溶骨性转移灶、肝病的骨营养障碍、肾功能衰竭或血液透析引起的骨代谢改变、骨折、骨科手术、妊娠、哺乳、神经性厌食、或制动者、非承重(non-weight bearing)者、营养不良者或减肥者)。

称为过氧化物酶体增殖因子活化的受体(PPAR)的转录因子家族在调节饮食脂肪的储存和代谢上起到了关键作用。PPAR被克隆还不到十年,是核受体基因家族的孤立成员,该基因家族包括类固醇、视黄醛、和甲状腺激素受体。已经建议将PPAR用作为开发治疗人代谢性疾病药物的分子靶点。
有3种PPAR亚型,它们是不同基因的产物并常被表示为PPARα、PPARγ、PPARδ。和其他核受体基因家族成员一样,PPAR是配体活化的转录因子。激动剂配体和受体的结合造成PPAR靶基因编码的mRNA表达水平的改变。这个过程被称为“反式激活”,并发展出以细胞为基础的测定方法用于测定这种功能活性。
通过获得的潜在的和选择性的受体配体已经揭示了PPAR大部分的生物学特点。通过使用结合和功能测定,一些小组已经报道确定和优化了3种亚型的每种亚型的配体。在“反向内分泌学(reverse endocrinology)”中使用的这些化学工具揭示了PPAR在人生理和疾病过程中的作用。
PPARγ是3种PPAR亚型中至今被研究最多的亚型。PPARγ是调节脂肪细胞分化的关键转录因子。已经发现在存在弱PPARγ激动剂下成纤维细胞内PPARγ的强迫表达造成了细胞向脂肪细胞的分化。迄今为止获得的数据表明PPARγ在脂肪形成的信号链中起到了中枢性作用,同时也表明受体可以影响它本身的激动剂的生成和细胞摄取。
已知天然的PPARγ配体包括噻唑烷、多利多不饱和脂肪酸、花生四烯酸衍生物和来源于PGD2的J系列前列腺素,这些配体在微摩尔浓度结合并激活受体。
尽管激活PPARγ与骨发生的减少和脂肪生成的增加相关,但是PPARγ的激活也有健康益处。研究最多的PPARγ激动剂的治疗应用是治疗2型糖尿病。其他已经进行的研究表明PPARγ激动剂降低了各种脂肪异常血症的动物模型中的血浆甘油三脂、胆固醇和游离脂肪酸浓度。此外,PPARγ激动剂可以有抗高血压、抗炎症和抗肿瘤作用。
Liang et al,″Suppression of inducible cyclooxygenase and nitric oxidesynthase through activation of PPARγ by flavonoids in mouse macrophages″,FEES Letters 496(2001)12-18,报道黄酮类的芹菜(甙)配基、白杨素以及山奈素可以作为能结合并激活PPARγ的变构效应物。然而该研究中10μM浓度的金雀异黄素没有激活PPARγ活性。没有进行PPARγ与金雀异黄素的结合研究。这说明这些黄酮类对炎性疾病可能有治疗作用,例如粥样硬化和类风湿关节炎。
US 5,506,211(The UAB Research Foundation)描述了一种用于抑制成骨细胞活性并造成骨退化的方法,包括将成骨细胞与由一定量的黄豆中天然存在的金雀异黄素-葡糖苷共轭化合物组成的组合物接触。采用的金雀异黄素-葡糖苷共轭化合物的每天剂量在2-50mg范围。
US 5,953,996(Taishi Food Company)描述了一种用于促进骨化和预防骨盐成分丢失的方法,包括给予哺乳动物它们所需要的有效剂量的组合物,该组合物含有协同有效剂量的金雀异黄素和锌盐。可以看出该本发明人已经发现金雀异黄素和锌能增强骨发生。所描述的组合物是优选食入的,以致食入1个小时后的血中金雀异黄素浓度可以是10-7M或更高。
WO 99/66913(Sigma-Tau Healthscience)描述了一种组合物,它包括作为活性成分的丙酰基L-肉毒碱和金雀异黄素,并用于预防和/或治疗骨质疏松症和更年期综合征。
WO01 74345 A(Ingram Jonathan)描述了一种抑制体重增加、引起体重丢失或造成胃部饱胀感的方法,它包括施用至少一种异黄酮,例如金雀异黄素。施用的异黄酮可以在5-500mg/天的范围内。
US 6 326 366 (Protein Teclmologies International)是关于一种激素替代治疗方案,它包括一起施用治疗有效剂量的哺乳动物雌激素和异黄酮(例如金雀异黄素)的混合物。采用的异黄酮的每天剂量为20-1000mg范围。
EP-A 0829 261涉及一种用于脂肪细胞的脂肪降解的组合物,它包括至少一种异黄酮和/或它们的衍生物。适宜的异黄酮选自包括大豆黄素、大豆黄甙、金雀异黄素和金雀异黄甙的组。在该专利申请书中,将脂肪细胞暴露于3-100μM金雀异黄素后,可以观察到剂量依赖性的脂肪分解。在0.3-1μM金雀异黄素中没有观察到作用。
Harmon et al.(Am J Physiol Cell Physiol.2001 Apr;280(4):C807-13)报道了异黄酮对3T3-L1的脂肪生成和脂肪分解的不同作用的研究结果。结论是黄酮类的金雀异黄素和柑桔素以时间和剂量依赖性的方式抑制脂肪前体细胞的增生。当将金雀异黄素加入到诱导分化2天的融合后的脂肪前体细胞时,发现金雀异黄素抑制了PPARγ的表达。在第C182页看到“假定金雀异黄素促进了细胞培养中的脂肪分解以及抑制了脂肪生成,我们预计金雀异黄素将在体内将有类似的作用并可能促进体内脂肪的丢失”。另外它表明:“非毒性的药用剂量的金雀异黄素,8mg/kg体重,能将血清金雀异黄素提高到10μM范围...,能有效抑制3T3-L1细胞脂肪生成的水平。”
Dang et al.(J.of Bone and Mineral Res.,2000 Sep.;15(1),S496)描述了金雀异黄素和17D-雌二醇对成骨细胞生成和脂肪生成的影响的研究结果。本发明的两位本发明人是该文章的作者。摘要显示了低浓度金雀异黄素对KS485细胞的影响的数据。尽管该实验结果提供了本发明的基础,但是当时还不知道更高浓度的金雀异黄素产生了与更低浓度所观察到的相反及相对的作用。
Wang et al.(J Biol Chem.1999 Nov 5;274(45):32159-66)报道了一个研究结果,它表明酪氨酸激酶抑制剂抑制了地塞米松和甲基异丁基黄嘌呤(methylisobutylxanthine)所引起的脂肪生成的能力,并提供了酪氨酸激酶Syk在Gsα介导的脂肪生成调节上起到了关键作用的显著证据。在文章中有以下的描述:“探讨了金雀异黄素对D/M所引起的脂肪生成的作用的剂量依赖性。100μM金雀异黄素有效地消除了脂肪生成转化。10和33μM浓度的酪氨酸激酶抑制剂对脂肪生成都有着显著的抑制作用。在3.3μM,测定的最小浓度,金雀异黄素仅仅表现出对诱导剂D/M所引起的3T3-L1细胞分化的微弱的抑制作用。在先前任何的技术文献中都没有发现关于异黄酮特别是金雀异黄素结合并激活PPARγ,也就是作为PPARγ激动剂的能力。尽管金雀异黄素成分已经被用于治疗骨质疏松症以及各种其他病变和疾病,但是对这些异黄酮的确切功能了解的还很少。金雀异黄素和骨质疏松症的量效关系还没有很好的建立,以及如下所示,这是本质缺陷,因为它们没有认识到金雀异黄素对PPARγ介导的脂肪生成的作用。
发明内容
本发明人已经发现,只要按照不会导致血浆金雀异黄素水平高到足以激活PPARγ受体的施用方案施用这些异黄酮,那么金雀异黄素成分可以适合用于治疗或预防各种疾病,包括骨质疏松症、肥胖症和/或X综合征(胰岛素抵抗综合征)。尽管大量的技术文献是有关于金雀异黄素对例如骨发生和/或脂肪生成的有益的作用,但是之前还没有认识到避免将金雀异黄素水平提高到能有效激活PPARγ受体的水平对于防止抑制成骨细胞的活性与分化以及阻止对脂肪生成的刺激是至关重要的。事实上根据本发明人的发现,这些文章中一些所推荐的金雀异黄素剂量和血浆浓度将产生显著的相反结果。本发明人已经开发出了具有高度的可重复性和敏感性的体外模型,该模型使得本发明人能发现金雀异黄素成分对例如骨发生及脂肪生成的作用是双相的,就是说当血浆金雀异黄素水平高于约0.02μM时,可以激活雌激素受体(ER);同时当血浆金雀异黄素水平高于4μM时,将激活PPARγ受体,这将启动脂肪生成。通过将血浆金雀异黄素水平保持在雌激素受体被持续激活同时避免激活PPARγ受体的水平,可以有效地治疗例如骨质疏松症和肥胖症的疾病。本发明人已经发现:低血浆浓度(0.02-3μM)的金雀异黄素刺激骨发生并抑制脂肪生成,这是通过:
1.对成骨前体细胞增殖的刺激;
2.对成骨细胞分化的刺激并减少脂肪细胞的定向分化,就是说刺激成骨细胞生成和抑制脂肪生成;
3.对成骨细胞活性的刺激。
这些作用是通过ER介导的。
本研究者进一步发现:在高浓度金雀异黄素(也就是大于4μM,尤其是大于6μM)情况下,金雀异黄素结合于PPARγ受体,就是说作用为真正的PPARγ受体激动剂。这造成对细胞增殖的抑制并减少了成骨细胞生成和骨发生。同时增加了脂肪生成。除了通过直接结合激活PPARγ受体的活性外,金雀异黄素抑制了MAPK活性,因此增强了PPARγ受体活性。尽管高浓度的金雀异黄素也激活ER,但是PPARγ受体的激活主导了生物学结果,也就是增加脂肪生成并减少成骨细胞生成。这造成PPARγ受体介导的效应掩盖了ER介导的效应。另外,在这些受体的活性之间存在交叉应答,相互抑制彼此的活性。
注意到先前提及的Anderson等的研究提示大剂量的金雀异黄素不及低和中等剂量有效。然而,上述研究中所采用的中等和大剂量都超过了本发明所包含的剂量。
上述提及的金雀异黄素的双相剂量效应对于将金雀异黄素应用于预防不仅是雌激素依赖性也是PPARγ活性依赖性的疾病有着重要意义。特别是对于更高剂量造成对细胞增殖和分化(例如向着脂肪细胞)非所需的抑制的情况下,必须小心地保证将金雀异黄素血浆水平保持在一个带宽内,该带宽由能有效地激活ER的最小剂量以及能有效地激活PPARγ活性的最小剂量来界定。如果采用的剂量不能有效地激活ER(也就是血浆浓度小于0.02μM),将不会有生理作用。然而,如果剂量太高(也就是高于4μM),将激活PPARγ以及所观察到的真实作用可能恰好与所需的作用相反。
既然在大量的食品中都存在着不同量的金雀异黄素,认识到个人饮食和/或使用营养补充将极大地影响血清金雀异黄素水平是重要的。已经报道在无黄豆饮食的人中,血浆金雀异黄素浓度是相当低的并常小于20nM。相反,在食用大量黄豆制品的日本人的血浆中,金雀异黄素浓度达到将近5μM。因此,在打算将血中金雀异黄素水平保持在能刺激骨发生并抑制脂肪生成的水平的方法中,使得施用的金雀异黄素量与个人饮食相适应是重要的,这可以避免超过激活PPARγ而观察到相反作用的上限水平。
具体实施方式
因此,本发明涉及一种控制哺乳动物的血浆金雀异黄素浓度以避免激活过氧化物酶体增殖因子活化的受体γ(PPARγ)的方法,上述方法包括以下步骤:
a.评价哺乳动物血清金雀异黄素浓度;
b.如果需要,给所述哺乳动物施用一种金雀异黄素成分,其量足以在每天至少8个小时内、优选在每天至少16个小时内将血清金雀异黄素浓度维持在0.02μM和3μM之间的水平,优选维持在0.05和2μM之间的水平;
c.在至少30天的时间内反复进行步骤a和b,且中间隔不超过3天。
术语“血清金雀异黄素浓度”指的是纯金雀异黄素和金雀异黄素的某些代谢物的混合血清浓度。Setchell et al,2001J.Nutr.131:1362-1375S描述了测定该血清(混合)金雀异黄素浓度的适宜方法。该方法能够测定纯金雀异黄素以及金雀异黄素代谢物例如金雀异黄素糖苷。术语血清金雀异黄素浓度和血浆金雀异黄素浓度在本文件全文中可以相互替换使用。
术语“金雀异黄素成分”包括金雀异黄素以及当根据本方法应用时能释放出金雀异黄素的金雀异黄素的前体物,以及有着相似功能的金雀异黄素的衍生物,也就是在相似的血清浓度也能激活ER和PPARγ。每当在本文中提及血清金雀异黄素浓度,都要知道这不仅指的是血清真正的金雀异黄素浓度,也指的是金雀异黄素和有着相似体内功能的金雀异黄素衍生物的混合血清浓度。
金雀异黄素的体内半衰期为5和10个小时之间并依赖于性别和个体的生理状态。这是优选的施用金雀异黄素成分每天至少一次(如果需要)的原因。如果采用更长的施用间隔,则需要相当大的剂量以持续保持血清金雀异黄素浓度在有效高的水平。如此相当大的剂量增加了血清金雀异黄素浓度超过3μM的危险,以及这样施用金雀异黄素成分可能有着与所预期的相反的作用的危险。特别优选为施用金雀异黄素成分每天至少两次。
在本发明的一个特别的实施方案中,包括每天施用一次含金雀异黄素成分的缓释配方。使用缓释配方所具有的优点是血清金雀异黄素浓度可以保持在相当持续的水平,而不需要每天2、3次或更多次地施用金雀异黄素成分。
应当理解的是如果在步骤(a)中对血清金雀异黄素浓度的测定显示上述浓度超过3μM,那么直到当测定的浓度在0.02和3μM范围内的那天才施用金雀异黄素。因此,本方法包括一种包括期间不施用金雀异黄素的施用间隔的施用方案。
可以通过分析的方法进行哺乳动物血清金雀异黄素浓度的测定或根据上述哺乳动物近期的饮食来估计上述浓度。因为进行常规的血清分析以测定血清金雀异黄素浓度经常是不现实的,优选的是根据哺乳动物近期的饮食来评价血清金雀异黄素浓度。这里近期饮食指的是在评价前不超过4天的时间内所食用的食品和营养补充品的品种。优选的每天至少进行一次上述的评价。
为了保持血清金雀异黄素浓度在3μM以下,通常应该施用的金雀异黄素成分的量是相当于每天每kg体重0和6微摩尔之间的总口服剂量,优选的是相当于每天每kg体重0和2.5微摩尔之间的口服剂量。在绝大多数个体中,为保持有效高的血清金雀异黄素浓度所需要施用的金雀异黄素的最小量要超过相当于每天每kg体重0.04微摩尔的口服剂量。
根据本发明,可以用不同的方式施用金雀异黄素成分,例如肠内或肠外。最优选的是口服施用金雀异黄素成分。缓释配方形式的金雀异黄素成分适合于口服施用,该形式以一定速度释放金雀异黄素成分,该速度使得血清金雀异黄素浓度保留在上述的临界范围内。这可以通过选择一种缓释配方来实现,该配方在施用后最初3个小时内释放出每kg体重小于1.3微摩尔金雀异黄素成分,更优选的是小于1微摩尔。
优选的按营养或药用的剂量单元施用金雀异黄素成分,它包括最少2.8微摩尔金雀异黄素成分,更优选的将近10-400微摩尔金雀异黄素成分和最优选的50-175微摩尔金雀异黄素成分。
在本方法中适合使用的药用剂量单元的实例包括片剂、胶囊、粉末等。在本发明一个有利的实施方案中,药用剂量单元是缓释配方形式。使用缓释配方形式所具有的优点是有助于防止血清金雀异黄素浓度的大波动。因此显著地减少了血清金雀异黄素浓度波动超出所需带宽的危险。合适的缓释配方的实例包括由含有金雀异黄素成分的核心所构成的片剂,该核心被半透膜包被(例如丙烯酸酯)所包裹。也可以将金雀异黄素置于胶囊中,该胶囊在胃肠道内预先设定的条件下解体。此外,将金雀异黄素成分结合于一药丸中是可行的,由于药丸的大小和水溶性,在通过胃肠道期间它将缓慢地解体。根据本发明适合使用的适宜的营养品剂量单元的实例包括麦片、营养棒和营养奶。
根据本发明的方法,在最近测定血清金雀异黄素水平后的1个小时内适合施用金雀异黄素成分。更优选的是在测定后的半小时内进行上述施用。
在本发明步骤(b)的优选的实施方案中包括如果需要施用金雀异黄素,施用量为能有效地保持血清金雀异黄素浓度在0.02和3μM之间的水平。优选的施用金雀异黄素成分的量为最初能有效地将血清金雀异黄素浓度提高到至少0.5μM,优选的至少0.7μM,以及保持上述浓度直到下次施用为至少0.3μM水平,更优选的为至少0.4μM。
通过刺激骨发生和抑制脂肪生成,本方法可以适合用于预防例如骨质疏松症和/或肥胖症的疾病。本方法对于预防和治疗骨质疏松症特别有效包括绝经后骨质疏松症,当将血清金雀异黄素浓度保持在设定带宽0.02-3μM时,不仅将刺激骨发生也将抑制脂肪生成。对于肥胖症(预防性)治疗而言,对脂肪生成的抑制可以有效地用于抑制体内脂肪形成。
本发明的作用在雌激素和PPARγ受体都大量存在的组织内最明显。这些组织的实例为乳腺、骨髓、前列腺、阴道、卵巢、子宫和脂肪组织,例如脑部、肝脏和胰腺内。
为了保证维持本发明方法的有利作用,主张持续本方法至少6个月时间,更优选的至少12个月。
对已经施用的金雀异黄素成分量是否是足够高的可靠提示可以从对骨发生影响的评价中获得。优选的,如果需要,施用的金雀异黄素成分的量能有效地将血清金雀异黄素浓度提高到骨发生被刺激的水平。最优选的,相对于施用金雀异黄素成分之前观察到的骨发生水平,该水平被提高了至少5%。
本发明一个基本部分是根据通过饮食摄入的金雀异黄素量调整每天金雀异黄素成分的剂量。特别的在那些饮食导致血清金雀异黄素浓度明显波动的情况下,本方法可以产生特别好的效果。优选的,在至少30天的时间内,施用的以每天为基础的金雀异黄素成分的量波动在最小量和最大剂量之间,两者相差至少3倍。更优选的,当在上述30天的时间内没有一天没有施用金雀异黄素成分时,观察到了这样的差异系数。
尽管本方法包括无施用的间隔的方案,在优选的实施方案中,每周至少施用一次金雀异黄素成分。更优选的,每周至少3天施用金雀异黄素成分。最优选的,在至少30天的时间内每天至少施用一次金雀异黄素成分。特别优选的是每天至少施用两次金雀异黄素成分的方法。优选的每天两次施用的方式包括施用时间间隔为8和16个小时之间。优选的根据施用之间的间隔时间调整施用剂量,意思是如果下次施用在例如16个小时后,那么使用的施用剂量是下次施用在8个小时后所施用剂量的2倍。在特别优选的施用方案中,早晨(例如6:00am和10:00am之间)施用一次剂量,并在夜间(例如5:00pm和9:00pm之间)施用另一次剂量。在夜间适合于施用每天金雀异黄素剂量的至少60mol%。
在本发明优选的具体实施方案中,金雀异黄素成分包括一种或多种以下式所表示的物质:
其中R5、R7和R4’各自独立为氢原子;饱和或不饱和的、直链、支链或环状的、任选地被取代的、具有1到6个碳原子的烷基;具有饱和或不饱和的、直链、支链或环状的、任选地被取代的、具有1到8个碳原子的烷基的酰基;硫酸酯基团;或单、双或三糖基团。
优选的R5、R7和R4’各自独立为氢原子;饱和、直链或支链的具有1到3个碳原子的烷基;具有有着1个到3个碳原子的饱和烷基的酰基;硫酸酯基团;或单、双或三糖基团。更优选的R5、R7和R4’各自独立为氢原子;甲基;具有有着1个到3个碳原子的饱和烷基的酰基;或单或双糖基团。
其中R5、R7和R4’中至少一个是烷基或酰基基团的金雀异黄素成分有着增加的稳定性和更高活性的优点。其中R5、R7和R4’中至少一个是单、双或三糖基团的金雀异黄素成分(也就是糖基化金雀异黄素成分)首先需要被代谢为有生理活性的代谢物。因此这些衍生物有着延迟作用的优点。
特别适合用于本方法的糖基化金雀异黄素成分包括7-O-βD-糖苷(金雀异黄素)。在本方法采用糖基化金雀异黄素成分的情况下,优选的至少50wt%的糖基化金雀异黄素成分为7-O-βD-糖苷。
如果在夜间,尤其是5:00pm和9:00pm之间施用金雀异黄素成分,那么这种延迟作用是特别有利的。然而,在早晨,也就是6:00am和10:00am之间,施用大量非糖基化金雀异黄素成分是有益的,因为对血清金雀异黄素水平而言将造成更能预期的效应。因此,在本方法优选的具体实施方案中包括施用步骤:
a)在5:00pm和9:00pm之间施用每kg体重至少0.02微摩尔金雀异黄素成分,其中至少70mol%的金雀异黄素成分为糖基化金雀异黄素,以及
b)6:00am和10:00am之间施用每kg体重至少0.02微摩尔金雀异黄素成分,其中最多30mol%的金雀异黄素成分为糖基化金雀异黄素。
从例如大豆粉或发酵大豆的天然资源中可以适当地得到金雀异黄素成分。本发明也包括应用合成的金雀异黄素以及特别合成的天然金雀异黄素的衍生物。最优选的本发明采用从植物材料中获得的金雀异黄素成分,优选的从植物提取物中获得金雀异黄素成分成分,更特别的是从蛋白质样植物提取物中获得。如前所述,个体饮食特别强烈地影响血清金雀异黄素成分浓度。特别的,以大豆为原料的食品对上述血清浓度有着严重影响。因此,在本方法优选的实施方案中,步骤(a)包括根据哺乳动物在过去的24小时内所消耗的以大豆为原料的主食产品的数量估计血清金雀异黄素浓度。通过所提供的关于这些主食产品的金雀异黄素内容物的信息以及考虑到在给定时间内所消耗的数量,估计血清金雀异黄素浓度以及据此为保持最适宜的血清金雀异黄素成分水平所需施用的金雀异黄素成分的量是可能的。
在本方法中,联合施用已知的抗骨质疏松症和肥胖症的活性物质可能是有益的。可以方便与金雀异黄素成分一起联合施用的抗骨质疏松症的适宜的活性物质包括多酚、维生素K、锌、钙、和/或维生素D。优选的,联合施用的维生素D量每天不超过1mg,更优选的,联合施用的维生素D量每天在2-10μg之间。每天的钙量要保持在60-1000mg之间,优选的为80-800mg之间的钙。以药用可接受形式适合施用的锌量每天不超过40mg,优选的每天不超过30mg。每剂(也就是施用剂量)的锌量优选的保持在15mg以下,因为在更高的剂量观察到了难闻的味道效应。优选的锌量每天要超过5mg。在预防肥胖症上,金雀异黄素可以与羟基柠檬酸以及葡萄籽提取物联合使用。
本发明的另一个部分涉及一种治疗或预防哺乳动物的骨质疏松症和/或肥胖症的方法,该方法包括如下步骤:
a)在5:00pm和9:00pm之间施用每kg体重至少0.02微摩尔金雀异黄素成分,其中至少70mol%的金雀异黄素成分为糖基化金雀异黄素;以及
b)在6:00am和10:00am之间施用每kg体重至少0.02微摩尔金雀异黄素成分,其中最多30mol%的金雀异黄素成分为糖基化金雀异黄素。
之前已经解释了这种施用方案的优点。本方法优选的实施方案与那些所描述的与先前的控制血清金雀异黄素浓度通过激活哺乳动物的PPARγ以刺激骨发生和抑制脂肪生成的方法所相关的实施方案基本一致。

优选的,口服剂量单元包含至少2.8微摩尔金雀异黄素成分,更优选的为至少10微摩尔。该剂量单元中的金雀异黄素成分的量优选的不超过150微摩尔。在优选的具体实施方案中,口服剂量单元是一种单独的固体或半固体的口服剂量单元,例如片剂或药棒(bar)。该口服剂量单元中的金雀异黄素成分量优选的在10-400微摩尔的范围内。
通过以下实施例的方式进一步阐述本发明。
实施例
实施例1
金雀异黄素对骨发生的作用
用不同浓度的金雀异黄素从第一天起连续处理KS 483细胞21天。金雀异黄素对骨发生有明确的双相作用。在0.1μM到10μM范围内的金雀异黄素有刺激碱性磷酸酶(ALP)活性、结节形成和Ca沉积的作用,并在1μM时有着最大作用。相反,25μM或更高浓度的金雀异黄素有抑制碱性磷酸酶(ALP)活性、结节形成和Ca沉积的作用。通过成骨标记物Cbfal、降钙素和甲状旁腺素(PTH)/甲状旁腺素相关肽(PTHrP)受体的mRNA的表达进一步证实了1μM增加骨形成以及25μM减少骨形成。
在鼠和人骨髓细胞培养物中也观察到了金雀异黄素对骨形成相似的刺激和抑制作用。和KS 483细胞一样,对鼠骨髓细胞培养物ALP活性和Ca沉积相似的刺激作用出现在0.1μM和10μM之间的浓度,而抑制作用也发现于25μM或更高的浓度。在人骨髓细胞培养物中,在0.001μM发现了刺激作用,而超过4μM观察到抑制作用。
这些发现表明金雀异黄素以双相方式影响前体细胞的骨发生,也就是低浓度增加骨发生以及高浓度抑制骨发生。
金雀异黄素对脂肪生成的作用
与骨发生相反,金雀异黄素抑制或刺激KS 483细胞、鼠和人骨髓细胞的脂肪生成。从第1天起用金雀异黄素处理的KS 483细胞的剂量相关的脂肪生成效应表明在0.1-1μM范围内金雀异黄素减少了脂肪细胞数目,而在10μM或更高浓度它增加了脂肪细胞数目。
通过脂肪细胞标记物PPARγ2、脂肪细胞蛋白2(aP2)和脂蛋白脂肪酶(LPL)的mRNA的表达进一步证实金雀异黄素对KS 483细胞脂肪生成的这些抑制和刺激作用。获得了不同剂量的金雀异黄素对鼠骨髓细胞的脂肪生成效应。暴露于25μM或更高浓度金雀异黄素的鼠骨髓培养物没有融合,以及在培养物中没有脂肪细胞。然而,在大于4μM的浓度观察到了脂肪细胞数目的增多,而在0.1μM和1μM浓度发现了脂肪细胞数目的减少。
同样,将人骨髓培养物暴露于不同剂量的金雀异黄素,在10μM浓度观察到脂肪细胞数目的增加,而在0.001μM、0.01μM和0.1μM浓度观察到脂肪细胞数目的减少。
这些数据清楚地表明金雀异黄素以双相方式影响前体细胞的脂肪生成,也就是低浓度抑制脂肪生成以及高浓度刺激脂肪生成。
金雀异黄素对脂肪生成和骨发生作用的机制:金雀异黄素对PPARγ反式激活、结合以及MAPK活性的影响
用荧光素酶报告子构建体短暂转染KS 483细胞,该构建体含有TATA前插有两个拷贝的连续的雌激素受体反应元件(ERE)或TATA前插有五个拷贝的连续的PPARγ反应元件(PPRE)或与编码人PPARγ2的表达质粒一起的空白TATA-荧光素酶报告子。发现剂量相关的ERE报告子活性的增加出现于0.1μM到50μM浓度范围的金雀异黄素。同样的,在1μM到50μM范围内发现了剂量相关的PPRE报告子活性的增加。
当荧光素酶报告子构建体包含有在最小的胸腺嘧啶激酶启动子前插有三个拷贝的连续的PPRE时,也发现了金雀异黄素诱导的剂量相关的PPRE报告子活性的增加。为了确定金雀异黄素是否通过与该受体直接接触以激活PPARγ,我们进行了膜结合PPARγ的结合测试。发现测定的金雀异黄素的Ki为5.7μM,这比得上已知的PPARγ配体LY 171883(4.2μM)以及其他报道的在微摩尔范围内结合的配体。
这些数据表明金雀异黄素能直接作用于PPARγ配体结合结构域并因此能被确定为一种PPARγ配体。已经显示通过p42和p44丝分裂素活化蛋白激酶(MAPK)对PPARγ的磷酸化调节了PPARγ活性。我们因此进行western印迹以测定微摩尔浓度的金雀异黄素是否抑制了p42和p44MAPK活性。发现在微摩尔范围内出现了p42和p44 MAPK活性的抑制。我们的结果表明金雀异黄素对ERE和PPRE的转录激活是剂量相关的但不是双相的。对p42和p44 MAPK活性的抑制似乎是增强了PPARγ活性。
另外,我们这里显示ER和PPARγ之间的相互作用可能是金雀异黄素抗雌激素作用的机制。我们显示当将KS 483细胞暴露于相同浓度的金雀异黄素时,共转染PPARγ2降低了ERE报告子活性。这些结果提示在ER和PPARγ之间可能存在共同配体金雀异黄素的竞争。PPARγ的激活下调了雌激素的转录活性也是有可能的,也就是在ER和PPARγ之间存在着交叉应答。将ERα共转染暴露于金雀异黄素的KS 483细胞造成了PPRE报告子活性的下调。这些结果说明或在ER和PPARγ之间存在着同一配体的竞争或ERα对PPARγ转录活性有抑制作用。不同于金雀异黄素对KS 483细胞骨发生和脂肪生成的双相作用,ERE和PPRE报告子活性都表现出剂量相关性增高并在50μM金雀异黄素浓度时达到最高水平。生物效应和转录活性之间的差异说明金雀异黄素的生物效应依赖于ER依赖性途径和PPARγ依赖性途径的激活作用之间的平衡。因此配体浓度在决定ER和PPARγ活化程度上起到了关键作用。
总而言之,我们的结果表明金雀异黄素对ERE和PPRE的转录激活是剂量依赖的但不是双相的,以及在ER和PPARγ之间存在着交叉应答。
实施例2
制备了两种用于治疗骨质疏松症和/或肥胖症方法的不同类型的片剂。一种片剂设计为在6:00am和10:00am之间早晨施用(“早晨片剂”),另一种为计划用于在5:00pm和9:00pm之间夜间施用的其他片剂(“夜间片剂”)。
这2种片剂的配方如下:
早晨片剂(1克):
金雀异黄素 14mg
维生素K 0.1mg
维生素D 5μg
碳酸钙 500mg
赋形剂 剩余剂量
夜间片剂(2克):
大豆提取物 500mg(36mg金雀异黄素,主要为糖基化的)
维生素K 0.1mg
维生素D 5μg
碳酸钙 400mg
硫酸锌一水化物 66mg
赋形剂 剩余剂量
实施例3
从以下成分制备一种用于治疗骨质疏松症和/或肥胖症方法的缓释片剂:
片剂核心
金雀异黄素 30mg
Metolose 60SH4000 189.1mg
硬脂酸镁 0.9mg
小计 220.0mg
包被物
ETHOCEL 10(Dow Chemcial) 5.0 mg
聚乙二醇6000 0.5mg
小计 5.5mg
总计 225.5mg
将Metolose 60SH4000和硬脂酸镁加入到金雀异黄素中,充分混合混合物。用装备有8mm直径、6.5R捣杵的连续压缩机在1吨的主压下将混合物直接压缩成每片重为220mg的片剂。然后,将获得的基片放置于盘式涂敷器(pan coater)(Freund Industry)上,将ETHOCEL 10以及在水-乙醇混合液(1∶9)中的聚乙二醇6000的溶液喷于其上,接着干燥得到膜包被的片剂。
Claims (12)
1.金雀异黄素成分在生产一种用于通过阻止骨发生的减低和脂肪生成的增加而治疗或预防骨质疏松症和/或肥胖症的组合物中的用途,其特征在于所述组合物含1.4-200微摩尔金雀异黄素成分,其中至少70mol%的金雀异黄素成分是糖基化的;或其中最多30mol%的金雀异黄素成分是糖基化的。
3.根据权利要求1或2的用途,其中金雀异黄素成分为适于经口服施用的形式。
4.一种药用或营养药剂盒,包括一个或多个单独的含1.4-200微摩尔金雀异黄素成分的口服剂量单元,其中至少70mol%的金雀异黄素成分是糖基化的;以及一个或多个单独的含1.4-200微摩尔金雀异黄素成分的口服剂量单元,其中最多30mol%的金雀异黄素成分是糖基化的。
5.一种口服剂量单元,含有1.4-200微摩尔金雀异黄素成分、2-10μg维生素D和25-3000μg维生素K。
6.根据权利要求1或2的用途,其中金雀异黄素成分为缓释配方的形式,所述缓释配方在施用后的最初3小时内释放的金雀异黄素成分低于每kg体重1.3微摩尔。
7.一种口服剂量单元,包含缓释配方形式的金雀异黄素成分,所述缓释配方在施用后的最初3小时内释放的金雀异黄素成分低于每kg体重1.3微摩尔,且所述缓释配方包含的金雀异黄素成分的量足以在每天至少8个小时内将血清金雀异黄素浓度维持在0.02μM和3μM之间的水平。
8.根据权利要求5的口服剂量单元,其中所述剂量单元进一步包含80-800mg的钙和5-15mg的锌。
9.根据权利要求4的药剂盒,其进一步含有说明书,该说明书指出:
a)在5:00pm和9:00pm之间施用每kg体重至少0.02微摩尔金雀异黄素成分,其中至少70mol%的金雀异黄素成分是糖基化的;以及
b)在6:00am和10:00am之间施用每kg体重至少0.02微摩尔金雀异黄素成分,其中最多30mol%的金雀异黄素成分是糖基化的。
10.根据权利要求9的药剂盒,其中所述说明书进一步指出在夜间施用所述金雀异黄素成分的每日剂量的至少60mol%。
11.根据权利要求7的口服剂量单元,其中所述口服剂量单元包含的金雀异黄素成分的量足以在每天至少16个小时内将血清金雀异黄素浓度维持在0.02μM和3μM之间的水平。
12.金雀异黄素成分在生产一种用于通过阻止骨发生的减低和脂肪生成的增加而治疗或预防骨质疏松症和/或肥胖症的组合物中的用途,其特征在于所述金雀异黄素成分为缓释配方形式,所述缓释配方在施用后的最初3小时内释放的金雀异黄素成分低于每kg体重1.3微摩尔,且所述缓释配方包含的金雀异黄素成分的量足以在至少8个小时内将血清金雀异黄素浓度维持在0.02μM和3μM之间的水平。
