Abstract

发明领域
本发明涉及眼内镜片(IOL)领域,更确切地说,涉及一种具有一植入后用于受控地或可判定的在荚膜囊(capsular bag)中轴向移动光学基体的改进了的接触部分(haptics)设计的新镜片。
发明背景
传统的眼内镜片,包括一光学基体和固定在该光学基体上面的接触部分,或者由与光学基体同一块材料整体制成的接触部分。对于这种镜片,由于聚甲基丙烯酸甲酯(PMMA)良好的光学特性和良好的生物适应性,早已被发现为优良的材料,特别是作为该镜片主体。相当多的努力集中在接触部分的设计是由于许多原因:眼内的组织对于外部作用力相当敏感,重要的还在于要保持镜片主体的光轴在眼睛的中心,正好说明应当考虑配合方面的问题。这个事实可以由多年以来公布的涉及各种接触部分设计的大量专利很好地说明。
在准备植入镜片时,固有的晶状体(natural lens)要从荚膜囊中取出,且在大多数情况下荚膜囊中是冲有高粘度溶液例如Healon(Pharmacia & Upjohn AB)的。然后通过切口经过虹膜将镜片植入荚膜囊中,并利用与荚膜囊的壁相接触的接触部分使其定位,并使光学基体在虹膜的前方对中。为了避免该光学基体与虹膜接触,通常的做法是让此接触部分相对于光学基体具有例如约5~10°的向前形成的一定角度。在植入之后,当此接触部分在荚膜囊中稍微被压缩时,镜片的拱顶便向后朝向后部的在取出固有晶体之后大多数情况下保留下来的荚膜壁。由于患者中间荚膜囊的尺寸差异都要求有新的人造晶状体(artificial lens),所以很容易实现让光学基体的位置从一个人到另一个人的差异。
包括荚膜的位置在内的眼睛的固定尺寸,是在外科手术之前测出来的,以便给出用于计算待植入的眼内镜片屈光度的适当基础。如果镜片主体的轴向位置是完全无法推断的,则高达每0.1mm位移约0.2屈光度的误差便会引入。因此,在植入镜片的过程中需要将镜片放在一予计的位置,且被保留在该位置。用于计算矫正屈光度的改进的方法,公开在待审的专利申请EP 9614495.5中。一个重要的参数是覆盖眼白平面(haptic plane)的位置,它可在准备外科手术中由眼科人员进行的测试中得到,但重要的是要考虑由于接触部分受到压缩造成的拱顶的影响。可植入镜片的支承元件即接触部分,已被很好地公开在各种用途的文献中。例如美国专利4,778,464公开了一种在距其光学部分一受控的距离上具有支承元件的眼内镜片,其与两个悬杆(rod)一起被放入(interpose)虹膜。这种类型的拟由虹膜固定的镜片具有如上公开的各种类型,它是本发明的主题,旨在植入荚膜囊中。虹膜固定型的其它一些眼内镜片,已由德国专利说明书DE 2437184和DE2538983所公开。
要求将眼内镜片放在荚膜囊中,其在白内障外科手术之后能提供更好的光学效果,不管荚膜囊的尺寸如何或者不同患者间的玻璃体压力如何显著地变化。
发明内容
本发明提供一种用于植入眼的荚膜囊中的眼内镜片,包括一居中的光学基体和接触部分,所述光学基体和接触部分分别具有光学平面和覆盖眼白的平面,其特征在于:
i)在所述接触部分与光学基体之间设置连接件,将覆盖眼白的平面与光学平面隔开一大于光学基体一半厚度的距离,其中所述连接件基本上垂直于该镜片的光学平面;
ii)所述接触部分与所述连接件之间的刚度比约为1比12;以及
iii)所述接触部分位于一个与光学基体的光轴垂直的平面内,使得:
a)达到所述眼内镜片的受控或可预测位移;以及
b)在该镜片被植入后,所述接触部分不会由于作用在荚膜囊中该接触部分上的压力和来自玻璃体的反作用力而沿光轴发生位移。

具体实施方式
我们现已发现,由于来自玻璃体的作用在接触部分上面的压力和反作用力引起光学元件沿光轴的受到控制的或者可推测的位移,可以通过具有相互以适当距离隔开的、平行的光学平面和覆盖眼白平面的眼内镜片来实现,如图1所示。镜片的总直径D通常在5至15mm的范围内,而且光学部分的直径d为4至8mm。接触部分伸出的长度被定义为a,它等于(D-d)/2。接触部分的两个相同的侧面部分一起构成覆盖眼白的平面。这种新的设计的特征在于使覆盖眼白的平面与光学平面隔开一距离b的连接件b。在图1给出的示意性表示中,该连接件是作为主要与光轴平行提供的,而且光轴基本上垂直于光学元件的平面和接触部分的平面。然而重要的特征在于产生在覆盖眼白平面和光学基体平面之间的距离,为了避免与虹膜接触,其最好大于光学元件厚度的一半。一般说来,眼内镜片最大可达2mm厚,可被用来计算出b在大多数情况下在0.2至1.0mm的范围内。
进一步具有重要性的是实现让接触部分a和连接件b的刚性,或者更正确地说是a和b的刚度比,以完成可予测的位移。假定最大拱顶为0.01是可以接受的,我们可以推断出,利用已经限定的镜片几何形状与荚膜囊几何形状的组合,最小刚度比a与b被发现约为1至12。
尽管这样的连接件非常适合且在实践中可能具有图1中给出的示意性设计,但对本领域的技术人员很清楚,改进的连接件特别是具有平滑弯曲的也可以采用,只要能将覆盖眼白的平面与光学平面隔开的关键性能能够达到。本发明的进一步实施例表示在图2中。
根据本发明的镜片,可以由硬质材料如PMMA以及其它生物相容的可折叠材料如水凝胶、软丙烯酸酯(acrylics)、氨基甲酸乙酯或这些材料的组合制成。
在本发明的一个实施例中,接触部分是由和光学基体相同的同一块材料例如PMMA制成的,这意味着它们与光学基体一体连接,通常称之为单片镜片。在另一实施例中,接触部分是由不同于光学元件的材料制成的,并且通过本身为已知的方法例如等离子体或者电晕放电焊结、夹紧、聚合或者粘结剂固定。在所谓的三片型镜片中,光学部分可以由硬性材料如PMMA制做,但最好由可折叠的材料制做。
Claims (6)
1.一种用于植入眼的荚膜囊中的眼内镜片,包括一居中的光学基体和接触部分,所述光学基体和接触部分分别具有光学平面和覆盖眼白的平面,其特征在于:
i)在所述接触部分与光学基体之间设置连接件,将覆盖眼白的平面与光学平面隔开一大于光学基体一半厚度的距离(b),其中所述连接件基本上垂直于该镜片的光学平面;
ii)所述接触部分与所述连接件之间的刚度比约为1比12;以及
iii)所述接触部分位于一个与光学基体的光轴垂直的平面内,使得:
a)达到所述眼内镜片的受控或可预测位移;以及
b)在该镜片被植入后,所述接触部分不会由于作用在荚膜囊中该接触部分上的压力和来自玻璃体的反作用力而沿光轴发生位移。
2.根据权利要求1的眼内镜片,其特征在于,所述接触部分与所述光学基体形成一体。
3.根据权利要求1的眼内镜片,其特征在于,该镜片是由聚甲基丙烯酸甲酯PMMA制成的单片镜片。
4.根据权利要求1的眼内镜片,其特征在于,该镜片是由可折叠材料制成的单片镜片。
5.根据权利要求1的眼内镜片,其特征在于,该镜片是三片可折叠镜片。
6.根据权利要求1的眼内镜片,其特征在于,所述接触部分包括两个相同的侧面部分。
