技术领域

背景技术
脑血管疾病是一组高发病率的疾病,是全球死亡和致残的主要原因之一。现代医学认为脑血管疾病主要分为缺血性和出血性卒中两种类型,其中缺血性卒中占卒中总发病量的87%。缺血性卒中后抑郁(Ischemia Post-stroke Depression,IPSD)是脑卒中常见的、重要的并发症,是卒中后以持续性的兴趣减退、情感低落为核心症状的心境障碍,多发生于缺血性脑卒中后1个月~1年。在临床治疗中,医生往往不够重视缺血性卒中后患者的情感障碍,对IPSD的诊断不够积极。缺血性卒中后抑郁患者的神经系统修复能力较差,它显著影响脑卒中患者的康复和转归,造成IPSD患者的神经功能缺损的恢复效果差,导致住院时间延长,医疗费用增加,降低其生活质量,给家庭、社会带来沉重的负担。目前关于缺血性卒中和卒中后抑郁的发病机制仍不十分明确,卒中后抑郁的治疗方案与原发性抑郁症的治疗几乎没有差别。因此进行缺血性卒中和卒中后抑郁的新机制研究,探寻治疗潜在的预后标志物,探索抗新靶点和药物,凸显其重要性。
水通道蛋白(Aquaporins,AQPs)是分子量介于26和34kDa之间的疏水性内在膜蛋白,有助于水的快速和被动转运,含有250~290个氨基酸。最初在1988年,由Agre等人从人血红细胞中分离发现了第一个AQP,并命名为AQP。从被发现至目前为止,水通道蛋白家族成员的数量持续增加,目前在人类和啮齿类动物中发现了13种AQP,即AQP0-AQP12。AQPs在神经系统的生理病理过程中起重要作用,研究结果显示,在脑组织中AQP1、AQP3、AQP4、AQP5、AQP6、AQP8、AQP9、AQP11有明显表达,其中水-甘油通道AQP3表达水平较高,说明AQP3可能在脑组织的生理及病理过程中扮演重要作用。AQP3在脑组织小神经胶质细胞中表达,AQP3基因敲除后显著降低小神经胶质细胞的迁移运动能力、胞内ATP的浓度、外周神经修复明显延迟。
AQP5可能是星形胶质细胞中重要的水通道,在各种脑损伤过程中差异表达。在永久性局灶性脑缺血和早产室内出血后,脑内AQP5的表达上调。在缺血诱导的大鼠脑梗死边界附近也检测到AQP5表达,AQP5水平可以通过缺氧和蛋白激酶A调节。此外,最近的研究还表明,AQP5的表达与脑膜瘤患者肿瘤周围水肿的发展和强度有关。在脑膜瘤患者的永久性局灶性脑缺血、脑室内出血、外伤和脑水肿期间,大脑中AQP5的表达增加。这些数据证明了AQP5在与脑缺血相关的脑水肿发展中的作用。
发明内容
针对上述现有技术,本发明提供水通道蛋白3和/或水通道蛋白5作为抑郁症药物靶点的用途。本发明的研究结果显示,AQP3和AQP5表达水平的变化与抑郁症症状的缓解密切相关,为后期治疗抑郁症药物研发提供了全新的药物靶点以及新的治疗手段和思路。
为了达到上述目的,本发明所采用的技术方案是,提供一种水通道蛋白3和/或水通道蛋白5作为药物靶点在筛选治疗脑卒中或卒中后抑郁的药物中的应用。
本发明还提供了一种水通道蛋白3的基因和/或水通道蛋白5的基因作为药物靶点在筛选治疗脑卒中或卒中后抑郁的药物中的应用。
在上述技术方案的基础上,本发明还可以做如下改进。

具体实施方式
下面通过动物实验对本发明的技术方案详细说明,但并不意味着对本发明的任何限制。
实验材料
1.实验动物:SPF级SD大鼠80只,体重(260±10)g,雄性,合格证号:SCXK(川)2020-030,成都达硕生物科技有限公司提供。动物饲养于动物房,温度24~26℃,相对湿度40%~60%,昼夜时间为12小时,自由摄食饮水,实验开始前适应性饲养1周。动物实验遵守国际实验动物伦理学要求。
2.试剂:制何首乌(云南七丹药业股份有限公司,1901001);盐酸氟西汀(PatheonFrance,J20130010)。
实施例1:建立模型
1.1.缺血性卒中(MCAO)模型
选取体重为280~300g的SD雄性大鼠,采用线栓法构建脑缺血再灌注损伤(MCAO)模型。造成大鼠右脑堵塞,缺血后2h拔出线栓进行再灌注,参考Zea-longa评分法评估大鼠神经功能缺损程度。
1.2.脑卒中后抑郁(PSD)模型
MCAO造模后7天,给予28天的慢性不可预见的温和性应激(CUMS),包括:45°倾斜鼠笼、2h行为限制、24h禁水禁食、24h潮湿垫料、1min夹尾、24h昼夜颠倒,每日选择1种刺激,相邻两天刺激不相同。假手术组不做处理。
实施例2:分组及给药
给药一组:将MCAO造模成功的50只大鼠随机分为模型组、制何首乌低剂量组(0.75g/kg)、制何首乌高剂量组(1.5g/kg)和阳性药氟西汀组(10mg/kg),每组10只,另随机选取10只钝性分离后直接缝合处理;不进行进栓,不进行PSD造模,其他操作相同的大鼠作为假手术组。假手术组、模型组于PSD造模开始时灌胃生理盐水,各给药组灌胃给药。
给药二组:将MCAO造模成功的50只大鼠随机分为模型组、芦荟大黄素组、虎杖苷组及阳性药氟西汀组(10mg/kg),每组10只,另随机选取10只钝性分离后直接缝合处理;不进行进栓,不进行PSD造模,其他操作相同的大鼠作为假手术组。假手术组、模型组于PSD造模开始时灌胃生理盐水,各给药组在PSD造模开始时灌胃给药。
实施例3:神经功能评分
根据Longa Z的5分制法进行神经功能损伤评价。对MCAO造模后1d、7d、14d、21d、28d的各组大鼠按表1中的记载进行神经行为学评分,并记录。

神经行为学评分能直观反映MCAO模型大鼠神经功能的恢复程度。评分越低,表示大鼠恢复程度越好。
如图1所示,随着抑郁症模型的进展,模型组大鼠神经功能损害有加重趋向,出现较严重的死亡,但各给药组的神经功能有所恢复,且死亡较少。在给药第21天,制何首乌(低)和制何首乌(高)剂量组与模型组相比,神经功能明显恢复(*P<0.05),且给药7天与给药21天相比,制首乌(高)剂量组神经功能显著恢复(**P<0.01)。以上均可说明,制首乌(高)剂量组具有明显的促进神经功能修复的作用。如图2所示,在PSD造模给药第21天(即MCAO造模第28天)后,与模型组相比,虎杖苷组的神经功能评分明显降低(*P<0.05),且芦荟大黄素组显著降低(**P<0.01),效果接近于假手术组。
实施例4:行为学评分(针对给药一组的大鼠)
1.糖水消耗实验
第一天给予每笼大鼠两瓶1%的蔗糖水,第二天禁水禁食,第三天分别给予每笼大鼠一瓶普通水和一瓶1%蔗糖水。每隔1h更换两个水瓶的位置。3h后记录每组各瓶水的消耗量。分别于PSD造模后的第1、7、14、21天测取实验数据。其中,蔗糖水消耗率通过以下公式计算得到:
蔗糖水消耗率=蔗糖水消耗量/(蔗糖水消耗量+普通水消耗量)×100%
蔗糖偏好(蔗糖水消耗率)可以反应大鼠抑郁程度,大鼠恢复状态越好,则糖水偏好值越高。给药一组大鼠的蔗糖偏好测定结果如图3所示,模型组在PSD造模第1天与第21天相比,糖水偏好程度明显降低(*P<0.05);制何首乌高剂量组在PSD造模第14天和21天,糖水偏好值明显高于模型组(*P<0.05),表明制首乌可以改善缺血性卒中和卒中后抑郁状态,修复神经功能损伤。
2.开场实验
在黑暗的环境中将大鼠放入70cm×70cm的纸箱中,纸箱内用记号笔划分为25个区域,记录大鼠穿越过的区域数和站立的次数。分别于PSD造模后的第1、7、14、21天测取实验数据。
开场实验可以反映大鼠抑郁程度,评分升高则说明大鼠抑郁程度减轻。给药一组大鼠的开场实验检测结果如图4和5所示。从图4中可以看出,制何首乌(高)剂量组在PSD造模给药的第21天,与模型组相比穿越格子数明显增多(*P<0.05)。从图5中可以看出,模型组在PSD造模第7天与21天相比,垂直站立次数显著减少(**P<0.01),而制何首乌(高)剂量组明显增多(*P<0.05)。且在PSD造模21天后,与模型组相比,制首乌(低)(高)剂量组的垂直站立次数明显增多(*P<0.05)(**P<0.01)。上述结果同样表明制首乌可以改善缺血性卒中和卒中后抑郁状态,修复神经功能损伤。
3.强迫游泳实验
将大鼠放入直径为50cm装有25cm水深的水桶中,先让大鼠在水中适应1min,然后记录大鼠在接下来的6min时间里的不动时间。分别于PSD造模后的第1、7、14、21天测取实验数据。
强迫游泳实验可以反应大鼠的抑郁程度,大鼠游泳时间越短,说明其抑郁症状越轻。给药一组大鼠的强迫游泳时间如图6所示,从中可以看出,制何首乌(高)剂量组在PSD造模第7天与第21天相比,强迫游泳时间非常显著的缩短(***P<0.001)。且在PSD造模第21天,制何首乌(高)剂量组比模型组的游泳时间显著减少(**P<0.01)。上述结果同样表明制首乌可以改善缺血性卒中和卒中后抑郁状态,修复神经功能损伤。
实施例5:组织病理学检测
1.脑梗死体积与脑含水量
取PSD造模后21天脑组织(全脑),于-20℃冷冻,均匀切成5片。用TTC染液避光染色后,4%多聚甲醛固定。其中,红色部分为正常脑组织区域,白色部分为脑梗死区域。脑梗死体积计算公式如下:
脑梗死体积(%)=(M白色部分/M全脑)×100%
将大鼠新鲜脑组织冲洗擦拭干净后称重(湿重),再将脑组织通过烘箱37℃烘干至恒重后称重(干重)。脑含水量计算公式如下:
脑含水量(%)=(M湿重-M干重)/M湿重×100%
TTC染色可以检测大鼠脑梗死体积。给药一组大鼠脑组织的TTC染色结果如图7~8所示。与模型组相比,各组脑梗死体积均有一定程度的减小,其中,制何首乌(高)剂量组脑梗死体积非常显著的减小(***P<0.001)。制何首乌(低)剂量组大鼠的脑梗死体积也显著减小(**P<0.01)。由此可说明,制何首乌可以减少大鼠脑梗死体积,降低脑组织损伤程度,缓解脑缺血和脑缺血后抑郁。给药二组大鼠脑组织TTC染色结果如图9~10所示,与模型组相比,各组脑梗死体积均极大减小,其中,芦荟大黄素组、虎杖苷组组脑梗死体积非常显著的减小(***P<0.001)。由此可说明,芦荟大黄素、虎杖苷可以减少大鼠脑梗死体积,降低脑组织损伤程度,缓解缺血性卒中和卒中后抑郁。
脑含水量用于评价脑卒中疾病发生后各组大鼠脑水肿的严重程度。给药一组大鼠脑含水量检测结果如图11所示。从中可以看出,制何首乌(高)及制何首乌(低)剂量组均能显著降低大鼠脑含水量(**P<0.01)。由此说明,制何首乌可以改善模型大鼠脑水肿现象,降低缺血造成的损伤。给药二组大鼠脑含水量检测结果如图12所示。从中可以看出,芦荟大黄素组、虎杖苷组均能显著降低大鼠脑含水量(**P<0.01,*P<0.05)。结果表明,芦荟大黄素、虎杖苷可以改善模型大鼠脑水肿现象,降低缺血造成的损伤。
2.苏木精-伊红(HE)染色
取PSD造模后21天脑组织(取全脑,切片为冠状面),固定、脱水、二甲苯清洗、石蜡包埋、切片。按照步骤进行HE染色,1%盐酸分化、氨水返蓝、伊红复染、水洗、梯度脱水、烘干后封片。
HE染色用于考察脑缺血抑郁后脑部微观组织的损伤情况。给药一组和给药二组大鼠大脑HE染色结果分别如图13和14所示。从图中可以看出,假手术组细胞饱满,排列紧密,细胞结构完整;模型组细胞间隙增大,出现空洞,组织结构破坏,细胞肿胀破碎,构造不清晰。与模型组相比,制何首乌各给药组、芦荟大黄素组、虎杖苷组脑部结构都较清晰,恢复程度接近于假手术组,对PSD模型大鼠的脑部结构修复具有明显的改善作用。
3.尼氏(Nissl)染色
方法类似HE染色,大鼠脑组织依次经过固定、包埋、切片及脱蜡后,1%甲苯胺蓝染色,95%酒精分化,梯度脱水、封片后显微观察。
脑缺血再灌注损伤会导致病理环境中神经元的死亡或凋亡,从而加重脑卒中后抑郁的发生,尼氏染色可用于考察病灶部位尼氏小体的数量,直观反应神经元的数量。给药一组和给药二组大鼠大脑的Nissl染色结果分别如图15和16所示,尼氏小体定量统计结果分别如图17和18所示。从图中可以看出,假手术组尼氏小体数量较多,且排列紧凑,颜色较深;模型组尼氏小体数目明显减少,出现固缩,边缘消失,出现明显的空洞裂缝。与模型组相比,制何首乌(高)剂量组、芦荟大黄素组、虎杖苷组的尼氏小体数量明显增加(*P<0.05),表明这些物质可以减少脑卒中后抑郁大鼠的神经元凋亡,减轻PSD大鼠神经功能损伤。
4.免疫组化
取PSD造模后21天脑组织(取全脑,切片为冠状面),经过石蜡切片,脱蜡,修复抗原,室温孵育后,分别加入AQP3、AQP5及BDNF抗体,孵育过夜;二抗孵育后进行DAB染色,苏木素复染,脱水,透明,封片。
给药一组和给药二组大鼠脑组织切片分别用AQP3抗体进行免疫组化染色,染色结果分别如图19~20和21~22所示。从图中可以看出,假手术组脑组织结构完整清晰,神经细胞排布密集,排布整齐;模型组脑组织结构松散,出现缺失,神经细胞数量减少,大量细胞排布混乱,细胞形状不清晰;各给药组脑组织结构较完整,神经细胞排布较整齐,细胞数量较多,分布较均匀。与模型组相比,制何首乌(低)剂量组与AQP3抗体结合的数目明显降低;制何首乌(高)剂量组、芦荟大黄素组、虎杖苷组与AQP3抗体结合的数目非常显著的降低,表明这些物质可以调节AQP3的表达,消除脑组织水肿,改善模型大鼠神经功能。
给药一组大鼠脑组织切片分别用AQP5和BDNF抗体进行免疫组化染色,结果分别如图23~24和25~26所示;给药二组大鼠脑组织切片分别用AQP5和BDNF抗体进行免疫组化染色,结果分别如图27~28和29~30所示。从图中可以看出,假手术组脑皮质神经元结构清晰完整,排列紧密,细胞核居中,核仁清楚;模型组部分神经元细胞出现凋亡坏死,脑组织部分区域核固缩,细胞裂解,水肿严重,脱水后出现空洞;制何首乌给药组可见部分神经元呈现浅棕色染色,神经元结构排列较整齐,细胞核比较完整。与模型组相比,制何首乌(低)及制何首乌(高)剂量组、芦荟大黄素组、虎杖苷组与AQP5抗体结合的数目均明显降低,与BDNF抗体结合的数目均明显上升(*P<0.05)。由此说明,制何首乌、芦荟大黄素、虎杖苷可以调节AQP5及BDNF的表达,改善神经元微环境,促进BDNF的表达,发挥神经保护作用。
4.ELISA检测血清AQP3、AQP4、AQP5及AQP11水平
在给药一组和给药二组大鼠的腹主动脉采血5ml,离心取血清,按照ELISA试剂盒说明书测定大鼠血清AQP3、AQP4及AQP5及AQP11水平,结果如图31和32所示。从图中可以看出,与模型组相比,各给药组大鼠血清中AQP3和AQP5的含量均显著降低。
5.Western blotting检测大鼠脑组织AQP3、AQP4、AQP5及BDNF蛋白表达
将给药一组大鼠的脑组织放入蛋白裂解缓冲液,匀浆,离心,提取蛋白,检测浓度。然后电泳、转膜、脱脂牛奶封闭1h,分别加入AQP3、AQP4、AQP5、及BDNF一抗,4℃孵育过夜。再用相对应的二抗室温孵育后采用化学发光法测量条带信号的强度,并用ImageJ软件进行灰度值分析。结果如图33和34所示。从图中可以看出,与模型组相比,各给药组大鼠脑组织中的AQP5的蛋白含量均有所降低。BDNF的含量均有所升高。制何首乌(高)剂量组可以明显降低大鼠脑组织中AQP5的蛋白含量,提高BDNF的蛋白含量(*P<0.05)。制何首乌通过调节模型大鼠脑组织中AQP5、BDNF的蛋白含量,改善其行为活动。
虽然结合实施例对本发明的具体实施方式进行了详细地描述,但不应理解为对本专利的保护范围的限定。在权利要求书所描述的范围内,本领域技术人员不经创造性劳动即可作出的各种修改和变形仍属本专利的保护范围。
Claims (9)
1.水通道蛋白3和/或水通道蛋白5作为药物靶点在筛选治疗脑卒中或卒中后抑郁的药物中的应用。
2.水通道蛋白3的基因和/或水通道蛋白5的基因作为药物靶点在筛选治疗脑卒中或卒中后抑郁的药物中的应用。
3.根据权利要求1或2所述的应用,其特征在于:所筛选的药物为抑制水通道蛋白3和/或水通道蛋白5表达水平的药物。
4.根据权利要求3所述的应用,其特征在于:所筛选的药物为同时抑制水通道蛋白3和水通道蛋白5表达水平的药物。
5.根据权利要求4所述的应用,其特征在于:所述药物以制何首乌、芦荟大黄素和虎杖苷中的至少一种为活性物质。
6.根据权利要求5所述的应用,其特征在于:所述药物的活性物质为制何首乌。
7.根据权利要求5所述的应用,其特征在于:所述药物的活性物质为芦荟大黄素。
8.根据权利要求5所述的应用,其特征在于:所述药物的活性物质为虎杖苷。
9.根据权利要求1或2所述的应用,其特征在于:所述脑卒中为缺血性脑卒中。
