背景技术
衰老的自由基理论表明衰老的生物过程会导致老年人中增加的氧化应激。细胞抵抗氧化应激损伤潜力的能力由氧化剂自由基的产生与可供细胞使用的抗氧化剂防御性系列之间的重要平衡决定。存在多种抗氧化剂防御系统,而其中谷胱甘肽(GSH)是整体抗氧化剂防御中最为丰富的细胞内组分。GSH是一种三肽,它由前体氨基酸谷氨酸、半胱氨酸和甘氨酸在谷氨酸半胱氨酸连接酶(GCL,也称为γ-谷氨酰半胱氨酸合成酶,EC 6.3.2.2)和γ-L-谷氨酰-L-半胱氨酸:甘氨酸连接酶(也称为谷胱甘肽合成酶,EC 6.3.2.3)所催化的两个步骤中合成,并且在细胞中从头进行GSH合成。

发明内容
本公开的实施方案涉及用于对与降低的细胞内GSH水平直接或间接相关的医学病症或身体状况进行治疗或预防或延迟发作的方法和/或组合物。本公开的实施方案还涉及用于对与降低的半胱氨酸和/或甘氨酸的血液水平或细胞内水平直接或间接相关的医学病症或身体状况进行治疗或预防或延迟发作的方法和/或组合物。在具体实施方案中,增加个体中的细胞内GSH的水平,从而以此对该医学病症或身体状况进行治疗、预防或延迟发作。在某些实施方案中,降低个体中的C反应蛋白(CRP)的水平,从而以此对该医学病症或身体状况进行治疗、预防或延迟发作。
在特定实施方案中,设想到提供来自谷胱甘肽、n-乙酰半胱氨酸和/或甘氨酸的有益效果的方法,并且在具体情况下,谷胱甘肽、n-乙酰半胱氨酸和/或甘氨酸各自单独地和/或共同地作出贡献。

在一个实施方案中,存在预防和/或治疗肌少症、肌少性肥胖或恶病质的方法。在恶病质的具体实施方案中,个体会因为潜在的医学病症诸如长期患病、携带HIV、患有癌症、患有COPD、在原本健康的个体中出现衰老(肌少症)等等而存在恶病质。在老年HIV患者的具体实施方案中,补充本文所设想的组合物会升高谷胱甘肽,且会增加瘦体重(肌肉)和减少脂肪量。
本公开的一个特定实施方案可以对由低GSH水平(包括已知富含谷胱甘肽的眼部晶状体中的低水平)直接或间接引起的眼部病症进行预防和/或治疗。例如,此类病症包括白内障和/或青光眼、老花眼(随着衰老而丧失近视力,需要老花眼镜)。或老年聋(随着衰老而丧失听力,需要助听器)。

在特定实施方案中,本公开的方法预防和减轻/治疗线粒体毒性,包括药物诱导的线粒体毒性。例如,为接受对线粒体功能有害的抗逆转录病毒HIV药物的HIV携带个体提供有效量的NAC和甘氨酸以改善线粒体功能。
在具体实施方案中,存在预防和/或治疗对乙酰氨基酚毒性的方法。具体地讲,为将要摄入对乙酰氨基酚或正在服用对乙酰氨基酚或具有对乙酰氨基酚毒性的个体提供有效水平的一种或多种本文所设想的组合物。具有对乙酰氨基酚毒性的个体可能是或可能不是对乙酰氨基酚的长期服用者。在具体实施方案中,该个体在服用增加个体中的GSH水平的一种或多种药剂的同时服用对乙酰氨基酚。

本公开的实施方案包括改善骨骼肌损失的方法,该骨骼肌损失因任何原因造成,包括恶病质、肌少症、不活动、应激(包括手术后、脓毒症后和创伤后)等等。该改善可为肌肉损失的速率或数量的减少和/或肌肉损失的逆转。

如本文所用,术语“氧化应激”是指个体、或个体的细胞或组织中活性氧的产生与解毒活性中间体或易修复生物系统中造成的损害的能力之间不平衡的状态。通过使用代谢能量恒定输入的过程来维持细胞内的天然还原环境,并且这种正常的氧化还原状态的干扰可通过产生例如破坏细胞组分诸如蛋白质、脂质和/或DNA的自由基和过氧化物而带来毒性效应。
II.一般实施方案
本公开的实施方案包括增加半胱氨酸和甘氨酸(例如,半胱氨酰甘氨酸)的血液水平以增加细胞内GSH、半胱氨酸和/或甘氨酸和/或减少CRP水平的方法。在一些实施方案中,作用机制涉及半胱氨酸和/或甘氨酸但不依赖于GSH。
在本发明的某些实施方案中,存在用于治疗由个体中的GSH水平不足直接或间接引起的医学病症的方法和组合物。该个体可为任何年龄或健康状况,但在特定实施方案中,该个体是老年人,易出现与GSH水平不足直接或间接相关的特定医疗病症或身体状况,或者具有与GSH水平不足直接或间接相关的医疗病症或身体状况。向该个体递送的组合物在此类情况下包含至少甘氨酸和n-乙酰半胱氨酸,特别是作为有利于升高该个体中的谷胱甘肽水平的前体氨基酸。例如,可以测量红血细胞GSH或可以通过肌肉活检测量细胞内GSH水平。细胞内GSH测量方法是本领域已知的(Rahman等人,2007年)。
在具体实施方案中,使用有效量的甘氨酸或其功能性衍生物和N-乙酰半胱氨酸或其功能性衍生物治疗或预防由降低的半胱氨酸、甘氨酸和/或GSH水平直接或间接引起的一种或多种医学病症。在特定实施方案中,该医学病症或身体状况是以下中的一种或多种:(a)肌肉损失;(b)失重的有害效应;(c)器官损伤;(d)心脏功能或衰竭;(e)癌症预防;(f)用于预防肥胖和/或糖尿病的后期发展的胎儿代谢规划;(g)妊娠糖尿病中的母体和胎儿健康;(h)运动能力和身体机能;(i)肥胖;(j)寿命;(k)肝毒性;(1)神经退行性疾病;(m)肾病的预防;(n)对乙酰氨基酚毒性的预防;(o)非酒精性脂肪性肝炎;(p)酒精宿醉;(q)听力损伤;(r)阿尔茨海默病;(s)帕金森氏病;(t)骨质疏松;(u)高血压;(v)多囊卵巢综合征(PCOS);(w)动脉粥样硬化;(x)冠状动脉疾病;(y)应激后心肌损伤;(z)接种疫苗后免疫力不足;(aa)囊性纤维化;(bb)创伤性脑损伤;(cc)脑震荡;(cc)脑震荡;(dd)耳毒性;(ee)耳鸣;(ff)眩晕;(gg)头晕;(hh)美尼尔氏病;(ii)创伤后恢复和存活(例如,手术后、脓毒症后、因事故或身体攻击造成的钝器伤或穿透伤后等);(jj)非酒精性脂肪肝病(NAFLD)以及(kk)它们的组合。该个体可被诊断为患有一种或多种此类病症,或可疑似患有一种或多种此类病症,或可易患一种或多种此类病症。除了本公开的方法之外,还可使用其他一种或多种疗法治疗该个体。
在具体实施方案中,出于升高GSH、半胱氨酸和/或甘氨酸的细胞内水平的明确目的并且因为已确定个体患有与此类水平直接或间接相关的病症,而为个体提供有效量的本文所述组合物。在具体情况下,本公开的方法包括一种或多种此类医学病症的诊断。
III.药物组合物
在特定实施方案中,本发明涉及用于对与降低的细胞内GSH水平直接或间接相关的医学病症或身体状况进行治疗、预防或延迟发作的药物组合物。在具体实施方案中,所述组合物由以下物质组成,基本上由以下物质组成,或包含以下物质:甘氨酸(或其功能性衍生物)和N-乙酰半胱氨酸(或其功能性衍生物)。甘氨酸的功能性衍生物被定义为在个体中单独起作用或与N-乙酰半胱氨酸(或其功能性衍生物)共同起作用以增加细胞内GSH水平的甘氨酸衍生物。N-乙酰半胱氨酸的功能性衍生物被定义为在个体中单独起作用或与甘氨酸(或其功能性衍生物)共同起作用以增加细胞内GSH水平的N-乙酰半胱氨酸衍生物。在具体实施方案中,可采用“半胱氨酸”衍生物,即,在个体中单独起作用或与甘氨酸共同起作用的半胱氨酸的功能性衍生物。
可一起或单独提供甘氨酸组分和N-乙酰半胱氨酸组分。在具体实施方案中,该组合物包含N-乙酰半胱氨酰甘氨酸;半胱氨酰甘氨酸及其所有形式,例如L-半胱氨酰甘氨酸等等。甘氨酸衍生物的示例包括至少D-烯丙基甘氨酸;N-[双(甲硫基)亚甲基]甘氨酸甲酯;Boc-烯丙基-Gly-OH(二环己铵)盐;Boc-D-Chg-OH;Boc-Chg-OH;(R)-N-Boc-(2′-氯苯基)甘氨酸;Boc-L-环丙基甘氨酸;Boc-L-环丙基甘氨酸;(R)-N-Boc-4-氟苯基甘氨酸;Boc-D-炔丙基甘氨酸;Boc-(S)-3-噻吩基甘氨酸;Boc-(R)-3-噻吩基甘氨酸;D-α-环己基甘氨酸;L-α-环丙基甘氨酸;N-(2-氟苯基)-N-(甲磺酰)甘氨酸;N-(4-氟苯基)-N-(甲磺酰)甘氨酸;Fmoc-N-(2,4-二甲氧基苄基)-Gly-OH;N-(2-呋喃甲酰基)甘氨酸;L-α-新戊基甘氨酸;D-炔丙基甘氨酸;肌氨酸;Z-α-膦酰甘氨酸三甲酯等等。
在特定实施方案中,所述药物组合物包含N-乙酰半胱氨酸(NAC)、L-甘氨酸、L-甘氨酸乙酯和/或二肽形式,例如半胱氨酰甘氨酸。
在具体实施方案中,在特定的时间段内以1.33mmol/kg/d施用甘氨酸并且以0.83mmol/kg/d施用NAC。治疗的持续时间可持续例如一天或多天、1周、2周、3周、一个月、两个月、三个月、四个月、五个月、六个月、一年、两年、五年、十年、十五年、二十年、二十五年、三十年等等。在一些情况下,该治疗持续该个体的剩余寿命。在具体实施方案中,该施用一直进行到未遗留该医学病症的可检测的症状。在具体实施方案中,该施用一直进行到出现至少一种症状的可检测的改善,并且在另外的情况下,继续保持改善。
在本发明涉及使用本发明的化合物进行治疗的情况下,可通过任何公认的施用模式进行本发明的化合物(必要时随合适的药物赋形剂一起)的施用。所述化合物可在适当时被包含在药物可接受的赋形剂中,该赋形剂可被认为是在施用给动物时不产生不利反应、变态反应或其他不良反应的分子实体和/或组合物。它包括任何和/或所有溶剂、分散介质、涂层、抗细菌和/或抗真菌剂、等渗和/或吸收延迟剂和/或类似物。此类介质和/或药剂在药物活性物质中的应用是本领域熟知的。除非任何常规的介质和/或药剂与活性成分不相容,都可以预期将其用于治疗组合物中。
因此,施用可以是例如静脉内、局部、皮下、经皮、肌肉内、经口、关节内、非肠道、腹膜、鼻内、膀胱内或通过吸入施用。因此合适的施用部位包括但不限于皮肤、支气管、胃肠、肛门、阴道、眼睛、膀胱和耳朵。制剂可采取固体、半固体、冻干粉或液体剂型的形式,诸如例如片剂、丸剂、胶囊剂、散剂、溶液剂、混悬剂、乳剂、栓剂、滞留型灌肠剂、霜剂、膏剂、洗剂、气溶胶剂等等,优选地为适于简单施用精确剂量的单位剂型。
所述组合物通常包含常规药物载体或赋形剂,并且可另外包含其他药剂、载体、辅料等等。优选地,该组合物将占本发明的一种或多种化合物的约5重量%至75重量%,并且其余部分由合适的药物赋形剂组成。可通过本领域熟知的方法为特定组合物和施用途径定制适当的赋形剂,例如REMINGTON′S PHARMACEUTICAL SCIENCES,18TH ED.,MackPublishing Co.,Easton,Pa.(1990)(《雷明顿药物科学》,第18版,宾夕法尼亚州伊斯顿的马克出版公司,1990年)。
对于经口施用而言,此类赋形剂包括药品等级的甘露糖醇、乳糖、淀粉、硬脂酸镁、糖精钠、滑石粉、纤维素、葡萄糖、明胶、蔗糖、碳酸镁等等。该组合物可采取溶液剂、混悬剂、片剂、丸剂、胶囊剂、散剂、持续释放制剂等形式。
在一些实施方案中,这些药物组合物采取丸剂、片剂或胶囊剂的形式,因此该组合物可连同生物活性缀合物包含以下中的任一种:稀释剂,诸如乳糖、蔗糖、磷酸二钙等;崩解剂,诸如淀粉或其衍生物;润滑剂,诸如硬脂酸镁等;以及粘结剂,诸如淀粉、阿拉伯树胶、聚乙烯吡咯烷酮、明胶、纤维素及其衍生物。
可将所述式的活性化合物配制成栓剂,其包含例如置于聚乙二醇(PEG)载体(例如,PEG 1000[96%]和PEG 4000[4%])中的约0.5%至约50%的本发明的化合物。
液体组合物可通过以下方式制备:将化合物(约0.5%至约20%)和任选的药用辅料溶解或分散在载体诸如例如盐水溶液(例如,0.9%w/v氯化钠)、右旋糖水溶液、甘油、乙醇等等中,以形成例如用于静脉内施用的溶液剂或混悬剂。也可将活性化合物配制成保留灌肠剂。
如果需要,待施用的组合物也可含有少量的无毒辅助物质,诸如润湿剂或乳化剂、pH缓冲剂诸如例如乙酸钠、脱水山梨糖醇单月桂酸酯或油酸三乙醇胺。
对于局部施用而言,该组合物以任何合适的形式诸如洗剂或透皮贴剂来施用。对于通过吸入递送而言,该组合物可经由喷雾器以干粉的形式(例如,吸入治疗剂)或以液体形式递送。
用于制备此类剂型的方法是已知的或对于本领域技术人员是显而易见的;例如,参见《雷明顿药物科学》,出处同上,以及类似的出版物。待施用的组合物在任何情况下都将包含药物有效量的前体药物和/或一种或多种活性化合物,以便在根据本发明的教导施用时使所治疗的病症得到缓解。
一般而言,本发明的化合物以治疗有效的量施用,即,足以实现治疗的剂量,该剂量将随个体和所治疗的病症的不同而不同。通常,治疗有效的日剂量为每天0.1至100mg/kg体重的药物。大多数病症对每天约1和约30mg/kg体重之间、或对于70kg的人而言每天约70mg和2100mg之间的总剂量的施用有应答。
缀合物的稳定性可进一步通过化学改变来控制,包括多肽链中的D氨基酸残基以及其他拟肽部分。此外,缀合物的稳定性也可通过非天然的碳水化合物残基来增强。
甘氨酸和N-乙酰半胱氨酸组分可按特定比率配制。在某些实施方案中,该制剂可包含以下示例性比率的所述组分:例如,1∶1、1∶2、1∶3、1∶4、1∶5、1∶6、1∶7、1∶8、1∶9、1∶10、1∶12、1∶15、1∶20、1∶25、1∶30、1∶35、1∶40、1∶45、1∶50、1∶55、1∶60、1∶65、1∶70、1∶75、1∶80、1∶85、1∶90、1∶95、1∶100、1∶150、1∶200、1∶300、1∶400、1∶500、1∶600、1∶750、1∶1000、1∶10,000等等。在特定实施方案中,该制剂可包含以下配制百分比的组分(每种组分的百分比相同或不同):例如,1%、2%、3%、4%、5%、6%、7%、8%、9%、10%、12%、15%、20%、25%、30%、40%、45%、50%、55%、60%、65%、70%、75%、80%、85%、90%、95%、97%或99%。
甘氨酸(或其功能性衍生物)和N-乙酰半胱氨酸(或功能性衍生物)可以在相同的组合物或不同的组合物中递送。在单独提供甘氨酸(或其功能性衍生物)和N-乙酰半胱氨酸(或功能性衍生物)的实施方案中,单独递送它们的方案可为任何合适的种类。在具体实施方案中,在N-乙酰半胱氨酸之前、与N-乙酰半胱氨酸同时、或在N-乙酰半胱氨酸之后向个体提供甘氨酸。单独递送可涵盖相同的施用途径但在不同时间递送,或者可为不同的施用途径。
IV.组合治疗
作为另外一种选择,本发明的治疗可在另一种治疗之前、之后从数分钟到数周范围的间隔,或者兼有这两种情况。在向个体单独提供一种或多种本发明组合物和其他药剂的实施方案中,通常会确保每次递送的间隔时间未经过显著的时间段,使得本发明组合物和其他药剂仍将能够对细胞发挥有利组合的作用。在此类情况下,可以设想到可在彼此间隔约12-24小时内、并且更优选地在彼此间隔约6-12小时内递送这两种模态。然而,在一些情形下,可能期望显著延长治疗的时间段,其中在各次施用之间经过数天(2、3、4、5、6或7)到数周(1、2、3、4、5、6、7或8)。
可采用各种组合,例如,其中本发明的治疗是“A”,并且用于如本文所述的本发明的医学病症的第二药剂诸如糖尿病治疗(仅作示例)是“B”:

向患者施用本发明的创造性组合物将遵循药物施用的一般方案,并考虑分子的毒性(如果有的话)。预期可以根据需要重复治疗周期。还可以设想到各种标准疗法以及手术介入可以与所述疗法联合应用。
V.试剂盒
与本发明的组合物相关的治疗试剂盒包括本发明的另一个方面。此类试剂盒一般将在合适的容器装置中容纳本发明的创造性组合物。该试剂盒可具有容纳本发明组合物的单个容器装置,或其可具有不同的容器装置,这些不同的容器装置用于本发明组合物以及其他可包括在此类试剂盒内的药剂。
试剂盒的各组分可以以一种或多种液体溶液的形式或一种或多种干燥粉末的形式提供。当各组分以液体溶液的形式提供时,该液体溶液为水性或非水性的溶液,且无菌的水性或非水性溶液是特别优选的。当试剂或组分以干燥粉末的形式提供时,该粉末可通过加入适当的溶剂来重构。可以预见,溶剂也可在另一个容器装置中提供。
所述容器装置一般将包括可在其中放入该组合物并且优选适当地等分该组合物的至少一个小瓶、试管、烧瓶、瓶子、注射器或其他容器装置。在提供第二药剂的情况下,该试剂盒一般也将含有可在其中放入该药剂的第二小瓶或其他容器。本发明的试剂盒通常也将包括用于以紧密密封的方式容纳药剂小瓶用于商业销售的装置。此类容器可包括例如将所需小瓶保持在其中的注塑或吹塑塑料容器。
本发明的试剂盒中,甘氨酸(或其功能性衍生物)和N-乙酰半胱氨酸(或其功能性衍生物)可单独提供或以混合物的形式一起提供。
VI.实施例
包括以下实施例以展示本发明的优选实施方案。本领域技术人员应当理解,以下实施例中所公开的技术代表发明人发现在本发明的实施中表现良好的技术,因此可以被认为构成其实施的优选模式。但是,根据本公开,本领域技术人员应当理解,在不脱离本发明的实质和范围的情况下,可在所公开的具体实施方案中进行许多改变,并且仍可取得相同或类似的结果。
实施例1
衰老过程中肌少性肥胖的代谢基础:谷胱甘肽的作用
老年人具有变得超重或患肥胖症的最高风险。与此同时,该群体中的肌肉量减少,因此老年人会出现肌少性肥胖的表型,肌肉力量减弱且生活质量下降,但尚未充分理解基本机制并且没有有效的疗法。人类和啮齿动物中的转化医学研究已获得了这样的发现:衰老过程中缺乏最为丰富的内源性抗氧化剂谷胱甘肽(GSH)与线粒体功能障碍有关联,并且在一些实施方案中,这为老年人患肌少性肥胖提供了机制解释。老年人的GSH缺乏由合成减少所引起,而合成减少的原因是其前体氨基酸半胱氨酸和甘氨酸的可利用性有限。短期补充这些氨基酸足以校正其自身的缺乏,并且还会恢复GSH的细胞内合成和浓度。与禁食的健康年轻人对照相比,禁食的GSH缺乏老年人存在线粒体脂肪酸氧化的重度损伤(这可促进脂肪存储),同时碳水化合物氧化增加(这可造成肌肉损失)。由于禁食状态下线粒体燃料偏好是脂肪酸而不是葡萄糖,禁食燃料偏好的这一异常逆转表明线粒体能量学受损。有趣的是,在2周内使用半胱氨酸和甘氨酸前体补充剂在这些老年人中恢复GSH合成,引起了禁食线粒体脂肪酸和碳水化合物氧化完全恢复到年轻人对照中所见的水平。基于这些数据,认为受损的线粒体脂肪酸氧化迫使燃料氧化转向葡萄糖以满足能量需求。由于在禁食状态下葡萄糖由糖异生提供且肌肉蛋白的贡献很大,这将导致肌肉蛋白(以及因此肌肉量)损失以及半胱氨酸和甘氨酸(已知的糖异生氨基酸)缺乏,从而进一步引发GSH缺乏。肌肉量损失继而将导致肌肉力量减弱。补充半胱氨酸和甘氨酸可以校正GSH缺乏并打破该负螺旋,从而校正线粒体脂肪酸氧化(并因此降低总的体脂肪),降低碳水化合物氧化(并因此免于肌肉蛋白损失以增加瘦体重)并且增加肌肉力量。对这种考虑的支持来自于对出现生物性衰老的HIV患者的研究,其中通过半胱氨酸和甘氨酸补充(所用剂量和持续时间与老年研究相同)对GSH缺乏的改善与禁食线粒体燃料氧化的恢复相关,在2周时间范围内总的体脂肪量减少3.5磅,瘦体重增加1.9磅,并且优势臂和非优势臂的肌肉力量显著增加。
实施例2
预防和治疗肌少症、肌少性肥胖、恶病质和肌肉消耗
肌少症是与衰老相关的骨骼肌质量、特质和力量的退行性损失。肌少症也可继发于废用和零重力或失重。恶病质是一种复杂的代谢消耗综合征,其特征在于体重下降、肌肉萎缩无力和疲劳,并且伴有一系列慢性病,包括癌症、HIVAIDS、COPD、退行性神经疾病诸如多发性硬化、充血性心脏病、肺结核和肾病。肌少症可与脂肪量增加相关(即,肌少性肥胖),并且恶病质可与脂肪量损失相关或不相关。
这些病症代表通过以下方式预防和治疗的主要目标:提供半胱氨酸加甘氨酸以在哺乳动物中升高细胞内GSH并改善肌肉健康。通过施用其前体半胱氨酸和甘氨酸对GSH的改善与可以在被HIV感染的老年患者中改善线粒体燃料氧化的生理模式,降低总的体脂肪、腰围和胰岛素抗性以及提高去脂体重和肌肉力量相关,从而表明该方法可预防和逆转肌少症、肌少性肥胖和恶病质。
实施例3
药物毒性和其他毒性的预防和治疗
多种药物会诱导线粒体毒性和/或肝毒性,包括例如对乙酰氨基酚和抗逆转录病毒药物。某些引起线粒体毒性的药物包括至少抗惊厥药、精神药物(抗抑郁药、安定药、巴比妥类药物和焦虑药物)、胆固醇药物、镇痛/抗炎药物、抗生素、抗心律失常药物、类固醇、抗病毒药物、抗逆转录病毒药物、癌症药物、糖尿病药物、β-阻滞剂和免疫接种。具体药物包括丙戊酸钠、阿米替林、阿莫沙平、氟西汀、西酞普兰、氯丙嗪、氟非那嗪、氟哌啶醇、利培酮、苯巴比妥、司可巴比妥、布他比妥、异戊巴比妥、戊巴比妥、阿普唑仑、安定、他汀类药物、胆汁酸-消胆胺、环丙贝特、非诺贝特、阿司匹林、对乙酰氨基酚、吲哚美辛、萘普生、双氯芬酸、四环素、米诺环素、氯霉素、替诺福韦、地瑞那韦、利巴韦林、特拉匹韦、氨基糖甙类、利奈唑胺、胺碘酮、干扰素、齐多夫定、多柔比星、顺铂、它莫昔芬和二甲双胍。
在特定实施方案中,向个体提供NAC和/或甘氨酸以预防、治疗或减少线粒体毒性和/或肝毒性的有害效应。在具体实施方案中,使用本公开的方法处理与氧化应激和/或GSH缺乏相关的其他毒性。
在具体实施方案中,提供了预防和治疗对乙酰氨基酚毒性的方法,诸如在肝毒性的情况下。在药物开发过程中以及对于许多已开发完善的药物的使用而言,肝毒性是一个严重的问题。例如,对乙酰氨基酚过量是目前美国急性肝衰竭最常见的原因。肝线粒体直接地或通过形成反应性代谢物间接地成为药物毒性的关键靶标。对乙酰氨基酚(醋氨酚、N-乙酰基-对氨基苯酚;APAP)是一种广泛使用的非处方镇痛药和解热药。它也常与氢可酮、丙氧芬、可待因和羟考酮结合用于一些处方麻醉药中。在治疗剂量下,对乙酰氨基酚具有类似于阿司匹林和布洛芬的镇痛和解热效应,但它的治疗窗口非常狭窄。即使在推荐范围内的剂量下,对乙酰氨基酚也是急性肝衰竭的主要原因。每年有数以万计的中毒控制中心呼叫和入院事件,以及数百例死亡。饮酒和空腹(由于疾病、厌食或营养不良)都大大增加了由于对乙酰氨基酚而造成肝损伤的风险。
诸如高龄、饮酒和空腹(例如由于疾病、厌食或营养不良)的条件,甚至对乙酰氨基酚本身的代谢物通过降低谷胱甘肽的水平大大增加了肝损伤的风险,其中谷胱甘肽是帮助肝脏分解对乙酰氨基酚的抗氧化剂。即使在标准剂量下,对乙酰氨基酚在人体内的代谢也会释放出少量的有毒物质N-乙酰基-苯醌亚胺(或NAPQI)。在过量剂量下,将形成更大量的这种毒素。对乙酰氨基酚的安全剂量与危险剂量之间相差很小,这意味着剂量略高于最大推荐剂量(4克/天)就可能会导致肝损伤。
在肝脏中利用最佳的细胞内谷胱甘肽浓度是针对对乙酰氨基酚毒性的合理预防和治疗方法。N-乙酰半胱氨酸施用通过其维持肝脏谷胱甘肽贮库的能力,已被用作针对由对乙酰氨基酚过量所触发的肝毒性的主要治疗方法。在具体实施方案中,即使以规定水平,在施用对乙酰氨基酚之前、同时和/或之后使用NAC/甘氨酸提高肝GSH水平能减轻对乙酰氨基酚的毒性效应。
实施例4
改善体能表现
本公开的方法和/或组合物可提供给个体以用于改善体能表现,通过增强运动的效果、从剧烈运动中恢复或者逆转由非疾病病症引起的瘦肌肉质量损失来防止肌肉质量的损失,所述非疾病病症会加速其他年轻的强健体魄的个体(诸如,宇航员(零重力)、马拉松选手、消防队员、优秀运动员等等)的衰老和肌肉损失。此外,耐力活动尤其会增加氧化应激,这在可能已经缺乏细胞内GSH的年长运动员中特别受到关注。因此,在具体实施方式中,本公开的方法预防和/或治疗运动的氧化应激。

使用半胱氨酸(以n-乙酰半胱氨酸的形式)和甘氨酸补充老龄小鼠的饲料足以提升抗氧化剂谷胱甘肽的水平。这些老龄小鼠体内的谷胱甘肽恢复导致线粒体燃料氧化的显著改善。因为这些有益的变化对于影响寿命的长短是有用的,所以测试小鼠饲料中补充半胱氨酸(以n-乙酰半胱氨酸的形式)和甘氨酸是否能延长它们的寿命。研究如下进行:将60周龄的小鼠分为2组(每组7只小鼠,均为2只雌鼠和5只雄鼠)进行研究,并且两组的性别、年龄和体重匹配。允许一组随意进食普通饲料,并给第二组饲喂含有额外的半胱氨酸(以n-乙酰半胱氨酸的形式)和甘氨酸的饮食。然而,两种饮食的饲料含量是匹配的,使得它们具有相同量的卡路里且在每克饲料中具有相同的蛋白氮,即两种饮食都是等热量和等氮的。饲料重量的监测显示出两组饲料食用量相似。允许动物自由获取它们各自的饮食和水,并且将寿命长短记录为主要结果测量标准。结果显示,接受半胱氨酸和甘氨酸补充饮食的小鼠的寿命平均延长了34周,这表示寿命增加了35%(图6)。
实施例6
老年人HIV
据报道,感染HIV且年龄超过50岁的患者的机能衰退速度加快,且与老年非HIV患者相比,肌肉质量减少、肌力下降、机能受限,但是这些缺陷的潜在机制尚未充分了解,并且缺乏有效的治疗手段。认识到这一点,疾病控制中心提出,HIV患者的“老年阶段”从50岁开始。
在本公开的具体实施方案中,老年HIV感染患者的机能衰退与线粒体功能受损相关。线粒体依靠抗氧化剂来防御破坏性的活性氧和氧化应激。谷胱甘肽(GSH)是最丰富的内源性细胞内抗氧化剂以及线粒体抗氧化剂防御的关键组分,已知其在HIV患者体内是不足的。对于导致老年HIV患者体内缺乏GSH的机制,发生这种情况的原因是由于其两个前体氨基酸半胱氨酸和甘氨酸的缺乏导致GSH合成严重减少。两周口服膳食补充半胱氨酸和甘氨酸纠正了这些氨基酸的缺乏,增加了GSH合成,提高了细胞内GSH浓度,并降低了ROS水平和氧化性损伤。在生理条件下,空腹状态下选择的燃料是脂肪酸(FA),而不是葡萄糖。缺乏GSH的老年HIV患者具有严重受损的空腹FA氧化和较高的空腹葡萄糖氧化,表明存在线粒体缺陷。GSH浓度的提高使得空腹线粒体FA氧化增加且葡萄糖氧化减少这一现象快速增加。这些变化与非脂肪质量和肌肉力量显著增加有关。有趣的是,当GSH水平升高时,这些患者的肌肉力量显著增加,而其在GSH缺乏状态下的肌肉力量与80岁的非HIV人类相当,随着GSH的增加,肌肉力量增加到与70岁人类相当。实际上,这些老年HIV患者通过改善GSH在2周的时间内“年轻”了10岁。
可调查GSH缺乏是否会导致老年HIV患者的肌肉质量、力量损失、机能受限且生活质量下降,并测试通过补充半胱氨酸和甘氨酸来纠正GSH缺乏是否会逆转这些缺陷。例如,可对年龄在50-60岁的10名老年HIV患者和10名非HIV对照(年龄、性别和BMI匹配)进行开放标签研究。根据公布的数据,需要8名受试者的样本量,并且可研究10名受试者以考虑20%的流失率。所有受试者都可进行基线研究,仅在接受半胱氨酸加甘氨酸治疗12周后才能再次研究HIV受试者。可测试与非HIV对照相比,老年HIV患者的GSH缺乏是否与空腹线粒体燃料氧化受损和肌肉蛋白质损失相关,以及补充半胱氨酸加甘氨酸是否能够逆转这些缺陷。
在老年HIV患者的具体实施方案中,GSH缺乏导致空腹线粒体燃料氧化缺陷、葡萄糖氧化升高和肌肉蛋白质损失,GSH恢复可逆转这些缺陷。尽管不受理论的限制,但GSH缺乏导致空腹线粒体NEFA氧化受损,迫使因能量需求而转变为葡萄糖氧化。由于空腹状态下的葡萄糖主要由肌肉蛋白质的糖异生提供,因此导致肌肉损失,并且半胱氨酸和甘氨酸缺乏(图1)。补充半胱氨酸加甘氨酸以纠正GSH缺陷将恢复空腹线粒体FA氧化并减少葡萄糖氧化,从而减少肌肉蛋白质向糖异生的损失,并增加肌肉质量。出于这样的考虑,可测量肌肉GSH、半胱氨酸和甘氨酸水平(HPLC)、空腹NEFA和葡萄糖氧化(量热法)、肌肉蛋白质损失(稳定同位素研究)、肌肉质量(DEXA,全身钾和氮扫描)。
可测试与非HIV对照相比,老年HIV患者的GSH缺乏是否与肌肉质量、肌肉力量和机能降低相关,以及GSH恢复是否会使肌肉力量和机能恢复到与匹配的非HIV对照相当。在具体实施方案中,老年HIV患者的GSH缺乏导致肌肉力量和机能的损失,并且GSH恢复可使力量和机能改善到与匹配的非HIV组相当。出于这样的考虑,可测量力量(诸如通过测力法进行前臂抓握)和机能(诸如,步行6分钟)。
老年HIV患者具有受损的线粒体氧化和肌肉蛋白质损失,但潜在机制未知。由于预期>50%的HIV患者在2015年进入老年阶段(>50岁),这些缺陷的并发症将显著增加人体负担和医疗成本。本公开预防和逆转老年HIV患者的肌肉损失、增加肌肉力量、改善身体机能和生活质量,并减少越来越多的老年HIV患者的医疗成本。在具体实施方案中,GSH缺乏是老年HIV患者肌肉损失的一个新的且重要的危险因素,并且可提供基于半胱氨酸加甘氨酸补充的疗法以纠正GSH缺陷并逆转肌肉损失。在具体实施方案中,可增加肌肉质量和力量、运动能力并改善生活质量。本公开的实施方案提供了使用半胱氨酸加甘氨酸纠正老年HIV患者GSH缺陷的新型、简单、安全、有效且经济的营养策略。
在具体实施方案中,在携带HIV的老年患者中,GSH缺乏导致空腹线粒体燃料氧化受损,肌肉质量、力量损失且机能受限,并促使机能衰退加速。可使用创新的基于稳定同位素示踪剂的方案、量热法、DXA、全身钾和氮扫描、测力法和机能测试在全身(NEFA和葡萄糖氧化以及肌肉损失)和组织(肌肉GSH和蛋白质损失)水平测量结果。结果表明,由于半胱氨酸和甘氨酸补充而获得有益效果。本公开的实施方案提供了使用半胱氨酸加甘氨酸纠正老年HIV患者的线粒体燃料氧化缺陷、肌肉蛋白质、肌肉质量和力量的损失并改善生活质量的新型、简单、安全、有效且经济的营养策略。
HIV和GSH缺乏:测量青年(30-40岁;n=10)和老年(50-60岁;n=20)HIV患者的RBC-GSH水平,并发现所有患者的GSH水平都较低,但年龄与甚至更低的GSH浓度显著相关(P<0.0001)。进一步分析表明,55岁的HIV患者的GSH水平与70岁的非HIV人类相当。
老年HIV患者的GSH动力学(图2、图3):在补充半胱氨酸加甘氨酸作为GSH前体之前和之后,对8名老年GSH缺陷型HIV患者(约55岁)研究GSH动力学。与历史GSH充足的非HIV对照(n=8)相比,结果表明,老年HIV患者中严重的细胞内半胱氨酸和甘氨酸缺乏通过补充得到了改善。如图所示,补充前的HIV受试者具有低58%的GSH-FSR水平和低57%的GSH水平(与对照相比)。补充后,GSH-FSR(其中FSR是分级合成速率)水平和GSH水平分别增加120%和53%。
老年HIV受试者的空腹燃料氧化(图4):在禁食16小时后,与非HIV对照相比,缺乏GSH的老年HIV受试者具有显著更低的NEFA氧化和更高的碳水化合物(CARB)氧化。恢复GSH合成使得NEFA氧化增加46%,且碳水化合物氧化减少49%。(*=p<0.05;Φ=p<0.01)。
GSH提高增加了非脂肪质量和肌肉力量:GSH的提高使得非脂肪质量(p=0.003)和前臂肌肉力量(p<0.01)显著增加了0.9kg。
因此,老年HIV患者由于合成量减少(因其前体半胱氨酸和甘氨酸的可用性降低引起)而导致GSH缺乏,并且与线粒体燃料氧化受损、肌肉质量和力量的丧失有关。补充半胱氨酸和甘氨酸2周提高了GSH水平。在具体实施方案中,更长的12周的补充持续时间使得GSH浓度完全恢复,并且逆转了老年HIV患者的肌肉损失和机能衰退。
实施例7
C反应蛋白
C反应蛋白(CRP)是存在于血浆中的急性期蛋白,并由肝脏合成。CRP水平响应于炎症而升高,因此它被认为是与炎症增加相关的病症的生物标志物。CRP也被认为是心血管疾病的生物标志物,其水平>3μg/ml时,被认为是不可取的,其水平<1μg/ml时是最佳的。CRP升高也与糖尿病、HIV和衰老有关。降低CRP水平的干预措施有限。在一类被称为他汀类药物的药剂中的强效降胆固醇药物可降低CRP水平。
缺乏谷胱甘肽的老年HIV患者具有高水平的CRP,并且当通过口服膳食补充半胱氨酸(以n-乙酰半胱氨酸的形式)和甘氨酸提高谷胱甘肽水平时,CRP水平显著下降(p<0.05)(图5)。
实施例8
药物摄取后线粒体缺陷的改善
某些药物会引起毒性,因为其作用机制导致线粒体功能障碍或损伤。在某些实施方案中,提供了通过向个体提供有效量的包含甘氨酸或其功能性衍生物和N-乙酰半胱氨酸或其功能性衍生物的组合物来中和或减轻药物诱导的线粒体功能障碍或损伤的方法。由药物引起的毒性可以是任何导致线粒体功能障碍或损伤的类型,但在具体实施方案中,药物是抗病毒药物,诸如HIV药物、肝炎药物等等。在某些实施方案中,药物毒性也可在治疗之前或之后由于GSH消耗而引起或加剧。
引起线粒体毒性的药物包括至少抗惊厥药;精神药物(抗抑郁药;安定药;巴比妥类药物;焦虑药物);胆固醇药物;镇痛/抗炎药物;抗生素;抗心律失常药物;类固醇;抗病毒药物;抗逆转录病毒药物;癌症药物;糖尿病药物;β-阻滞剂;和免疫接种。
在特定情况下,该药物是丙戊酸钠(双丙戊酸钠);阿米替林(盐酸阿米替林);阿莫沙平;氟西汀(百忧解);西酞普兰(喜普妙);氯丙嗪(托拉嗪);氟非那嗪(氟奋乃静);氟哌啶醇(氟哌丁苯);利培酮(理思必妥);苯巴比妥;司可巴比妥(速可眠);布他比妥(异丁巴比妥);异戊巴比妥(阿米妥);戊巴比妥(耐波他);阿普唑仑(赞安诺);安定(烦宁、地西泮);他汀类药物;胆汁酸-消胆胺;环丙贝特;ASA(阿司匹林);对乙酰氨基酚(泰诺);吲哚美辛(消炎痛);萘普生(萘普生钠);双氯芬酸;四环素、米诺环素;氯霉素;氨基糖甙类;利奈唑胺(利奈唑烷);胺碘酮;干扰素;齐多夫定;多柔比星(阿霉素);顺铂;它莫昔芬;二甲双胍;cystuc;或其混合物。
在个体由于线粒体损伤或GSH减少而需要服用已知或怀疑具有药物毒性的药物的情况下,还可向个体提供包含甘氨酸或其功能性衍生物和N-乙酰半胱氨酸或其功能性衍生物的组合物。在具体情况下,在给予个体甘氨酸或其功能性衍生物和N-乙酰半胱氨酸或其功能性衍生物同时和/或之前和/或之后,给予个体具有线粒体毒性的药物。
实施例9
增加GSH浓度的生理有益效果
1.HIV和TNFα:8名携带HIV的患者在补充半胱氨酸和甘氨酸以增加GSH浓度之前和2周后测量TNF-α的血浆浓度。数据显示,TNF-α从34.6±7.5降低到27.8±4.7(p=0.00049)。
2.神经认知数据:3名HIV患者在补充半胱氨酸和甘氨酸以增加GSH浓度之前和12周后进行神经认知评估。数据显示,神经认知功能的改善如下所示:

3.心脏舒张功能障碍的改善:
将雄性小鼠(30-35个月大)分成2组进行研究,一组饲喂普通饲料饮食(对照组-CON),并且另一组的饲料中补充半胱氨酸加甘氨酸(NacGly)。比较饮食之前和7周后的主动脉流出、二尖瓣血流、主动脉僵硬度以及左心室和左心房的解剖结构和功能的超声心动测量的无创测量值(每组n=4)。与未改变的对照相比,NacGly小鼠表现出二尖瓣血流参数的显著改善。NacGly小鼠也显著改善了等容舒张时间(Con 23.1+2.5对NacGly 19.2+0.7msec,p<0.05)、等容收缩时间(Con 26.3+4.6对NacGly 13.9+0.3msec,p<0.05)、早期充盈速率峰值(Con 67+4对NacGly 78+5cm/sec,p<0.05)。本研究的结论是,膳食补充半胱氨酸(以n-乙酰半胱氨酸的形式)和甘氨酸改善老年小鼠的舒张功能。

5.糖尿病小鼠的肝脂肪:研究暴露于严重不受控的糖尿病1年后的两组小鼠。从诱导糖尿病时开始,一组(治疗组)接受半胱氨酸(以n-乙酰半胱氨酸的形式)和甘氨酸的补充,而另一(对照)组接受与第一组等氮和等热量的对照饲料。组织学评估显示,接受等氮/等热量饮食的对照组的脂肪肝患病率为95%-100%,而食用半胱氨酸/甘氨酸饮食的治疗组的发病率仅有2%-5%的脂肪肝。在经过治疗的小鼠中,肝脂肪的量化结果显示出显著降低的量。
5.肌肉蛋白质分解:为3名老年HIV患者补充半胱氨酸(以n-乙酰半胱氨酸的形式)和甘氨酸之前和12周后,使用示踪剂3-甲基组氨酸研究肌肉蛋白质分解。结果显示,肌原纤维蛋白质分解明显减少。这些数据表明,使用半胱氨酸和甘氨酸改善老年患者的谷胱甘肽可减少肌肉分解并预防肌少症。

在小鼠研究中,n-乙酰半胱氨酸和甘氨酸的作用改善了老年小鼠的线粒体功能和肌肉力量,并且在具体实施方案中,这通过谷胱甘肽得以实现。在一些实施方案中,补充n-乙酰半胱氨酸和甘氨酸减少了小鼠的肝脂肪。
在特定方面,为HIV患者补充n-乙酰半胱氨酸和甘氨酸改善了感染HIV的患者的轻度神经认知缺陷,改善了肌肉力量和运动能力;并且/或者使谷胱甘肽恢复到与年龄匹配的对照相当。
在某些实施方案中,在对老年人进行的研究中,补充n-乙酰半胱氨酸和甘氨酸在至少4周内改善了认知缺陷。
实施例11
HIV身体和神经认知数据
据报道,感染HIV的患者的身体机能衰退加速了衰老。还有报道称,HIV患者的认知功能严重损伤。为评估半胱氨酸和甘氨酸补充的影响,我们研究了在补充半胱氨酸(以n-乙酰半胱氨酸的形式)和甘氨酸之前和12周后的8名HIV患者,并且比较对照组是8名年龄、性别和BMI匹配的HIV阴性人群。结果测量包括身体机能(步行速度)和神经认知功能(连线测试和MAEIII)。
结果显示,与非HIV对照组相比,感染HIV的患者的步行速度显著较低(1.3±0.1对1.06±0.04m/s,p<0.001),并且通过连线测试A(34.6±3.6对62.6±6.1秒,p<0.01)和连线测试B(53.8±7.2对117.5±5.0秒,p<0.01)以及多语言失语检查III(41.0±3.5对28.9±3.2字,p<0.01)测得,严重的认知损伤。补充12周后,与补充前的水平相比,HIV患者的步行速度已经恢复(1.06±0.04对1.30±0.04m/s,p<0.01),该水平与HIV阴性对照相似和相当,表明补充半胱氨酸和甘氨酸可以逆转HIV患者的加速衰老。这进一步通过连线测试A(62.6±6.1对46.4±4.4秒,p<0.01)和B(117.5±5.0对69.8±5.4秒,p<0.01)以及MAEIII(28.9±3.2对34.6±2.0字,p<0.01)的得分的提高所看出的认知功能显著改善(补充前与补充后水平)得到支持。
结论:补充半胱氨酸和甘氨酸可逆转HIV患者的谷胱甘肽缺乏,并逆转机能衰退和认知功能。总的来说,这些数据支持半胱氨酸和甘氨酸补充逆转感染HIV的患者加速衰老的观点。
