Abstract

技术领域
本发明涉及分子生物检测技术领域,涉及一种检测肾细胞癌甲基化生物标志物的引物的应用,特别是涉及在评估肾细胞癌患者对免疫检查点阻断治疗的反应的应用,还涉及评估肾细胞癌患者对免疫检查点阻断治疗的反应的检测工具及其使用方法。
背景技术
近年来,越来越多的研究表明,异常的DNA甲基化参与了各种疾病和肿瘤的发生和发展。据报道,肿瘤细胞中甲基化水平在抗肿瘤免疫和免疫治疗效果中起着至关重要的作用。目前,越来越多的研究关注DNA甲基化对肾细胞癌患者的免疫治疗效果的影响。
肾细胞癌(Renal Cell Carcinoma,简称RCC)是最常见的肾脏恶性肿瘤。肾细胞癌的发病率占所有癌症的3%,尤其是在西方国家。近年来,免疫治疗取得了长足的进步,研究表明肾细胞癌具有高免疫原性,对免疫治疗敏感。然而,仍有部分肾细胞癌患者在初诊时已发生转移,免疫治疗效果有限,此类患者的生存率低于其他患者。因此,为提高对肾细胞癌患者对免疫治疗效果评估的准确性,寻找新的DNA上的生物标志物对评估肾细胞癌患者对免疫检查点阻断治疗的反应尤为关键。
发明内容
基于此,本发明的目的在于,提供一种检测肾细胞癌甲基化生物标志物的引物的应用,通过使用该引物检测肾细胞癌中甲基化生物标志物的甲基化水平,这对于评估肾细胞癌患者对免疫检查点阻断治疗的反应具有重要意义。
为实现上述发明目的,一方面,本发明提供了检测肾细胞癌甲基化的生物标志物,所述生物标志物生物标志物位于IL18基因启动子上,具体为cg09122223、cg11304234或cg26534425中的任一种。
具体地,IL18基因在GenBank中的登记号为3606,在染色体的位置:11号染色体长臂23.1(11q23.1),由5个内含子和6个外显子组成,其编码区长582bp,编码194个核苷酸。IL18基因启动子区序列全长2000bp(在染色体的位置:chromosome11:112141260-112143259)。
进一步地,所述IL18基因的转录起始点在112143482,所述cg09122223位于IL18基因启动子上游20969bp处;所述cg11304234位于IL18基因启动子上游20595bp处;所述cg26534425位于IL18基因启动子上游20719bp处。
另一方面,本发明提供了一种检测上述生物标志物的引物的应用,用于制备评估肾细胞癌患者对免疫检查点阻断治疗的反应的检测工具。其中,所述引物为任意可用于检测上述生物标志物甲基化水平的引物。
再一方面,本发明还提供了一种评估肾细胞癌患者对免疫检查点阻断治疗的反应的检测工具,包括引物,所述引物用于检测肾细胞癌甲基化的生物标志物。
进一步地,所述生物标志物为位于所述IL18基因启动子上的cg09122223、cg11304234或cg26534425中的任一种。
进一步地,所述评估肾细胞癌患者对免疫检查点阻断治疗的反应的检测工具为单独试剂或试剂盒。
还有一方面,本发明还提供了上述检测工具的使用方法,利用所述检测工具,通过焦磷酸测序检测样本的上述肾细胞癌甲基化生物标志物的甲基化水平,并基于所述生物标志物的甲基化水平评估肾细胞癌患者对免疫检查点阻断治疗的反应。
与现有技术相比,本发明的有益效果在于:
(1)本发明分析了IL18基因在肾细胞癌中的表达情况,并首次发现了IL18基因的表达水平、IL18启动子甲基化水平与肾细胞癌中免疫细胞浸润密切相关,具有临床诊断和指导意义;
(2)本发明还首次发现了IL18基因的表达水平与启动子甲基化位点cg09122223、cg11304234或cg26534425可作为一种新的针对肾细胞癌的生物标志物,对评估肾细胞癌患者对免疫检查点阻断治疗的反应有重要的价值。

下面结合具体实施例,对本发明作进一步详细的阐述,下述实施例仅用于说明本发明,不用于限制本发明。以下实施例中所使用的实验方法如无特殊说明,实施例中未注明具体条件的实验方法,通常按照常规条件,下述实施例中所使用的材料、试剂等,如无特殊说明,均可从商业途径得到。
IL18是一种内毒素诱导的血清因子。它是IL1家族的成员之一,由24Kda前体产生,然后被caspase-1切割成成熟形式。人体中的大多数正常细胞,尤其是巨噬细胞和树突状细胞,都可以产生IL18。有趣的是,在不同的细胞环境中,IL18可以是抗肿瘤因子,也可以是致癌因子。最近已有越来越多的研究集中在IL18对癌症的影响上。迄今为止,关于IL18在肿瘤中作用的研究主要集中在IL18通过调节相关信号通路促进或已知肿瘤发展或免疫浸润。然而,IL18基因启动子甲基化对肿瘤尤其是肾细胞癌的发生、发展以及免疫浸润的影响仍然不清楚。本发明人通过研究肾细胞癌IL18基因以及IL18基因启动子甲基化状态对肾细胞癌患者对免疫检查点阻断治疗的反应的影响,在此基础上完成了本发明。
如本文所用,术语“IL18基因”是指表达IL18血清因子的脱氧核苷酸序列。本领域的普通技术人员可以使用常规方法获得IL18基因的序列,如从NCBI获得。
如本文所用,术语“启动子甲基化”是指甲基与DNA的特定核苷酸的生物化学偶联或化学偶联。在本发明的上下文中,“启动子甲基化”指的是位于CpG-二核苷酸区段中的一个胞嘧啶(5-甲基胞嘧啶)的第五碳原子存在甲基基团。
启动子甲基化的检测方法属于现有技术,这些方法在本领域相关文献是已知的。本发明具体采用焦磷酸测序对样本进行检测,所述的焦磷酸测序包括以下步骤:
S1:提取DNA样本;
S2:对步骤S1提取的DNA进行转化、纯化;
S3:以步骤S2转化、纯化得到的DNA样本为模板,利用引物进行PCR扩增;
S4:对PCR产物,采用焦磷酸测序对样本进行测序分析,得到样本的甲基化水平。
肿瘤微环境(TME)
肿瘤微环境即肿瘤细胞生长的局部内环境,是肿瘤细胞赖以生存和发展的复杂环境。这一内环境中除肿瘤细胞外,还包括各种免疫和炎症细胞因子、细胞间质、微血管及其他生物分子等。免疫抑制微环境是肿瘤微环境中起抑制免疫功能的部分,其组成包括免疫抑制性细胞和抑制性细胞因子。
如本文所用,“免疫细胞浸润”与“肿瘤的免疫浸润”可以互换使用,是指由肿瘤细胞与免疫细胞之间相互作用所形成免疫微环境,肿瘤的内部和周围往往聚集着大量的免疫细胞,这些免疫细胞与肿瘤细胞存在复杂相互作用和调节。
如本文所用,术语“免疫检查点阻断治疗”一种治疗肿瘤的方法,其基本原理是基于免疫细胞T细胞的激活机制。程序性死亡受体表达在T细胞表面,其配体表达在肿瘤细胞和髓源性抑制细胞表面。程序性死亡受体与其配体结合可使T细胞衰竭而无法正常杀伤肿瘤细胞,肿瘤细胞便可逃脱宿主的免疫监视。因此,程序性死亡受体及其配体被称为“免疫检查点”。基于程序性死亡受体及其配体的免疫检查点阻断治疗通过使用如免疫检查点抑制剂等物质,来抑制二者的结合,从而提高宿主免疫系统对肿瘤细胞的攻击性。
实施例1
本实施例研究IL18基因在肾细胞癌患者中的表达情况。
本发明人基于TCGA(癌症基因组图谱数据门户)数据库和CPTAC(美国国家癌症研究所的临床蛋白质组学肿瘤分析联盟)分析IL18基因在肾细胞癌肿瘤组织和配对的正常相邻组织中的表达,结果发现,与正常组织相比,IL18基因在肾细胞癌肿瘤组织中高度表达。其中,图1A示出TCGA数据库中肾细胞癌肿瘤组织和邻近正常组织之间IL18基因的mRNA的相对表达。图1B示出CPTAC数据库肾细胞癌肿瘤组织和邻近正常组织之间IL18基因的mRNA的相对表达。同时,本发明还发现IL18基因的表达在不同病理分级和GRADE分级的肾细胞中存在差异表达,且IL18基因的表达与病理分级呈正相关。其中,图1C和图1E示出TCGA数据库中肾细胞癌患者不同TNM分期中IL18基因的表达情况。图1D和1F示出TCGA数据库中肾细胞癌患者不同病理分级中IL18基因的表达情况。

本发明人通过TCGA数据库下载以TPM表示的RNA序列数据,使用R软件包(edgeR)和R版本4.0.3软件对相关数据进行归一化和处理,分析从Infinium人甲基化450珠芯片珠获得的差异甲基化位点对其中317例肿瘤组织进行甲基化和转录组分析,并将其与正常组织IL18基因启动子上甲基化位点进行甲基化程度差异分析比对,鉴定得到cg04100971、cg05687149、cg04929355、cg09122223、cg11304234和cg26534425是甲基化差异程度显著的位点。为了验证肿瘤组织中的IL18启动子甲基化,除TCGA数据库外,本发明还分析另外两个GEO数据集(GSE70303和GSE105260)中IL18基因启动子中的cg04100971、cg05687149、cg04929355、cg09122223、cg11304234和cg26534425这6个甲基化位点。其中,在不同数据库中取样数量以及分析方法如下方表1所示:

结果发现,与正常组织相比,CpG位点cg04100971和cg05687149在肿瘤中显着高甲基化;而cg04929355、cg09122223、cg11304234和cg26534425与正常组织相比是低甲基化的。其中,图2A示出TCGA数据库中肿瘤组织和正常组织在CpG位点cg04100971、cg04929355、cg05687149、cg09122223、cg11304234和cg26534425的甲基化差异;图2B示出GSE70303数据库中肿瘤组织和正常组织在CpG位点cg04100971、cg04929355、cg05687149、cg09122223、cg11304234和cg26534425的甲基化差异;图2C示出GSE105260数据库中肿瘤组织和正常组织在CpG位点cg04100971、cg04929355、cg05687149、cg09122223、cg11304234和cg26534425的甲基化差异。
同时,基于TCGA数据库中肾细胞癌患者的数据分析还发现,IL18基因启动子的甲基化水平在肾细胞癌患者的不同TNM分期和病理分级中存在差异。图3A-3F示出TCGA数据库中CpG位点cg04100971、cg04929355、cg05687149、cg09122223、cg11304234和cg26534425的甲基化水平与肾细胞癌患者的TNM分期的关系。图3G-3L示出TCGA数据库中CpG位点cg04100971、cg04929355、cg05687149、cg09122223、cg11304234和cg26534425的甲基化水平与肾细胞癌患者的病理分级的关系。由图可见,TNM分期越晚、病理分级等级越高,cg04100971和cg05687149的甲基化水平越高,但cg04929355、cg09122223、cg11304234和cg26534425的甲基化水平越低。
这些结果表明:IL18基因启动子在肾细胞癌患者中异常甲基化,而IL18基因启动子的甲基化水平也与肾细胞癌的TNM分期、病理分级密切相关。
实施例3
本实施例研究IL18基因启动子甲基化水平与肾细胞癌中的IL18基因表达的关系。
本发明人通过分析TCGA数据库中IL18基因的表达与IL18基因启动子中6个CpG位点cg04100971、cg04929355、cg05687149、cg09122223、cg11304234和cg26534425的甲基化水平的关系,发现除了cg05687149位点外,其他5个位点与IL18基因的表达有关。其中,图4A-4F示出TCGA数据库中肾细胞癌肿瘤组织中的CpG位点cg04100971、cg04929355、cg05687149、cg09122223、cg11304234和cg26534425的甲基化水平与IL18基因表达的相关性。由图4A-4F可见,发现除了cg05687149位点外,其他五个甲基化位点也与IL18表达有关。其中,cg04100971的甲基化与IL18基因表达呈正相关,表明cg04100971甲基化水平越高,IL18基因表达程度越高;cg04929355、cg09122223、cg11304234和cg26534425的甲基化与IL18基因呈负相关,表明cg04929355、cg09122223、cg11304234和cg26534425的甲基化水平越低,IL18基因表达程度越高。
这些结果表明:IL18基因启动子的异常甲基化可能导致肾细胞癌中IL18基因的异常高表达。
实施例4
本实施例研究IL18基因的表达、IL18基因启动子甲基化水平与肾细胞癌中的免疫细胞浸润相关性。
本发明人通过分析了包括B细胞以及CD4和CD8 T细胞在内的23种免疫细胞系中IL18基因的表达与IL18基因启动子甲基化的相关性,发现IL18基因的高表达与更高程度的免疫浸润。与此同时,本发明人还发现IL18基因启动子的甲基化水平与肿瘤细胞的免疫浸润相关。而在CpG位点cg04100971、cg04929355、cg05687149、cg09122223、cg11304234和cg26534425中,cg04100971的高甲基化以及cg0912223的低甲基化与肿瘤细胞的免疫浸润显著相关(请参阅图5A)。其中,cg04100971甲基化水平越高,肿瘤细胞的免疫浸润程度越高;而cg0912223甲基化水平越低,肿瘤细胞的免疫浸润程度越高。图5A示出TCGA数据库中肾细胞癌患者中IL18基因表达及其启动子差异甲基化位点与23种免疫细胞的相关热图。
为进一步分析关键的免疫相关通路,本发明选择了17条生物学通路,分别是Angiogenesis,Antigen processing machinery,CD8T effecter,Co-inhibition APC,Co-inhibition T cell,Co-stimulation APC,Co-stimulation T cell,CytolyticActivity,EMT1,EMT2,EMT3,Immune checkpoint,MHC-IHLA,MHC-HLA,Pan-F-TBRS,TypeIIFN Response以及Type IFN Reponse,并分析这17条生物学通路与IL18基因表达以及IL18基因启动子甲基化之间的相关性,其中,图5B-5H分别示出根据IL18基因表达中位数与在17条免疫相关通路中CpG位点cg11304234、cg26534425、cg05687149、cg04100971、cg04929355和cg09122223甲基化的中位数、高组以及低组的相对富集分数,结果发现:
(1)较低的IL18基因表达与较高的血管生成评分相关(请参阅图5B),表明较低的IL18基因表达时,血管生成越多,这意味着具有IL18基因低表达的肾细胞癌患者可能对抗血管生成治疗敏感。
(2)较低的IL18基因表达与较低的抗原加工水平、CD8T效应子、抗原溶细胞活性和MHC-HLA评分有关,这表明对于具有IL18基因低表达的肾细胞癌患者,其肾细胞癌肿瘤微环境中免疫细胞浸润程度较低。
(3)较高的IL18基因表达与较高的共抑制APC、共刺激APC、共抑制T细胞特征免疫检查点活性和EMT3显著相关(请参阅图5B)。这些这些结果表明,具有高IL18表达的肿瘤细胞的免疫浸润中丰富,使得肿瘤细胞与免疫细胞相互作用的微环境中含有更多免疫细胞,这意味着该免疫浸润含有高浓度的免疫检查点,具有更强的免疫检查点特征。
其中,图5B-5H示出根据IL18基因表达中位数与CpG位点cg04100971、cg04929355、cg05687149、cg09122223、cg11304234和cg26534425甲基化的中位数、高组以及低组的相对富集分数。
以上结果表明,IL18基因表达水平是肾细胞癌患者对免疫检查抑制剂治疗反应的潜在分子生物标志物,即可通过检测IL18基因表达水平评估肾细胞癌患者对免疫治疗的反应。
此外,本发明人还分析了IL18基因启动子甲基化水平与上述17种免疫相关途径的关系(请参阅图5C-5H)。图5C-5H分别示出CpG位点cg04100971、cg04929355、cg05687149、cg09122223、cg11304234和cg26534425与上述17种免疫相关途径的关系。其中,由于图5C和5D可见,位点cg11304234和cg26534425低甲基化与较低的血管生成相关,但较高地共抑制T细胞特征、免疫检查点活性、共刺激APC。这些结果表明,IL18基因的高表达、IL18基因启动子低甲基化与免疫细胞浸润呈正相关,免疫细胞浸润程度高使得免疫细胞浸润含有高度富集的免疫检查点,这意味着可能对使用免疫检查点抑制剂的免疫检查点阻断治疗产生敏感反应。
以上结果表明,IL18基因的表达水平、IL18基因启动子的甲基化水平是肾细胞癌患者对免疫检查抑制剂治疗反应的潜在分子生物标志物,这意味着可通过检测IL18基因启动子的甲基化水平评估肾细胞癌患者对免疫检查点阻断治疗的反应。
实施例5
本实施例研究IL18表达和启动子甲基化与肾细胞癌中关键免疫调节剂的表达的相关性。
本发明人通过使用TCGA数据库分析IL18基因表达、IL18基因启动子甲基化于74种关键免疫调节剂之间的关系,发现cg04100971和cg0912223的低甲基化与大多数表达调节剂的表达相关(请参阅图6A),其中表达调节剂包括ENTPD1,TLR4,SELP和HMGB1等。
基于免疫检查点抑制剂对各种肿瘤的不同治疗效果,本发明通过使用TCGA数据分析了IL18基因表达与CTLA4、LAG3和PDCD1这三个免疫检查点分子的联系,同时分析了cg04100971、cg05687149、cg04929355、cg09122223、cg11304234和cg26534425这六个位点与CTLA4、LAG3和PDCD1的联系,结果发现:IL18基因的表达与CTLA4、LAG3和PDCD1的表达呈显着正相关(请参阅图6B),而cg05687149、cg04929355、cg11304234和cg26534425甲基化与CTLA4、LAG3和PDCD1的表达呈负相关(请参阅图6C-6F)。
这些结果表明:IL18基因的表达、IL18基因启动子的甲基化与肾细胞癌中的免疫抑制微环境有关。
以上所述实施例仅表达了本发明的几种实施方式,其描述较为具体和详细,但并不能因此而理解为对发明专利范围的限制。应当指出的是,对于本领域的普通技术人员来说,在不脱离本发明构思的前提下,还可以做出若干变形和改进,则本发明也意图包含这些改动和变形。
Claims (8)
1.一种检测肾细胞癌甲基化生物标志物的引物的应用,其特征在于:用于制备评估肾细胞癌患者对免疫检查点阻断治疗的反应的检测工具。
2.根据权利要求1所述的应用,其特征在于:
所述生物标志物为位于所述IL18基因启动子上的cg09122223、cg11304234或cg26534425中的任一种;
通过所述引物分析所述生物标志物的甲基化水平,并基于所述生物标志物的甲基化水平评估肾细胞癌患者对免疫检查点阻断治疗的反应。
3.根据权利要求2所述的应用,其特征在于:
所述cg09122223位于IL18基因启动子上游20969bp处;
所述cg11304234位于IL18基因启动子上游20595bp处;
所述cg26534425位于IL18基因启动子上游20719bp处。
4.一种评估肾细胞癌患者对免疫检查点阻断治疗的反应的检测工具,其特征在于:包括引物,所述引物用于检测肾细胞癌甲基化的生物标志物。
5.根据权利要求4所述的检测工具,其特征在于:所述生物标志物为位于所述IL18基因启动子上的cg09122223、cg11304234或cg26534425中的任一种。
6.根据权利要求5所述的检测工具,其特征在于:
所述cg09122223位于IL18基因启动子上游20969bp处;
所述cg11304234位于IL18基因启动子上游20595bp处;
所述cg26534425位于IL18基因启动子上游20719bp处。
7.根据权利要求4所述的检测工具,其特征在于:所述检测工具为单独试剂或试剂盒。
8.一种基于权利要求4~7任一所述的检测工具的使用方法,其特征在于,利用所述检测工具,通过焦磷酸测序检测样本的肾细胞癌甲基化生物标志物的甲基化水平,并基于所述生物标志物的甲基化水平评估肾细胞癌患者对免疫检查点阻断治疗的反应。
