Description

Traditional Chinese medicine composition for treating thyroid diseases and preparation method thereof
Technical Field
The invention relates to the technical field of traditional Chinese medicines, in particular to a traditional Chinese medicine composition for treating thyroid diseases and a preparation method thereof.
Background
In recent years, clinical physical examination data show that the thyroid nodule detection rate of adults in China is higher, and the thyroid nodule detection rate becomes an environmental health problem which needs to be highly valued by the health administration department in China. Thyroid disorders are most commonly referred to as thyroid nodules, and any thyroid disorder can exist in the form of nodules, so thyroid nodules are involved in almost all thyroid disorders. Epidemiological studies have shown that in iodine-rich areas, 5% of women and 1% of men have reachable thyroid nodules; after high resolution ultrasound examination, 19% -67% of randomly selected populations had thyroid nodules. At present, the average prevalence of thyroid nodules of residents in China is 32.4%, and the average prevalence of thyroid nodules is a year-by-year growing trend.
At present, the incidence rate of thyroid diseases in China is high, the current situations of low awareness rate, low diagnosis rate and low treatment rate exist generally, and the overall treatment rate of the thyroid diseases is less than 5%. Thyroid nodule patients have no obvious clinical symptoms in early stages, and most patients are found on physical examination. Modern Western medicine has no special treatment means for benign thyroid nodule. If severe compression, malignant transformation tendency, tumor in special position and other conditions, can be treated by surgery, some other conservative treatment methods, but in domestic and foreign guidelines are not recommended to be used as conventional treatment means, which is also the main cause of low treatment rate of thyroid nodule diseases. Because there is no good treatment method and the early symptoms of the patient are not obvious, the treatment of the disease is ignored. It is therefore desirable to find a way to solve the above problems.
The traditional Chinese medicine has long history in preventing and treating thyroid nodule, can avoid adverse reactions such as hypothyroidism, local tissue damage, severe pain and the like caused by traumatic treatment such as surgery and the like, and is helpful for relieving psychological pressure of patients. The existing medicine for treating thyroid nodule contains very common Chinese medicinal materials such as herba Euphorbiae Humifusae, herba Senecionis Scandentis, radix Cicadae, herba Desmodii Styracifolii, and white snail shell, or toxic Chinese medicinal materials such as Mylabris and Catharsii Molossi. The used very common medicinal materials may have insufficient resources or have incomplete researches, and the like, are difficult to mass produce and have certain dangers; the use of toxic components increases the risk of adverse reactions, leading to allergic or toxic conditions.

Thyroid diseases mentioned in the present invention include various benign thyroid nodules and thyroiditis. For thyromegaly caused by light and moderate benign thyroid nodule and thyroiditis, including people with confirmed physical examination of thyroid nodule, asymptomatic or unobvious symptoms, or people who refuse oral medication or are hesitant to treatment, the external preparation provided by the invention can be independently and locally applied. The acupoint application can be used for biological regulation of skin and acupoints, promoting topical microcirculation improvement, and promoting nodule dissipation and absorption. On the basis of using the oral preparation provided by the invention, the external preparation provided by the invention is applied locally to the severe or malignant thyroid nodule and thyroiditis complicated with thyroid dysfunction, so that the external preparation can directly reach the focus, the local symptoms and the nodule can be quickened to subside, the clinical curative effect is enhanced, and the effective rate of disease treatment is improved.
Thyroid diseases correspond to "goiter" in traditional Chinese medicine. Goiter is a general term for lump diseases occurring on both sides of the anterior cervical junction, and is clinically characterized in that both sides of the anterior cervical junction are diffusely swollen or agglomerated, and can move up and down along with swallowing actions. The traditional Chinese medicine considers that qi, fire, phlegm and blood stasis are important pathological factors of thyroid nodule and penetrate through the whole disease process. The applicant uses the general regulation method to treat liver, spleen and kidney dysfunction and qi fire phlegm stasis, combines two treatment means of oral administration and external application, treats thyroid nodule simultaneously, or only uses external application for treating mild patients, and proves that the clinical curative effect is safe and reliable through a great deal of clinical practice verification. Observing symptoms before and after treatment, detecting thyroid function and thyroid color Doppler ultrasound, and evaluating curative effect; detecting blood routine, liver function and kidney function, and performing safety evaluation.
(2) The diagnosis standard is prepared by referring to China's header "diagnosis and treatment guidelines for thyroid nodule and differentiated thyroid cancer" which are jointly compiled by the national health publishing society 2018 (ninth edition) and the China's China medical society endocrinology and science conference, the China's society of surgery and head and neck tumor professional committee of China's anticancer society, and the China's society of nuclear medicine conference:
(1) medical history: the onset of the disease is hidden, the disease course is different, and the disease is not easy to be found.
(2) Symptoms: most cases have no clinical manifestations, and when thyroid dysfunction is combined, symptoms of hyponychium such as hypodynamia, inappetence and the like can appear, or clinical manifestations of hyperthyroidism such as palpitation, tremble hands, hyperhidrosis and emaciation can appear. Some patients have symptoms of oppression and discomfort of neck, hoarseness, compressed air feeling, unsmooth breathing, dysphagia and the like, which are caused by the fact that the nodule is enlarged to press surrounding tissues. When inflammation, hemorrhage or cancer occurs, there may be local symptoms such as neck pain and tenderness, and systemic manifestations such as fever.
(3) The physical sign is as follows: palpation can detect one or more tumors in the thyroid gland on one side or both sides, and can move up and down with swallowing movements.
(4) Auxiliary inspection: thyroid ultrasound examination can discover the presence of one or more nodules within the thyroid gland and determine the location, texture, shape, clarity of the boundaries, integrity of the envelope, presence of calcification foci, and blood supply around and within the nodules. Thyroid function examination may be in the presence of elevated, normal or reduced TSH, TGAb, TPOAb may be negative or positive.
(3) The diagnosis standard of the Chinese medicine syndrome refers to the national standard of the people's republic of China, the Chinese medicine clinical diagnosis and treatment terminology and syndrome section (GB/T16751.2-1997), and the Chinese medicine syndrome type is determined to be qi stagnation, phlegm coagulation and blood stasis type.
(4) The curative effect is comprehensively evaluated according to the changes of main symptoms and signs before and after the treatment of patients:
traditional Chinese medicine syndrome curative effect judgment standard: formulation by referring to the 2002' guidelines for clinical research on New Chinese medicine
Clinical recovery: the clinical symptoms and signs of the traditional Chinese medicine disappear or basically disappear, and the integral of symptoms is reduced by more than or equal to 95 percent.
The effect is shown: the clinical symptoms and signs of the traditional Chinese medicine are obviously improved, and the integral of symptoms is reduced by more than or equal to 70 percent and is less than 95 percent;
the method is effective: the clinical symptoms and signs of the traditional Chinese medicine are all improved, and the integral of symptoms is reduced by more than or equal to 30 percent and is less than 70 percent;
invalidation: the clinical symptoms and physical signs of the traditional Chinese medicine are not obviously improved or even aggravated, and the integral of symptoms is reduced by less than 30 percent.
Note that: calculation formula (nimodipine method): [ integration before treatment-integration after treatment ]
Total effective rate= (clinical cure + onset + efficacy)/total number of cases 100%.
(5) Therapeutic results
The results show that each group of treatment achieves better clinical effects, wherein the traditional Chinese medicine composition of the embodiment 3 has more remarkable effects in improving symptoms and preventing the development of thyroid nodules. See table 1:
table 1 comparison of clinical efficacy of internal and external treatment groups and Chinese medicine oral administration group
Figure BDA0003789749690000071
(6) Adverse reactions
None of the patients in the treatment groups 1-3 showed serious adverse reactions, of which 3 showed skin allergic reactions during the use of the external patch, and were relieved by themselves without special treatment, and no other reactions were found.
