技术背景

本文所述的方法利用JAK1途径抑制剂,特别是JAK1选择性抑制剂。JAK1选择性抑制剂是相对于其他Janus激酶优先抑制JAK1活性的化合物。JAK1在许多细胞因子和生长因子信号转导途径中起着核心作用,这些途径在失调时会导致或造成疾病状态。在自身免疫疾病和癌症中,活化JAK1的炎性细胞因子的全身水平升高也可能造成疾病和/或相关症状。因此,患有如结节性痒疹的自身免疫相关疾病的患者可能会受益于JAK1抑制。JAK1的选择性抑制剂可能是有效的,同时避免抑制其他JAK激酶的不必要和潜在不期望的影响。与健康对照相比,在PN患者中,活化的T细胞已显示出增加的IL-22细胞因子表达(Belzberg等人,JInvest Dermatol,141(9):2208-2218.e14(2021))。

在一些实施方案中,额外治疗剂选自JAK抑制剂。额外的JAK抑制剂可包括ATI-50002(JAK1/3选择性)。额外的JAK抑制剂可包括PF-06651600(JAK3选择性)。额外的JAK抑制剂可包括PF06700841(JAK1/TYK2选择性)。额外的JAK抑制剂可包括巴瑞克替尼(baricitinib)(JAK1/JAK2选择性)。额外的JAK抑制剂可包括TYK2选择性抑制剂。
在一些实施方案中,额外治疗剂选自抗氧化剂。抗氧化剂可选自假过氧化氢酶、维生素E、维生素C、泛醌、硫辛酸、白绒水龙骨(Polypodium leucotomos)、过氧化氢酶/超氧化物歧化酶组合和银杏(Ginkgo biloba)。在一些实施方案中,抗氧化剂还可与光疗联合施用。在光疗期间或之前施用抗氧化剂旨在于抵消由UV辐射本身引起的氧化应激,从而提高光疗效果。
在一些实施方案中,额外治疗剂包括抗组胺。
在一些实施方案中,额外治疗剂是抗代谢物。抗代谢物可包括5-氟尿嘧啶。
在一些实施方案中,额外治疗剂选自局部用皮质类固醇、免疫调节剂、钙调神经磷酸酶抑制剂和光疗。在一些实施方案中,额外疗法是全身性类固醇或免疫抑制剂。
在一些实施方案中,额外治疗剂包括类固醇(例如,经口施用类固醇),包括全身性类固醇。类固醇治疗可包括经口类固醇微脉冲疗法(例如,使用倍他米松(betamethasone)和/或地塞米松(dexamethasone))。

当用作药物时,JAK1途径抑制剂或其药学上可接受的盐可以药物组合物的形式施用。这些组合物可以制药领域中熟知的方式制备,并且取决于是需要局部治疗还是全身治疗以及待治疗的区域,可通过多种途径施用。施用可为局部施用(包括透皮、表皮、眼部和粘膜施用,包括鼻内、阴道和直肠递送)、肺部施用(例如,通过吸入或吹入粉末或气溶胶,包括通过喷雾器;气管内或鼻内施用)、经口或肠胃外施用。肠胃外施用包括静脉内、动脉内、皮下、腹膜内、肌内或注射或输注;或颅内施用,例如鞘内或脑室内施用。肠胃外施用可呈单次丸剂剂量的形式,或可例如通过连续灌注泵来进行。用于局部施用的药物组合物和制剂可包括透皮贴剂、软膏剂、洗剂、霜剂、凝胶、滴剂、栓剂、喷剂、泡沫、液体和粉末。常规的药物载体、水性、粉末或油性基体、增稠剂等可能是必要或期望的。
本发明还包括药物组合物,所述药物组合物含有作为活性成分的本文所述的JAK1途径抑制剂或其药学上可接受的盐与一种或多种药学上可接受的载体(赋形剂)的组合。在一些实施方案中,所述组合物适合于局部施用。在制备组合物时,活性成分通常与赋形剂混合、由赋形剂稀释、或包封在这种载体内,所述载体是呈例如胶囊、药囊、纸或其他容器的形式。当赋形剂充当稀释剂时,其可为固体、半固体或液体材料,其用作活性成分的媒介物、载体或介质。因此,组合物可呈片剂、丸剂、粉末、锭剂、药囊、扁囊剂、酏剂、悬浮液、乳液、溶液、糖浆、气溶胶(作为固体或处于液体介质中)、含有例如高达10重量%活性化合物的软膏剂、软明胶胶囊和硬明胶胶囊、栓剂、无菌可注射溶液和无菌包装粉末的形式。
在制备制剂时,可研磨活性化合物以提供适当粒度,之后将其与其他成分组合。如果活性化合物基本上不溶,则可将其研磨成小于200目的粒度。如果活性化合物基本上可溶于水,则可通过研磨来调整粒度,以在制剂中提供基本上均匀的分布,例如约40目。
可使用已知的诸如湿磨的研磨程序来研磨JAK1途径抑制剂,以获得适合于片剂形成和其他制剂类型的粒度。可通过本领域中已知的工艺制备JAK1选择性抑制剂的细分(纳米颗粒)制剂,例如参见国际申请号WO 2002/000196。
组合物可被配制为单位剂型,每个剂量含有一定量的呈游离形式或盐形式的活性成分。术语“单位剂型”是指作为单位剂量适合于人类受试者和其他哺乳动物的物理离散单位,每个单位含有经计算以联合合适的药物赋形剂一起产生期望的治功效果的预定量的活性材料。
在本发明的方法和用途中,可使用相似剂量的本文所述的化合物。
活性化合物可在宽剂量范围内有效,并且通常以药学上有效的量施用。然而,应当理解,实际施用的化合物的量通常由医师根据相关情况确定,所述相关情况包括待治疗的疾患、选择的施用途径、所施用的实际化合物、个体患者的年龄、体重和反应、患者症状的严重程度等。
为了制备诸如片剂的固体组合物,将主要活性成分与药物赋形剂混合以形成含有本发明化合物的均质混合物的固体预配制组合物。当将这些预配制组合物称为均质时,活性成分通常均匀地分散在组合物中,使得该组合物可容易地细分成同等有效的单位剂型,诸如片剂、丸剂和胶囊。然后将这种固体预制剂细分为上文所述类型的含有例如约0.1至约1000mg本发明的活性成分的单位剂型。
可将本发明的片剂或丸剂包衣或以其他方式混配以提供具有延长作用优势的剂型。例如,片剂或丸剂可包含内部剂量组分和外部剂量组分,后者是呈在前者之上的包膜的形式。两种组分可由肠溶层分开,所述肠溶层用于抵抗胃中的崩解并且准许内部组分完整地进入十二指肠或延迟释放。多种材料可用于此类肠溶层或包衣,此类材料包括多种聚合酸和聚合酸与诸如虫胶、鲸蜡醇和乙酸纤维素的材料的混合物。
可掺入本发明化合物和组合物以经口施用或通过注射施用的液体形式包括水溶液、适当调味的糖浆、水性或油性悬浮液和具有诸如棉籽油、芝麻油、椰子油或花生油等食用油的调味乳液、以及酏剂和类似药物媒介物。
用于吸入或吹入的组合物包括在药学上可接受的水性或有机溶剂或其混合物中的溶液和悬浮液、以及粉末。液体或固体组合物可含有如前所述合适的药学上可接受的赋形剂。在一些实施方案中,组合物通过经口或经鼻呼吸途径施用以产生局部或全身效果。组合物可通过使用惰性气体进行雾化。雾化溶液可直接从雾化装置吸入,或者雾化装置可附接到面罩、帷罩或间歇式正压呼吸机。溶液、悬浮液或粉末组合物可从以适当方式递送制剂的装置经口或经鼻施用。
局部制剂可含有一种或多种常规载体。在一些实施方案中,软膏剂可含有水和一种或多种选自以下各者的疏水性载体:例如液体石蜡、聚氧乙烯烷基醚、丙二醇、白凡士林等。霜剂的载体组合物可基于水与甘油和一种或多种其他组分的组合,所述其他组分例如甘油单硬脂酸酯、PEG-甘油单硬脂酸酯和鲸蜡硬脂醇。可使用异丙醇和水,适当时与其他组分(例如像甘油、羟乙基纤维素等)组合来配制凝胶。
向患者施用的化合物或组合物的量将根据施用什么药物、施用目的(诸如预防或治疗)、患者状态、施用方式等而变化。在治疗应用中,可以足以治愈或至少部分遏制疾病及其并发症的症状的量向已经罹患疾病的患者施用组合物。有效剂量将取决于所治疗的疾病状况以及主治医生根据诸如疾病的严重程度、患者的年龄、体重和一般状况等因素所作出的判断。
向患者施用的组合物可呈上文所述药物组合物的形式。这些组合物可通过常规灭菌技术灭菌,或者可进行无菌过滤。可包装水溶液以按原样使用或将其冻干,将冻干制剂在施用之前与无菌水性载体组合。化合物制剂的pH通常在3与11之间,更优选地为5至9,且最优选地为7至8。应当理解,使用某些前述赋形剂、载体或稳定剂将导致形成药用盐。
本发明化合物的治疗剂量可根据例如治疗欲取得的特定用途、化合物的施用方式、患者的健康状况以及处方医师的判断而变化。本文所述的化合物在药物组合物中的比例或浓度可根据许多因素而变化,所述因素包括剂量、化学特性(例如,疏水性)和施用途径。剂量可能取决于诸如以下各者的变量:疾病或病症的类型和进展程度、特定患者的总体健康状况、所选化合物的相对生物学功效、赋形剂的配制及其施用途径。有效剂量可由来源于体外或动物模型测试系统的剂量-反应曲线中推断出。
本发明的组合物还可包含一种或多种额外药剂,诸如化学治疗剂、类固醇、抗炎化合物或免疫抑制剂,在本文中列出了其实例。
药盒
本发明还包括可用于例如治疗和/或预防结节性痒疹的药用药盒,所述药用药盒包括一个或多个容器,所述一个或多个容器含有包含治疗有效量的本文所述的化合物的药物组合物。如果需要,此类药盒还可包括一种或多种不同的常规药用药盒组件,例如像具有一种或多种药学上可接受的载体的容器、额外容器等,如本领域技术人员容易显而易见的。药盒中还可包括作为插页或标签的说明书,所述说明书指示待施用的组分的量、施用指南和/或用于混合组分的指南。
实施例
本发明将通过具体实施例来进行更详细的描述。以下实施例出于说明目的而提供,而不意图以任何方式限制本发明。本领域技术人员将容易认识到,可改变或修改各种非临界参数来产生基本上相同的结果。根据本文所述的至少一种测定,已发现实施例的化合物为JAK抑制剂。
实施例A:体外JAK激酶测定
根据Park等人,Analytical Biochemistry 1999,269,94-104中描述的以下体外测定来测试可用于治疗细胞因子相关疾病或病症的JAK1途径抑制剂对JAK靶标的抑制活性。使用昆虫细胞中的杆状病毒表达具有N端His标签的人类JAK1(a.a.837-1142)、JAK2(a.a.828-1132)和JAK3(a.a.781-1124)的催化域,并且加以纯化。通过测量生物素化肽的磷酸化来测定JAK1、JAK2或JAK3的催化活性。通过均相时间分辨荧光(HTRF)来检测磷酸化肽。在具有100mM NaCl、5mM DTT和0.1mg/mL(0.01%)BSA的50mM Tris(pH 7.8)缓冲液中含有酶、ATP和500nM肽的40μL反应物中,测量化合物对每种激酶的IC50。对于1mM IC50测量,反应物中的ATP浓度为1mM。使反应在室温下进行1小时,然后用测定缓冲液(Perkin Elmer,Boston,MA)中的20μL 45mM EDTA、300nM SA-APC、6nM Eu-Py20停止所述反应。与铕标记的抗体结合40分钟,并且在Fusion读板器(Perkin Elmer,Boston,MA)上测量HTRF信号。在这个测定中测试表1中的化合物,并且显示出其IC50值也存在于表1中。
实施例B:
JAK1介导的PN病理生理学药理学抑制
获得活动性疾病,即未经治疗的结节性痒疹的全厚(4mm)皮肤活检物。对于每个病变,获得两(2)个4-mm穿刺活检物(参见图1)。将活检物纵向分成两块,总共四(4)个2-mm全厚切片,然后将其置于培养物中。图1绘示了用于JAK1介导的PN病理生理学药理学抑制的皮肤穿刺活检的表示。然后在媒介物对照(DMSO)或小分子JAK1抑制剂化合物(在此特定情况下为化合物1)的存在下将活检物培养8天。每2天收集一次上清液以用于细胞因子/蛋白质分析。分析所收集的上清液以量化通过培养物中的皮肤外植体分泌的趋化因子、细胞因子和生长因子。通过Procarta Multiplex Immunoassay(Thermo Fisher,Waltham,MA)在培养物上清液中检测和量化总共51种蛋白质。将上清液和标准品在4℃下孵育过夜。在Luminex200仪器(Luminex Corporation,Austin,TX)上读取测定板。从每种分析物的抗原标准曲线外推浓度。将每种分析物的抑制百分比确定为具有DMSO的培养物中的蛋白质浓度(CD)与具有JAK1抑制剂(化合物1)的那些培养物中的蛋白质浓度(CJ)的差异,或(CD–CJ)/CD,参见图2。
已显示化合物1能抑制关键疾病途径中涉及的关键趋化因子、细胞因子和生长因子(参见表2和图2)。这些关键趋化因子、细胞因子和生长因子与炎症、瘙痒(发痒)或可能与PN相关联的病变发展有关。例如,当与非病变对照皮肤相比时,已显示PN病变皮肤的CXCL8、CXCL10和干扰素γ的水平增加(Tsoi等人,J Allergy Clin Immunol 149,1329-39(2022))。
此外,也已显示诸如IL-31和IL-17等其他细胞因子在PN中发挥着作用。例如,与非病变对照皮肤相比,已显示CD4+T细胞显著浸润病变PN皮肤(Wong等人,J InvestigativeDermatology,140(3),702-706.e2(2020))。CD4+T细胞可表达白介素31(IL-31),所述白介素已显示为是瘙痒(PN的重要症状)的介导体(等人,N Engl J Med,382:706-716(2020))。所表达的IL-31也显示出在PN的发展中发挥着作用,其中通过萘莫利珠单抗抑制IL-31导致PN患者的瘙痒和皮肤病变有所改善(/>同上)。当PN皮肤用萘莫利珠单抗(一种IL-31抑制剂)培养时,观察到病变皮肤中的IL-17水平的降低,从而表明抑制IL31R信号转导可影响IL-17表达(Tsoi,同上)。此外,与对照皮肤相比,PN病变皮肤真皮中表达IL-17的细胞增加(Wong,同上)。此外,IL-2对浸润性CD4+T细胞的发育、维持和功能至关重要(Furtado,J Exp Med,196(6):851-7(2002)),并且是一种强瘙痒介导体(Xie等人,JDermatol,46(3),177–185 2019))。此外,已显示PN皮肤的表皮呈STAT6阳性,STAT6是诸如IL-5和IL-31等Th2细胞因子的标志物(Mullins等人,NLM,StatPearls Publishing,https://www.ncbi.nlm.nih.gov/books/NBK459204),2021年9月14日))。最后,角质细胞与PN的发病机制有关,特别是在瘙痒周围(Zhong等人,Acta Dermato-Venereologicavolume99,579-5861(2019年)5月)。在皮肤中,角质细胞产生IL-34。
因此,化合物1已显示能抑制参与PN和/或与PN相关联的潜在炎症和瘙痒的多种关键趋化因子、细胞因子和生长因子。

实施例C:化合物1的2期研究
研究设计
描述了JAK抑制剂化合物1治疗结节性痒疹患者的功效和安全性的2期随机化、双盲、安慰剂对照剂量范围研究。图3绘示了使用化合物1进行的用于治疗患有结节性痒疹的参与者的研究。该研究可包括患有结节性痒疹的参与者的24周双盲延长期。参与者可包括结节总计≥20、结节在≥2个不同身体区域、IGA≥3和严重瘙痒的患有结节性痒疹(例如,临床诊断达到至少3个月)的参与者。该研究可包括至少18岁的男性和女性。
参与者将参与长达48周,包括长达4周的筛查、长达40周的治疗(安慰剂对照期16周和延长期24周)和4周的安全性随访。大约105名参与者将按1:1:1随机分配到3个治疗组中的1个(剂量A(45mg(例如,3个化合物1 15mg片剂和2个安慰剂片剂))、剂量B(75mg(例如,5个化合物1 15mg片剂))或安慰剂(例如,5个安慰剂片剂))。化合物1或匹配安慰剂将经口服用,每日一次。基于第16周时的功效反应,参与者将额外接受2个剂量的活性研究药物中的1个剂量,持续24周。例如,如果根据取得瘙痒NRS中≥4点的降低(基于每周平均瘙痒评分)和IGA-TS(评分为0或1,相对于基线改善≥2级),某人对治疗有所反应,则其为反应者,而如果其不符合反应者的定义,则其是无反应的。
在一些实施方案中,参与者被包括在研究中的标准包括以下各者中的任一者:≥18岁或以上的男性和女性;临床诊断结节性痒疹达到至少3个月;≥20个结节;结节在≥2个不同身体区域上;IGA≥3;以及严重瘙痒。严重瘙痒可被定义为:筛查前一周平均瘙痒NRS评分≥5;并且第1天前一周的平均瘙痒NRS≥5(注意:参与者必须具有基线前7天中的至少4天瘙痒数据才能计算平均值并进行随机化)。在参与研究的持续时间内,参与者必须愿意采取适当、药学上可接受的避孕措施,以避免妊娠或生育孩子。另外,女性参与者在筛查时必须具有阴性血清妊娠试验,并且在第1天随机化之前具有阴性尿妊娠试验。

在一些实施方案中,本文公开的治疗方法的功效可基于研究者总体评定(IGA)而确定。IGA是0至4的总体严重程度评级,如表6所提及。在一些实施方案中,IGA-TS(研究者总体评定治疗成功)被定义为0或1的IGA评分,相对于基线改善≥2级。如果通过评估在指定时间点(例如,第16周)取得IGA-TS(IGA为0或1,降低2点)的参与者比例,可确定功效。

在一些实施方案中,本文公开的治疗方法的功效可基于结节评定而确定。在一些实施方案中,将在每次诊所内访视时对结节的数量进行计数,并且将经由PAS评定疾病活动。在一些实施方案中,本文所述的化合物1和/或使用方法导致相对于基线,参与者的PAS反应有所改善。PAS(痒疹活动评分)根据顶部有表皮剥脱/结痂(反映主动抓挠)的瘙痒性病变的百分比来评定疾病活动,并且通过PAS的“活动”子项来测量治愈的瘙痒性病变的百分比,以便量化结节性痒疹皮肤病变的变化。
在一些实施方案中,本文公开的治疗方法的功效可基于受影响区域的摄影而确定。访视时将获得受PN影响的身体区域的摄影。所有场所都将使用2维摄影来拍摄含有PN结节的身体区域。
在一些实施方案中,本文公开的治疗方法的功效可基于患者报告结果(PRO)而确定。在一些实施方案中,本文公开的治疗方法的功效可基于皮肤病生活质量指数(DLQI)而确定。在一些实施方案中,本文所述的化合物1和/或使用方法导致相对于基线,参与者的DLQI反应有所改善。DLQI是经过验证的问卷(例如,10个问题),其用于度量在过去7天内,皮肤问题对参与者的影响程度,如在SoA中所概述。参与者可用(1)非常多,(2)很多,(3)很少,或(4)完全没有来回答问卷。问卷可在6个标题下进行分析:症状和感受;日常活动;闲暇;工作和上学;个人关系;以及治疗。
在一些实施方案中,本文公开的治疗方法的功效可基于患者总体印象变化(PGIC)而确定。在一些实施方案中,本文所述的化合物1和/或使用方法导致相对于基线,参与者的PGIC反应有所改善。PGIC是参与者的自我报告测量,其反映了他们对治疗功效的看法。PGIC是描述参与者对整体改善评级的7点量表,并且在现场访视期间记录。例如,参与者将回答以下问题:“自从您在本研究中接受治疗以来,在接受研究药物治疗的区域中,您的PN为:(1)极大地改善,(2)大为改善,(3)稍微改善,(4)无变化,(5)稍微恶化,(6)大为恶化,以及(7)极大地恶化。”

在一些实施方案中,可基于参与者的EQ-5D-5L问卷的改善而评估功效。在一些实施方案中,本文所述的化合物1和/或使用方法导致相对于基线,参与者的EQ-5D-5L问卷反应有所改善。EQ-5D-5L问卷是标准化的经过验证的工具,以用作健康结果的量度。EQ-5D-5L问卷将提供用于经济模型和分析的数据,包括开发健康效用或质量调整寿命年(QALY)。EQ-5D-5L问卷由以下2个部分组成:EQ-5D描述系统和EQ VAS。描述系统包括五个维度:活动能力、自我护理、日常活动、疼痛/不适和焦虑/抑郁。
每个维度有5个等级:1级为“没有问题”,2级为“轻微问题”,3级为“中度问题”,4级为“严重问题”,并且5级为“极端问题”。EQ-5D-5L问卷的这个部分提供了可用于生成健康状态概况的描述性概况。例如,处于“健康状态12345”的参与者在活动能力方面没有问题、在自我护理(洗涤或穿衣)方面有轻微问题、在进行日常活动方面有中度问题、有严重疼痛或不适以及极度焦虑或抑郁。基于健康状态的社会偏好权重,可潜在为每个健康状态分配一个汇总指数评分。这些权重(有时被称为效用)通常用于计算QALY以用于健康经济分析。健康状态指数评分的范围通常为小于0(其中0为相当于死亡的健康状态值;负值表示比死亡更糟糕的值)至1(完全健康值)的范围内,评分越高指示健康效用越高。健康状态偏好通常代表国家或地区值,并且因此可能因国家/地区而异。EQ VAS在垂直视觉模拟量表(0至100)上记录参与者的自评健康状况,其中端点被标记为“您能够想象到的最佳健康状况”(100评分)和“您能够想象到的最差健康状况”(0评分)。
将分析若干群体。在一些实施方案中,将分析意向治疗(ITT)。ITT群体包括所有随机参与者。这个群体的治疗组将根据随机化时的治疗分配进行定义。在一些实施方案中,将分析安全群体。安全群体包括接受至少1剂研究药物的所有参与者。这个群体的治疗组将根据参与者在第1天接受的实际治疗来确定。在一些实施方案中,将分析PK/PD可评估群体。PK/PD可评估群体包括接受至少1剂研究药物(化合物1和/或安慰剂)并提供至少1次给药后PK/PD样品/评定的参与者。研究药代动力学家将审查参与者施用和样品记录的数据列表,以鉴别将排除在分析之外的参与者。
统计分析
在一些实施方案中,初步分析将基于ITT群体。将使用精确逻辑回归测试主要替代假设(在第16周取得瘙痒NRS方面4点改善的参与者(反应者)比例方面,化合物1组与安慰剂组相比有优势)。这个模型将包括治疗组和分层因子。-每个化合物1组与安慰剂组之间的未调整p值将与预先指定的显著性水平进行比较。也将提供优势比和相应95%置信区间。安慰剂对照期中的所有无反应者,以及所有缺少基线后值的参与者都将被定义为无反应者插补分析的无反应者。
在一些实施方案中,将对ITT群体进行二次功效分析。所有二次功效变量都将使用描述性统计进行汇总。对于二元端点,即在第16周取得IGA-TS(IGA为0或1,降低2点)的参与者比例,汇总统计数据将包括样品量、频率和百分比。对于事件发生时间端点,即在瘙痒NRS方面的相对于基线改善≥2点的时间,将按治疗组呈现Kaplan-Meier曲线。参与者的数量、事件的数量和审查的数量将按治疗组进行汇总。中位时间的KM估计将以其95%CI呈现。
在一些实施方案中,将对安全群体进行安全分析。TEAE是第一次报告的任何AE,或者在第一剂研究药物后直至最后一剂研究药物后28天内早先存在事件的恶化。AE分析将限于TEAE,但数据列表将包括所有AE,而不管其相对于研究药物施用的时间如何。不良事件将按MedDRA优选项和系统器官类别制成表格。AE的严重程度将基于国家癌症研究所CTCAEv5.0(使用1至5级)。
研究者认为与研究药物有关系的AE子集将被视为治疗相关AE。如果研究者未指定AE与研究药物的关系,则AE将被视为与治疗相关。AE和治疗相关AE的发生率将被制成表格。
临床实验室数据将使用汇总统计进行分析;没有计划进行正式治疗组比较。将基于临床参考实验室的正常范围而评定超出正常范围的实验室测试值的严重程度。异常实验室值的发生率和相对于基线的偏移表将被制成表格。将在每个评定时间确定生命体征(血压、脉搏、呼吸率和体温)的描述性统计数据和相对于基线的平均变化。将在每个评定时间为每个ECG参数确定描述性统计数据和相对于基线的平均变化。
在一些实施方案中,将使用描述性统计数据汇总所有探索性功效变量。对于分类测量,汇总统计数据将包括样品量、频率和百分比。对于连续测量,汇总统计数据将包括样品量、平均值、中位数、标准偏差、最小值和最大值。如果适用,将为基线、每次访视时的实际测量以及每次访视时相对于基线的变化和百分比变化提供连续测量的汇总统计数据。对于事件发生时间端点,参与者的数量、事件的数量和审查的数量将按治疗组进行汇总。中位时间的KM估计将以其95%CI呈现。
在一些实施方案中,药代动力学分析将在PK可评估群体中进行。化合物1血浆浓度数据将通过群体PK建模方法进行分析。此类数据可与来自临床开发项目中其他研究的数据相结合,以开发或完善群体PK模型,其中将评估健康参与者、化脓性汗腺炎参与者和/或白癜风参与者的群体,并且如果显著,则将其纳入模型作为协变量。这个模型可用于评估内在和外在协变量对化合物1的PK的影响,并且确定单独血浆暴露的测量值(例如稳态峰、谷和/或时间平均浓度)。将单独报告数据分析计划和群体PK分析结果。
在一些实施方案中,将在PK/PD可评估群体中探索药代动力学/药效学分析。将分析诸如血小板计数等临床反应。将单独报告数据分析计划和群体PK/PD分析结果。

Claims (18)
1.一种用于治疗受试者的结节性痒疹的方法,所述方法包括向所述受试者施用治疗有效量的JAK1途径抑制剂或其药学上可接受的盐。
2.如权利要求1所述的方法,其中所述JAK1途径抑制剂或其药学上可接受的盐对JAK1的选择性高于JAK2、JAK3和Tyk2。
3.如权利要求1所述的方法,其中所述JAK1途径抑制剂是4-[3-(氰甲基)-3-(3',5'-二甲基-1H,1'H-4,4'-联吡唑-1-基)氮杂环丁烷-1-基]-2,5-二氟-N-[(1S)-2,2,2-三氟-1-甲基乙基]苯甲酰胺或其药学上可接受的盐。
4.如权利要求1所述的方法,其中所述JAK1途径抑制剂是4-[3-(氰甲基)-3-(3',5'-二甲基-1H,1'H-4,4'-联吡唑-1-基)氮杂环丁烷-1-基]-2,5-二氟-N-[(1S)-2,2,2-三氟-1-甲基乙基]苯甲酰胺磷酸盐。
5.如权利要求1所述的方法,其中所述JAK1途径抑制剂是{1-{1-[3-氟-2-(三氟甲基)异烟碱酰基]哌啶-4-基}-3[4-(7H-吡咯并[2,3-d]嘧啶-4-基)-1H-吡唑-1-基]氮杂环丁烷-3-基}乙腈或其药学上可接受的盐。
6.如权利要求1所述的方法,其中所述JAK1途径抑制剂是{1-{1-[3-氟-2-(三氟甲基)异烟碱酰基]哌啶-4-基}-3[4-(7H-吡咯并[2,3-d]嘧啶-4-基)-1H-吡唑-1-基]氮杂环丁烷-3-基}乙腈己二酸盐。
7.如权利要求1-6中任一项所述的方法,其中所述JAK1途径抑制剂或其药学上可接受的盐以基于游离碱为约5mg至约95mg的日剂量施用。
8.如权利要求1-6中任一项所述的方法,其中所述JAK1途径抑制剂或其药学上可接受的盐以基于游离碱为约45mg或75mg的日剂量施用。
9.如权利要求1-8中任一项所述的方法,其中所述JAK1途径抑制剂或其药学上可接受的盐与另外的治疗剂联合施用。
10.如权利要求9所述的方法,其中所述另外的治疗剂是神经激肽1受体拮抗剂。
11.如权利要求10所述的方法,其中所述神经激肽1受体拮抗剂是阿瑞匹坦。
12.如权利要求9所述的方法,其中所述另外的治疗剂是抗IL-4/IL-13抗体。
13.如权利要求12所述的方法,其中所述抗IL-4/IL-13抗体是度匹鲁单抗、来瑞组单抗或曲罗芦单抗。
14.如权利要求9所述的方法,其中所述另外的治疗剂是抗IL-5抗体。
15.如权利要求14所述的方法,其中所述抗IL-5抗体是贝那利珠单抗、美泊利珠单抗或瑞利珠单抗。
16.如权利要求9所述的方法,其中所述另外的治疗剂是抗IL-31抗体。
17.如权利要求16所述的方法,其中所述抗IL-31抗体是萘莫利珠单抗。
18.如权利要求1-17中任一项所述的方法,其中所述施用包括施用所述JAK1途径抑制剂或其药学上可接受的盐,以及至少一种药学上可接受的载体或赋形剂。
