狼疮抗凝物(LA)是免疫球蛋白并且属于获得性自身抗体的类型。它们引起抗磷脂综合征(APS),这是最常见的自身免疫性疾病之一,并且引起血栓形成、复发性流产以及妊娠期并发症。狼疮抗凝物免疫球蛋白是所谓的抗磷脂抗体(APA),其结合阴离子磷脂、蛋白质或蛋白质/磷脂复合物。与某些蛋白和磷脂形成复合物的抗磷脂抗体是自身抗体的非常多样化(异质,heterogeneous)的组,其可以针对多种抗原,诸如针对载脂蛋白β2-糖蛋白I(β2GPI)、心磷脂、凝血酶原、蛋白C、蛋白S、血栓调节蛋白、因子XII等,并且针对这些蛋白与磷脂的复合物。
所谓的狼疮抗凝物是抗磷脂抗体,根据定义,该抗体延长某些凝血测试(例如APTT)的凝血时间。矛盾的是,狼疮抗凝物在体外会引起凝血反应的抑制;而在体内,增加的凝血反应(高凝性)与抗磷脂综合征(APS)相关。
抗磷脂综合征(APS)的实验室诊断因抗磷脂抗体的多样性(异质性,heterogeneity)而复杂化。为了直接检测抗体,使用免疫学方法。然而,在此还检测到许多在体内不具有凝血作用的抗体。对于抗体的间接功能检测,使用凝血测试。在凝血测试中,患者的血浆样品通常与凝血激活剂、磷脂和钙离子混合,并且测量的是直至凝块形成的时间。基于确定DRVVT(稀释的Russell氏毒液时间)的特别测试显示与抗磷脂抗体的凝血功效相对良好的相关性。狼疮诊断是非常复杂的,因为每个患者样品必须经过多个分析步骤(关于综述,参见:Devreese,K.and Hoylaerts,M.F.,Challenges in the diagnosis of theantiphospholipid syndrome.Clin.Chem.2010,56(6):930-940)。
通常,使用至少一种凝血测试的两种变体来分析患者样品,以诊断狼疮抗凝物。在对狼疮抗凝物敏感的第一变体中,在相对低浓度的磷脂的存在下进行测试(筛选测试);在对狼疮抗凝物不敏感的第二变体中,在存在相对高浓度的磷脂的情况下进行相同的测试(确认测试)。以下两者的结合指示狼疮抗凝物的存在:在第一测试变体(筛选测试)中,与正常样品(不含狼疮抗凝物)相比,延长的凝血时间;在第二测试变体(确认测试)中,与正常样品相比,延长的凝血时间。
特别常用的测试是DRVVT测试,其中对狼疮抗凝物敏感的第一变体用含有Russell氏毒液和相对低浓度的磷脂的活化试剂(LA1筛选试剂)进行,对狼疮抗凝物不敏感的第二变体用含有Russell氏毒液和高浓度的磷脂的活化试剂(LA2确认试剂)进行。如果LA1测试是阴性的,即与比较标准相比没有测量到延长的凝血,则不再需要进行LA2测试。另一种常用的测试是APTT测试。
因此,不利的是,狼疮抗凝物的间接功能性检测总是要求进行至少两次凝血测试,这伴随着所要求的材料、时间和工作的增加。
发明内容
本发明的目的是提供一种用于间接功能性检测体液样品中狼疮抗凝物的方法,该方法避免了上述缺点。
已经发现,可以通过确定用狼疮抗凝物敏感性APTT试剂进行的单一凝血反应的凝血时间和评估反应曲线的某些参数来检测狼疮抗凝物。
因此,本发明提供了用于检测患者血浆样品中的狼疮抗凝物的方法。该方法包括以下步骤:
通过向血浆样品中添加狼疮抗凝物敏感性APTT试剂提供反应混合物,并且开始凝血反应,
测量反应混合物随时间(t)的测量变量S,得到时间依赖性测量值的函数S(t),以及
确定凝血时间。
根据本发明,在该方法中另外进行的是,
a)确定函数S(t)的最大反应速度vmax和/或最大反应加速度amax,以及
b)确定测量开始时的第一测量值SB与测量结束时的第二测量值SE之间的差值的绝对值(|ΔS|),并且然后
c)分别使用以下公式确定相对最大反应速度vmax rel和/或相对最大反应加速度amax rel:
vmax rel=vmax/|ΔS|,以及
amax rel=amax/|ΔS|。
如果与预定参考值相比凝血时间延长,并且相对最大反应速度vmax rel和/或相对最大反应加速度amax rel在预定狼疮抗凝物特异性值范围内,则最终检测到狼疮抗凝物。
患者的血浆样品优选是人的低血小板计数血浆样品。优选地,该低血小板计数血浆样品获得自柠檬酸化的全血。
APTT(活化的部分凝血活酶时间)是检查固有凝血系统的测试。添加到待测试样品中的APTT试剂通常包含磷脂(“部分凝血活酶”)、表面活性物质(所谓的“接触活化剂”),诸如鞣花酸、高岭土或二氧化硅以及任选地钙离子。与狼疮抗凝物不敏感性APTT试剂相比,狼疮抗凝物敏感性APTT试剂通常包含降低浓度的磷脂,这意味着与正常(无狼疮抗凝物)样品相比,使用这种试剂测量含有狼疮抗凝物的样品的凝血时间导致延长的凝血时间。狼疮抗凝物敏感性APTT试剂可以包含另外的组分,诸如某些二价金属离子产生物质,这些物质促进凝血时间-延长作用的作用(例如,参见EP 3076178 A1)。可替代地,可以在单独的试剂中提供钙离子,除了狼疮抗凝物敏感性APTT试剂之外,将该试剂添加到待测试的样品中。添加钙离子开始反应混合物中的凝血反应。
测量反应混合物随时间(t)测量变量S,得到时间依赖性测量变量的函数S(t),通常从添加钙离子开始,或者在添加钙离子之后立即或之后不久开始。
例如,作为凝血反应的结果随时间改变的反应混合物的典型测量变量S是作为反应混合物中纤维蛋白形成的结果的反应混合物的浊度或粘度,并且它们可以借助光学或机械方法定量测定。在某个时段上测量变量的连续测定得到时间依赖性测量值的函数S(t),即反应曲线。根据测量变量的性质,测量变量可与凝血反应成比例或反比例地改变。
反应混合物的凝血时间的确定可以用任何常规的评估方法进行。通常,术语“凝血时间”应理解为意指从凝血反应开始,通过向样品中添加相关试剂直到纤维蛋白凝块的有形形成(以秒计)的时间范围(跨度,span)。凝血时间优选基于反应曲线和合适的评估方法来确定。
将针对该样品或反应混合物确定的凝血时间与预定参考值进行比较,这将正常凝血时间与延长的凝血时间进行区分。通过例如使用狼疮抗凝物敏感性APTT试剂确定统计上显著数量的正常(无狼疮抗凝物)血浆样品和/或一个或多个正常血浆池的凝血时间来预先确定所述参考值。
函数S(t)的最大反应速度vmax可通过确定函数S(t)的一阶导数(dS(t)/dt)的最大值或最小值来确定,这取决于测量变量是否与凝血反应成比例或反比例地改变。
函数S(t)的最大反应加速度amax可通过确定函数S(t)的二阶导数(d2S(t)/dt2)的最大值或最小值来确定,这取决于测量变量是否与凝血反应成比例或反比例地改变。
此外,确定测量开始时的第一测量值SB与测量结束时的第二测量值SE之间的差值的绝对值(|ΔS|)。众所周知,凝血反应基本上以三个阶段进行。在第一阶段中,在测量开始时,即从样品已经与凝血时间试剂和钙离子混合并且因此已经开始凝血反应的时间点开始,信号没有显著变化,即,在某个时间段内,测量变量S没有变化可被识别,即,该反应曲线基本上平行于x轴(t)延伸。在随后的第二阶段中,可以识别的是信号的变化,其变化速率在已经达到最大值之后减小之前首先增大。在随后的第三阶段中,在测量结束时,信号电平已经达到最大值,并且在信号中不再有任何进一步的变化,即,该反应曲线再次平行于x轴(t)延伸,但是是在与第一阶段中不同的信号电平下。因此,第一测量值SB是来自凝血反应的第一阶段的测量值;因此,第二测量值SE是来自凝血反应的第三阶段的测量值。第一测量值SB和第二测量值SE在各自情况下可以是单独的测量值或在各反应阶段内的多个(例如2个、3个、4个、5个或更多个)连续测量值的平均值。
此外,检查对样品或反应混合物确定的相对最大反应速度vmax rel和/或相对最大反应加速度amax rel是否在预定狼疮抗凝物特异性值范围内。通过例如以下方式预先确定所述狼疮抗凝物特异性值范围:通过添加狼疮抗凝物敏感性APTT试剂,在统计上显著数量的含有狼疮抗凝物的血浆样品和正常(无狼疮抗凝物)血浆样品(和/或对于一个或多个正常血浆池)中开始凝血反应,测量反应混合物随时间(t)的测量变量S,其产生时间依赖性测量值的函数S(t),并且然后确定相对最大反应速度vmax rel和/或相对最大反应加速度amax rel,如以上在步骤a)至c)中所述。以此方式可以确定的是对于包含狼疮抗凝物的样品特异的值范围。
已经发现,通过根据本发明的方法,含有狼疮抗凝物的样品可以可靠地区别于正常样品和含有延长凝血时间的其他因子的样品,诸如肝素、凝血因子缺乏和直接口服抗凝剂。特别有利的是,单一凝血反应的评估使得能够进行这种区分,其结果是,可以省去根据现有技术所要求的包含不同试剂的第二凝血反应。
本发明进一步提供自动分析仪,该自动分析仪被配置成使得其进行根据本发明的上述方法。
用于自动处理和评估凝血测试的已知的自动分析仪至少包括:(i)一个或多个移液装置,用于将样品体积和至少一个试剂体积转移到反应容器中以制备反应混合物,(ii)测量装置,用于测量反应容器中反应混合物随时间(t)的测量变量S,(iii)数据存储器,用于存储对样品或反应混合物测量的时间依赖性测量值的函数S(t),以及
(iv)评估装置,被配置成使得其使用来自该数据存储器的时间依赖性测量值的函数S(t)来计算凝血时间。
根据本发明的自动分析装置的不同之处在于,该评估装置被另外配置成使得其
a)确定函数S(t)的最大反应速度vmax和/或最大反应加速度amax,以及
b)确定测量开始时的第一测量值SB与测量结束时的第二测量值SE之间的差值的绝对值(|ΔS|),并且然后
c)分别使用以下公式确定相对最大反应速度vmax rel和/或相对最大反应加速度amax rel:
vmax rel=vmax/|ΔS|,以及
amax rel=amax/|ΔS|,
d)将所确定的凝血时间与预定参考值进行比较,以及
e)将相对最大反应速度vmax rel和/或相对最大反应加速度amax rel与各预定狼疮抗凝物特异性值范围进行比较,以及
f)如果与预定参考值相比凝血时间延长并且相对最大反应速度vmax rel和/或相对最大反应加速度amax rel在各预定狼疮抗凝物特异性值范围内,则作为结果输出在样品中存在狼疮抗凝物。
优选地,将样品中存在狼疮抗凝物的结果输出至显示介质,例如监测器、移动装置或打印机,通过该显示介质可以将结果传达至使用者。
用于测量反应混合物的测量变量S的测量装置可以是用于测量光学特性(诸如吸收)的装置,例如光度计。
附图说明
图1示出了Sysmex CS-2100分析仪上的典型的APTT凝血曲线。曲线1示出了在180s时间段内反应混合物以任意单位[AU]随时间的光透射率/传输(S)。可以在软件中读取测量结束时(在时间点t=180s的测量值SE)和测量开始时(在时间点t=0s的测量值SB)的透射率之间的差值Delta S(ΔS)。通过确定测量值对应于差值DeltaS(ΔS)的50%的时间点来确定APTT凝血时间。曲线2示出了相对于时间的数值一阶导数(dS/dt),其是反应速度v的量度。其在最高反应速度vmax下具有最小值。曲线3示出了相对于时间的数值二阶导数(d2S/dt2),其是反应加速度a的量度。其在最高反应加速度amax下具有最小值。最大反应速度vmax和最大反应加速度amax的数值可以经由软件读取。通过将最大速度和最大加速度的绝对值除以ΔS的绝对值得到相对最大速度vmax rel和相对最大加速度amax rel的数值。
具体实施方式
以下将基于示例性实施方式来阐明本发明。
实施例:在根据本发明的每种情况下在单一APTT凝血混合物中的狼疮抗凝物阳性样品的鉴定
测试以下类型的低血小板计数血浆样品:
样品类型 描述
正常 来自健康个体的>100次捐赠的池,稳定;
肝素 来自健康个体的>100次捐献的池,稳定并与未分级或低分子量肝素(UF或LMW肝素)混合;
因子VIII缺陷 缺乏因子VIII的血浆和正常池的混合物;
狼疮抗凝物(LA) 来自个体供体的血浆,在基于稀释的Russell氏毒液(DRVVT)测试的测试系统中的狼疮抗凝物阳性。
对于APTT的测量,使用在Sysmex CS-2100分析仪(Sysmex Corp.)上批准的测试设置。将50μL样品与50μL含有磷脂和鞣花酸作为活化剂的狼疮抗凝物敏感性APTT试剂(ActinFSL试剂,Siemens Healthcare Diagnostics Products GmbH)混合。在+37℃温育180s后,加入50μL 25mM CaCl2以开始反应,并开始获取测量值。
APTT凝血时间是通过确定所测量的反应混合物的光透射率是测量结束时(在时间点t=180s处的测量值SE)与测量开始时(在时间点t=0s处的测量值SB)的透射率之间的差值ΔS的50%的时间点来确定的。超过30s的凝血时间结果被认为是延长的。
相对最大反应速度vmax rel通过首先确定最大反应速度vmax,通过确定反应动力学关于时间的一阶导数(dS/dt)的最小值并且最后将vmax除以ΔS的绝对值来确定。
通过首先确定最大反应加速度amax(通过确定反应动力学相对于时间的二阶导数(d2S/dt2)的最小值)并且最后将amax除以ΔS的绝对值来确定相对最大反应加速度amax rel。
每个样品的结果显示在表1中。

如所预期的,肝素样品、缺乏因子VIII的样品(具有固有体系的因子缺陷)以及狼疮抗凝物阳性样品示出延长的凝血时间(>30s),而正常样品未示出延长的凝血时间。作为对狼疮抗凝物的筛选测试,在此实验中的APTT具有100%的灵敏度。
此外,变得清楚的是,对于狼疮抗凝物阳性样品,如上所述的参数vmax rel在1.1至2.3 10-3s-1的值范围内,该范围对于狼疮抗凝物阳性样品是特异性的。此外,变得清楚的是,对于狼疮抗凝物阳性样品,同样如上所述的参数amax rel在1.3至2.3 10-5s-2的值范围内,该范围对于狼疮抗凝物阳性样品同样是特异性的。其他样品(正常样品、肝素样品、缺乏因子VIII的样品)各自具有参数vmax rel和amax rel的较大数值,该数值在狼疮抗凝物特异的vmax rel和amax rel的值范围之外。
因此,仅源自单一APTT测量并且包括凝血时间的确定和参数vmax rel和/或amax rel的确定的两步法适合作为狼疮抗凝物的证据。
在本实验中,检测狼疮抗凝物的方法具有100%的灵敏度和100%的特异性。
Claims (7)
1.一种用于在患者的血浆样品中检测狼疮抗凝物的方法,所述方法包括以下步骤:
通过向所述血浆样品中添加狼疮抗凝物敏感性APTT试剂提供反应混合物,并开始凝血反应,
测量所述反应混合物随时间的测量变量S,得到时间依赖性测量值的函数S(t),以及
确定凝血时间,
其特征在于,另外地,
a)确定函数S(t)的最大反应速度vmax和/或最大反应加速度amax,以及
b)确定测量开始时的第一测量值SB与测量结束时的第二测量值SE之间的差值的绝对值(|ΔS|),并且然后
c)分别使用以下公式确定相对最大反应速度vmaxrel和/或相对最大反应加速度amaxrel:
vmaxrel=vmax/|ΔS|,以及
amaxrel=amax/|ΔS|,
如果与预定参考值相比所述凝血时间延长,并且所述相对最大反应速度vmaxrel和/或所述相对最大反应加速度amaxrel在预定狼疮抗凝物特异性值范围内,则检测到狼疮抗凝物。
2.根据权利要求1所述的方法,其中,所述狼疮抗凝物敏感性APTT试剂包含磷脂、接触活化剂以及任选地钙离子。
3.根据权利要求1所述的方法,其中,所述最大反应速度vmax对应于所述时间依赖性测量值的函数S(t)的一阶导数的最大值或最小值。
4.根据权利要求1所述的方法,其中,所述最大反应加速度amax对应于所述时间依赖性测量值的函数S(t)的二阶导数的最大值或最小值。
5.一种自动分析仪,包括:
(i)一个或多个移液装置,用于将样品体积和至少一个试剂体积转移到用于制备反应混合物的反应容器中,
(ii)测量装置,用于测量所述反应容器中所述反应混合物随时间(t)的测量变量S,
(iii)数据存储器,用于存储对样品或反应混合物测量的时间依赖性测量值的函数S(t),以及
(iv)评估装置,配置成使得其使用来自所述数据存储器的所述时间依赖性测量值的函数S(t)计算凝血时间,
其特征在于,
所述评估装置被另外配置成使得其
a)确定所述函数S(t)的最大反应速度vmax和/或最大反应加速度amax,以及
b)确定测量开始时的第一测量值SB与测量结束时的第二测量值SE之间的差值的绝对值(|ΔS|),并且然后
c)分别使用以下公式确定相对最大反应速度vmaxrel和/或相对最大反应加速度amaxrel:
vmaxrel=vmax/|ΔS|,以及
amaxrel=amax/|ΔS|,
d)将所确定的凝血时间与预定参考值进行比较,以及
e)将所述相对最大反应速度vmaxrel和/或所述相对最大反应加速度amaxrel与各预定狼疮抗凝物特异性值范围进行比较,并且
f)如果与所述预定参考值相比所述凝血时间延长并且所述相对最大反应速度vmaxrel和/或所述相对最大反应加速度amaxrel在各预定狼疮抗凝物特异性值范围内,则作为结果输出在所述样品中存在狼疮抗凝物。
6.根据权利要求5所述的自动分析仪,其中,所述测量装置适用于测量反应混合物的光学测量变量S。
7.根据权利要求6所述的自动分析仪,其中,用于对反应混合物的光学测量变量S进行测量的所述测量装置是光度计。
