An embodiment of another aspect of the present application provides a method for preparing a patch of a Chinese medicinal composition for treating gout and gouty arthritis, including the steps of:
s1, mixing and grinding the raw material components in parts by weight to obtain drug particles;
s2, mixing the obtained drug particles with a blending agent according to the mass ratio of 1: (0.5-0.7) to obtain a blend;
s3, coating the obtained blend on a carrier to obtain the patch.
The preparation method of the patch for treating gout and gouty arthritis provided by the invention has simple conditions, can be completed only by a pulverizer (or other machines with crushing functions), and can be completed under simple conditions.
In the embodiment of the application, S1 comprises the steps of mixing, grinding, mixing and grinding the raw material components in parts by weight to 100-200 meshes to obtain the drug particles. The active ingredients of the drug particles of the above particle size are easily absorbed at the time of use.

According to another aspect of the invention, the invention also provides application of the traditional Chinese medicine composition for treating acute gouty arthritis in preparing a medicine for treating acute gouty arthritis.
The patch provided by the embodiment achieves a satisfactory effect on treating acute gouty arthritis (damp-heat accumulation) through clinical application, is used for treating 40 cases of acute gouty arthritis (damp-heat accumulation) patients, and after 5 days of treatment, 39 cases of patients are effectively treated, and the total treatment effective rate is 97.50%; and the pain symptoms of the patients treated by the detoxification pain relieving plaster are obviously improved, and the CRP level is obviously reduced compared with that of the patients before treatment.
Examples
The following examples more particularly describe the disclosure of the present application, which are intended as illustrative only, since numerous modifications and variations within the scope of the disclosure will be apparent to those skilled in the art. Unless otherwise indicated, all parts, percentages, and ratios reported in the examples below are by weight, and all reagents used in the examples are commercially available or were obtained synthetically according to conventional methods and can be used directly without further treatment, as well as the instruments used in the examples.
Example 1
The patch for treating acute gouty arthritis is prepared from 15g of honeysuckle stem, phellodendron bark, rhubarb, glabrous greenbrier rhizome, radix trichosanthis, rhizoma curcumae longae, angelica dahurica, rhizoma atractylodis and rhizoma corydalis according to the following weight ratio:
(1) Weighing 15g of honeysuckle stem, phellodendron bark, rhubarb, glabrous greenbrier rhizome, radix trichosanthis, curcuma longa, angelica dahurica, rhizoma atractylodis and rhizoma corydalis respectively, fully mixing, crushing into 100 meshes by using a wall breaking machine, and uniformly mixing to obtain mixed medicinal powder for standby;
(2) Adding purified water with the weight of 0.5 times of the mixed powder in the step (1), fully and uniformly mixing to be pasty, uniformly coating on a patch, and controlling the thickness to be 2-3 mm to obtain the patch.
Example 2
(1) Weighing 12g of honeysuckle stem, 15g of phellodendron bark, 12g of rheum officinale, 15g of rhizoma smilacis glabrae, 15g of radix trichosanthis, 15g of curcuma zedoary, 15g of radix angelicae, 15g of rhizoma atractylodis and 15g of rhizoma corydalis, fully mixing, crushing into 150-mesh powder by using a wall breaking machine, and uniformly mixing to obtain mixed medicinal powder for later use;
(2) Adding the mixed powder obtained in the step (1) into green tea with the weight of 0.7 times of the powder, fully and uniformly mixing to be pasty, uniformly coating on a patch, and controlling the thickness to be 2-3 mm to obtain the patch.
Example 3
(1) Weighing 15g of honeysuckle stem, 15g of phellodendron bark, 15g of rheum officinale, 10g of rhizoma smilacis glabrae, 10g of radix trichosanthis, 10g of curcuma zedoary, 10g of radix angelicae, 10g of rhizoma atractylodis and 10g of rhizoma corydalis, fully mixing, crushing into 200-mesh powder by using a wall breaking machine, and uniformly mixing to obtain mixed medicinal powder for standby;
(2) Adding purified water with the weight of 0.7 times of the mixed powder in the step (1), fully and uniformly mixing to be pasty, uniformly coating on a patch, and controlling the thickness to be 2-3 mm to prepare the patch.
The patches prepared in examples 1 to 3 were subjected to clinical studies as follows:
1. case inclusion and exclusion criteria
The patient accords with the diagnosis standard of acute gouty arthritis in the 'primary gout diagnosis and treatment guide'; the patient accords with the syndrome differentiation standard about damp-heat accumulation in the traditional Chinese medicine syndrome diagnosis curative effect standard issued by the medical administration in 1994.
Exclusion criteria: (1) those with non-acute onset; (2) allergic persons to the drugs used in the study; (3) combining severe primary disease patients; (4) skin-damaged person; (5) and combining the mental diseases.
Using a random number method, 40 patients are listed in an observation group, and 40 patients are listed in a control group; patients in the observation group were treated with the traditional Chinese medicine patch of this example, and patients in the control group were treated with diclofenac diethylamine emulsion agent as usual.
2. Therapeutic effect criterion
Referring to the "clinical guidelines for new traditional Chinese medicine", the therapeutic effect index is calculated by nimodipine scoring method [ therapeutic effect index= (pre-treatment score-post-treatment score)/pre-treatment score x 100% ], and comprehensive evaluation is performed on the therapeutic effect: a. clinical control: the symptoms completely disappear, the joint functions are normal, and the curative effect index is more than or equal to 95%; b. the effect is shown: the symptoms are basically disappeared, the joint function is basically improved, and the curative effect index is more than or equal to 70% and less than 95%; c. the method is effective: the symptoms are improved, the joint function is improved, and the curative effect index is more than or equal to 30% and less than 70%; d. invalidation: no improvement of symptoms, no change of joints and a curative effect index less than 30 percent. Total effective rate = (clinical control + efficacy) number of cases/total number of cases x 100%.
3. Treatment details.
All patients strictly manage diet (low in purine) and properly increase water intake; oral meloxicam tablet (manufacturer: yangzui pharmaceutical Co., ltd.; approval mark: national drug standard character H20010207), 1 tablet/time, 1 time/d.
The treatment group was treated with the patch prepared in example 1, and applied to the red, swelling, heat and pain sites of patients for 4 hours, 2 times/d. The control group was treated with diclofenac diethylamine emulsion (manufacturer: beijing Nohua pharmaceutical Co., ltd.; approval mark: national drug standard H20140361), and the pain area was externally applied with an appropriate amount of the drug 2 times/d.
Both groups of patients were treated for 5d.
4. Therapeutic effects
Of the 40 patients in the treatment group, 7 patients had clinical control of the disease condition, 9 patients had significant treatment effects, and 23 patients had effective treatment, with a treatment effective rate of 97.50%. Of 40 patients in the control group, 3 patients had clinical control of the disease condition, 7 patients had significant treatment effects, and 24 patients had effective treatment, with a treatment effective rate of 85.00%.
5. Typical cases
Case (1): wu Mou Man, 87 years old
Initial diagnosis: bilateral knee redness and swelling pain for 7 days, VAS pain scores: and 6 minutes. Blood uric acid level is 623umol/L, and acute gouty arthritis is diagnosed.
Examination of the sample: the right knee joint is significantly red (3 degrees). Bilateral knee joint tenderness is obvious and walking is difficult.
Treatment: the affected part of the wet compress patient was wrapped with the patch prepared in example 2, 1 time per day, and the single application time was 3 hours.
And (5) re-diagnosis: the gouty arthritis treatment is carried out for 5 days, the symptoms of red swelling and pain of the left foot are disappeared, the blood uric acid level is 409umol/L, and the examination is carried out: the knee joints at the two sides have no red swelling and tenderness and limited movement.
Case (2): liao Mou Man, 87 years old
Initial diagnosis: left ankle joint red swelling pain for more than 1 month, VAS pain scores were: 7 minutes. The blood uric acid level is 740umol/L, and the acute gouty arthritis is diagnosed.
Examination of the sample: the left ankle joint was significantly red and swollen (3 degrees). Joint tenderness is obvious and walking is difficult.
Treatment: the affected part of the wet compress patient was wrapped with the patch prepared in example 31 time a day.
And (5) re-diagnosis: gouty arthritis treatment was repeated for 5 days, red swelling and pain symptoms of left ankle had disappeared, blood uric acid level was 285umol/L, and examination of the examination was performed: the joints have no red swelling and tenderness, and the movement is not limited.
Case (3): kang Mou Male, 64 years old
Initial diagnosis: in the past, there is a history of gouty arthritis, and the right wrist joint is obviously red and swollen (2 degrees). VAS pain scores were: 4 minutes. The uric acid level in blood was 519umol/L.
Examination of the sample: the right wrist joint is red, swollen and painful and difficult to flex and stretch.
Treatment: the traditional Chinese medicine powder prepared in the step (1) of the embodiment 2 is prepared by purified water, and then the traditional Chinese medicine powder is wrapped by gauze and wet-applied to the affected part of a patient for 1 time per day.
And (5) re-diagnosis: the symptoms of red swelling and pain of the wrist joint are obviously improved after the treatment is carried out for 5 days, the symptoms of the pain of the wrist joint are basically disappeared after 7 days, the movement is free, and the blood uric acid level is 196umol/L.

The traditional Chinese medicine composition for treating acute gouty arthritis is characterized by comprising the following raw material components in parts by weight: 10-15 parts of honeysuckle stem, 10-15 parts of amur corktree bark, 10-15 parts of rhubarb, 10-15 parts of glabrous greenbrier rhizome, 10-15 parts of Mongolian snakegourd root, 10-15 parts of turmeric, 10-15 parts of dahurian angelica root, 10-15 parts of swordlike atractylodes rhizome and 10-15 parts of rhizoma corydalis.

The traditional Chinese medicine composition for treating acute gouty arthritis according to claim 1, which is characterized by comprising the following raw material components in parts by weight: caulis Lonicerae, cortex Phellodendri, radix et rhizoma Rhei, rhizoma Smilacis Glabrae, radix Trichosanthis, rhizoma Wenyujin Concisa, radix Angelicae Dahuricae, rhizoma Atractylodis, and rhizoma corydalis.
