本发明公开了血小板在制备抑制大别班达病毒感染导致免疫细胞焦亡药物中的应用,属于医药技术领域。本发明提供了血小板在制备抗大别班达病毒感染引发免疫细胞焦亡导致的发热伴血小板减少综合征的治疗药物方面的新用途,本发明首次公开血小板对DBV诱导的巨噬细胞焦亡具有抑制作用,包括抑制焦亡标志性基因和多种细胞因子基因的表达、抑制焦亡标志性蛋白的表达,同时血小板对多种炎性细胞因子的分泌也具有抑制作用。相较于SFTS传统治疗方法,本发明提供的靶点治疗具有如下优点:①抑制免疫细胞的焦亡使免疫细胞在抗DBV感染中持续发挥作用;②抑制焦亡导致的器官损伤和细胞因子风暴,为重症SFTS的预防和治疗提供更有效的新方法。
Description
技术领域
本发明涉及医药技术领域,具体涉及血小板在制备抑制大别班达病毒感染导致免疫细胞焦亡制剂中的新用途,进而应用于制备预防和/或治疗抗大别班达病毒感染的药物。
背景技术
发热伴血小板减少综合征(severe fever with thrombocytopenia syndrome,SFTS)是由大别班达病毒(Dabie bandavirus,DBV)感染引起的急性自然疫源性传染病。DBV属于布尼亚病毒目白蛉纤细病毒科班达病毒属的新成员。SFTS主要散发于山区丘陵地区,夏秋季居多,常与蜱虫叮咬有关,临床以发热伴血小板减少为主要特征。部分感染患者病情危重,可迅速发展为以呼吸衰竭、全身弥散性血管凝血症、细胞因子风暴和严重炎症反应综合征为主要临床表现的重症或危重症型,最终因多器官功能衰竭而导致死亡,死亡率高达30%。由于SFTS的高死亡率和存在潜在大流行的可能性,世界卫生组织已将该疾病列为需要深入研究和优先关注的重要传染病。
目前SFTS的发病机制、病理特征及致死原因仍不明确,临床上尚无针对大别班达病毒感染的特异性药物或疗法,感染患者主要采取对症支持治疗。先前的研究表明,SFTS患者的细胞因子风暴与疾病的严重程度密切相关,尤其是IL-1β在致命的SFTS病例中表现出独特的升高模式,并且患者血清中的病毒载量与IL-1β等细胞因子相关。此外,严重感染患者的免疫细胞显著减少,通常因免疫细胞耗竭而导致预后不良甚至死亡。作为免疫和抗原呈递细胞的一部分,次级淋巴器官中的巨噬细胞等免疫细胞被认为是致命DBV感染末期的主要靶点。DBV在进一步复制后,病毒进入人体循环,形成病毒血症,进一步破坏其他免疫细胞,诱导机体产生细胞因子风暴,引发严重炎症反应综合征。
然而目前还未有相关药物用于抑制DBV感染引发的免疫细胞死亡和细胞因子风暴。
发明内容
本发明的目的在于提供一种能够有效抑制大别班达病毒感染引发免疫细胞焦亡的药物,用于解决现有技术中针对发热伴血小板减少综合征缺乏有效治疗手段的问题。
为实现上述目的,本发明采用如下技术方案:
本发明提供了血小板在制备抑制大别班达病毒感染导致免疫细胞焦亡药物中的应用。
本发明研究表明,大别班达病毒(DBV)感染会导致免疫细胞通过caspase-1途径发生焦亡,具体表现为细胞膜穿孔、胞体膨大、胞质变透明等细胞形态变化以及细胞因子的分泌水平显著上升。
临床表现上,所述大别班达病毒感染引发免疫细胞焦亡导致重症发热伴血小板减少综合征,所述重症发热伴血小板减少综合征具有如下(1)~(3)任一项中的特征:
(1)出现持续性外周血小板减少,凝血异常;
(2)免疫功能失调,且免疫细胞具有焦亡特征;
(3)具有重症发热伴血小板减少综合征的临床表现,且出现细胞因子风暴或过度炎症反应症状。
进一步的,所述免疫细胞包括巨噬细胞。
进一步的,所述免疫细胞焦亡的特征包括:炎症小体激活、焦亡标志性蛋白表达或炎性细胞因子分泌。其中焦亡标志性蛋白包括:半胱天冬氨酸酶-1、成孔蛋白D;炎性细胞因子包括:IL-1β、TNF-α、IL-6、IL-8。
进一步的,本发明提供了DBV感染引发免疫细胞焦亡的细胞模型或动物模型应用于筛选抗DBV感染的药物。具体的,利用细胞模型或动物模型筛选抑制免疫细胞焦亡的药物,通过测定焦亡相关基因或蛋白表达水平、细胞因子分泌水平来评判待测药物活性。
本发明研究发现合用血小板可显著抑制DBV感染后免疫细胞中焦亡的发生。机理研究表明,合用血小板可以显著抑制DBV感染导致的炎症小体激活、焦亡标志性蛋白表达、炎性细胞因子包括IL-1β、TNF-α、IL-6和IL-8等的分泌,从而遏制SFTS患者器官损伤的病理进程和细胞因子风暴的形成,起到减轻SFTS患者症状的效果。
进一步的,所述血小板包括但不限于:新鲜血小板、促进血小板生成的制剂。新鲜血小板是指新鲜离体血液中分离的血小板;促进血小板生成的制剂是指促进机体血小板生成的药物。
具体的,促进血小板生成的药物包括但不限于:血小板生成素、白细胞介素-11。促进血小板生成的药物通过增加血液中血小板用于抑制细胞焦亡,从而发挥抑制因大别班达病毒感染引发巨噬细胞焦亡和细胞因子分泌的功能。
本发明提供了一种抑制大别班达病毒感染导致免疫细胞焦亡的药物,包括有效剂量的血小板或有效剂量的促进血小板生成的制剂,以及药学上可接受的载体。
所述药物通过抑制DBV感染诱导免疫细胞焦亡的发生进而减轻细胞因子风暴,达到治疗发热伴血小板减少综合征的目的。
本发明提供了所述的药物在制备预防和/或治疗DBV感染引发免疫细胞焦亡导致发热伴血小板减少综合征的药物中的应用。
所述药物中,血小板或促进血小板生成的制剂为发热伴血小板减少综合征治疗药物的唯一有效成分或有效成分之一。
所述药学上可接受的载体包括但不限于:增稠剂、稀释剂、缓冲剂。药物载体是本领域技术人员已知的,包括在生理pH下的溶液,例如无菌水、盐水和缓冲溶液。
本发明提供的药物可通过常规的方式施用于所需的对象(如人和非人哺乳动物)。代表性的制剂形式包括但并不限于:注射制剂,包括静脉注射、静脉滴注、肌肉注射或皮下注射等中的一种或多种;口服制剂,包括胶囊、片剂、粉剂。使用药物时,是将安全有效量的药物施用于所需的对象。当然,具体剂量和方法还应考虑给药途径、病人健康状况等因素,这些都是熟练医师技能范围之内的。
本发明具备的有益效果:
(1)本发明提供了血小板在制备抗大别班达病毒感染引发免疫细胞焦亡导致的发热伴血小板减少综合征的治疗药物方面的新用途,本发明首次公开血小板对DBV诱导的巨噬细胞焦亡具有抑制作用,具体表现为:抑制焦亡标志性基因和多种细胞因子基因的表达、抑制焦亡标志性蛋白的表达,同时血小板对多种炎性细胞因子的分泌也具有抑制作用,为治疗SFTS提供了新的方法。
(2)相较于SFTS传统治疗方法,本发明提供的靶点治疗具有如下优点:①抑制免疫细胞的焦亡使免疫细胞在抗DBV感染中持续发挥作用;②抑制焦亡导致的器官损伤和细胞因子风暴,为重症SFTS的预防和治疗提供了更有效的新方法。
(3)本发明挖掘了血小板在抗DBV诱导免疫细胞焦亡方面的新的作用机制,其在抑制炎症反应中起到重要作用,为SFTS的治疗及新药开发提供新的思路。

图4为血小板能抑制DBV感染诱导的巨噬细胞焦亡。其中,A为细胞活性检测;B为western blot检测炎症小体、焦亡标志性蛋白的表达水平。
图5为血小板能抑制DBV感染诱导的巨噬细胞细胞因子分泌,其中A、B、C、D为不同感染复数条件下血小板对IL-1β分泌水平的影响;E-I分别为相同感染复数条件下血小板对TNF-α、IL-6、IL-8、IL-10、IL-12分泌水平的影响。
具体实施方式
下面结合具体实施例对本发明做进一步说明。以下实施例仅用于说明本发明,不用来限制本发明的适用范围。
下述实施例中所使用的试验方法如无特殊说明,均为常规方法;所使用的材料、试剂等,如无特殊说明,为可从商业途径得到的试剂和材料。
巨噬细胞(浙江美森细胞科技有限公司)、DBV(由浙江省疾病预防控制中心提供)、血小板(由浙江省血液中心提供)、LPS(Sigma)和Nigericin(Sigma)、caspase-1抗体(CellSignaling Technology)、cleaved GSDMD抗体(Cell Signaling Technology)、cleavedIL-1β抗体(Cell Signaling Technology)、NLRP3抗体(成都正能生物技术有限公司)。
术语说明:
“DBV”是指属于白蛉纤细病毒科班达病毒属的大别班达病毒(Dabie bandavirus,DBV),是急性感染性疾病发热伴血小板减少综合征(severe fever withthrombocytopenia syndrome,SFTS)的病原体。
“PLT”是指人血小板,能抑制DBV感染导致的巨噬细胞焦亡。
“LPS”和“Nigericin”分别是脂多糖和尼日利亚菌素,可诱导巨噬细胞焦亡,作为阳性对照组。
“NLRP3”是指炎症小体,“caspase-1”是指半胱天冬氨酸酶-1,“GSDMD”是指成孔蛋白D,是细胞焦亡标志性的蛋白质。
“IL-1β、TNF-α、IL-6、IL-8、IL-10、IL-12”是细胞因子,“IL”是指白细胞介素,“TNF”是指肿瘤坏死因子。
实施例1、DBV感染介导巨噬细胞焦亡
1、将巨噬细胞(1×106/孔)使用DBV(MOI=0.1、1)感染作为实验组、将巨噬细胞使用LPS(1mg/mL)处理4h后再使用Nigericin(2μM)处理半小时作为阳性对照组、不处理作为阴性对照组,在37℃,5% CO2的培养箱中培养72h,然后使用场发射扫描电子显微镜和倒置显微镜进行观察。
结果显示,被DBV感染的巨噬细胞和阳性对照组的巨噬细胞呈现出细胞膜穿孔和细胞质变透明、胞体膨大等焦亡特异性细胞形态(图1A-B)。
2、将巨噬细胞(1×105/孔)接种到96孔板中,并使用DBV(MOI=0.01、0.1、1)感染作为实验组,LPS和Nigericin联合处理作为阳性对照组,不处理作为阴性对照组。在37℃,5% CO2的培养箱中培养特定的时间后加入CCK-8试剂(碧云天),然后使用酶标仪检测450nm下的吸光度,对各组的巨噬细胞活性进行检测。在特定的时间点取各组上清液,加入LDH检测试剂(碧云天),然后使用酶标仪检测490nm下的吸光度,对各组的巨噬细胞LDH释放率进行检测。
结果显示,DBV感染能导致巨噬细胞细胞活性下降,并且细胞的存活率与DBV的剂量、感染时间呈正相关;DBV感染能导致巨噬细胞LDH释放,LDH的释放率与DBV的剂量、感染时间也呈正相关关系(图1C-F)。
实验结果证实,DBV感染诱导了巨噬细胞发生焦亡。
实施例2、DBV感染介导巨噬细胞焦亡基因和焦亡蛋白表达、IL-1β分泌
1、为了进一步证实DBV感染引发巨噬细胞焦亡,我们进行了以下实验。将巨噬细胞(1×106/孔)使用DBV(MOI=0.01、0.1、1)感染作为实验组、使用LPS和Nigericin联合处理作为阳性对照组、不处理作为阴性对照组。在特定的时间使用RNeasy Mini Kit(Qiagen)提取细胞中的总RNA,然后使用PrimeScrip RT Master Mix(TaKaRa)逆转录成cDNA,再使用TBPremix Ex TaqTMII(TaKaRa)试剂盒配制实时荧光定量PCR反应体系,其中引物序列如表1所示,委托北京擎科生物科技有限公司合成。最后在ABI7500 Fast Real-Time PCR(Applied Biosystems)系统进行实时定量PCR扩增反应,确定各组巨噬细胞中焦亡基因的表达情况。

实验结果显示,感染DBV的巨噬细胞中caspase-1、GSDMD、IL-1β这三个焦亡相关基因的mRNA表达量显著提升了(图2A-F)。
2、在特定的时间点收集巨噬细胞上清液,然后12000rpm离心10分钟取上清去除细胞碎片,用于检测IL-1β的分泌水平。将样本与检测试剂按照ELISA试剂盒(Abcam)说明书进行孵育,然后使用酶标仪检测450nm下的吸光度,最后计算出各组上清中的IL-1β的浓度。
结果显示,DBV感染的巨噬细胞上清中IL-1β显著提升并在感染时间为72h时达到峰值,IL-1β的浓度与DBV的剂量呈正相关(图2G)。
3、在特定的时间点使用RIPA裂解液(索莱宝)提取细胞中的总蛋白,然后加入5×蛋白上样缓冲液(索莱宝),95℃金属浴10分钟使蛋白变性,然后采用western blot检测DBV感染巨噬细胞后不同时间点caspase-1、GSDMD、IL-1β的相对表达量。
结果显示,DBV感染的巨噬细胞中,焦亡标志性蛋白caspase-1、cleaved GSDMD和cleaved IL-1β的表达均显著提升了(图2H)。
以上结果证实了DBV感染的巨噬细胞通过caspase-1途径发生了焦亡。
实施例3、DBV感染诱导细胞因子分泌
按照实施例2步骤对巨噬细胞进行处理,在感染时间为72h时收取各组的上清液,然后12000rmp离心10分钟取上清去除细胞碎片,用于检测TNF-α、IL-6、IL-8、IL-10、IL-12的分泌水平。将样本与检测试剂按照TNF-α、IL-6、IL-8、IL-10、IL-12的ELISA试剂盒(北京索莱宝生物科技有限公司)分别进行孵育,然后使用酶标仪检测450nm波长下的吸光度,最后计算出各组上清中的细胞因子浓度。
结果显示,DBV感染能诱导巨噬细胞分泌TNF-α、IL-6、IL-8、IL-10、IL-12,并且TNF-α、IL-6、IL-8这三种细胞因子的水平有非常显著的上升(图3)。
实施例4、血小板能抑制DBV感染诱导的巨噬细胞焦亡
1、按照实施例1步骤,巨噬细胞与DBV(MOI=0.01、0.1、1)和血小板(5×107)进行孵育,巨噬细胞与DBV(MOI=0.01、0.1、1)孵育作为对照,在72h时检测巨噬细胞的细胞活性。
结果显示,血小板能显著抑制由于DBV感染导致的巨噬细胞活性降低(图4A)。结果提示,血小板抑制了DBV导致的巨噬细胞焦亡。
2、按照实施例2步骤,巨噬细胞与DBV(MOI=1)和血小板(5×107)进行孵育,巨噬细胞与DBV(MOI=1)孵育作为对照,在特定的时间点使用RIPA裂解液(索莱宝)提取巨噬细胞的总蛋白,使用Western blot检测DBV感染巨噬细胞后不同时间点焦亡标志性蛋白的相对表达量。
结果显示,血小板能显著抑制炎症小体NLRP3、焦亡效应蛋白cleaved caspase-1、cleaved GSDMD、cleaved IL-1β的表达(图4B)。
结果证实血小板能显著抑制DBV感染诱导的巨噬细胞焦亡。
实施例5、血小板能抑制DBV感染诱导的巨噬细胞细胞因子分泌
按照实施例3步骤,巨噬细胞与DBV(MOI=0.01、0.1、1)和血小板(5×107)进行孵育,巨噬细胞与DBV(MOI=0.01、0.1、1)孵育作为对照,在特定时间点收集上清液。然后12000rmp离心10分钟取上清去除细胞碎片,用于检测IL-1β、TNF-α、IL-6、IL-8、IL-10、IL-12的分泌水平。将样本与检测试剂按照IL-1β、TNF-α、IL-6、IL-8、IL-10、IL-12的ELISA试剂盒(索莱宝)分别进行孵育,然后使用酶标仪检测450nm波长下的吸光度,最后计算出各组上清中的细胞因子浓度。
结果显示,血小板在各时间段各剂量DBV组中均能显著抑制巨噬细胞分泌IL-1β(图5A-D),在感染时间为72h时血小板能显著抑制DBV感染导致的TNF-α、IL-6、IL-8分泌,但对IL-10和IL-12的抑制效果不明显(图5E-I)。
实验结果证实,血小板能显著抑制DBV感染导致的巨噬细胞细胞因子分泌。
本发明是基于广泛而深入的研究,发现DBV感染能够诱导巨噬细胞焦亡,具体表现为细胞膜穿孔、胞体膨大、胞质变透明和IL-1β分泌。血小板能抑制DBV感染诱导的巨噬细胞焦亡和细胞因子分泌。依据研究结果,进一步开发与血小板有关的焦亡抑制剂,对SFTS患者的器官损伤和细胞因子风暴提供新的治疗方法,为SFTS患者的临床救治提供有效的策略和选择。
以上的实施例是为了说明本发明公开的实施方案,并不能理解为对本发明的限制。此外,本文所列出的各种修改以及发明中方法、组合物的变化,在不脱离本发明的范围和精神的前提下对本领域内的技术人员来说是显而易见的。虽然已结合本发明的多种具体优选实施例对本发明进行了具体的描述,但应当理解,本发明不应仅限于这些具体实施例。事实上,各种如上所述的对本领域内的技术人员来说显而易见的修改来获取发明都应包括在本发明的范围内。

1.血小板在制备抑制大别班达病毒感染导致免疫细胞焦亡药物中的应用。
2.如权利要求1所述的应用,其特征在于,所述血小板包括新鲜血小板或促进血小板生成的制剂。
3.如权利要求1所述的应用,其特征在于,所述大别班达病毒感染引发免疫细胞焦亡导致的重症发热伴血小板减少综合征,具有如下(1)~(3)任一项中的特征:
(1)出现持续性外周血小板减少,凝血异常;
(2)免疫功能失调,且免疫细胞具有焦亡特征;
(3)具有重症发热伴血小板减少综合征的临床表现,且出现细胞因子风暴或过度炎症反应症状。
4.如权利要求1-3任一项所述的应用,其特征在于,所述免疫细胞包括巨噬细胞。
5.如权利要求1-3任一项所述的应用,其特征在于,所述免疫细胞焦亡的特征包括:炎症小体激活、焦亡标志性蛋白表达或炎性细胞因子分泌。
6.一种抑制大别班达病毒感染导致免疫细胞焦亡的药物,其特征在于,包括有效剂量的血小板或有效剂量的促进血小板生成的制剂,以及药学上可接受的载体。
7.如权利要求6所述的药物,其特征在于,所述药学上可接受的载体为增稠剂、稀释剂或缓冲剂。
8.如权利要求6所述的药物,其特征在于,所述药物的制剂形式为注射制剂或口服制剂。
9.如权利要求6-8任一项所述的药物在制备预防和/或治疗大别班达病毒感染引发免疫细胞焦亡导致发热伴血小板减少综合征的药物中的应用。
